Airway management through submental derivation: a safe and easily reproduced alternative for patients with complex facial trauma

  • Objectives: Airway management in patients with panfacial trauma is complicated. In addition to involving facial lesions, such trauma compromises the airway, and the use of intermaxillary fixation makes it difficult to secure ventilation by usual approaches (nasotracheal or endotracheal intubation). Submental airway derivation is an alternative to tracheostomy and nasotracheal intubation, allowing a permeable airway with minimal complications in complex patients. Materials and Methods: This is a descriptive, retrospective study based on a review of medical records of all patients with facial trauma from January 2003 to May 2015. In total, 31 patients with complex factures requiring submental airway derivation were included. No complications such as bleeding, infection, vascular, glandular, or nervous lesions were presented in any of the patients. Results: The use of submental airway derivation is a simple, safe, and easy method to ensure airway management. Moreover, it allows an easier reconstruction. Conclusion: Based on these results, we concluded that, if the relevant steps are followed, the use of submental intubation in the treatment of patients with complex facial trauma is a safe and effective option.

  • The law reflects the situation of the times, understands the society, and shows the will of the state and the community. The Means of Egress should be maintained from design to construction, supervision, as well as use, in order to protect the lives and property of the residents as well as the safe use of the facilities. However, Interior Architects are think that evacuation and safety regulations are complex elements that change frequently and may inhibit the idea of design. The purpose of this study is to propose a design method for the use of safe facilities in the interior architectural design on the evacuation regulations affecting the actual design by the IBC(the International Building Code) and NFPA(National Fire Protection Association) LSC (Life Safety Code). The research method is to investigate and analyze the provisions related to the evacuation of interior architecture in the US and to understand the current regulations and the evacuation regulations. We suggest to design method to the details of the hallway, corridors, aisle accessway, door way, exit path, In particular, the design of furniture, tables and chairs layout that could be obstacles to evacuation situations is presented.

농약안전사용(農藥安全使用) 및 중독경험(中毒經驗)에 관(關)한 연구(硏究) (A Study on Safe use of Pesticides and Pesticidal Poisoning Among the Korean Farmers)

  • This study was conducted with 478 farm households located in Pyongtaek Gun of Gyonggi Do, Chongju City and Chongwon Gun of Chung Buk Do, and Hongchon Gun of Gangwon Do for the period of 10, September through 20, October 1980. It dealt with general features of the farmhouseholds, their practices of handling and using pescides, and relationship between safe use of pesticides and poisoning experiences. The results of the study are summarized as follows: 1. Of the total, 63.2% purchased pesticides mainly through the Agricultural Coops, and 95.4% transported them home separately from other goods. 2 Pesticides were sold contained mainly in glass bottles and paper bags. 3. Mixing of the respodents (87.5%) stored pesticides in sheds, warehouses and boxes. 4. Mixing containers and measuring tools were not used by the majority of the group studied. This indicates that there exist serious problems of safety and accuracy with regard to use of pesticides. 5. As to pesticide protective devices, gloves, respirators, goggles and boots were not used in general. Even in case of using them, their quality was not satisfactory. 6. Among the interviewees, 4.2% did not read pesticide label, 33.9% did not wash immediately when pesticide was spilled on the skin, 48.4% did not care the wind direction during pesticide application, and 17.0% did not wash hands when smoking a cigarette while pesticide is being handled. 7. Of the total, 40.4% were found to harvest vegetables within 10 days from the date of pesticide application, which indicates their unawareness of possible hazards by pesticide residues. 8. Empty pesticide containers or bags were not properly disposed of by majority of the respondents. Furthermore, impacts on the environment by the misuse or abuse of pesticides were not understood by 67.6% 9. Of the interviewees, 48.7% were found to have not received instructions on safe use of pesticides. 10. The rate of pesticidal poisoning experience was about 28.0% if minor symptoms excluded, and it rose to about 44.0% if included. Meanwhile, the rate of pesticidal poisoning tends to increase with the size of farm. 11. The study failed to show a statistically significant relationship between the rate of pesticidal poisoning and use of mixing container and/or measuring tools. However, use of gloves showed a statistical significance on the rate of pesticidal poisoning. 12. Among the poisoned cases, 19.0% were cared for in either hospitals or clinics.

내압방폭을 위한 Safe Gap의 측정에 관한 연구 (A Study on the Safe Gap for Explosion-proof)

  • 가연성 가스가 존재하는 위험 분위기에서 전기기기를 사용할 경우 전기 스파크에 의한 폭발위험성이 존재하기 때문에 점화원을 격리시키거나 고립시키는 것이 필요하지만 현실적으로 점화원의 고립이 불가능하므로 폭발을 방지하기 위한 일반적인 방법으로 내압 방폭형전기기기를 사용하고 있다. 따라서 내압방폭기기의 내부에 침투한 가연성 가스가 폭발하여도 화염이나 열이 틈새를 통과하여 외부의 가연성 분위기를 점화시킬 수 없는 최대 틈새크기를 찾아야 할 필요가 있다. 본 논문에서는 수소-공기 혼합기와 메탄-공기 혼합기에 대하여 실험적 최대틈새크기(MESG)를 찾아내어 기존의 결과와 비교하고, MESG에 영향을 미치는 요인들을 찾아내고자 하였다. 실험장치는 내용적 8${\iota}$의 구형용기를 사용하였으며 실험 변수들로는 전화위치, 혼합기의 농도, 초기압력 등이었다. 실험결과 각각의 변수들에 의해 영향을 받으며 특히 농도와 초기압력에 크게 영향을 받는 것을 알 수 있었으며, 당량 농도 가까이에서 최소값을 나타내었으며 초기압력의 상승과 함께 MESG는 감소하였다.

스마트시티의 주민참여형 안전도시 계획을 위한 빅데이터 활용에 관한 고찰 (A Study on big data utilization for implementation of the resident participation type safe community planning of the smart city)

  • 기존의 도시계획은 내부 소수의 의사결정자들에 의해 이루어졌으나 스마트시티에서는 ICT 기술 진화를 통해 개인이 접할 수 있는 정보가 많아지고 채널이 다양화 되면서 다양한 방식으로 의사결정과정에 참여할 수 있게 되었다. 이에 본 연구에서는 스마트시티의 안전도시 계획 과정을 주민과 직간접으로 소통하는 협력적인 계획과정으로써 데이터 활용 방안을 제시하고자 한다. 이를 위해 WHO 국제안전도시 인증을 받은 일본의 마츠바라시(松原市)의 데이터의 내용과 활용을 국내와 비교하여 국제안전 도시의 승인을 준비하는 지역에서 주민참여 방안의 보조적 역할로 데이터의 활용성을 높이고자 한다.

  • 본 본문은 NOMS 와 CMOS 집적회로에서 발생 가능한 물리적 결점에 의한 결함에 대해서 Fail-safe 시스템에서 사용할 목적이며, 첫 번째 VLSI 회로 상에서 다양한 물리적 결점을 반영할 수 있는 PLA에 대한 결함 모델을 제시한다. PLA에 근거한 설계 이유는 VLSI칩에서의 물리적 결점을 세부적으로 분식하는 것이 너무 복잡하기 때문이다. 두 번째 본문에서는 2단 AND-OR 또는 NOR-NOR 회로로 구현한 설계가 최적의 크기를 갖는다는 것을 보여준다. 또한 NOR-NOR PLA로 구현한 비교기가 제시한 단일 결함 모델에 대해서 자체시험성을 갖는다는 것을 증명한다. 최종적으로 Fail-safe 가산기에 대해 빌딩블럭으로 자체시험 비교기의 적용을 논한다.

안전한 수혈을 지원하는 모바일 헬스케어시스템의 설계 및 구현 (Design and Implementation of Mobile Healthcare System supporting Safe Transfusion)

  • 바코드 시스템과 전자태그(RFID)부터 저 전력 블루투스 기술을 활용한 비콘(Beacon)까지 진료 및 검사 단계를 자동화하고 스마트 기기와 연계된 진료 정보를 이용하는 등 정보기술과 보건‧의료 분야의 용합이 널리 확산되고 있다. 본 논문에서는 환자에게 혈액을 수혈하는 과정에서 다른 혈액형이 수혈되는 사고를 예방하기 위해 안전한 수혈관리를 위한 모바일 헬스케어 시스템을 설계하고 구현하였다. 환자 팔찌, 채혈 용기, 혈액백 및 의료진 신분증의 바코드에 저장된 정보를 스마트 기기로 인식하여 환자의 정보와 매칭시켜 안전하고 효율적인 수혈이 가능하도록 구현하였다. 본 논문에서 제안하여 구현한 수혈관리 모바일 헬스케어 시스템을 종합병원의 수혈 과정에 적용한 결과, 안전하고 효율적인 수혈이 가능하여 수혈 과정에서 발생하는 사고의 예방이 가능한 것으로 확인되었다.

코로나19 대유행 시기 국민안심 병원 간호사의 감염관리 수행도, 감염관리 피로도, 사회적 지지가 소진에 미치는 영향 (The Effects of Infection Control Performance, Fatigue and Social Support on Burnout among Nurses Working in National Safe Hospitals During the COVID-19 Pandemic)

  • Purpose: The purpose of this study was to identify the effects of infection control performance, infection control fatigue, and social support on the burnout of nurses working at National Safe Hospitals during the coronavirus disease (COVID-19) pandemic, and to use that data to suggest methods to reduce burnout among nurses. Methods: From June 30 to September 30, 2021, data were collected from 144 nurses who both consented to participate in the study and had worked for more than six months in National Safe Hospitals with 300 or more beds in the Gyeonggi-do Province in Korea. Data were analyzed using multiple regression analysis. Results: The factors affecting nurses' burnout at National Safe Hospitals during the COVID-19 pandemic were found to be infection control fatigue (β=.60, p<.001) and social support (β=-.27, p<.001). The explanatory power of these factors was 37.0%. Conclusion: Nurses' burnout at National Safe Hospitals was found to be at a moderate level. Burnout should be reduced through human, material, and administrative support, all of which can reduce infection-control fatigue. It is also necessary to develop both a non-face-to-face social-support-reinforcement program, considering restrictions mandated by infectious-disease pandemics, and a long-term intervention strategy to prevent burnout.

  • Increasing Public Concern : Pesticide residue is probably one of the fastest growing problems in regard to environmental contamination. Pesticide use in agriculture in this century has produced certain benefits, including a decrease in crop waste and an increase in crop yields and food quality. However, pesticide use also creates problems of having effects on the environment and remaining in food chain. The presence of pesticide residue in food, water, and soil has aroused public concern over potential health hazards. Despite information provided by national and private level agencies suggesting that food is safe, consumer groups worldwide are demanding assurance as to the safely of agricultural products.
