Design of a Reusable Secret Sharing Scheme in a Hierarchical Group (비밀조각의 재사용이 가능한 권한 위임 비밀분산법의 설계)

  • A secret sharing scheme is a cryptographic Protocol that a dealer distributes shares about a secret to many participants and authorized subsets of the participants can reconstruct the secret. Secret sharing schemes that reflect various access structure were proposed. We propose a new reusable secret sharing scheme in a hierarchical group. Participants have priority about restoration of secret from high position level of tree. And when participants who belong in high position level are absent, they can delegate restoration competence of the secret transmitting delegation ticket to child nodes that it belongs in low rank level. By participants reuse own share and take part in different secret restoration, they who belong on hierarchical group can be possible different secret restoration by each participant's single share.

A case of tacrolimus-induced encephalopathy after kidney transplantation

  • We present a case of tacrolimus-induced encephalopathy after successful kidney transplantation. An 11-year-old girl presented with sudden onset of neurologic symptoms, hypertension, and psychiatric symptoms, with normal kidney function, after kidney transplantation. The symptoms improved after cessation of tacrolimus. Magnetic resonance imaging (MRI) showed acute infarction of the middle cerebral artery (MCA) territory in the right frontal lobe. Three days later, she had normal mental function and maintained normal blood pressure with left hemiparesis. Follow-up MRI was performed on D19, showing new infarct lesions at both cerebral hemispheres. Ten days later, MRI showed further improvement, but brain single photon emission computed tomography (SPECT) showed mild reduction of uptake in both the anterior cingulate gyrus and the left thalamus. One month after onset of symptoms, angiography showed complete resolution of stenosis. However, presenting as a mild fine motor disability of both hands and mild dysarthria, what had been atrophy at both centrum semiovale at 4 months now showed progression to encephalomalacia. There are two points of interest in this case. First, encephalopathy occurred after administration of tacrolimus and improved after discontinuation of the drug. Second, the development of right-side hemiplegia could not be explained by conventional MRI; but through diffusion tensor imaging (DTI) and diffusion tensor tractography (DTT) of white matter tract, visualization was possible.

Panner's Disease Occurred in a Five-year-old Child: A Preliminary Case Report

  • Panner's disease, osteonecrosis of the capitellum of the elbow, was first reported by Panner in 1927. The disease occurs mainly in boys between 6 and 15 years old and shows unilateral distribution. Pain, stiffness, localized tenderness over the lateral condyle of the elbow, and decreased range of motion are the typical clinical symptoms. Conservative treatment is generally recommended for patients in the early stage of this disease. A few cases of Panner's disease have been reported and few are related to long-term follow-up results. To the best of our knowledge, all reported cases were over six years. Therefore, we report on a five-year-old boy diagnosed as Panner's disease that showed resorption and regeneration of the humeral capitellum with no limitation of motion over three years. The current study was exempted from review by the institutional review board because it was a single retrospective case report. Informed consent was obtained from the patient's guardian.

Long-term Outcome of Laparoscopic Hernia Sac Transection and Intracorporeal Ligation in Children: A Single Center Cohort Study (복강 내 탈장낭 절개 및 봉합법을 이용한 소아 복강경 서혜부 탈장 수술의 장기 추적관찰 결과: 단일기관 코호트 연구)

  • Purpose: Laparoscopic hernia repair in children is still controversial. The aim of this study was to report our long-term results of the laparoscopic hernia technique, which is based on the same surgical principles as conventional open herniotomy. Methods: Five hundred fourteen pediatric patients with inguinal hernia were included in this study under informed consent. All patients underwent a laparoscopic technique of sac transection and intracorporeal ligation. The asymptomatic contralateral inguinal ring was routinely explored and repaired if a patient had patent processus vaginalis on the contralateral side. Patients were prospectively followed for 5 years. Those who were lost to follow-up were excluded from the study. Perioperative complications and recurrences were evaluated. Results: The mean follow-up period was 29 months. Mean operation time was 27.5 minutes. Forty one percent of the patients had contralateral patent processus vaginalis. Only one hernia recurred (0.19%). We had one case of contralateral metachronous hernia (0.21%) during follow-up period. Conclusion: The long-term follow-up results of our study revealed that laparoscopic hernia sac transection and ligation can be a safe and effective alternative for conventional herniorraphy.

Early Experience of Laparoscopic Adhesiolysis in Children with Postoperative Intestinal Obstruction (소아에서 수술 후 발생한 장폐색에 대한 복강경 유착박리술의 초기 경험)

  • The purpose of this study is to analyze the early experience of the laparoscopic adhesiolysis for the intestinal obstruction due to postoperative adhesion. Seven patients were included in this study. The median age of those patients was 13, and there were 3 males and 4 females. Previous diagnosis and surgical procedure were various in seven cases, including small bowel resection with tapering enteroplasty, Boix-Ochoa fundoplication, Ladd's procedure with appendectomy, mesenteric tumor resection with small bowel anastomosis, ileocecal resection and anastomosis, primary gastric repair, and both high ligation. A successful laparoscopic adhesiolysis was performed in one who had high ligation for inguinal hernia and had a single band adhesion. Six out of 7 (86%) cases needed to convert open surgery due to multiple and dense type of adhesion. In conclusion, laparoscopic approach with postoperative small bowel adhesion seems safe. However, it might be prudently considered because of high rates of conversion in children.

Effects of Social Supports and Health on the Depression of the Rural Elderly: Focus on Household Patterns (가구유형별 농촌 노인의 사회적지지 및 건강이 우울감에 미치는 영향)

  • The purpose of this study was to investigate the effects of social supports and health on the depression of the rural elderly according to their household patterns. Data were collected from 1,000 of the elderly over 65 living in rural Korea. A questionnaire was used with direct interviews and was analyzed through descriptive statistics, ANOVA, and a multiple regression analysis. The major findings of this study are as follows. First, the general characteristics, social supports, and health of the rural elderly are significantly different according to their household patterns. Second, depression in the rural elderly is also significantly different according to household patterns; elderly people in a single household report greater depression than elders in a coupled household or those in a household with a child. Third, satisfaction with health conditions and nutrition management variables has a significant influence on the depression of the rural elderly in all household patterns. Fourth, there are differences among variables that affect depression of the rural elderly according to their household patterns.

  • The 18q-syndrome is a deletion disorder that occurs in humans. Clinical symptoms are mental retardation, craniofacial anomalies, skeletal deformity, seizure, and hearing loss. 18q- deletion occurs over a broad region, spanning the interval from 18q22.2 to 18qter rather than a single critical region containing 18q. We experienced a case of 18q-syndrome in a male child. It was diagnosed by clinical and chromosomal study. He was a 15month-old infant who was admitted because of prolonged fever and vomiting. And he manifested a depressed midface, esotropia, anhydrosis, and developmental delay. Peripheral blood chromosome studies showed deleted chromosomal material at the distal part of the long arm of chromosome 18. He showed right hydronephroureterosis on IVP. So, he was diagnosed as 18q-syndrome with right hydronephroureterosis and anhydrosis. We report this syndrome with a review and related literature.

English Medium Instruction in Higher Education: Does It Promote Cultural Correction or Cultural Continuity?

  • This study investigates English medium instruction (EMI) in an institution of higher education in Seoul, Korea to see whether this course creates cultural correction (reproduction of inequitable relations of power in EMI settings) or cultural continuity (opportunities for transporting students into a third space and enabling them to explore cultural diversity and to create new knowledge for themselves). A single site where EMI is carried out, a class on fairy tales and child education taught by a native English speaking professor, was chosen because it was hypothesized that the professor would display some of her unconscious dominant cultural orientation. The results of the study show that there more cases of cultural correction than there were of cultural continuity. Cases of cultural correction included lack of knowledge about the local context, fixing Korean classroom discourse as if it were American classroom discourse, and reproducing orientalism in the local educational setting. Cases of cultural continuity included using comparison to consider the cultural reality of the milieu, creating new knowledge for the local milieu, and learning as a dynamic ongoing process. Implications of this research are discussed including the important realization that EMI should be managed by subject specialists who are trained in language education and have knowledge of the students' needs and discourse in the L1 and in the local context.

  • The main purpose of this study was to evaluate the anticariogenic effect of argon laser. Histological observations on lesion initiation and progression were performed under the polarized microscope. The results from the present study can be summarized as follows; 1. The specimens of laser cured group were shown to have more irregular and discontinuous lesion body in general than visible light cured group with rather continuous positive birefringence. 2. With lesion initiation and progression, almost all the specimens showed deeper body of lesion with shallower intact surface zone in the visible light cured group than the laser cured group(p<0.05). When the comparision was made between the two argon laser cured groups, the single-cure group showed deeper lesion body and the shallower surface layer than double-cure group. 3. Based upon the above mentioned results of this study, it can be assumed that the use of argon laser in the procedure of resin polymerization may provide the child and adolescent patient population with anticariogenic effect as well as efficient polymerization. Further studies using various materials and experimental conditions are being encouraged.

Association Between Unpredictable Work Schedules and Depressive Symptoms in Korea

  • Backgrounds: Irregular and unpredictable work schedules have become more common in most societies. The purpose of this study was to investigate the association between unpredictable work schedules and depressive symptoms in Korea. Methods: Data from 34,486 workers who participated in the Korean Working Condition Survey in 2017 were used. Unpredictable work schedules were measured by questions about the frequency of changes in work schedule and limited advanced notice. Depressive symptoms were assessed by a single item asking if the participants had depressive symptoms over the last 12 months. Multivariable-adjusted logistic regression analysis was used to calculate odds ratios (ORs) for high depressive symptoms. Results: The OR for depressive symptoms was significantly higher in the workers with unpredictable work schedules compared to those with predictable work schedules after controlling for age, sex, education, salary, marital status, occupation, contract period, full-time versus part-time, shift work, weekly working hours, and having a child under the age of 18 years (OR = 2.43, 95% confidence interval 1.93-3.07). Conclusion: Unpredictable work schedules were associated with depressive symptoms controlling for the other dimensions of precarious employment in a representative working population in Korea.
