종합병원의 경피적 관상동맥조영술 후 환자의 수면의 질과 수면방해요인 (Sleep Quality and Factors affecting Sleep Disturbance among Percutaneous Coronary Angiography Patients in a General Hospital)

  • Purpose: The purpose of this study was to investigate the variance of sleep quality and factors affecting sleep disturbance among patients with percutaneous coronary angiography in a general hospital. Methods: The subjects of this study was comprised of 101 patients with percutaneous coronary angiography in a general hospital located in B city. Data were collected by a structured questionnaire from November to December 2013. The data were analyzed with descriptive statistics, t-test, ANOVA, Pearson's correlation coefficient and stepwise multiple regression. Results: The levels of the participants' sleep quality and sleep disturbance were 4.35 and 5.91 in order of each out of the total score, 10. There was no significant difference in the quality of sleep between intensive care unit and internal medicine unit. Factors influencing sleep quality were 'uncomfortable bed, gown and pillow'(${\beta}=-.279$, p=.003), 'living alone'(${\beta}=-.273$, p=.003), and 'toilet problem' (${\beta}=-.222$, p=.016), which explained 21.5% of the variance (F=10.03, p<.001). Conclusion: The results of this study will provide a basis for improving the sleep quality of patients with percutaneous coronary angiography.

폐쇄성수면무호흡증후군의 무호홉-저호흡 지수에 따른 수면양상의 비교 (Comparison of Sleep Pattern According to Apnea-Hypopnea Index with Obstructive Sleep Apnea Syndrome)

  • Obstructive sleep apnea syndrome (OSAS) is defined by sleep apnea with decreased oxygen saturation, excessive snoring with daytime sleepiness, and frequent awakening during the night time sleep. The present study was performed to investigate how apnea-hypopnea, that possibly causes breathing disturbance during sleep, can affect sleep pattern in patients with OSAS. We included 115 patients (92 men, 23 women) who underwent a polysomnography from January 2006 to May 2007. As the frequency of sleep apnea-hypopnea increases, the proportion of non-rapid eye movement (REM) sleep (p<0.001), and stage I sleep (p<0.001) increased, while that of stage II sleep (p<0.001), stage III and IV sleep (p<0.01), and REM sleep (p<0.05) decreased. Furthermore, sleep apnea-hypopnea was closely correlated with REM sleep (r=0.314, p<0.001), stage I sleep (r=0.719, p<0.001), stage II sleep (p=-0.342, p<0.05), stage III and IV sleep (r=-0.414, p<0.001), and REM sleep (r=-0.342, p<0.05). Stage I sleep could account for the 51% of the variance of apnea-hyponea. Our study shows sleep apnea-hypopnea affects sleep pattern in pattern with OSAS significantly, and the change of stage I sleep is the most important factor in estimating the disturbance of sleep pattern.

아로마 마사지가 편마비 환자의 견통, 우울 및 수면장애에 미치는 효과 (Effects of the Aroma Massage on Shoulder Pain, Depression, Sleep Disturbance in Hemiparesis Patients)

  • Purpose: The purpose of this study is to find out the effects of the aroma massage on shoulder pain, depression, and sleep disturbance in hemiparesis patients. Method: The research design was a nonequivalent control group nonsynchronized design. The data was collected from July 1 to August 31, 2003 at D medical Center in Busan. The subjects were 37 patients that they were assigned to two groups, nineteen subjects in the experimental group and seventeen subjects in the control group. The oil made from a blend of lavendar, clarysage, bergamot at a ratio 3:1:2 used for relieving shoulder pain, depression, sleep disturbance. The instruments used for this study were Visual Analogue Scale(VAS) for shoulder pain, Zung' depression scale, 4 score's scale for sleep disturbance. The data were analyzed using SPSS and hypotheses were examined with ANCOVA. Results: Shoulder pain, depression, and sleep disturbance were significantly lower in the experimental group compared than the control group. Conclusion: The aroma massage using selected essential oils is an effective intervention for relieving shoulder pain, depression, sleep disturbance in hemiparesis patients. However differences between aroma massage and massage were not clear, further study needs to be done.

수면장애 연구와 실무를 위한 한국어 자가보고 수면 측정도구 (Korean Versions of Self-reported Sleep Questionnaires for Research and Practice on Sleep Disturbance)

  • Purpose: To identify Korean versions of self-reported sleep questionnaires for cross-cultural research and practice on sleep disturbance, we searched self-reported sleep questionnaires which have been developed to measure general sleep patterns. They should be qualitative, easy to take permission, do not obligate to pay, do not need any skill or professional help for scoring and have been reported high reliability and validity. Methods: General Sleep Disturbance Scale (GSDS), Leeds Sleep Evaluation Questionnaire (LSEQ), and Pittsburgh Sleep Quality Index (PSQI) were identified based on all criteria. Korean version of GSDS, LSEQ, and PSQI were developed according to the methods combined the guidelines for the process of cross-cultural adaptation of self-reported measures with decentering method. Results: Three Korean versions of self-reported sleep questionnaires were suitable for epidemiological study. They are not full length and easy to read. It requires less than 10 minutes for each subject to complete as well as for researchers to score. Conclusion: Psychometric analysis study is necessary to evaluate the reliability and validity of those Korean versions of self-reported sleep questionnaires.

순환교대근무 간호사의 일주기유형에 따른 근무조별 수면시간, 사회적 시차, 주관적 수면장애 비교 (Comparison of Sleep Duration, Social Jetlag, and Subjective Sleep Disturbance in Rotating Shift Nurses According to the Chronotype)

  • Purpose: The purpose of this study was to investigate the sleep duration, social jetlag (SJL), and subjective sleep disturbance according to the individual chronotype in rotating shift nurses. Methods: A total of 344 rotating 3-shift nurses (mean age 28.67 years) were recruited at one university affiliated hospital. They completed the following questionnaires, which were used to assess their chronotype and sleep: the morningness-eveningness questionnaire (MEQ), self-reported sleep duration of work days (SDWshift) and free days (SDFshift) in each shift (day [D], evening [E], night [N]), and sleep disturbance (Insomnia severity index, ISI). SJLshift was calculated as the difference in midsleep (MS=sleep onset+1/2 sleep duration) between work days (MSW) and free days (MSF). Results: Subjects were divided into 3 chronotype groups according to the MEQ; morning (MG, 4.4%) intermediate (IG, 57.8%), and evening groups (EG, 37.8%). SDWD was shortest (4.68 hr) and SDFE was longest (8.93 hr) in the EG. SJLD was longest in the EG (3.77 hr), and SJLN was longest in the MG (7.37 hr). The prevalence of sleep disturbance was 33.3% (MG), 29.6% (IG), and 40.0% (EG), respectively, without any statistical significance. Conclusion: In order to improve the sleep of shift workers, it is recommended that the evening chronotypes should reduce the day shifts and the morning chronotypes should reduce the night shifts. We also propose a study to determine whether these shift assignments can improve the sleep in shift nurses.

피츠버그 수면척도를 이용한 사상체질별 수면 비교 (Comparison of Sleep by Sasang Constitutional Type Using Pittsburgh Sleep Quality Index)

  • Objectives The purpose of this study was to identify differences in sleep by Sasang constitutional type using Pittsburgh Sleep Quality Index(PSQI). Methods The subjects of this study were 3,578 people who had information on Sasang constitutional type and PSQI among the data established at the Korean Medicine Data Center. We compared each item score in PSQI (subjective sleep quality, sleep latency, sleep duration, habitual sleep efficiency, sleep disturbance, use of sleep medication, daytime dysfunction) and PSQI global scores by Sasang constitutional type using analysis of variance. Logistic regression was conducted to calculate the odds ratio of poor sleep by Sasang constitutional type. Results & Conclusions The SE type showed statistically significantly higher scores than the TE type and SY type in subjective sleep quality, sleep latency, habitual sleep efficiency, sleep disturbance, and PSQI global score, indicating poor sleep compared to other constitutional types. The odds ratio for poor sleep also shows that even after adjusting sex, age, height, and weight, the SE type was significantly higher than other constitutional types, indicating that the SE type could be a risk factor for poor sleep.

Noise in hospital rooms and sleep disturbance in hospitalized medical patients

  • Objectives Hospitalized patients are vulnerable to sleep disturbances because of environmental stresses including noise. While most previous studies on hospital noise and sleep have been performed for medical machines in intensive care units, there is a limited data for patients hospitalized in medical wardrooms. The purpose of present study was to measure noise level of medical wardrooms, identify patient-perceived sources of noise, and to examine the association between noise levels and sleep disturbances in hospitalized patients. Methods Noise dosimeters were used to measure noise level in 29 inpatient wardrooms at a university hospital. Sleep pattern and disturbance were assessed in 103 hospitalized patients, using the Pittsburgh Sleep Quality Index (PSQI) and Leeds Sleep Evaluation Questionnaire. Results The mean equivalent continuous noise level for 24 hours was 63.5 decibel A (dBA), which was far higher than 30 dBA recommended by the World Health Organization for hospital wardrooms. Other patients sharing a room were perceived as the most common source of noise by the patients, which was usually preventable. Of the patients in the study, 86% had bad sleep as assessed by the PSQI. The sleep disturbance was significantly correlated with increasing noise levels in a dose response manner. Conclusions Systemic organizational interventions are needed to keep wardrooms private and quiet to reduce sleep disturbance.

공황장애의 수면과 야간 공황발작 (Sleep in Panic Disorder and Nocturnal Panic Attack)

  • Sleep disturbance is a one of common complaints among patients with panic disorder. However, clinicians and researchers did not give much attention to the sleep symptoms of panic disorder yet. Several previous studies suggested that the sleep disturbance in panic disorder is mediated by nocturnal panic attack. In terms of the pathophysiology of panic disorder, nocturnal panic attack seems to be closely associated with the sleep problems in panic disorder. In this article, the authors reviewed various previous studies about sleep of panic disorder and intended to give importance of evaluating sleep disturbances and nocturnal panic attack in panic disorder for both clinical and research purpose.

  • Smoking is one of the most harmful causes of disease. Many previous researches have shown that cigarette smoking leads to cardiovascular, respiratory, oncologic, and cerebrovascular diseases. In addition to such adverse effects, the literature indicates that cigarette smoking can worsen sleep quality and induce sleep disorders. This review focuses on the relationship between smoking/nicotine and sleep and sleep disorders of insomnia, obstructive sleep apnea, and restless legs syndrome. Because smoking is a behavioral pattern that can be changed, it is important to quit smoking to improve overall health and sleep.

The effect of stabilization exercise on pain-related disability, sleep disturbance, and psychological status of patients with non-specific chronic low back pain

  • Background: Sleep disturbance and depression are becoming more recognized as important symptoms among individuals with chronic low back pain. This study evaluated the effect of stabilization exercise on pain-related disability, sleep disturbance and the psychological status of patients with non-specific chronic low back pain (NSCLBP). Methods: A total of 26 patients (M/F = 17/9) with NSCLBP with a mean age of $50.0{\pm}15.5$ took part in this study. Participants were recruited from selected hospitals in Lagos state. Ethical approval was sought and obtained from the health research and ethics committee of Lagos University Teaching Hospital Idi-araba, Lagos, Nigeria. Participants performed stabilization exercise for eight weeks consecutively and were assessed for pain-related disability, anxiety, depression, and sleep disturbance using the pain-disability index, hospital anxiety and depression scale, and the insomnia severity index at baseline, the 4th week, and the 8th week, postintervention. Results: The participants studied recorded significant reduction in pain-related disability (P = 0.001). There was also improvement in the sleep quality (P = 0.001), depression level (P = 0.001), and anxiety level (P = 0.001), post intervention. Conclusions: This study revealed that stabilization exercise is very useful in the management of sleep disturbance, pain-related disability, depression, and anxiety in NSCLBP patients.
