Radiotherapy of Para-Aortic Node Metastases in Carcinoma of the Uterine Cervix (자궁경부암에서 대동맥 임파절 전이의 치료)

  • Forty one patients with para-aortic node metastases from carcinoma of the uterine cervix treated with radiotherapy at Department of Radiation Oncology, Yonsei University, College of Medicine from January 1982 to December 1987 were retrospectively analyzed. Eleven patients were diagnosed at the time of diagnosis of carcinoma of the cervix (early diagnosis) and 30 patients were diagnosed during follow up period after definitive radiotherapy of primary site (late diagnosis). The most important factors affecting the survival in this study were time of diagnosis and dose of irradiation. Overall 5 year actuarial survival rate of 41 patients was $25.7\%$. Five year survival rate for early diagnosis was $60.3\%$, but late diagnosis was $16.9\%$. And survival rate for high dose (over 4000 cGy) radiation group and low dose radiation group were $42.2\%\;and\;8.9\%$ respectively The most leading cause of death was para-aortic node failure, so early diagnosis and maximum palliation with full dose radiotherapy (over 4000 cGy) is necessary to improve the survival and the quality of life.

  • In medical institutions, there are radiation-related workers such as radiological technologists, physicians, dentists, and dental hygienists who handle diagnostic radiation generators. Also, there are work assistants, such as nurses and assistant nurses, who assist in radiation treatment or transfer patients to the radiation examination room. Radiation exposure management for radiation-related workers is carried out under the 「Medical Service Act」, but there is no legal basis for work assistants, etc. And the management of radiation exposure for diagnosis is regulated by the 「Medical Service Act」, and the management of radiation exposure by therapeutic radiation and nuclear medical examination is governed by the 「Nuclear Safety Act」. Thus, to improve the management of radiation exposure for diagnosis, the regulations on radiation exposure management for diagnosis under the 「Medical Service Act」 were compared and reviewed with those of the 「Nuclear Safety Act」. As a result, the main contents are as follows. First, it is necessary to legislate to include nurses, assistant nurses, and clinical practice students who are likely to be exposed to radiation besides radiationrelated workers as subjects of radiation exposure management for diagnosis. Second, when a radiation-related worker for diagnosis is confirmed to be pregnant, the exposure dose limit should be defined. Third, it is necessary to revise the regulations on the types of personal exposure dosimeters in the 「Rules on the Safety Management of Radiation Generators for Diagnostics」. Fourth, it seems that health examination items for radiation-related workers, radiation workers, and frequent visitors should be the same. Fifth, It is necessary to unify and regulate diagnostic radiation and all medical radiation, including therapeutic radiation and nuclear medicine, in one legal system.

Problems of the Act and Subordinate Statutes Related to the Regulation of Radiation Safety for Diagnosis (진단용 방사선 안전관리 법령의 문제점에 관한 연구)

  • The use of diagnostic radiation in medical institutions is rapidly increasing. Accordingly, the collective effective dose is on the rise every year. Therefore, it is necessary to reduce the radiation exposure of the person undergoing the radiation examination as low as reasonably achievable. And we must establish a legal system to perform the safe management of radiation for diagnosis efficiently. In this way, I went over the problems of the Act and Subordinate Statutes regarding radiation safety management for diagnosis. As a result, the main contents are as follows. First, in the 「Medical Service Act」, there is no basis for the Safety Inspection Institute of Radiation and Radiation Exposure Measuring Institutes. And there are no provisions concerning delegation of administrative disposition. Therefore, it is necessary to secure legal justification by providing the basis for the Safety Inspection Institute of Radiation along with Radiation Exposure Measuring Institutes and the basis for administrative dispositions against these institutions in the 「Medical Service Act」. Second, the 「Rules on the Installation and Operation of Special Medical Equipment 」 should be integrated with the 「Rules on the Safety Management of Radiation Generators for Diagnostics」 to unify administrative procedures such as reporting for radiation special medical equipment for diagnosis. Third, in the case of violating the diagnostic radiation safety management standards in the 「Rules on the Safety Management of Radiation Generators for Diagnostics」, it is necessary to supplement the insufficient sanctions such as administrative disposition. Fourth, regulating diagnostic radiation and therapeutic radiation used in medical institutions with the dual legal system of the 「Medical Act」 and the 「Nuclear Safety Act」 is not efficient in the safety management of diagnostic radiation. Therefore, it is necessary to uniformly regulate diagnostic radiation and all medical radiation, including therapeutic radiation and nuclear medicine, in the 「Medical Service Act」 system.

A Study on recognition for Medical radiation of health and non-health for radiation safety (방사선안전을 위한 보건/비보건계열의 의료방사선 인식도 분석)

  • This study implemented the survey and analysis of the exposure to radiation by using the questionnaire targeting H Health College, located in Daejeon from September 1st, 2014 to October 15th. A total of 400 copies of the questionnaire was distributed and among them, 385 copies, excluding 15 omitted ones (total collection ratio: 96.3%), were used for the analysis. The analysis results are as follows. For the harmfulness of radiation for diagnosis, the average of the health-related was 3.15 and the average of the health-unrelated was 2.82, which the health-related students recognized the harmfulness of radiation for diagnosis higher (p<.001). The necessity of radiation for diagnosis was appeared higher from the health-related students as the average of the health-related was 3.07 and 2.52 for the average of the health-unrelated (p<.001). The recognition on the prevention of the exposure to radiation was higher from the health-related students as the average of the health-related was 3.13 and 1.84 for the average of the health-unrelated (p<.001). From this study, the necessity of using radiation from the medical field and the recognition on its harmfulness appeared to have a big difference between the health-related and health-unrelated. For such, the accurate understanding of the recognition on radiation and the education to improve recognition on radiation are considered to be required.

  • Opened a court in February 10, 2006, a rule of safety management of the diagnosis radiation system was promulgated for safety of the radiation worker, patients and patients' family members. The purpose of this rule is to minimize the risk of being exposed to radiation during the process of handling X-ray. For this reason, we manufactured shielding device of mobile X-ray unit collimator for diminution of skin dose. Shielding device is made to a thickness of Pb 0.375mm. For portable chest radiography, we measured skin dose 50cm from center ray to 200cm at intervals of 20cm by Unfors Xi detector. As a result, a rule of safety management of the diagnosis radiation system has been strengthened. But there are exceptions, such as ER, OR, ICU to this rule. So shielding device could contribute to protect unnecessary radiation exposure and improve nation's health.

The Radiation Safety Management in the Animal Hospital Using Inspection Standard of Diagnosis Radiation System (진단용 방사선발생장치의 검사기준을 적용한 동물병원의 방사선 안전관리)

  • The purpose of this study was to investigate the actual conditions of radiation safety supervision in animal clinics using inspection standard of X-ray generator for diagnosis. The surveys for inspection standard system, equipment condition, and safety supervision were carried out in 18 animal clinics randomly. The inspection standard included reproducibility of dose exposure, kVp, mAs, collimator accuracy test, collimator luminance test, X-ray view box luminance test, grounding system equipment test and external leakage current test. The surveys of equipment condition and safety supervision used one-on-one interview with 5 points measurement. As a result, 44.44% of reproducibility of dose exposure was proper, 81.25% of kVp test was good, and 100% of mAs test was appropriate. Also, 66.66% of collimator accuracy test was proper, 61.11% of collimator luminance test was good, 53.13% of X-ray view box luminance test was suitable. In addition, only 5.55% of grounding system equipment and ground resistance was proper, 63.64% of external leakage current test was appropriate in grounding system equipment test. The 100mA electric capacity of X-ray generator for diagnosis was popular with 44.44%, and its 55.56% was purchased used equipment. Monthly average of less than 50 times (61.11%) was top frequency in use, and no animal clinic had a thermo-luminescence dosimeter(TLD). The 16 animal clinics with radiation safety zone and 2 without radiation safety zone were appeared.

  • Four cases of sphenoid sinus carcinoma have been observed for last 10 years and we reviewed English literatures about sphenoid sinus carcinoma. The sphenoid sinus carcinoma is rare and the diagnosis is difficult. In the early stage, the non-specific deep constant headache is the only symptom but if the sinus wall is penetrated, the neuro-ophthalmologic symptoms and signs may appear. The extension of lesion is identified by radiologic imaging and the diagnosis requires direct biopsy. In case of deep constant headache combined with neuro-ophthalmologic symptoms and signs the sphenoid sinus carcinoma should be considered. Our small data reveals that the radiation treatment offers a possibility of relatively good outcome, although most of the cases are advanced already on initial diagnosis.

  • Radiation recall pneumonitis is an inflammatory reaction of previously radiated lung parenchyma triggered by systemic pharmacological agents (such as chemotherapy and immunotherapy) or vaccination. Patients present with non-specific symptoms such as cough, shortness of breath, or hypoxia soon after the initiation of medication or vaccination. Careful assessment of the patient's history, including the thoracic radiation treatment plan and timing of the initiation of the triggering agent, in conjunction with CT findings, contribute to the diagnosis. Once a diagnosis is established, treatment includes cessation of the causative medication and/or initiation of steroid therapy. Differentiating this relatively rare entity from other common post-therapeutic complications in oncology patients, such as recurrent malignancy, infection, or medication-induced pneumonitis, is essential for guiding downstream clinical management.

Fate of pulmonary nodules detected by computer-aided diagnosis and physician review on the computed tomography simulation images for hepatocellular carcinoma

  • Purpose: To investigate the frequency and clinical significance of detected incidental lung nodules found on computed tomography (CT) simulation images for hepatocellular carcinoma (HCC) using computer-aided diagnosis (CAD) and a physician review. Materials and Methods: Sixty-seven treatment-$na{\ddot{i}}ve$ HCC patients treated with transcatheter arterial chemoembolization and radiotherapy (RT) were included for the study. Portal phase of simulation CT images was used for CAD analysis and a physician review for lung nodule detection. For automated nodule detection, a commercially available CAD system was used. To assess the performance of lung nodule detection for lung metastasis, the sensitivity, negative predictive value (NPV), and positive predictive value (PPV) were calculated. Results: Forty-six patients had incidental nodules detected by CAD with a total of 109 nodules. Only 20 (18.3%) nodules were considered to be significant nodules by a physician review. The number of significant nodules detected by both of CAD or a physician review was 24 in 9 patients. Lung metastases developed in 11 of 46 patients who had any type of nodule. The sensitivities were 58.3% and 100% based on patient number and on the number of nodules, respectively. The NPVs were 91.4% and 100%, respectively. And the PPVs were 77.8% and 91.7%, respectively. Conclusion: Incidental detection of metastatic nodules was not an uncommon event. From our study, CAD could be applied to CT simulation images allowing for an increase in detection of metastatic nodules.

Lack of any Association between Season of Diagnosis and Survival of Gastric Cancer Cases in Kayseri, Turkey

  • Background: The influence of season of diagnosis on cancer survival has been an interesting issue for many years. Most studies have shown a possible association between seasonality and survival in some cancers. We aimed to investigate whether there is an association between season of diagnosis and survival in patients with gastric cancer. Materials and Methods: We reviewed retrospectively the files of 279 histologically proven gastric cancer patients. According to diagnosis date, the patients were grouped into 4 seasons of diagnosis groups, spring, summer, autumn, and winter. Results: There was no significant differences when the overall survival rates of the patients were compared according to the patients' season of diagnosis (p: 0.871). Median overall survival rates were 22.0 (14.5-29.5) months for the patients who were diagnosed in spring, 24.0 (12.4-35.6) for summer, 18.0 (9.96-26.0) for autumn and 21.0 (16.3-25.7) for winter. Median disease-free survival rates were 66.0 (44.1-68.1) months for the patients who were diagnosed in spring, 28.0 (17.0-39.0) for summer, 22.0 (0-46.4) for autumn and 23.0 (17.5-28.5) for winter. While the rate was best for the patients diagnosed in spring the differences were not statistically significant (p= 0.382). Conclusions: On the basis of the above results the season was not suggested as contributing to prognosis in gastric cancer cases in Kayseri, Turkey.
