시판(市販) 불소계(弗素系) 살서제(殺鼠劑)의 성분특성(成分特性) (On the Chemical Properties of Commercial Organofluoro Rodenticide)

  • 시판중인 유기불소계 살서제인 키라톨-F 제의 화학적(化學的)특성을 알아보기 위해 본 실험을 수행하였다. 시료는 제조회사에 따라 두 종류를 비교 검토하였으며, 아울러 실험실내 합성을 통해 주성분을 확인하고자 시도하였으며 그 중요한 결과는 아래와 같다. 1. 시판 키라톨-F제의 주성분으로 제시된 ${\beta}$-fluoroethanol acetate 는 확인할 수 없었으며, 그대신 ${\beta}$-fluoroethanol 이 주성분으로 밝혀졌다. 2. 시판 키라톨-F제는 수용액이며 chloro ethanol 이 소량 함유되어 있다. 3. 실험 실내에서 합성한 fluoroethanol은 ${\alpha}$-와 ${\beta}$-fluoroethanol 의 혼합물로 추정되며 합성수율은 광조건이 암조건보다 훨씬 높은 것으로 나타났다. 4. 키라톨-F제의 희석 배율에 따른 경시변화를 조사한 결과, 주성분의 분해는 일어나지 않았으며, 따라서 2차독성의 가능성이 매우 높은 것으로 생각된다.

  • Rats and Cockroaches are the most problematic pests of our living environment, Rats generally fall under three categories, namely, the Norway rats, the roof rats and the mouses, Among these three types, the roof rats are increasing rapidly in number and their habitats are spreading quickly in the city area, where as the Norway rats are decreasing both in number and habitat. There are numerous damages that these pests create in our society, The rat induce damages to computers and other information processing equipments, sometimes cause light-out, and other financial losses and other sanitary problems. The cockroaches, apart from inducing serious sanitary problems, is an insect that creates insectophobia. decently, it has been revealed that cockroaches are also the main carriers of allergic diseases. Among the pesticides employed against rats in Korea, Beta - fluoro acetate(1080) was initially used after the National Liberalization. Later, the Korean government imported anticoagulant rodenticide for its control. This was followed by a brief, unsuccessful utilization of Zinc Phosphide. Since the begin of 1980s, with the advent of pest control companies, first generation anticoagulant rodenticide were developed in Korea and recently followed by a widespread use of pesticide that utilizes second generation anticoagulant material, the "Brodifacoum" . Among insecticides employed against cockroaches, "Fenitrothion" "Dusban" "DDVP" "Diazinon" and others have been used. They were gradually being replaced by the use of synthesised pyrethroid insecticides until recently. Today there is a wide spread use of bait poisons, the "hydramethylnon" Nevertheless, full prevention and control can not be realized with the sole use of the pesticides. In the future, hygine and proofing, combined with affective pest management, must be incorporated in order to bring about more satisfactory and fruitful preventive measures.

Beta-fluoroethyl acetate 음독 1례 (A Case of ${\beta}-fluoroethyl$ Acetate Poisoning)

  • The emergence of anticoagulant rodenticides in the 1950's resulted in an epochal reduction in the use of all other agents. It is estimated that anticoagulant rodenticides constitute 95 percent of all rodenticides now in use. However, the advent of rodent resistance to these agents has led to a return to use of some other non-anticoagulant types of rodenticides. Older non-anticoagulant rodenticides have been stored in basements, garages, and barns and are still in use in some regions of the United States and in other countries. We report a case of non-anticoagulant rodenticide (beta-fluoroethyl acetate) poisoning.

  • Many losses in domestic animals have bern reported in this country, since sodium monofluoroacetate (Compound 1080) was used as a rodenticide. Lesions described inclubed multiple petechiae in epi- and endocardium, lung and intestinal mucosa, or superficial necrosis of the gastric mucosa. In this studies as to the poisoning 7 rabbits were administered 0.6mg (Group I), 0.4mg (Group II), and 0.3mg (Group III) of sodium monofluoroacetate per kilogram of body weight. and the results obtained were as follows: In addition to the changes mentioned above fatty degeneration in central parts of hepatic lobule or nut meg liver, haemorrhagic feci in cerebral cortex and leptomeninges and fatty degeneration in kidney and cardiac muscles were found.

반추수(反芻獸)의 살서제중독(殺鼠劑中毒)에 관한 병리학적(病理學的) 연구(硏究) (Pathological Studies on the Experimentally Induced Rodenticide Poisoning in Ruminant)

  • This paper dealt with the pathological and clinical findings on the experimentally induced rodenticide (fluoroacetate, zinc phosphide, thallium sulfate, coumarin) and NaCN poisoning of ruminants (Holstein cattle and/or Korean native goat) for the purpose of the diagnosis in the accidental rodenticide poisoning of cattle. The results observed are summarized as follows: Fluoroacetate poisoning (cattle and goat): in the clinical signs, there were depression, convulsion, dyspnea, groan, grinding of the teeth, vomiting, opisthotonus and post-mortem tympany. In the macroscopical findings, the blood was more or less poor coagulative and dark red, bloody fluid with foam in the trachea, hyperemia and hemorrhage of tracheal mucosa and lung, cloudy swelling and hyperemia of kidney, epicardial hemorrhage(cattle), and hyperemia of abomasum, intestine and brain were observed. In the microscopical findings, there were pulmonary edema and hemorrhage, necrosis of convoluted tubular epithelium and interstitial hemorrhage of kidney, focal coagulative necrosis of myocardium, hemorrhage of pancreas and spleen, dilatation of Virchow-Robin space and hyperemia of brain, and necrosis with desquamation of mucosal epithelia of abomasum and upper small intestine. In the histological lesions of the liver, lobular peripheral hyperemia, centrilobular necrosis and cytoplasmic inclusion bodies of the hetatic cells were observed. The cytoplasmic inclusion body of the hepatic cells was not seen in the affected goat, but hydropic degeneration of the hepatic cells was marked. Zinc phosphide poisoning (cattle and goat): clinically, the affected animals died in recumbent position after ataxia, dyspnea and convulsion. In the macroscopical findings, hyperemia and hemorrhage of lung, cloudy swelling and hyperemia of liver and kidney, hemorrhage of spleen (cattle), and catarrh of abomasum and small intestine were observed. In the microscopical findings, necrosis of the convoluted tubular epithelium and hyperemia of kidney, hemorrhage of spleen, hyperemia of lung, hyperemia or hemorrhage of heart, cloudy. swelling and fatty changes of hepatic cells, dilatation of hepatic central vein, hyperemia of brain, and catarrh of abomasal and small intestinal mucosae were observed. Thallium sulfate poisoning (cattle): in the macroscopical findings dark red color of blood, hyperemia and hemorrhage of lung, bloody fluid with foam in the tracheal mucosa, petechiae of tracheal mucosa, cloudy swelling and hemorrhage of liver, necrotic lesions and hemorrhage of renal cortex and epicardial hemorrhage were observed. In the microscopical findings, severe hemorrhages of the lung, cloudy swelling and necrosis of hepatic cells, hyperemia and hemorrhage of liver, focal coagulative necrosis of mycordium, necrosis of the convoluted tubular epithelium and hyperemia of kidney, hyperemia and hemorrhage of spleen and dilatation of Virchow-Robin apace in brain were observed. Coumarin poisoning (goat): the poisoned animals died in the state of groan and depression. In the macroscopical findings, poor coagulation of blood, hemorrhage of lung, cloudy swelling and severe hemorrhages of liver, cloudy swelling and hemorrhage of kidney, abomasal hemorrhage, catarrh of small intestine, and hyperemia and hemorrhage of the other organs were observed, In the microscopical findings, hyperemia and hemorrhage of lung and kidney, cloudy swelling of the convoluted tubular epithelium of kidney, severe hepatic hyperemia, cloudy swelling and hydropic degeneration of heptatic cell, and hyperemia and hemorrhage of brain and spleen were observed. NaCN poisoning (cattle and goat): clinically, there were convulsion, severe dyspnea, paresis of hind limb, depression and then rigor of four limbs. In the macroscopical findings, bright red color of blood, hyperemia and bright and red tinge of lung cloudy swelling of kidney and liver, and hyperemia of abomasum were observed. In the microscopical findings, cloudy swelling and hydropic degeneration of hepatic cell, hyperemia and edema of lung, necrosis and degeneration of the convoluted tubular epithelium and hemorrhage in kidney, dilatation of Virchow-Robin space of brain and hemorrhage of spleen were observed.

급성 직업성 농약중독 및 손상에 대한 감시체계: 미국의 경험 (Surveillance of acute Occupational Pesticide-related Illness: The US Experience)

  • Pest control is required for protecting the food supply and for controlling disease vectors. Unfortunately, there is no perfectly safe form of pest control. Pesticides are commonly used for pest control. Pesticides are defined under the US Federal Insecticide Fungicide and Rodenticide Act(FIFRA) as any substance or mixture of substances intended to prevent, destroy, repel, or mitigate pests, and any substance or mixture of substances intended for use as a plant regulator, defoliant, or dessicant(40 CFR Part 152). Currently in the United States, there are 890 active ingredients registered as pesticides. Approximately one billion pounds of active ingredient are used in the US per year. Unlike most chemicals(anti-neoplastic and anti-micobial medications are the principal exceptions), pesticides are specifically designed to kill and cause harm. Because society allows these chemicals to be disseminated into the environment, it is important to monitor the health effects associated with these releases. This represents an important justification for establishing and maintaining surveillance systems for acute pesticide-related illness and injury. A comprehensive, national surveillance system for acute pesticide-related illness and injury does not currently exist in the US. Although the United States has several surveillance systems for this condition, none provide a complete understanding of the problem of acute pesticide-related illness and injury. The Toxic Exposure Surveillance System(TESS) and Bureau of Labor Statisitics(BLS) are useful for assessing magnitude and trends. The state-based surveillance systems are more useful for timely identification of outbreaks and emerging problems. Efforts are underway to increase the number of states that conduct surveillance, and to broaden the use of the standardized case definition to facilitate aggregation of data across states. Through such efforts, a comprehensive, national surveillance system may be attainable.

일 지역 119구급대를 통한 약물중독 사례 고찰 - 서울시 서초지역 중심 - (A Survey of Drug Intoxication Patients Transferred by 119 Ambulance Service - Seocho area in Seoul -)

  • This study aims to provide information about the acute poisoning patients transferred by 119 ambulance service for nurses and emergency medical technicians. The data is based on 119 ambulance services patients charts of Seocho area in Seoul. The results were as follows: 1. Total number of emergency patients all over the country has been nearly fourtimes in 1995 compared to 1991. Total number of emergency patients were 322.051 in 1994. 2. About $35\%$ of total E.R. patients were transferred by 119 ambulance services in Seoul in the last 5years. Acute drug intoxication patients occupied $2-3\%$ of E.R. patients in Seoul. 3. About $4\%$ of E.R. patients in Seoul were transferred by 119 E.R. services of Seocho area in Seoul in 1995. Among them $2.4\%$ patients were drug intoxication patients. 4. Data were collected from available patients of 119 ambulance services from Jan. 1993 to Dec. 1995 in Seocho area. Total poisoning patients were 184. - The female/male ratio was 69.7:30.4 and most of patients$(69.7\%)$ were in the age of 20th and 30th. - The busy time of calling 119 ambulance service was from 6P.M. to midnight$(37.5\%)$ - It took within 10 minutes for patients$(62.8\%)$ from notification to arrival in hospital. - Regarding poisoning substances. hypnotics $(22.8\%)$, tranquilizer$(14.7\%)$, agricultural agents$(6.0\%)$ rodenticide$(27\%)$ and others $(10.3\%)$ were in order and unknown were $43.5\%$. - Most of the patients or protector $(72.8\%)$ chose hospitals. Among them $(87.3\%)$ were general hospital. - The most predominant symptoms were coma$(22.3\%)$. dizziness$(6.5\%)$ and then allergy. vomiting. gastrointestinal cramps etc. - Airway management and oxygen administration together was the main medical control of prehospital emergency medical services$(33.7\%)$. It is proposed that first, a systematic survey of drug intoxication patients must be conducted to give an appropriate prehospital emergency care for the emergency medical technician and second, a wide and regular public education to improve understanding of first aids should be undertaken.

인삼성분이 살서제(Vacor)로 유발시킨 쥐의 당뇨에 미치는 영향 (EFFECTS OF GINSENG COMPONENTS ON RODENTICIDE VACOR-INDUCED DIABETES MELLITUS IN RATS)

  • 고려홍삼에서 추출한 성분(RGF-3)이 살서제 Vacor로 유발된 당뇨병에 어떤 영향을 미치는가를 흰쥐에서 관찰하였다. 홍삼 RGF-3 분획의 복강내투여 (20mg/kg)는 allo-xan-유발고혈당을 저하시키고 소량(10mg/kg)의 Vacor로 유발된 당불내성을 개선시켰다. 소량의 Vacor로 중독된 흰쥐는 정상대조군에 비하여 높은 혈중 insulin치를 나타내는 경향이었으며, Vacor투여 3시간 전에 복강내 투여한 RGF-3은 정상대조군에 있어서보다 더 높은 혈중 insulin치의 증가를 가져 왔다. 흰쥐의 부고환지방세포의 glucose대사[(1-$^{14}C$-glucose 의 $^{14}CO_{2}$에로의 전환]와 glucose로부터의 지방합성 [(1-$^{14}C$) glucose로부터의 $^{14}C$-lipid에로의 전환]은 Vacor중독에 의하여 저하되었으나, RGF-3 투여에 의하여 정상수준까지 회복되었으나, 그러나 정상지방세포에 대해서는 RGF-3은 별다른 영향을 주지 아니하였다. 한편 이상과 같은 말초지방세포의 glucose 대사는 insulin의 in vitro 첨가에 의하여 촉진되었으나, Vacor 중독에 의하여 감약되었고, RGF-3은 Vacor 중독에 의한 감약된 반응을 어느정도 회복시켰다. 홍삼의 수침추출물 (30mg/kg)과 RGF-3분획 (20mg/kg)의 $10{\~}20$일간 격일 투여는 그 후 대량(25mg/kg)의 Vacor 중독으로 유발되는 고혈당 발생에는 별 영향을 주지 아니하였다. 이와 간은 실험결과에 비추어 홍삼의 RGF-3분획은 Vacor 중독으로 저하된 말초조직의 당대사를 개선시켜 소량의 Vacor 중독시의 당불내성을 정상으로 회복시키는 것이라 사료되었다.\alpha}$-tocopherol의 항 산화작용을 더욱 효과적으로 촉진 시킬 것으로 생각된다.-L(독성 호르몬-L)의 작용을 억제함으로서 암환자의 체중 감소를 방지하고, 식욕감퇴를 개선할것으로 생각된다.해되었으며, $Rb_{1}$은 장내의 효소와 tetracycline-resistantant bacteria에 의해 Rd와 2 종류의 미확인 물질로 분해되었다.xA_{2}$ synthetase 억제제인 imidazole의 효과와 유사하였다. 4. G-Re는 $1{\times}10^{-5}g/ml$ 이하의 농도에서는 효과가 없으나 $1{\times}10^{-4}g/ml$ 이상의 농도에서 농도의존적으로 유의성 있는 $PGE_{2},\;PGF_{2}{\alpha},\;TXB_{2}$의 생성억제와 함께 6-keto-$PGF_{1}{\alpha}$ 증가를 보였다. 이는 prostacyclin synthetase를 자극하는 serotonin의 효과와 같은 작용으로서 prostacyclin synthetase 억제제인 tranylcypromine에 대하여 길항효과를 보였다. 5. $TxB_{2}$생성억제 작용을 나타내는 ginsenoside들의 효과를 뒷받침하기 위하여 인삼 saponin 성분을 전처치한 patelet rich plasma에서 혈소판 응집시험 결과, ADP로 유도된 혈소판 응집반응에는 모든 인삼 saponin 성분들이 효과가 없었으나 arachidonic acid로 유도된 혈소판 응집반응에는 $G-Rb_{2}$, G-Rc, G-Re의 순으로 농도 의존적인 억제현상을 보였다. 이상의 결과와 같이 인삼 saponin 성분들은 arachidonic acid로부터 cy

1990년대와 2000년대의 원주시 소아에서 발생한 급성약물중독과 이물질섭취 환아의 비교분석 (Clinical analysis of acute drug intoxication and foreign body ingestion in Wonju: comparison between the 1990s and the 2000s)

  • 목 적: 본 연구는 1990년대와 2000년대의 급성 약물 중독과 이물질 섭취로 내원한 환자의 변화를 보기 위하여 실시하였다. 방 법: 1991년 1월 1일부터 1994년 12월 31일까지와 2001년 1월 1일부터 2004년 12월 31일까지 원주기독병원 소아과에 급성 약물 중독으로 입원한 환아와 이물질 섭취로 입원한 환아를 대상으로 하였다. 이들의 의무기록을 기초로 하여 후향적 조사를 하였으며, 10년간의 시간의 차이에 따라 약물과 이물질의 종류, 연령별 빈도, 성별 빈도, 재원일수 등을 비교 분석하였다. 결 과: 급성 약물 중독은 1990년대에는 43례, 2000년대 35례였고, 이물질 섭취 환아는 1990년대 47례, 2000년대 22례 였다. 이물질 섭취의 종류로는 1990년대의 경우, 동전(46.8%), 기타(31.9%), 바둑알(14.9%), 플라스틱 장난감(6.4%) 순이었으며, 2000년대의 경우, 기타(58.2%), 수은건전지(23.8%), 못(9%), 핀(9%) 순이었다. 중독 유발 약물의 종류로는 1990년대의 경우, 의약품(37.2%), 기타(23.2%), 농약(13.9%), 일산화탄소(11.6%), 살서제(9.3%), 살충제(9.3%) 순이였고, 2000년대의 경우, 기타(34.3%), 의약품(31.4%), 농약(20%), 세척제(11.4%), 살충제(2.9%), 일산화탄소(2.9%) 순이었다. 연령별 분포는 이물질 섭취의 경우, 1990년대에 가장 많은 빈도를 차지하는 연령대는 1-3세로 46.8%였고, 2000년대 역시 1-3세에서 40.9%로 가장 많은 빈도를 차지하였다. 약물 중독의 경우, 1990년대에 가장 많은 빈도를 차지하는 연령대는 1-3세로 55.8%였으며, 2000년대 역시 1-3세(54.3%)이었다. 사망이나 심각한 후유장애를 남긴 경우는 이물질 섭취의 경우, 1990년대 콩의 섭취중 기관지로의 흡입에 의한 사망 1례, 2000년대 옥수수알 흡입에 의한 사망 1례가 있었고, 약물중독의 경우, 1990년대 농약(paraquat계)중독에 의한 사망 1례와 일산화탄소 중독에 의한 저산소성뇌증과 편측사지마비를 보인 경우가 1례 있었고, 2000년대 농약(paraquat계)중독에 의한 사망 1례 있었다. 결 론: 급성약물중독과 이물질섭취 모두 1-3세가 대부분을 차지하여 이 연령대의 소아에게 특히 주의 깊은 관찰이 필요할 것으로 생각된다. 또한 섭취된 약물과 이물질의 종류도 시간이 지나면서 다양해지고 있어 이에 대한 부모교육이 필요할 것으로 생각된다.
