체형특성(體型特性)이 체온조절반응(體溫調節反應) 및 온열쾌적감(溫熱快適感)에 미치는 영향(影響) (Effects of Somatotype Characteristics on Body Temperature Control Reaction & Thermal Sensation)

  • This study is the first part of the research to reveal the effects of somatotype characteristics on body temperature control reaction as well as thermal sensation. Nine healthy female collegians (classified into 3 body types of thin, normal, and obese according to Rohrer index) living in Busan were chosen as the subjects. The following are the results: Significant differences of skin temperature appeared in the parts of epigastrium (thin/normal>obese), anterior forearm (normal>thin/obese), and anterior leg (obese > thin/normal) as well as mean skin temperature. Mean skin temperature temporarily dropped owing to the exercise but tended to recover as time went by. Skin temperature of normal/thin shows higher than obese type. The change of skin temperature was noticed in the order of forehead > epigastrium > anterior forearm > anterior leg > anterior thigh (obese type) ; epigastrium > forehead > anterior forearm > anterior thigh > anterior leg (normal type) ; epigastrium > forehead > anterior forearm > anterior thigh > anterior leg (thin type, before and after exercise); epigastrium > forehead > anterior forearm > anterior leg > anterior thigh (thin type, during exercise). Significant differences were shown in the temperature change inside clothes according to somatotypes. No significant differences were revealed in thermal sensation, moisture sensation, and comfortable sensation according to body types and time.

Statistically Designed Enzymatic Hydrolysis for Optimized Production of Icariside II as a Novel Melanogenesis Inhibitor

  • Three kinds of prenylated flavonols, icariside I, icariside II, and icaritin, were isolated from an icariin hydrolysate and their effects on melanogenesis evaluated based on mushroom tyrosinase inhibition and quantifying the melanin contents in melanocytes. Although none of the compounds had an effect on tyrosinase activity, icariside II and icaritin both effectively inhibited the melanin contents with an $IC_{50}$ of 10.53 and $11.13{\mu}M$, respectively. Whereas icariside II was obtained from a reaction with ${\beta}$-glucosidase and cellulase, the icariin was not completely converted into icariside II. Thus, for the high-purity production of icariside II, the reaction was optimized using the response surface methodology, where an enzyme concentration of 5.0mg/ml, pH 7, $37.5^{\circ}C$, and 8 h reaction time were selected as the central conditions for the central composite design (CCD) for the enzymatic hydrolysis of icariin into icariside II using cellulase. Empirical models were developed to describe the relationships between the operating factors and the response (icariside II yield). A statistical analysis indicated that all four factors had a significant effect (p<0.01) on the icariside II production. The coefficient of determination $(R^2)$ was good for the model (0.9853), and the optimum production conditions for icariside II was an enzyme concentration of 7.5mg/ml, pH 5, $50^{\circ}C$, and 12 h reaction time. A good agreement between the predicted and experimental data under the designed optimal conditions confirmed the usefulness of the model. A laboratory pilot scale was also successful.

자외선에 의한 피부반응 (Effects of Ultraviolet Radiation on the Skin)

  • 자외선이 피부에 조사되면 다양한 피부반응을 초래한다. 이러한 반응은 크게 1) 급성 반응과 2) 만성반응으로 나눌 수 있다. 급성반응으로서는 피부가 붉어지는 홍반반응, 피부색이 검어지는 색소반응, 그리고 피부두께의 변화 등을 들 수 있다. 홍반반응은 피부의 주된 세포인 각질형 세포를 비롯하여 진피세포들이 광여하여 진피혈관을 확장시키는 반응을 말한다. 홍반반응은 자외선의 파장, 광량, 피부의 조건, 환경조건 등에 따라 달라지며 피부에는 각질형성세포가 변형된 일광화상세포가 출형한다. 홍반반응은 가장 흔한 주된 반응이나 그 기전에는 명확하지 않은 점이 아직도 많이 있다. 색소반응은 멜라닌세포가 관여하는 반응으로서 자외선 조사 후 즉시 피부가 검어지는 즉시세포침착과 수일 후 검어지는 지연색소침착으로 나눌 수 있다. 색소반응과 피부두께의 변화 등은 자외선 조사로부터 몸을 보호하기 위한 방어작용의 일종이다. 자외선 조사에 의한 만성반응으로는 광노화현상과 광 발암현상을 들 수 있다. 광노화란 장기간에 걸친 광노출로 인한 피부의 노화를 말하며 생리적인 노화와는 임상적, 조직학적으로 다른 점이 많다. 자외선의 장기노출은 피부암을 발생시키는 중요한 원인의 하나로서 우리나라도 피부암 발생이 점차 증가하고 있다. 서울대학교병원 피부과에서의 피부암 발생빈도는 70년대에는 외래환자의 0.2% 정도였으나 80년대에는 0.4-0.8%를 차지하고 있다. 앞으로 우리나라의 경제성장과 함께 수명의 증가, 레져나 스포츠의 증가, 피부노출의 증가와 함께 오존층 파괴로 인하여 피부에 조사되는 자외선의 절대량의 증가는 자외선에 의한 피부의 급 만성반응을 더욱 증가시킬 것으로 예상되며 이에 대한 적절한 대책이 요구된다.

성인(成人) 남자(男子)의 발한(發汗) 반응(反應)에 관한 연구 - 운동선수(運動選手)와 비운동선수(非運動選手)를 중심(中心)으로 - (Sweating Reaction of Men Adults - Centered on Athletes and Non-Athletes -)

  • The present study aims to reveal the sweating reaction of male adults, focused on athletes. With six subjects (3 athletes and 3 non-athletes) in two different conditions of ambient temperature (I : $25\pm1.0^{\circ}C$, II : $29.5\pm1.0^{\circ}C$), their total sweat rate, local sweat rate, skin temperature, physiological reaction (rectal temperature, blood pressure, and pulse rate), and psychological reaction (thermal, moisture, comfort, and perceptive sweat sensations) were measured. The comparison gave the following results: Total sweating rate was greater in non-athletes, while the two groups had more perspiration in ambience II. Local sweating rate in both ambiences was the greatest in the central breast area (athletes) and the infrascapular area (non-athletes). The mean skin temperature had more changes of increase and decrease in athletes. As to physiological reaction, non-athletes had lower rectal temperature and blood pressure as well as higher pulse rate. As for psychological reaction in Ambience II, the 4 sensations were mostly 'hot', 'humid', 'uncomfortable', and 'sweaty'.

체형별 발한 반응에 관한 연구 (A Study of Sweating Reaction by the Somato Types)

  • To investigate the difference of sweating reaction by the somato types, we measured total sweat rate, local sweat rate, skin temperature, physiological reaction and psychological reaction at $25{\pm}1^{\circ}C$ and $29{\pm}1^{\circ}C$ under laboratory conditons. Nine healthy adult females were divided into three somato types : slender (3), normal (3) and obese (3). The results were as follows ; Total sweat rate was highest in the obese type, followed by the normal and slender types in order. Local sweat rate was highest in the infrascapular area, and then came breast, the back of the hand, upper ann, anterior leg, and anterior thigh in all somato types. Mean skin temperature was highest in the slender type, and followed the normal and obese types. Rectal temperature, blood pressure and pulse rate were highest in the obese type. Psychological reaction appeared 'hot', 'humid', 'sweaty' as ambient temperature went up. Somato types made little difference in psychological reaction.

  • Objective: This study was carried out to evaluate the inhibitory effect on immme hypersensitive reaction by Yangdokbackhotang extract. methods: To study, We divided 4 weeks old rats into three groups of 20, which are the constrasive group with nothing treated, the skin transplated only, and the Yangdokbackhotang extract fed before skin transplantation. And did the experiment. Results: 1. The ear swelling in mice treated with YB noticeably decreased than only DNCB treated mice. 2. As shown in Table3, also results in a significant decrease of YB group than ST group in the number of positive cells of $NF-{\kappa}B$, COX-2, IL-12B, and ICAM-1. From the above results, it could be concluded that Yangdokbackhotang showed benificial immunosupressive effect on the hypersensitive reaction.

EWHA Breast Device(EBD)의 제작 및 유용성 평가 (The evaluation of custo-made EWHA Breast Device(EBD))

  • 목적 : 3D conformal, IMRT와 같은 치료기술의 개발로 인하여 CT를 이용한 최신의 치료들이 적용되어지고 있는 것이 지금의 현실이지만, 유방암 환자에서는 일반 CT의 hole size의 제약으로 인하여 일부의 경우는 기존의 환자고 정기구들이 사용되어지지 못하고 있다. 또한 simulation 시에 수술한 팔을 장시간 올리고 있는 자세를 유지함으로써 환자들이 pain을 호소하고, SCL (supraclavicular Imphnodes) 치료 시에 skin reaction의 문제점이 제기되었다. 이에 본원에서는 이러한 문제점을 개선하기 위하여 Ewha Breast device (EBD)를 자체 제작하여 사용하였으며, 이번 논제에서는 EBD의 제작과 유용성의 평가에 대하여 논하고자 한다. 대상 및 방법 : CT data를 이용하여 머리의 크기를 분석하였으며, 일반 CT의 hole size, 자세의 안정성, SCL field에서의 피부의 겹침(fold) 등을 고려하여 아크릴로 device를 제작하였으며 기존의 device를 사용하였을 경우와 set-up의 재현성, RTP상의 dose분포, SCL field에서의 skin reaction 등을 비교 분석하여 EBD의 유용성에 대하여 평가하였다. 결과 : 제작한 EBD를 사용 시 CT simulator가 아닌 hole size가 작은 일반 CT에서도 팔의 부딪힘이 없이 자세를 유지할 수 있었고, 위로 올린 팔의 pain도 많이 감소하였다. 그리고, 기존의 device와 비교하였을 경우에 set-up의 재현성에서 큰 차이를 보이지 않았으며, RTP상에서는 3D segment field를 이용한 plan으로 device의 tilting이 되지 않은 문제점은 해결할 수 있었으나 기존보다 팔을 많이 위로 올리지 않는 관계로 axillary region에서 이전보다 약$10\%$ 정도의 선량이 더 들어감을 알 수 있었다. SCL field에서의 skin reaction은 접힌 부분(fold)을 펼치기 위해 팔을 조정하여 위치를 잡는 관계로 이전보다 월등히 적은 skin reaction이 발생함을 알 수 있었다. 결론 : 비용이 저렴한 EBD의 제작으로 유방암 환자에서 CT를 이용한 3D 치료가 용이하게 되었으며, simulation 시에 팔의 pain을 경감시킬 수 있었고, skin reaction의 감소 등으로 치료에 EBD를 유용하게 사용할 수 있었다. 앞으로 EBD를 사용하여 나타난 device의 문제점 등을 보완 수정한다면 치료에 더 많은 도움이 되리라 생각된다.

동종진피에 사람진피 섬유모세포와 각질세포를 적용한 인공피부의 실험적 제작 (Application of Human Dermal Fibroblast and Keratinocyte on Allogenic Dermis(AlloDerm®))

  • Purpose: Large skin defect by various causes, should be covered by autologous skin graft. But, the donor site of autologous skin graft is limited and leaves permanent donor scar and contracture. There have been our trial to engineer artificial skin using allogenic dermis (AlloDerm) with basement membrane. Methods: Dermal and epidermal layer were separated by immersing in dipase solution for 30 minutes, and the separated layers were treated with 0.05% trypsin for 10 minutes. And then each layer was cultivated to fibroblasts and keratinocytes on a culture medium. Fibroblasts were first penetrated into basement membrane of allogenic dermis facing down, then allogenic dermis was flipped over to face up and keratinocytes were transplanted to allogenic dermis. Results: Observing artificial skin fabricated in vitro, we found following: 1) The artificial skin opened in air for 5 days formed epidermal layer. In dermal layer, fibroblast was distributed evenly among all. 2) The artificial skin opened in air for 30 days formed thicker and thicker, and it formed basement membrane, spinous and granular layers. PAS stain to confirm existence of basement membrane showed positive reaction. 3) Cytokeratin 10 stain to confirm the formation of epidermal layer showed positive reaction. 4) The formation of thick keratin, lamellar body and desmosome similar to human skin were observed in result of an electron micrograph. Conclusion: As a result of research, the structure seen in normal skin such as rete ridge, is found in reproduced artificial skin. This type of artificial skin can be used as a useful model for investigating skin disease and for clinical application also.

방사선 치료 시 조사야 내에 위치할 수 있는 이물질이 체표선량에 미치는 영향 (Effect of skin dose by materials located in treatment field)

  • Purpose : In radiotherapy, various materials are used to located in treatment field unintentionally. It increases the dose delivered to the skin by interactions of the X-ray within the materials and occurs unwanted skin reaction.(due to the dose build-up effect) This aim of the this study is to measure the increase in skin dose when 13 materials are located in treatment field. Methods : Photon beam measurements were made using an plane-parallel chamber (Markus, PTW-Freiburg) in a polystyrene phantom. skin dose were measured using various overlaying 13 materials. a fixed geometry of a $10{\times}10cm$ field, a SSD=100cm and photon energy 4MV on Varian CLINAC 600C accelerator were used for all measurements. Results : There is an increase in skin dose for all materials($16.4{\sim}160.1\%$). As a percentage of maximum dose, the lowest skin dose were measured for the underwear with silk($43.2\%$) and the highest were measured for the 100m1 fluid-bag($96.6\%$) Conclusion : There is a significant increase in skin dose with 13 materials in the treatment field. a significant increase in skin dose can occur which could produce unwanted skin reaction. considerations for placement of 13 materials to be outside the treatment field whenever possible should be used to keep skin dose to a minimum level.

Delayed Allergic Reaction to Secondary Administrated Epidural Hyaluronidase

  • We are reporting a rare case of a delayed hypersensitivity reaction caused by hyaluronidase allergy following a lumbar transforaminal epidural block. Using an intradermal skin test, we have provided evidence that the systemic allergic reaction resulted from hypersensitivity to hyaluronidase. To our knowledge, this is a rare case of a delayed hypersensitivity reaction to epidural hyaluronidase, comprised of an initial exposure to hyaluronidase with no subsequent allergic response in prior block followed by a subsequent delayed reaction to hyaluronidase during a second epidural block.
