Analysis of factors related to breastfeeding practice of breastfeeding mothers (모유수유부의 모유수유실천에 따른 관련요인 분석)

  • The purpose of this study is to identifY the problems and factors which are closely related to the breastfeeding practice in Korea. The subjects are breastfeeding mothers who have participated in the First Healthy Breastfeeding Child Contest. The 50.8% of subjects are resided in Seoul. The 68.3% of subjects are in their twenties and the 50% of them are high school graduates. Also, 81.4% of the subjects have no job. The 50% of subjects consider that the breastfeeding practice is good for the physical development and health of the child. The 73.9% of the subjects believe that the promotion of breastfeeding practice can be achieved through breastfeeding education through mass media. The 50.4% of subjects consider that the appropriate length of breastfeeding and duration is 9 months. Subjects did proper care of their breast and nipples and did proper measures for the breastfeeding practice. The 95.8% of subjects practiced breastfeeding voluntarily and their breastfeeding practice was not inhibited through factors which were given in the questionnaire lists. In conclusion, the practice of breastfeeding is mostly dependent on the efforts and the positive attitude of breastfeeding mothers. Also the friendly breastfeeding environments in terms of social norms and social practice are the most pertinent factors in promotion of breastfeeding practice.

Silencing of COX-2 by RNAi Modulates Epithelial-Mesenchymal Transition in Breast Cancer Cells Partially Dependent on the PGE2 Cascade

  • In order to prove whether downregulation of COX-2 (Cyclooxygenase-2) could modulate the epithelial-mesenchymal transition (EMT) of breast cancer, celecoxib and siRNA were respectively used to inhibit COX-2 function and expression in MDA-MB-231 cells. The EMT reversal effect in the RNAi treated group was better than that of the celecoxib group while there were no obvious differences in the medium $PGE_2$ levels between the two groups. The results show that COX-2 pathways may contribute considerably to EMT of breast cancer cells, partially dependent on the PGE2 cascade. Akt2, ZEB2 and Snail were measured to clarify the underlying mechanisms of COX-2 on EMT; COX-2 may modulate EMT of breast cancer by regulating these factors. This finding may be helpful to elucidate the mechanisms of selective COX-2 inhibitor action in EMT modulation in breast cancer.

Determinants affecting patients loyalty in medical service: An empirical study on comparison with in-patients and out-patients using multi-group structural equation model (대학병원환자의 의료서비스 충성도에 영향을 미치는 요인에 관한 연구: 멀티그룹 공분산구조분석을 이용한 입원환자와 외래환자의 인식차이 비교분석)

  • The purpose of this research is to identify causalities among service quality, service value, customer satisfaction and customer loyalty and to compare the causalities between in-patients and out-patients. A multi-group structural equation model was used to test the hypothesis of conceptual model. Empirical results indicate that two factors such as assurance and empathy have direct impact on service value and customer satisfaction in both the patients. In the results of multi-group comparison test, however, the path coefficients from service value to customer loyalty and from customer satisfaction to customer loyalty were different across the two groups. In out-patients, customer loyalty is more positively related with service value. In in-patients, customer loyalty is more positively related with customer satisfaction. It is proven that service quality influences customer loyalty. Therefore To compete successfully in today's volatile & competitive medical markets, the hospital needs to manage the service quality, considering the type of patients.

  • A conflict of interest is usually defined as a set of circumstances that creates a risk that professional judgments or actions regarding a primary interest will be unduly influenced by a secondary interest. In general, a conflict of interest can be classified as either a financial or a non-financial one. A financial conflict of interest reportedly occurs frequently in medical (and psychiatric) research and practice and is associated with a greater likelihood of reporting results favorable to the intervention being studied. A non-financial conflict of interest may occur in psychiatric research and practice as a result of the so-called "allegiance effect." It is very important that clinicians and researchers in the field of psychiatry be aware that their field is vulnerable to both financial and non-financial conflicts of interest. Considering that it is impossible in modern psychiatry to be completely free from possible conflicts of interest, the best way to manage a conflict of interest is disclosure based on the principles of transparency. Actions that might lead to conflicts of interest should be avoided as much as possible.

A Study on the Effect of Aroma Massage Therapy on Normal Vaginal Delivery (아로마 마사지요법이 초산모의 정상분만에 미치는 영향)

  • Objective : As this is a quasi-experimental study by the post-test design for a nonequivalent control group, it was attempted to verify the effect of massage using aroma oil on whether or not the normal vaginal delivery of primiparous mothers. Methods : As for the experimental treatment, it applied the aroma oil massage to the experiment group when the cervical dilatation was $4{\sim}5cm$, and as for the data analysis, it conducted the significance test using the program of SPSS 12K for Windows, with Chi-square test $(X^2)$ and t-test. Results : The experiment group that carried out the aroma massage therapy, was high in the cases of normal vaginal delivery(P<0.05), compared to the control group that did not carry out the aroma massage therapy. Conclusion : The experiment group, which received the aroma massage therapy, had significant difference in the cases of normal vaginal delivery, compared to the control group that did not receive the aroma therapy, thus the aroma massage therapy is thought to be a nursing-intervention plan, which can be usefully applied as to the primiparous mothers aiming at normal vaginal delivery.

A Study on the Stress of the Family Caregivers for Patients with Stroke (뇌졸중환자 가족간병원의 스트레스에 관한 연구)

  • Purpose: The purpose of this study was to provide the basic data to decrease the stress of the family caregivers. Methods: The questionnaire was administrated to the family caregivers of 156 patients with stroke who consented to be interviewed. Among 156 caregivers, we analyzed 120 caregivers. We evaluated on the stress of the family caregivers. Results: First, the general characteristics of patient with CVA were high at 82.55% in male, 55.83% in infarction, 53.33% in the left hemiplegia, 47.50% in sixties, 80.83% in 1 recurrence rate, 27.50% in 1-2 years of disease period. Second, the general characteristics of family caregivers were high in above sixties, female in gender, existence in religion, married in marital status, high school in educational level, below 1 million-won in monthly income, spouse in relationship with patient, below 1 years in total caring period. Third, it showed that stress of family caregivers above 3.0 score has 8 items. Forth, the patient's characteristic that have the influence on the stress was significant in the period of disease(p<0.05). Finally, the characteristics of family caregivers that have the influence on the stress were the monthly income, caring period(p<0.05). Conclusion: These findings indicated that the stress of the family caregivers was correlated with the time in hospital, the income and nursing period.

A Study on Pain Characteristics in Parts of the Hand among Estheticians (피부미용사의 손 부위 통증에 관한 연구)

  • Objectives: The purpose of this study was to investigate estheticians' self-reported pain symptoms and pain levels according to the parts of the hand. Methods: Data were collected through a questionnaire survey among 116 estheticians from 31 skin care centers and spas. Information about general characteristics, job characteristics and work characteristics were based on data from the self-administered questionnaire. To determine the self-reported pain symptoms and pain levels according to the parts of the hand, a Cornell Musculoskeletal Discomfort Questionnaire was used. Results: The subjects had the greatest self-reported pain symptoms in the thumb(Right: 53.4%, Left: 46.6%) and felt the highest level of pain in the wrist(Right: 4.87 points, Left: 4.71 points). Physical burdens and age had statistically significant effects on the pain level in the right hand, while physical burdens and the manager rather than the director had statistically significant effects on the pain level in the left hand. Conclusions: Since the thumb and wrist have the greatest subjective pain symptoms in the hand, the use of proper tools is recommended in order to reduce the usage of the thumbs during massage, as is regular stretching to protect the wrists.

Changes in HbA1c According to Education & Consultation Intervention of Type 2 Diabetes Workers (교육 및 상담 중재에 따른 제2형 당뇨병 근로자의 당화혈색소 변화)

  • Objectives: The purpose of this study was to investigate changes in HbA1c through an education and consultation intervention with diabetic workers. Methods: The participants were 65 workers with Type 2 diabetes. The data were collected from May to October 2014 using questionnaires. The intervention program included six monthly sessions on exercise, dietary habits, stress management, and diabetes knowledge. Changes in HbA1c were evaluated. The data were analyzed using descriptive statistics and paired t-tests with SPSS WIN23.0. Results: The mean of HbA1c was 7.67% before intervention and 7.28% after intervention, and this difference was statistically significant (P<0.01).The mean level of HbA1c was significantly different depending on job position, working hours, work duration, and smoking habit. Conclusion: In conclusion, these findings support the usefulness of intervention programs in reducing HbA1c. Therefore, appropriate intervention programs involving exercise, dietary habits, stress management and diabetes knowledge should be developed and provided to diabetic workers.

  • Problem based learning(PBL) is one of the learning strategies from the constructivism. It is a learning centered students. The tutors are facillitators as activators, helpers and cooperators not organizer in the classrooms. PBL makes that students learn creativity, independence, reasoning skits, communication and collaboration for problem solving. As the PBL process, students get the problems that are in real situation, discussed with others for brain storming, self directed study and revisited to the situation. They think critically and apply to the real situation. When students are to be physical therapists, they are easy to adopt their job and efficient to manage well. But inspite of a lot of advantages to them, there are much conflict to use as the learning strategies. Students perceived one of best learning method that they have experienced, but there are stress, burden, anxiety, timeless to prepare, lack of information and so on. PBL is effective to learning health oriented subjects, problem solving, even a lot preparation and processing for learning. It is reduced the differences between theories in colleges and practices in the fields. In processing of PBL, students get more many skills than the conventional learning. As trying many times to the classrooms, we can fixed to PBL with mistakes and conflict for better the development of the teaching and learning.

Cyt1Aa from Bacillus thuringiensis subsp. israelensis Enhances Mosquitocidal Activity of B. thuringiensis subsp. kurstaki HD-1 Against Aedes aegypti but not Culex quinquefasciatus

  • The Cyt1Aa protein of Bacillus thuringiensis subsp. israelensis is known to synergize mosquitocidal proteins of B. thuringiensis and Bacillus sphaericus strains. Cyt1Aa is highly lipophilic, and after binding in vivo to the midgut microvillar membrane serves as a "receptor" for mosquitocidal Cry proteins, which subsequently form cation channels that kill mosquito larvae. Here we report that Cyt1Aa can serve a similar function for lepidopteran-specific Cry proteins of B. thuringiensis in certain mosquito larvae. Engineering Cyt1Aa into the HD-1 isolate of B. thuringiensis subsp. kurstaki enhanced toxicity against $4^{th}$ instars of Aedes aegypti, but not against $4^{th}$ instars of Culex quinquefasciatus.
