The formation of Sogal concept and classification in Korean Traditional Medicine (한국 한의학에서 소갈 분류의 형성과정)

  • To overcome the limits of prevention and treatment against Diabetes Mellitus(DM) in Western medicine, there have been tendency finding solutions in traditional medicine based on Sogal. But Sogal had been so various concepts, classification and names. As a result there has been confusion in applying Sogal treatment to DM. So in order to clarify, it is necessary to study Sogal concepts and classification historically. The results of studying changes of Sogal concepts and classification are following : Untill AD 8 century, Sogal had not been only syndrome but also disease with throat and urinating difficulties. From 10c to 13c, Sogal had been divided three types in addition to the theories of internal organs, Samcho and complications. From 13c to 14c, the three types of Sogal theory was improved by various medical theory. But still Sogal covered the concepts of syndrome and disease. After 16c, in Chosun Dynasty. concepts of syndrome was eliminated and concepts of disease was strengthend by accounts on pathology, prognosis. complications and malignities. This tendeny was showed well in ${\ulcorner}DongEuiBoGam{\lrcorner}$ and connected to post doctors in Chosun. It was distiction with Chinese Traditional Medicine's view regarding Sogal as syndrome and disease as well, up to the present.

  • Type 2 diabetes mellitus (T2DM) is a progressive disorder caused by a combination of insulin resistance and 𝛽 cell dysfunction. Sogal(消渴) is a traditional Korean medical term referring to a condition pertaining 3 major symptoms - thirst, polyphasia, polyuria. Sogal has been reported to have similar characteristics with DM. This case report demonstrates a patient with T2DM complaining of typical Sogal symptoms. We diagnosed him as So-yang person Sogal and treated him with acupuncture and herbal medicine.

The Fundamental Study of Blood Glucose Lowering Effect of Medications Recored on the Sogal Part of DONGUIBOGAM (동의보감(東醫寶鑑) 소갈문(消渴文)에 수재(收載)된 처방(處方)들의 혈당강하효과(血糖降下效果)에 대(對)한 초보적(初步的) 검색(檢索))

  • In order to investigate the effect of lowering blood glucose level, 27 medications of Sagal part in Donguibogam and the contraindicated medications to Sogal including Ijintang, Dodamtang, Chukdamtang and other six medications which were used to treat Diabetes in the 6th I.M of Kyung Hee Oriental Medical Center were given to the Alloxan induced diabetic mouse and blood glucose level were observed in the 2nd and 4th day. The result of this stqdy are as follows: 1. The 17 medicatlons(63% of 27 medicine) of Sogal part decreased' the glucose level significantly.(Singihwan, Baekhotang, Jowiseunggitang, Yukamijihwangwon, Palmihwan, Kamijeonssibaekchultang, Insamsukgotang, Sacngjinyanghyultang, Insambokryungsan, Hwalhyulyunjosaengjineum, Chungsinbogitang, Hwanggitang, Hwangryunjihwangtang, Ojeupokchunhwan, Yongbongwon, Joosahwangryunwon, Kagambaekchulsan) 2. Dry herbs like Banha, Namsung are the contraindicated herbs in the Sogal part of Donguibogam but Ijintang and Dodamtang which include those herbs also decreased the blood glucose level significantly. 3. The three medications-Bangpungtongsungsan, Bohyulansintang, Bogantang from the six me dications which were used to treat Diabetes in the 6th I.M. of Kyung Hee Oriental Hospital also decreased the blood glucose level significantly.

The Comparative Study between the Transformations(傳變症) of Sogal(消渴) and the Complications of Diabetes Mellitus (소갈(消渴)의 전변증(傳變症)과 당뇨병(糖尿病)의 만성합병증(慢性合倂症)에 대한 비교고찰(比較考察))

  • Acute complications of diabetes mellitus were diminished after Banting and Best discovered insulin. But chronic complications of diabetes mellitus have been increased. The main complications of diabetes mellitus are diabetic retinopathy, diabetic nephropathy, diabetic neuropathy, diabetic foot lesion and macrovascular complication. These complications can result in renal failure, loss of sight, cerebral infarction and myocardial infarction. So it is very difficult to treat the complications of diabetes mellitus. In oriental medicine, the transformations(傳變症) of Sogal(消渴) are edema, carbuncle, loss of sight and so on. The comparative study between the trcmsformations(傳變症) of SogaI(消渴) and the complications of diabetes mellitus has come to the following conclusions. 1. In oriental medicine, diabetic retinopathy was expessed as loss of sight and the treament of diabetic retinopathy should be started at an early stage, to prevent vitreous hemorrhage and traction retinal detachment. 2. In oriental medicine. diabetic nephropathy was expressed as edema and the treatment should be started at an early stage of renal injury when the protein comes from urine.3. Symmetrical distal polyneuropathy is the main part of diabetic neuropathy and it was expressed as weakness of the lower limbs and pain of joints in the symptoms of Haso(下消). In Oriental medicine, acupuncture and herb medicine which effect is SopungHwalHyul can treat polyneuropathy. 4. Chief macrovascular complications are coronary artery disease and cerebrovascular disease, The cause of macrovascular complication is atherosclerosis. So the method of treating atherosclerosis should be studied in oriental medicine. 5. Diabetic foot were expressed as carbuncle and its main causes are decreasing perfusion of fool, diabetic neuropathy and infection. So these causes should be studied in oriental medicine. 6. The complications of diabetes mellitus afe very similar to the transfonnatiuns of Sogal(消渴).The control of blood glucose is indispensable to prevent and delay the complication of diabetes mellitus.

A Case Study of Two Type II Diabetic Patients Diagnosed with Sogal (소갈(消渴)로 진단된 2형 당뇨병 환자 치험(治驗) 2례)

  • Objectives: This study aimed to explore and describe how two patients diagnosed with type II diabetes mellitus were successfully cured solely through the use of herbal medicine. Methods: The patients were administered the herbal medicine to reduce symptoms of stress-induced fever with dryness. The prescribed herbal medicines included Yangkyuksanwha-tang, Hyungbangsabaek-san, Hyungbangjihwang-tang, and Hyungbangdojeock-san. Results: The therapeutic outcomes showed that all numerical levels of symptoms such as thirst, polyuria, weight loss, blood sugar, and glycated hemoglobin (HbA1c) recovered to within normal levels. Conclusions: The results of this study suggest that our medical treatment of type II diabetes mellitus diagnosed as Sogal was a valid treatment. We hope that our team's new evidence and findings will shed positive light on the treatment of type II diabetes mellitus using herbal medicine.

Screening of Insulin-like Substances from Traditional Herbs of Diabetes Prescription in Donguibogam (동의보감 당뇨 처방에 사용되는 한약재에서 인슐린성 물질(Insulin-like substances)의 탐색)

  • In order to search for insulin-like substances from the constituted herbs of Sogal prescriptions, we selected 19 traditional herbs, based on a review of the Donguibogam. The effects of the hot-water extract from the selected herbs on the proliferation and the differentiation of 3T3-L1 fibroblasts were tested. The various water-extracts from Pinellia ternata, Magnolia obovata, Rheum palmatum, Acanthopanax sessiliflorun, Atractylodes japonica and Strychnos ignatii inhibited the proliferation of 3T3-L1 fibroblasts, did not influence entirely in differentiation of 3T3-L1 fibroblasts. Treatment of 3T3-L1 fibroblasts with the extract from Ephedra sinica, Trichosanthes kirilowii, Scrophularia buergeriana and Sophora flavescens significantly increased the differentiation of the cells. In conclusion, these may contain such compounds that play a role of insulin-like action.

A Case Study of Type-II Diabetes Mellitus: Two Patients with Taeumin's Dry-febrile Symptom (태음인(太陰人) 조열증(燥熱證)으로 진단된 2형 당뇨병 환자 치험(治驗) 2례)

  • Objectives: This study describes how two patients diagnosed with type-II diabetes mellitus were successfully treated using herbal medicine. Methods: The patients were given Yuldahanso-tang to reduce the effects of Taeumin's dry-febrile symptom. Results: Type-II diabetes mellitus symptoms experienced by patients with Taeumin's dry-febrile symptom included thirst, fatigue, weight loss, and numbness in the hands and feet. After administering Yuldahanso-tang, all numerical levels for blood sugar, glycated hemoglobin (HbA1c), glycosuria, and proteinuria recovered to within normal levels. Conclusions: Treatment of patients with Taeumin's dry-febrile symptom and type-II diabetes mellitus with Yuldahanso-tang was effective, and this new evidence supports the use of herbal medicine for treating such patients.

  • 1. Objectives The purpose of this study was to investigate the effects of administration of Indongdeungjikolpi-tang on the Soyangin Jungso(中消). 2. Methods To heal the Soyangin Jungso, we used Indongdeungjikolpi-tang(忍冬藤地骨皮湯). 3. Results Jungso of a Soyangin patient responded well to the treatment with Indongdeungjikolpi-tang. 4. Conclusions This case-study shows an efficient results by using Indongdeungjikolpi-tang in treatment of Soyangin Jungso(Diabetes Mellitus).

A Study on the Food Cure in Eumsunjungyo Singnyojaebyeong (『음선정요(飮膳正要)·식료제병(食療諸病)』의 식치(食治)에 관한 연구(硏究))

  • Objectives : This study examined the food cures in Eumsunjungyo Singnyojaebyeong. The diseases described in Singnyojaebyeong still occur frequently in modern times. This study also investigated the recipe of food cures for applying to the modern time treatment. Methods : I categorized the major symptoms of the sixty-one food therapies in Singnyojaebyeong, and sorted the main ingredients of each symptoms, and organized the efficacies of food sources based on third book of Eumsunjungyo. Results : Singnyojaebyeong, which organized the treatment methods curing diseases with foods, provided information about the foods that are good for twenty diseases, such as Heoro, Shinheo, Pung, Yotong, Heoyak, Biwui Heoyak, Seupbi, Sujong, Sogal, Sobyeonbultong, Gakgi, and Chijil. The food therapy methods were used to cure diseases using the food sources suitable for each major symptom. Conclusions : The food therapies in Eumsunjungyo Singnyojaebyeong proves that people in all regions have used food to cure diseases from old times. And to cure diseases, people have used the characteristics, tastes, and cooking forms of food ingredients for a long time. Thus, this study can be a reference for creating another food therapy.

Treatment of Insomnia in Hyungsang Medicine (불면증의 형상의학적 고찰)

  • Insomnia describes not only a state of a lack of sleep but also a causative factor for both psychological and physical disorders. Most people tend to underestimate the effects of insomnia on psychological problems, stroke, Sogal or other life-threatening conditions. insomnia may occur when 1) the flow of Jung(精), Ki(氣), Shin(神) and Hyul(血) is not smooth; 2) the five Jang organs and six Bu organs are in disharmony; 3) Shin(身) and Hyung(形) are inconsistently developed; 4) Young and Wi(衛) do not work in concert; 5) and other factors such as age or exposure to Cold are involved. I observed my patients suffering from insomnia according to Hyungsang medicine, which is based on 'Dongyuibogam'(東醫寶鑑). Insomnia due to the problems with Jung, Ki. Shin and Hyul was found in the Shin type(神科) and the Ki type(氣科). When it comes to the organs, sleep disorder significantly correlates to weakened Spleen, Liver and Kidneys. In terms of the inconsistent development of Shin and Hyung, patients who were shortsighted or experienced surgery showed insomnia. Patients exposed to Cold showed false type agitation. With respect to age, while children showed a weakened Gallbladder, the aged had deficient Blood. Sleep disorder caused by disharmony of Young and Wi occurs at dusk or dawn.
