Current trends in intravenous sedative drugs for dental procedures

  • Anxiety and phobia in dental procedures are common deterrents for patients visiting the dental care unit. For these individuals, procedural sedation may aid in completion of dental treatments. In most cases, the patients are conscious during sedation, thereby allowing spontaneous ventilation. Intravenous sedation (IVS) is widely used during dental treatment to relieve patient anxiety. IVS is the most effective route of administration to achieve this goal, but it requires advanced training, more than that provided during undergraduate education. During IVS, rapid onset, repetitive drug administration, easy titration, and rapid recovery from sedation can be achieved. However, conscious sedation during IVS can result in deep sedation that can cause respiratory and cardiovascular depression. Therefore, the characteristics of intravenous sedatives should be known. The purpose of this review is to discuss the characteristics and usage of intravenous sedatives currently used for dental procedures.

  • Dental treatment under sedation requires various sedation depths depending on the invasiveness of the procedure and patient drug sensitivity. Inappropriate sedation depth may cause patient discomfort or endangerment. For these reasons, patient-controlled sedation (PCS) pumps are commonly used. Patients are able to control the sedation depths themselves by pushing the demand button after the practitioner sets up the bolus dose and lock-out time. Dexmedetomidine is an ${\alpha}$-2 adrenoreceptor agonist with sedative, analgesic, and anxiolytic properties. It has been widely used for sedation for its minimal respiratory depression; however, there are few studies on PCS using dexmedetomidine. This study assessed the applicability of dexmedetomidine to PCS.

  • Pharmacological activities of Paeoniae Radix alba were studied with water extract (F-I), parched preparation (F-II) and 25% ethanol-treated preparation (F-III) of it. The results were summerized as follows; 1) Analgesic actions were recognized with F-I, F-II and F-III. 2) Sedative actions were recognized with F-I and F-III. 3) Prolonged effect of sleeping time were recognized significantly with F-I and F-III. 4) Antipyretic actions were recognized with F-1, F-II and F-III in rabbits. 5) F-I, F-II and F-III showed the antagonic action against contraction of isolated-ileum induced by acetylcholine, barium chloride and histamine in mice and guinea-pigs.

Role of dexmedetomidine in pediatric dental sedation

  • Dexmedetomidine is a highly selective ${\alpha}2$-adrenoceptor agonist with a vast array of properties, making it suitable for sedation in numerous clinical scenarios. Its use was previously restricted to the sedation of intensive care unit patients. However, its use in pediatric dental sedation has been gaining momentum, owing to its high suitability when compared with conventional pediatric sedatives. Its properties range from sedation to anxiolysis to analgesia, due to its sympatholytic properties and minimal respiratory depression ability. Because dexmedetomidine is an efficacious and safe drug, it is gaining importance in pediatric sedation. Thus, the aim of this review is to highlight the properties of dexmedetomidine, its administration routes, its advantages over the commonly used pediatric sedatives, and especially its role as an alternative pediatric sedative.

  • Acanthopanax species (Arelaceae) are widely distributed in Asia. which used as tonic and sedative as well as a drug with ginseng-like activities from olden time. There are many reports on the studies of these plants. but there seems no reported about components from the leaves of Acanthopanax sessiliflorus. which is indigenous plant to Korea. We have now characterized three triterpenoid compounds from MeOH extract of the leaves of this plant. (omitted)

Analysis of behavioral management for dental treatment in patients with dementia using the Korean National Health Insurance data

  • Background: The global population is aging rapidly, and accordingly, the number of patients with dementia is increasing every year. Although the need for dental treatment increases for various reasons in patients with dementia, they cannot cooperate during dental treatment. Therefore, behavioral management, including sedation (SED) or general anesthesia (GA), is required for patients with dementia. Thus, this study aimed to investigate the trends and effects of SED or GA in patients with dementia undergoing dental treatment in South Korea based on the Korean National Health Insurance claims data. Methods: This study utilized customized health information data provided by the Health Insurance Review and Assessment Service. Among patients with records of using sedative drugs during dental treatment from January 2007 to September 2019, patients with the International Classification of Diseases-10 code for dementia (F00, F01, F02, F03, and G30) were selected. We then analyzed the full insurance claims data for dental care. Age, sex, sedative use, and dental treatment of patients were analyzed yearly. In addition, the number of cases of GA or SED per year was analyzed, and changes in behavioral management methods with increasing age were investigated. Results: Between January 2007 and September 2019, a total of 4,383 (male, 1,454; female, 2,929) patients with dementia received dental treatment under SED or GA. The total number of SED and GA cases were 1,515 (male, 528 ; female, 987 ) and 3,396 (male, 1,119 ; female, 2,277) cases, respectively. The total number of cases of dental treatment for 4,383 patients with dementia was 153,051 cases, of which 2.22% were under GA and 0.98% were under SED. Midazolam was the most commonly used drug for SED. Conclusion: Although gingivitis and pulpitis were the most common reasons for patients with dementia to visit the dentist, GA or SED for patients with dementia was frequently used in oral and maxillofacial or periodontal surgery.

소아치과 진정법에 사용되는 약제 (Agents Used for Pediatric Dental Sedation)

  • 오늘날 소아치과 진료에 있어서 진정법은 필수 불가결한 요소로 받아들여지고 있다. 이에 대한소아치과학회는 학회원들의 보다 안전하고 효율적인 진정법 시행을 돕기 위해 진정법 위원회를 결성하고 2014년 소아치과 개원의들의 진정법 사용에 대한 실태조사를 실시 한 바 있다. 이 설문조사 결과, 진정법을 시행하는 교육 코스가 만들어 진다면 참여할 의사가 있는지에 대한 질문에 응답자의 86%가 참여의사를 밝혔으며 진정법 교육과정에서 배우고 싶은 내용으로 안전한 약물용량과 가이드라인이 2위를 차지하여 1위인 응급상황 시 대처법 다음으로 교육받고 싶은 내용으로 나타났다. 그러나 약물을 선택하고 용량을 결정하기 위해서는 여러 가지 요소를 고려해야 하므로 용량을 일률적으로 정하여 지침을 제정하는 것은 현실적으로 어려운 일이고 실제적으로도 그 어떤 진정법 지침에도 약물의 용량을 제시해 놓지는 않았다. 성공적인 진정법을 위해 소아치과의사는 각 과정마다 개별적인 치료 계획을 수립해야 하며, 각 진정제의 고유의 약효뿐 만 아니라 다른 약제들 간의 상호 작용에 대하여 충분히 숙지하는 것이 중요하다. 이 종설에서는 문헌 고찰을 통해 소아치과 임상에서 자주 사용되는 진정제들에 대한 정보를 되새기고 의사용 처방편람(Physicians' Desk Reference: PDR) 및 소아치과 분야의 다양한 교과서의 추천용량을 정리하여 소개하고자 한다.

Conscious sedation in dentistry: knowledge and practice among dental professionals in Tanzania

  • Background: Conscious sedation is a useful adjunct in the treatment of patients in dentistry; however, a lack of knowledge among the dental profession regarding sedation is a restricting factor in the practice of dental sedation. Therefore, this study was conducted to assess the knowledge and practice of sedation in dentistry among dental professionals in Tanzania. Methods: This was a cross-sectional study conducted for five months targeting all practicing dental professionals in Tanzania. A modified questionnaire contained 14 questions regarding knowledge about sedative agents and a section on the practice of sedation. The data obtained from this study were coded and entered into a computer program and analyzed using SPSS software version 23.0. The data are presented as frequencies and percentages in tables and charts. Statistical significance was set at P < 0.05. Results: The age range of participants was between 24 and 63 years (mean 36.6 ± 7.7 years). There were 107 men (78.1%), and the male-to-female ratio was 3.6:1. The majority (76.6%) of participants only had an undergraduate dental degree. Thirty-one percent of participants only worked in publicly owned health facilities. Slightly more than half (59.9%) of participants had satisfactory knowledge regarding sedation in dentistry. There was no statistically significant association between the level of sedation-related knowledge and the demographic characteristics of the participants. Only 21.9% reported using sedation in their practice, and the most commonly used sedative drug was diazepam. The reasons for not using dental sedation in clinical practice included a perceived lack of knowledge on sedation, lack of equipment, and cost. Conclusion: Most dental professionals in Tanzania have basic knowledge of sedation in dentistry, although knowledge regarding sedative agents is generally low. The practice of sedation in dentistry in Tanzania is very low compared to that in middle- and high-income countries. Inadequate knowledge, lack of equipment, and the cost of practicing sedation are the main reasons for not practicing sedation.

신약 개발을 위한 참당귀(Angelica gigas Nakai) 추출 Decursin과 Decursinol Angelate의 약리 작용 (Pharmacological Effect of Decursin and Decursinol Angelate from Angelica gigas Nakai)

  • Traditionally, Cham dang-gui (Angelica gigas Nakai) is one of the most popular herbal medicines in Asian countries including Korea. A. gigas has been used as a functional food product for treatment anemia, women's health care, a sedative, an anodyne or a tonic agent. Decursin and decursinol angelate isolated from the roots of A. gigas are pyranocoumarin compounds. Recently, as the global herbal medication market is increasing, investigations about pharmacological effects of decursin and decursinol angelate are rapidly increasing. We summarized previous studies about pharmacological effects of decursin and decursinol angelate, and reviewed relation with pharmacological effects of decursin and decursinol angelate on human disorder, focused on the approach for new drug development. Pharmacological effects of decursin and decursinol angelate were classified as anti-tumor activity, anti-bacterial activity, improvements of the circulating system, inhibition of cytochrome P-450 activity, anti-inflammation activity, anti-oxidant activity and cognitive-enhancing activites. The activity of A. gigas with improvement of the circulating system may have wide therapeutic potential for circulatory diseases, including diabetes, hyperlipidemia and atherosclerosis. Also, anti-inflammation activity A. gigas may be beneficial for the treatment and prevention of asthma, atopic dermatitis and rheumatism arthritis. This relation could potentially lead to the development of herbal new drugs. In order to development a new drug containing decursin and decursinol angelate, it is also necessary to consider the safety profile, and the information in this review would contribute to development a new drug from herbal medicine.
