여대생의 금연행위에 영향을 미치는 요인에 관한 연구 (Determinants of Smoking-Cessation Behaviors in Female University Students)

  • This study was done to determine the factors influencing smoking-cessation behavior in female university students. A total of 534 students participated in this cross sectional study by answering a questionnaire. The data collection was done between September 1 and October 31, 1997 The measurement tools used in this study were the self help change process scale (Cronbach's alpha=.9930 : developed by Oh & Kim, 1996) for smoking-cessation behaviors, the self efficacy scale(Cronbach's alpha=.8250 : developed by Sherer et al, 1982), the sex role acceptance scale (KR-20=.7757 : developed by Kim, 1991) and the social support scale(Cronbach's alpha=.9172 : developed by Park, 1985). The summarized results are follows : 1. The mean scores for smoking-cessation behaviors in smokers (N=150) was 91.72 that was considered a middle score compared to the total possible score of measurement tool (150.0). The mean score for smoking-cessation behaviors by smoking-cessation step showed significant different between the groups(F=11.71, p=.000). 2. The group with no experience in smoking(N=332) showed a high general self efficacy score (t=5.24, p=.000), and more openness to sex role acceptance(t=-2.15, p=.032) compared to the group with smoking experience (N=202). 3. General self efficacy, sex role acceptance, and social support were not different significantly between the groups according to the steps in smoking-cessation. 4. Significant factors influencing smoking-cessation behavior (total, sub concepts) were religion, sex role acceptance, social support, smoking duration, smoking attitude, time of smoking onset, amount of smoking, drinking, and perception of health status. 5. Smoking-cessation behaviors which explained 11% of the variance were smoking attitude, and smoking duration. In conclusion, this study identified factors influencing smoking-cessation behavior. Thereby it will help in the development of smoking-cessation intervention strategies. For future research, exploration other determinants of smoking cessation behaviors, evaluation of intervention efficiency, and comparative study by gender characteristics are needed.

경피적 관상동맥 중재술을 받은 환자의 금연교육이 흡연에 미치는 효과 (Effects of Smoking Cessation Education for Patients with Percutaneous Coronary Intervention)

  • Purpose: The purpose of this study was to investigate the effects of a smoking cessation education for patients with percutaneous coronary intervention. Methods: The study was a quasi-experimental study with a non-equivalent control group non-synchronized design. The Sixty subjects were assigned to either the treatment group. The treatment intervention was a onetime program during hospitalization. Data were collected during several time periods: before treatment, immediately after treatment, two weeks later, and one month after the treatment. Data were analyzed with the SPSS 14.0 program with an independent t-test, $x^2$ test, repeated measures ANOVA. Results: There were significant differences between the two groups in terms of knowledge and attitude. The significance was absent for the testing two weeks post group intervention and then one month later the significance was again detected in the amount of smoking and smoking behavior. Conclusion: The results of this study suggest that smoking cessation education could be utilized for inpatients with percutaneous coronary intervention as nursing intervention in hospitals effectively. Further research is recommended in order to understand the change in significance in the different time periods.

국내 청소년 흡연과 금연에 관한 선행연구 분석 (An Analysis of Articles Related to Smoking and Smoking Cessation of Korean Adolescents)

  • Purpose: The purpose of this study was to analyze the contents and trends of smoking and smoking cessation of adolescents in Korea. Methods: The subjects were 315 researches from 1997 to 2008 in Korea. They were examined in terms of subjects, research design, key words, types of intervention, times of intervention, and dependent variables for experimental research, and in terms of subjects, concepts, research questions and research methods for qualitative research. Results: Of the studies 52.4% were master's theses, and 22.9% of the subjects were male high school students. The most common research design was descriptive design. Major concepts studied were related to emotional cognitive variables. Among 80 experimental researches, 70% were about smoking cessation programs. The number of qualitative studies was only 6. Their concepts were smoking experience, motives for smoking cessation, smoking process, and motives for smoking. A major research question was: What does smoking mean to adolescents? Conclusion: More research should be done for the female students' smoking and smoking cessation. Also, research that adapts educational programs for smoking and smoking cessation needs to be performed through at least a 10 times longer period and in regular basis to get desirable results.

여고생을 대상으로 한 개별화 금연 프로그램의 효과 (Effects of a Tailored Smoking Cessation Program in High School Girls)

  • Objectives: This study was conducted to evaluate the effects of a tailored program based on the Transtheoretical Model to smoking cessation in high school girls. Methods: A quasi-experimental research design was used in this study. The participants were 35 industrial high school girls who have been smoking. The girls were allocated to an experimental group(18) and a control group(17) by randomization. For intervention, the experimental group received the group smoking cessation program and individual program which tailored according to the stage of change. A common group smoking cessation program was given to the control group. Data were collected before the program, immediately after and 4 weeks after the program was completed and were analyzed with repeated measure ANOVA. Result: In the experimental group, the daily smoking amount, nicotine dependency and decisional balance(Pros) score were significantly decreased and the self-efficacy score were significantly increased at one month after the intervention in comparison with before and immediately after it. The decisional balance(Cons) and processes of change score of the experimental group were significantly increased at immediately after the intervention and one month after it in comparison with before it, and at one month after it in comparison with immediately after it. A significant difference in the daily smoking amount, urine cotinine, nicotine dependency, decisional balance(Pros, Cons) and processes of change score between the both groups was found after one month of intervention. Conclusions: The tailored smoking cessation program was more effective, compared to the common smoking cessation program on smoking behavior, self-efficacy, decisional balance, processes of change. This program can be used for favorable changes in high school girl's smoking behavior.

일부 지역 의사들의 흡연실태와 금연지도활동에 대한 조사연구 (Smoking Status and Smoking Cessation Activity among Physicians in a Community)

  • Objectives : The purposes of this study were to assess the smoking status, knowledge and attitude related to smoking and smoking cessation activity of the physicians in a community, and to identify their predictors of smoking cessation activity. Method : All physicians employed by various health facilities in a community were surveyed using a structured questionnaire. Of the physicians surveyed,523 (69.6%) returned completed questionnaires. Results : The smoking rate of physicians was 29.3% (34.2% in males, 3.6% in females) and the knowledge and attitude scores to smoking were $22.5{\pm}2.4\;and\;65.4{\pm}0.9$, respectively. The self-efficacy score was $3.4{\pm}1.0$ and the smoking cessation activity score was $65.4{\pm}6.9$. The smoking cessation activity was statistically significant with working place, specialty, knowledge and attitude to smoking and self-efficacy. In stepwise multiple regression, smoking cessation activity was predicted by doctors' working place, specialty, attitudes related to smoking issues, and self-efficacy of counseling knowledge and stills. Conclusion : Physicians need to participate routinely and actively in smoking cessation activity. For doctors to effectively counsel and intervene in patients regarding smoking cessation, it is essential to integrate education un smoking cessation intervention into curricula in formal education and to offer continuing education including smoking cessation intervention.

산업장 남성 근로자의 금연행위 변화단계별 특성 (The Characteristics of Smoking Cessation Behavior by the Stage of Change in Industrial Workers)

  • Purpose: This study was to identify the stages of change in smoking cessation behavior and factors associated with the stages of smoking cessation behavior according to the trans-theoretical model. Methods: The subjects were 154 industrial workers working at H Industry in N City, Chonnam Province who were currently smoking and had smoked in the past. Data were analyzed by descriptive statistics, ANOVA, and Duncan's multiple comparison test using SAS Version 10.0. Results: The subjects were distributed among the stages of change in smoking cessation behavior: there were 28 subjects (18.2%) in the precontemplation stage, 71 (46.1%) in the contemplation stage, 21 (13.6%) in the preparation stage, 8 (5.2%) in the action stage, and 26 (16.9%) in the maintenance stage. The amount of smoking per day, self-efficacy, and advantages (pros) of smoking were significantly associated with the stage of change in smoking cessation behavior. Conclusion: This study suggested that the stage of change in smoking cessation behavior of the subject should be identified prior to the application of intervention programs, nursing intervention strategies should be considered to reduce the amount of smoking per day, and the disadvantages of smoking should be perceived.

  • Objectives: The aim of this study was to examine factors related to smoking behavior, and to develop multilevel communication strategies for smoking cessation. Methods: This paper reviewed theories and empirical findings with currents ecological models to develop communication strategies. Theory comparison was also performed to identify important mediators in the process of smoking cessation. Results: Factors that have been identified to influence smoking behavior ranges from individual perception, attitudes and self efficacy toward smoking to organizational norms, regulations, community capacity, media advocacy and public smoking regulation policy. In order to address these multi-level determinants of smoking behavior, objectives and strategies for smoking cessation intervention were developed utilizing ecological perspectives to cover intrapersonal, interpersonal(mainly family member and peers), organizational and community/public policy level factors. Conclusion: Multilevel approaches have advanced the existing knowledge on determinants of health behaviors. New direction of research focusing on testing multilevel intervention approaches should be expanded to inform the efficacy of applying social ecological models to health behavior change process.

국내 흡연과 금연관련 간호 연구동향 및 미래 방향 (Analysis of Smoking and Smoking Cessation Related Nursing Research in Korea and its Future Direction)

  • Purpose: The purpose of this study was to analyze the contents and trend of smoking and smoking cessation related nursing research in Korea and to suggest the directions for future research. Method: Eight nursing professional Journals published by the Korean Academic Nursing Society and by 7 - adult, community, psychiatric, fundamental, obstetrics, pediatrics, administrative- nursing societies from beginning edition to October 2005 were selected. They were examined for the proportion of published research, participants, research designs, key words, measurement tools, and the intervention outcomes in the case of the experimental research. Result: Of the research analyzed, 87 studies were smoking and smoking cessation related research. The Journals, which had published the most number of smoking and smoking cessation research were J of Korean Academic Society, J of Korean Adult Nursing Society, J of Korean Community Nursing Society. The most popular research design was an descriptive design. The major concepts studied were related psychosocial variables. Among 87 studies, only 11 were experimental research. Conclusion: Research on smoking and smoking cessation in the nursing discipline are increasing. However more research to test the effectiveness of nursing intervention programs are needed.

외래환자에서 금연 성공의 결정요인 (Predictors of smoking Cessation in Outpatients)

  • Objective : This study was conducted in order to investigate predictors of smoking cessation in outpatients. Method : Subjects were 40f adult smoking patients who saw their doctors in the outpatient setting at a university hospital, regardless of their willingness of otherwise in smoking cessation. Physicians delivered a brief, stop smoking prompt to all patients who smoked one or more cigarettes a day. Then they referred to on-site counselors who provided a brief, nurse assisted intervention with a survey to a randomly assigned intervention group (200 smoking patients), whom the counselors telephoned later to prevent relapse or promote the motivation to quit, or gave only a survey to a control group (201 smoking patients). After at least 5 months, self-reported current smoking cessation was confirmed later using cut-off values of 7 ppm or less in expired alveolar air after breath holding portable CO analyzer. Results : After 5 months, subjects in the intervention group were 1.56 times (95% C.I. 0.89-2.73) more likely to quit smoking than those in the non-intervention group (14.0% vs. 9.0%). Willingness to quit smoking in a month, scheduled admission in a month, self efficacy score and FTND (Fagerstrom Test for Nicotine Dependence) score were all significantly related with smoking cessation. In stepwise multiple logistic regression, previous attempts to quit smoking were significant instead of self efficacy score. In the intervention group who had willingness to quit smoking in a month (132 smoking patients), FTND score, whether quit date was today, and whether quit promise paper was submitting were all significantly related with smoking cessation. In stepwise multiple logistic regression, scheduled admission in a month and whether quit date was today were significant predictor variables, Smoking cessation treatment should be tailored to individual smoking patients considering these predictors.

청소년 흡연자의 금연의도 및 금연행위 예측을 위한 계획적 행위이론(Theory of Planned Behavior)의 검증 (Testing of the Theory of Planned Behavior in the Prediction of Smoking Cessation Intention and Smoking Cessation Behavior among Adolescent Smokers)

  • Objectives: The purpose of this study was to test the Theory of Planned Behavior (TPB) in the prediction of smoking cessation intention and smoking cessation behavior among adolescent smokers, in order to provide basic data to develop a future smoking cessation program as a nursing intervention. Method: The study subjects were 80 adolescent smokers who had smoked one cigarette and attended a five-day school smoking cessation program. The data were collected from October 24 to December 21, 1999. The instruments used in this study were the tools developed by Jee (1994) to measure TPB variables such as attitude toward smoking cessation behavior, subjective norm, perceived behavioral control, smoking cessation intention, and smoking cessation behavior. The data were analyzed with the SAS/PC program using descriptive statistics, hierarchical multiple regression, and logistic multiple regression. Results: 1. Attitude toward smoking cessation behavior, subjective norm, and perceived behavioral control were partially significant in predicting smoking cessation intention. 2. Smoking cessation intention and perceived behavioral control toward smoking cessation behavior did not significantly predict smoking cessation behavior. 3. There were partial interaction effects among the attitude toward smoking cessation behavior, subjective norm, and perceived behavioral control in the prediction of smoking cessation intention. 4. There were partial interaction effects between smoking cessation intention and perceiver behavioral control toward smoking cessation behavior in the prediction of smoking cessation behavior. Conclusion: This study partially demonstrated support for the TPB model that was partially useful in predicting smoking cessation intention and smoking cessation behavior among adolescent smokers. Therefore, it is recommended that attitude toward smoking cessation behavior and perceived behavioral control should be considered in developing smoking cessation programs and implementing nursing interventions to change the smoking behavior of adolescent smokers.
