보행 시 여성 노인의 하지 관절 근력과 국부 동적 안정성과의 관계 (Relationship between Strengths of the Lower Extremity's Joints and Their Local Dynamic Stability during Walking in Elderly Women)

  • Objective: The objective of the present study was to analyze the relationship between strength of the lower extremity's joints and their local dynamic stability (LDS) of gait in elderly women. Method: Forty-five elderly women participated in this study. Average age, height, mass, and preference walking speed were 73.5±3.7 years, 153.8±4.8 cm, 56.7±6.4 kg, and 1.2±0.1 m/s, respectively. They were tested torque peak of the knee and ankle joints with a Human Norm and while they were walking on a treadmill at their preference speed for a long while, kinematic data were obtained using six 3-D motion capture cameras. LDS of the lower extremity's joints were calculated in maximum Lyapunov Exponent (LyE). Correlation coefficients between torque of the joints and LyE were obtained using Spearman rank. Level of significance was set at p<.05. Results: Knee flexion torque and its LDS was negatively associated with adduction-abduction and flexion-extension movement (p<.05). In addition, ratio of the knee flexion torque to extension and LDS was negatively related to internal-external rotation. Conclusion: In conclusion, knee flexion strength should preferentially be strengthened to increase LDS of the lower extremity's joints for preventing from small perturbations during walking in elderly women.

선박 복원성 평가에 관한 항해사의 경험적 인지도 분석 연구 (Study on the Empirical Awareness Analysis of Navigational Officers on the Evaluation of Ship Stability)

  • 선박의 안전운항을 도모하기 위한 필수적 요소 중 하나인 복원성 확보에 대한 항해사의 해기지식은 선박의 대형화 및 자율운항선박 출현 등 선박 기술의 진보와 더불어 향상되고 개선되어야 한다. 이에 따라 본 연구에서는 항해사를 대상으로 복원성에 관한 설문조사를 수행하였고, 일반적 특성을 이용하여 경험적 인지도를 통계분석하였다. 분석 결과, 복원성 기준의 이해도에서 상위직급에서는 높은 이해도를 갖고 있었으며, 특정 선종에 대한 특별 기준에 대한 이해는 부족한 것으로 나타났다. 전체 응답자의 87.6%가 복원성 평가의 수단으로 Loading computer를 활용하고 있었다. 현장에서 복원성 평가 방법으로 GM 활용이 평균 3.891/5.000점으로 가장 높았으며, 주로 GM과 복원성 기준으로 복원성 확보여부를 판단하였다. 복원성 부족 시, 항해사는 주로 평형수 보충을 통하여 개선하였으며, 소각도 타각 사용으로 안전을 확보하는 운항적 경향도 있었다. 본 연구 결과는 선박 복원성에 대한 항해사의 경험적 인지도를 평가함으로써, 향후 운항자 중심의 복원성에 관한 교육 개선 및 연구에 중요한 자료로 활용하고자 한다.

현수보조장치를 이용한 과제 지향적 훈련이 경직형 양하지 뇌성마비아동의 체간 안정성과 대동작기능에 미치는 영향 (The Effects of Task Oriented Training with Suspension Device on Trunk Stability and Gross Motor Function of Children with Spastic Diplegia Cerebral Palsy)

  • PURPOSE: The purpose of this study was to examine the effect of using a suspension device for arm reaching activity on trunk stability and gross motor function of children with spastic diplegia cerebral palsy. METHODS: The subject in this study consisted of 11, GMFCS(Gross Motor Function Classification System) III~IV children with spastic diplegia cerebral palsy, all of whom agreed to participate in the study. All subjects were divided into two groups: the experimental group using a suspension device, and the control group using no suspension device. For each group, a thirty-minute intervention was done twice per week during 8 weeks. Before and after intervention, each test was measured using TIS(Trunk Impairment Scale), GMFM (Gross Motor Function Measure) and PRT(Pediatric Reaching Test) to change trunk stability, gross motor function and arm reaching activity. The data were analyzed with the Wilcoxon signed rank test. RESULT: All two groups had a meaningful increase in GMFM-Sit data measured before and after intervention. The experimental group had a significant increase from an average of 78.83 to an average of 84.83 in GMFM-Crawling. For both groups, there was a substantial increase in the change in sitting position and arm reaching. CONCLUSION: According to the results of this study, the arm reaching activity using suspension device had an effect on trunk stability and gross motor function and it changed arm reaching activity.

Relationship between Center of Pressure and Local Stability of the Lower Joints during Walking in the Elderly Women

  • Objective: The purpose of this study was to determine the relationship between center of pressure (CoP) and local stability of the lower joints, which was calculated based on approximate entropy (ApEn) during walking in elderly women. Method: Eighteen elderly women were recruited (age: $66.4{\pm}1.2yrs$; mass: $55.4{\pm}8.3kg$; height: $1.56{\pm}0.04m$) for this study. Before collecting data, reflective marker triads composed of 3 non-collinear spheres were attached to the lateral surface of the thigh and shank near the mid-segment to measure motion of the thigh and shank segments. To measure foot motion, reflective markers were placed on the shoe at the heel, head of the fifth metatarsal, and lateral malleolus, and were also placed on the right anterior-superior iliac spine, left anterior-superior iliac spine, and sacrum to observe pelvic motion. During treadmill walking, kinematic data were recorded using 6 infrared cameras (Oqus 300, Qualisys, Sweden) with a 100 Hz sampling frequency and kinetic data were collected from a treadmill (Instrumented Treadmill, Bertec, USA) for 20 strides. From kinematic data, 3D angles of the lower extremity's joint were calculated using Cardan technique and then ApEn were computed for their angles to evaluate local stability. Range of CoP was determined from the kinetic data. Pearson product-moment and Spearman rank correlation coefficient were applied to find relationship between CoP and ApEn. The level of significance was determined at p<.05. Results: There was a negative linear correlation between CoP and ApEn of hip joint adduction-abduction motion (p<.05), but ApEn of other joint motion did not affect the CoP. Conclusion: It was conjectured that ApEn, local stability index, for adduction/abduction of the hip joint during walking could be useful as a fall predictor.

Clinical changes of TMD and condyle stability after two jaw surgery with and without preceding TMD treatments in class III patients

  • Background: This study are to identify the symptomatic changes and condylar stability after 2 jaw surgery without preceding treatments for Temporomandibular joints(TMJ) in class III patients with the TMJ symptoms; and to assess therapeutic effect of 2 jaw surgery and the necessity of preceding treatment for alleviation of TMJ symptoms. Methods: 30 prognathic patients with preexisting TMJ symptoms were divided into 2 groups according to presence or absence of preceding treatments before the surgery. We evaluated symptomatic changes on both TMJ by questionnaires and clinical examinations. And we reconstructed 3D cone beam computed tomography images before 2 jaw surgery, immediately after the surgery, and 6 months or more after the surgery with SimPlant software, and analyzed the stability of condylar position on 3D reconstruction model. Significances were assessed by the Wilcoxon signed rank test on SPSS ver. 20.0. Results: Both groups had favorable changes of TMJ symptoms after orthognathic surgery. And postoperative position of condyle had good stability during follow-up period. Conclusion: 2 jaw surgery without preceding treatments for TMD can have therapeutic effect for TMD patients with class III malocclusion.

외항선원 고용형태 결정요인에 관한 연구 (Determinants of Seafarers' Employment Stability)

  • 승선 환경의 폐쇄성과 이사회성, 선내 규율의 엄격함 등으로 청년들은 선원직을 기피하고 있다. 우리나라는 승선근무예비역제도라는 특수한 제도가 존재해 청년들을 선원직으로 유인하고 있지만, 이 제도마저 존립이 위태롭다. 최근의 선원 고용유형은 선박관리회사로부터 고용되어 선사에 파견되는 비정규직 형태가 대부분이다. 선박관리회사(SM)와 선원 간에 임시 고용계약을 맺고, SM이 선사에 선원을 파견하는 비정규직 고용형태가 유행하고 있다. 선원직 매력도가 떨어지고 있는 상황에도 2018년 발표된 국가 해운재건 5개년 계획에는 선박과 화물의 확보 방안만 담겨 있다. 이 논문의 핵심 내용은 이항로짓모형을 이용해 선원의 고용형태 결정요인을 분석하는 것이다. 분석 결과, 선원의 최종학력이 고용형태를 결정하는 요소로 나타난 반면, 소속선사 유형, 승선 선종, 소속 선박관리회사 유형, 근무 분야, 해기사 면허 소지 여부 등은 선원 고용형태 결정에 유의미한 변수가 아닌 것으로 나타났다.

모유수유 저해요인에 대한 산모와 신생아실 간호사간의 인식정도 비교 연구 (A Comparative Study on the Perception of the Impediment Factors between Mothers and Nurses in Nursery)

  • The condition of nutrition during infancy will greatly affect infants' physical, emotional growth, especially breast feeding is important in their growth and development, and emotional stability, too. Despite such advantages of breast-feeding, its rate has continued to fall year after year in Korea : the 95% rate in 1960's has fallen to 25.4% in 1990. It is known that such a downfall of breast-feeding rate is associated with various factors. The purpose of the study is to examine mother's and nurses' in nursery perception of the impediment factors of breast-feeding : to compare those between of breast-feeding : to compare those between two groups : to provide fundamental data for developing strategies for increasing breast-feeding. The subjects were 45 new mothers from one hospital and the same number of nurses sampled from 3 university hospitals, in Seoul. The data were collected for 11 days from April 12 to April 23, 1994 and a questionnaire was developed based on the interview with 14 nurses and 10 mothers and the literature reviews. Liker's Five-Point Scale was used as measurement. The Data were analyzed using SPSS / PC and descriptive statistics, t-test, ANOVA. The results of study are as follows : 1) Most new mothers get the information about breast-feeding from their mothers, books or newspaper, and relatives in rank than professionals(nurses or doctors). 2) The impediment factors the breast-feeding are categorized as mothers, hospital system, medical personnels and social factors. The most frequently mentioned impediment factors from mothers are 'Difficulty by operational wounds'(3.13), 'Lack of will for breast-feeding'(3.47), 'Insufficient rest and sleep'(3.52) and 'Opposition from husband or his family members'(4.77)in rank. On the other hand those factors from nurses are 'Inadequate nipple condition'(2.37), 'Decreasing milk secretion given medicine after operation'(2.63), 'Mothers knowledge deficit by poorly prepared education'(2.79) and 'Mothers abhorance of breast-feeing'(3.87) in rank. 3) As for the hospital system, the highly perceived impediment factors by mothers are 'Seperation of baby from mother after birth'(2.78), 'Lack of space for breast-feeding in the hospital'(2.93), 'Lack of facility for informing the time for breast-feeding'(3.18) and 'Because of babys' preference artificial nipple by being accustomed to it in hospital(3.97), in rank. Meanwhile, those from nurses are "Seperation of baby from mother after birth",(1.92), "Inconsistency between hospital's nursing time and mother's breast secretion time" scretion time(2.97), "Lack of space for breast-feeding in the hospital"(3.39), and "Lack of facility for informing the time for breast-feeding"(3.74) 4) As for the medical personnels, the highestly perceived Impediment factor from mothers in "Lack of professional nurses for breast-feeding"(2.96), and the lowestly perceived one is "Doctors' reluctance"(4.75). Nurses perceived same as mothers, too. 5) As for the social factors, the highestly perceived impediment factor by mothers and nurses is "Inconvience of social activities"(2.83) and the lowestly ones are "The sense of self-sacrifice"(4.22) by mothers, and "The sense of old fashioned"(4.13) by nurses. 6) The difference of the perception of impediment factors between mothers and nurses is statistically significant only in mother factor.

반월상 연골의 관절경적 봉합시 관절안정성에 따른 결과 (Clinical results of arthroscopic meniscal repair according to joint stability)

  • 목적 : 관절경적 반월상 연골 봉합술시 인대 손상 동반 여부에 따른 결과를 분석하였다. 대상 및 방법 : 20례를 대상으로 하였으며 평균 추시 기간은 59.7개월이었다. 환자의 연령은 평균 32.3세였다. 동반 인대 손상이 없는 경우와 동반 손상인대를 치료한 17례를 안정 관절군으로, 동반 손상을 치료하지 않은 3례를 불안정 관절군으로 비교하였다. 안정 관절군은 급성기와 만성기로 분류하였다. 결과는 Lysholm 및 IKDC 평가법으로 판정하였다. 정상 및 아정상군은 만족으로, 비정상 및 장애군은 불만족으로 구분하였다. 통계적인 방법은 Wilcoxon rank sum test, fisher's exact test(p<0.05)를 이용하였다. 결과 : 동반 손상인대를 동시에 치료한 8례는 만족 $87.5\%$, Lysholm score는 평균 90.9을 보였고, 동반 손상을 치료하지 않은 3례는 만족 $66.7\%$, Lysholm score는 평균 77.7을 보였다 동반 인대 손상이 없는 9례는 만족 $88.9\%$, Lysholm score는 평균 91.4을 보였다. 급성기에 치료한 12례는 만족 $91.7\%$, Lysholm score는 평균 92.5를 보인 반면, 만성기의 5례는 만족 $80\%$, Lysholm score는 평균 88.6을 보였다. 결론 : 수상 후 급성기에 파열된 반월상 연골 및 동반 손상인대의 관절경적 복원술을 동시에 시행하는 것이 좋은 결과를 얻을 수 있을 것으로 사료된다.

다중회귀분석을 통한 경남 지방도로 절취사면의 안정성평가 (Risk Assessment of the Road Cut Slopes in Gyeoungnam based on Multiple Regression Analysis)

  • 본 연구는 절리암반사면의 현장상태 및 위험도에 대한 현장조사를 기초로 사면의 상대적 위험도를 판단 할 수 있는 토대를 마련하기 위하여 개별요인이 사면안정성에 미치는 영향을 고찰하였으며 사면붕괴에 영향을 미치는 요인들에 대한 통계적 분석을 수행하였다. 사면의 붕괴요인으로는 기존에 제시된 인장균열, 지반변형, 구조물 변형, 붕괴발생규모, 절리주향, 절리경사, 절리상태, 사면경사, 강우 및 지하수위, 절취상태, 배수조건, 보호보강상태 등 12가지 항목을 고려하였다. 본 연구를 위한 조사지역은 경상남도 지방도로에 위치한 절취사면 중 붕괴 이력이 있거나 다소 불안감을 보이는 위험 절개지 233개소이다. 조사지역의 사면안정성평가는 각각의 평가요인(붕괴요인)에 따른 사면의 결함지수와 사면위험등급에 의해 수행되었다. 조사된 총 233개소의 사면 중 토사사면을 제외한 암반사면 126개소를 선별하여, 각각의 붕괴요인들과 사면위험등급과의 통계적 연관성분석을 실시하였다. 또한, 다중회귀분석을 수행하고 적용성을 고찰하여 사면결함지수와 위험등급을 예측할 수 있는 통계모형을 제시하였다.

용제 중 염산온단세트론의 용해성 및 안정성 (Solubility and Physicochemical Stability of Ondansetron Hydrochloride in Various Vehicles)

  • The solubility and stability of ondansetron hydrochloride (OS) in various vehicles were determined. The effect of cyclodextrins (CD) on the solubility of OS in water was determined by equilibrium solubility method. The solubility of OS at $32^{\circ}C$ increased in the rank order of isopropyl myristate (IPM) < propylene glycol laurate (PGL) ${\ll}$ propylene glycol monolaurate < propylene glycol monocaprylate (PGMC) < poly(ethylene glycol) 400 < diethylene glycol mono ethyl ether (DGME) < ethanol < poly(ethylene glycol) 300 < water (36.1 mg/ml) ${\ll}$ propylene glycol (PG) (283 mg/ml). The addition of PG or DGME to non-aqueous vehicles such as IPM, PGL and PGMC markedly increased the solubility of OS. The addition of CDs in water increased the solubility. Apparent stability constant for the CD complexation with OS was calculated to be $25.5\;M^{-1}$ for $2-hydroxypropyl-{\beta}-CD\;(2HP{\beta}CD)$. Twenty mM ${\beta}-CD$, 69.4 mM sulfobutyl ether ${\beta}-CD$ and 115.4 mM $2HP{\beta}CD$ increased the aqueous solubilty of OS 1.27, 2.18 and 1.85 times, respectively. OS was stable in buffered aqueous solution (pH 5.0). However, OS was relatively unstable in non-aqueous vehicles in the order of PG
