Effects of Brain-Timing Training on Reading Abilities: A Retrospective Comparative Study (두뇌타이밍 훈련이 읽기 능력에 미치는 효과: 후향적 비교연구)

  • Objective : The purpose of this study was to investigate the effect of brain-timing training using an Interactive Metronome (IM) on the reading abilities of children with low brain-timing abilities. Methods : A single-group pretest and posttest (retrospective study) were conducted in 8 children whose timing abilities were lower than that for their age average and who were trained from 2019 to 2021. Brain-timing training was conducted 2 or 3 times per week, with 70 sessions in total and 40-50 minutes per session, depending on the developmental characteristics of the child. Changes in brain-timing ability before and after the training were measured using the IM Long-Form Assessment (LFA), and reading ability was measured using the Basic Academic Skills Assessment:Reading (BASA:R). Results : On the basis of the results of the brain-timing ability evaluation using the LFA, the reaction times of all the children during motor tasks were decreased, resulting in statistically significant improvements in their brain-timing abilities (p < .05). Moreover, the raw scores in the BASA:R reading and fill-in-the-blank tests were significantly improved (p < .05). Conclusion : This study is meaningful in that it presents a clinical basis for brain-timing training to improve the reading abilities of children.

Effect of Aquatic Ramp Exercise on Gait And Activity Of Daily Living In Child With Spinal Muscular Atrophy Type II: Single Subject Research (수중 경사로 보행 훈련이 제2형 척수성근위축증 아동의 보행 및 일상생활동작에 미치는 효과: 개별실험연구)

  • The purpose of this study was to determine the effect of aquatic ramp walking exercise on the activity of the quadriceps, gait and activity of daily living in child with Spinal Muscular Atrophy (SMA) type II. A 5 years-old girl with SMA type II participated in this study. This study used single-subject reverse(A-B-A) design study. There are 12 sessions(4weeks 3 times a week) each during the baseline phase(A), the intervention phase(B), the follow up phase(A). During the baseline phase and the follow up phase performed general aquatic therapy, the intervention phase additional performed walking activity on ramp in pool (60m). Surface electromyogram, Timed Up and Go (TUG) test, ACTIVLIM were used as outcome. During the intervention phase, there were decrease on the activity of the quadriceps. In modified TUG test, gait time reduced during the intervention phase. The ACTIVLIM logit score increased during intervention phase by comparison with the baseline phase. These findings suggest that an aquatic ramp walking exercise activities have the therapeutic possibility on the quadriceps activity and gait ability for child with SMA type II.

SINGLE PHOTON EMISSION COMPUTED TOMOGRAPHY FINDINGS IN ATTENTION DEFICIT- HYPERACTIVITY DISORDER (주의력결핍 ${\cdot}$ 과잉운동장애의 단일광자방출 전산화단층촬영 소견에 관한 연구)

  • The neural mechanisms involved in attention deficit hyperactivity disorder are largely unknown. In order to investigate the neuroanatomical lesions of attention deficit hyperactivity disorders and their relationships with psychopathology, Single Photon Emission Computed Tomography(SPECT) using HMPAO was performed in 46 ADHDS and Yale Children's Inventory(YCI), Conners Parent Questionaire and DSM-III-R Questionaire for Disruptive Behavior Disorder were used to assess the psychopathology of ADHDS The results are summarized as follows; 1) 30.4% (14/46) of this series revealed decreased perfusion In SPECT. 2) Regions of hypoperfusion were seen in cerebral cortex(17.4%, 8/46), thalamus(13.0%, 6/46), deep gray matter(8.7%, 4/46), basal ganglia(6.5%, 3/46) and cerebellum(2.2%, 1/ 46). 3) The mean scores of the total YCI revealed significant difference between the two groups(SPECT abnormal versus normal group), and among the subscales, hyperactivity, language and fine-motor subscales showed significant differences between the two groups. Although the relationship between the abnormal findings and specific symptom clusters of ADHDS remains unclear, we can suggest that these abnormal findings could be associated with ADHD, and based on these findings, the ADHDS can be subclassified into two groups. This study can be said to reinforce the current conception of heterogeneity of ADHD.

  • The pathophysiology and neural mechanism involved in Tourette's and chronic motor tic disorder are highly controversial. In order to investigate the functional abnormalities of brain. In Tourette's and chronic motor tic disorder, 42 children with Tourette's and chronic motor disorder underwent single photon emission computed tomography(SPECT) using Tc-99m-HM-PAO. The results are summarized as follows : 1) 31.0% (13/42) of this series revealed perfusion defect in cerebral cortex. 2) 4.8% (2/42) revealed perfusion defect in basal ganglia. 3) 4.8(2/42) revealed perfusion defect in thalamus. 4) 16.7%(7/42) showed perfusion defect in cerebellum. 5) The frequency of abnormal perfusion showed no significant difference between tic with and without attention deficit hyperactivity disorder. 6) The frequency of abnormal perfusion showed no significant difference between Tourette's and chronic motor tic disorder. These findings support the hypothesis of a possible involvement of brain dysfunction in the production of Tourette's and chronic motor tic disorder, and quantification of blood flow and co-registration with magnetic resonance imaging will increase the validity of this study.

A Study on the Application of Welfare Service Type on the Basis of the Tenants Needs and Locational Characteristics in Each Housing (단지별 거주자의 요구 및 입지특성에 따른 복지 서비스 유형 적용에 관한 연구)

  • Within twenty years, Permanent Affordable Housing provided for people's security of housing in late 1980's yields the principles of equity with tenant's in new one supplied, thus being confronted by the requirements that existing welfare service for tenants should be improved. Reflecting on these, this study means to prepare the basic data for the welfare policy as applying welfare service types on basis of housing locational characteristics, tenants tier compared with welfare service needs of permanent affordable housing tenants. For achieving this aim, the range and mixed use mechanism of welfare service are investigated as reviewing the related research and findings. Based on this investigation, 7 types of welfare service(Independence, Half-support, Child care, Juvenile, Self-support, Social-mix, and Single household) are established. On the analysis of survey, the welfare service needs are investigated as grouping with locational characteristics and survey target. In addition, the welfare service types are reorganized by analysis of tenant's demographic and locational characteristic on each housing and are compared with tenants needs. Although, the demographic of permanent affordable housing generally shows that the tenants are constituted with aged people, the housing with high ratio of children and juvenile can be applied with Child Care type and Juvenile type. As a result of comparison, the ratio of independence type, juvenile type, and single household type are more on the large housing and housing on metropolitan area, and the ratio of half-support type and child care type are more on the small housing and housing on small city area.

Clinical characteristics of children with 2009 pandemic influenza A (H1N1) admitted in a single institution

  • Purpose: This study aims to investigate the clinical characteristics of children diagnosed with the novel influenza A (H1N1) in the winter of 2009 at a single medical institution. Methods: Out of 545 confirmed cases of influenza A (H1N1) in children, using the real time RT-PCR method at Kosin University Gospel Hospital from September to December of 2009, 149 patients and their medical records were reviewed in terms of symptoms, laboratory findings, complications and transmission within a family. Results: Median age of subjects was 7 years (range: 2 months-18 years). New cases increased rapidly from September to reach a peak in November, then declined rapidly. Most frequently observed symptoms were fever (96.7%), cough (73.2%), rhinorrhea (36.9%) and sore throat (31.5%). Average body temperatures on the 1st, 2nd and 3rd hospital day were $38.75{\pm}0.65^{\circ}C$, $38.08{\pm}0.87^{\circ}C$ and $37.51{\pm}0.76^{\circ}C$, respectively. Complete blood counts and biochemical tests performed on the first admission day showed within the reference values in most cases. Of the 82 patients with simple chest radiography, 18 (22%) had pneumonic lesions; multi-focal bronchopneumonia in eleven, single or multi-segmental lobar pneumonia in five, and diffuse interstitial pneumonia in two patients. All of the 149 patients improved from their symptoms and discharged within 9 days of admission without any late complication. Conclusion: Children with 2009 pandemic influenza A (H1N1) at our single institution displayed nonspecific symptoms and laboratory findings, resembling those of common viral respiratory illnesses, and did not appear to develop more severe disease.

A Case Study on Therapeutic Music Making for Enhancement of Communications in Single-Mother Families (여성 한부모 가족의 의사소통 증진을 위한 치료적 음악 만들기 사례연구)

  • The purpose of this study is to discover how therapeutic music making affects the way of communications in single-parent families. To do this, the study conducted a therapeutic music making program in two families consisting of a mother and a child. Songwriting, Ostinato-creating, improvisation imitation, and ensemble were used as musical interventions. The results of this study are following. First, the participant group B improved from 59 to 69.5 point after the program. However, the group A decreased from 81 to 75.5 point mainly due to frequent absences, differences in applying music. Second, the analysis of the results of verbal, nonverbal and musical responses from the program suggests that the group B gradually improved functional communications as each session develops. For the case of group A, at the initial stage, dysfunctional communications in verbal and nonverbal ones could be found. After the eighth session however, functional communications had increased. The result of the study shows that overall therapeutic music making has been an effective musical intervention in improving communication in single-parent families.

Clinical application of genome-wide single nucleotide polymorphism genotyping and karyomapping for preimplantation genetic testing of Charcot-Marie-Tooth disease

  • Purpose: Preimplantation genetic testing for monogenic disorders (PGT-M) has been successfully used to prevent couples with monogenic disorders from passing them on to their child. Charcot-Marie-Tooth Disease (CMT) is a genetic disorder characterized by progressive extremity muscle degeneration and loss of sensory function. For the first time in Korea, we report our experience of applying single nucleotide polymorphism genotyping and karyomapping for PGT-M of CMT disease. Materials and Methods: Prior to clinical PGT-M, preclinical tests were performed using genotypes of affected families to identify informative single-nucleotide polymorphisms associated with mutant alleles. We performed five cycles of in vitro fertilization PGT-M in four couples with CMT1A, CMT2A, and CMT2S in CHA Fertility Center, Seoul Station. Results: From July 2020 through August 2021, five cycles of PGT-M with karyomapping in four cases with CMT1 and CMT2 were analyzed retrospectively. A total of 17 blastocysts were biopsied and 15 embryos were successfully diagnosed (88.2%). Ten out of 15 embryos were diagnosed as unaffected (66.7%). Five cycles of PGT-M resulted in four transfer cycles, in which four embryos were transferred. Three clinical pregnancies were achieved (75%) and the prenatal diagnosis by amniocentesis for all three women confirmed PGT-M of karyomapping. One woman delivered a healthy baby uneventfully and two pregnancies are currently ongoing. Conclusion: This is the first report in Korea on the application of karyomapping in PGT-M for CMT patients. This study shows that karyomapping is an efficient, reliable and accurate diagnostic method for PGT-M in various types of CMT diseases.

Effects of Perceptual Motor Program on Visual Motor Integration Skill and Motor Skill of a Child With Attention Deficit Hyperactivity Disorder: Single Subject Research Design (지각운동프로그램이 주의력결핍과잉행동장애아동의 시각운동통합기술과 운동기술에 미치는 영향)

  • Objective : The purpose of this study is to explore the effectiveness of a perceptual motor program in promoting visual motor and motor control outcomes in a child with Attention-Deficit/Hyperactivity Disorder (ADHD) who is enrolled in an elementary school. Methods : The subject is 8-year-old boy who was diagnosed with ADHD but did not receive medication. Research design is a single-subject A-B-A(follow-up). Baseline and follow-up phase were 3 sessions and intervention phase were 8 sessions. After performing 40 minutes of free play on the baseline A and follow-up A, we used the copying subtest of the Korean Development Test of Visual Perception 2 (K-DTVP-2) for evaluating visual motor integration skill and the rope jumping subtest for motor skill. After providing the perceptual motor activity in intervention phase B, copying and ropejumping were performed. Results : The standard scores of the Copying for measuring visual motor integration skill were poor at an average of 4.7 points at baseline phase A, but maintained an average of 9.6 points at intervention phase B and an below average of 7.7 points at the follow-up phase A. In the Rope jumping for the motor skill, it was increased by 4.3 times in the baseline phase A and 5.9 times in the intervention phase B, but slightly decreased by 5 times in the follow-up phase A. Conclusion : This study suggests that perceptual motor program has a positive effect on visual motor and motor function of ADHD children.

The Household Characteristics and Underage Children's Dental Clinic Use - Focusing on the Mother's Characteristics - (가구특성과 미성년 자녀의 치과이용 - 모의 특성을 중심으로 -)

  • The purpose of this study was to use the Korea Health Panel to determine how the mother's characteristic and household characteristic variables were associated with dental clinic use of children in childhood and adolescence. The logistic regression analysis found that mother's education level was statistically significantly positively correlated with underage child's dental clinic use and children in single-parent family were less likely to use a dental clinic than those in two-parent family. As for dental clinic use by diseases, the higher level of mother's education, the more likely to use a dental clinic due to dental caries and children in single-parent family were less likely to use a dental clinic due to dental caries than those in two-parent family. No difference in dental clinic use due to tooth extraction was found by any of mother's characteristics or household characteristics, probably because age has an absolute impact during the period between milk teeth and permanent teeth. Lastly, mother's education level and household income were significantly positively correlated with dental clinic use due to orthodontics. On the basis of these results, it is necessary to develop a customized strategy reflecting children's growth stages and household characteristics in making a plan for promoting oral health of children and adolescents.
