Primary Sternoclavicular Septic Arthritis in a Healthy Adult - A Case Report - (건강한 성인에서 발생한 일차성 흉쇄 관절 화농성 관절염 - 증례 보고 -)

  • Sternoclavicular septic arthritis is a rare condition and it is usually related to predisposing conditions like intravenous drug abuse, diabetic mellitus, trauma and so on. A delayed diagnosis of this disease may cause severe complications like mediastinitis and chest wall abscess. Computed tomography or magnetic resonance imaging is needed to evaluate the complications. If the above complications are present, then joint resection should be considered. We report here on a case of a 52-year-old man who was diagnosed with primary sternoclavicular septic arthritis and he had no predisposing conditions. The pathogen on the aspiration-culture was S. aureus and it was susceptible to cefminox. The patient was cured with administering only antibiotic therapy for 6 weeks; intravenous cefminox therapy for 4 weeks followed by oral cefminox therapy for 2 weeks.

A Case Report of Focal Ossification of the Auricular Cartilage (귀 연골조직에서 발생한 부분적 골화)

  • Purpose: The focal ossification of auricular cartilage is an unusual clinical entity in which the ear becomes partially or totally rigid and immalleable. This condition may result from cold injury, local trauma, inflammation, or various systemic diseases. Patients may feel mild discomfort, but there are usually no other serious symptoms. We present a case of focal ossification of auricular cartilage in which the cause is unknown. Methods: A healthy 58-year-old man presented with a 2-year history of hard mass of right posterior auricular area. He denied any precipitating historical events like cold injury and inflammation. Routine testing did not demonstrate systemic abnormalities. Ultrasonographic examination revealed a $22{\times}10{\times}11mm$ sized heterogenous isoechoic mass showing an acoustic shadow. Results: Excisional biopsy was performed under local anesthesia. Histological examination revealed the ossification with deposition of trabecular bone in normal elastic cartilage. The patient was healed without any problems and satisfied with the result. Conclusion: We report clinical experience of focal ossification of auricular cartilage, which is quite a rare clinical entity. It should be considered that there is the possibility of ossification of cartilage when it meets the benign mass of the ear.

Myocardial Preserving Effect of Fructose-1, 6-diphosphate in Isolated Rat Heart (적출된 쥐심장을 이용한 Fructose-1, 6 diphosphate의 심근보호 효과에 관한 실험)

  • Myocardial protection against ischemic and reperfusion injuries is still in troublesome eventhough couples of the way of myocardial protection have been applied since 1970's. One of the possibility in myocardial protection is adding Fructose-l,6-diphosphate(FDP) in cardioplegic solution. It is assumed that FDP can promote ATP production under anaerobic condition as well as inhibiting the supressing effect of lactate on phosphofructokinase. We compared the myocardial protecting effects of FDP in crystalloid cardioplegic solution (St. Thomas formula, 10$^{\circ}C$, pH = 7.4) and reperfusate using isolated rat hearts in modified Langendorf apparatus by the parameters of preischemic and post reperfusing heart rate, time to first beat, occurance of arrhythmia, time to stabilization, and the rate of left ventricular pressure developing. Group A (n = 10), containing no FDP in cardioplegic and reperfusing solutions was control. Group B (n = 5), containing FDP in cardioplegic solution, showed statistically significant superiority of postischemic left ventricular pressure development than the control group. Group C (n = 5), containing FDP in reperfusate, showed statistically significant myocardial depressing effect than the controls. Other parameters were unremarkable. The cause is uncertain, but it is assumed that the negative feedback inhibition of FDP in energy metabolism or unknown blocking effect of FDP on certain transmembrane ionic currents is present. In conclusion, 1) FDP in cardioplegic solution has beneficial effect on postischemic left ventricular preservation. 2) FDP is strong acid when is hydrolyzed, so precise acid titration is neccessary. 3) FDP in reperfusate has negative left ventricular preservation, otherwise the mechanism is still uncertain.

  • Broncholithiasis is defined as a condition in which calcified material is present within the bronchial lumen. It is a rare but troublesome disease that can cause life-threatening complications such as massive fatal hemoptysis. Therefore, pulmonary resection is frequently required to remove the broncholiths and irreversibly damaged parenchyma. We experienced 3 cases of broncholithiasis. In one case, a 36 year old female patient suffered from coughing, massive hemoptysis with lithoptysis caused by intrinsic obstructive broncholiths in the right middle and lower lobe. In the 2nd case, a 41 year old male patient complained of long-standing blood tinged sputum and frequent pneumonic symptoms for 10 months because of extrinsic broncholithiasis where the calcified peribronchial lymph node eroded into the bronchial lumen of the right lower lobe. The remaining case involved a 30 year old female patient who complained of intermittent blood-tinged sputum induced by intrabronchial broncholith in the orifice of the right middle lobe bronchus. Two patients underwent bilobectomy(right middle and lower lobe) for removal of the broncholiths, damaged bronchi and parenchyma. The other patient was treated with right middle lobectomy and stone removal by bronchotomy of bronchus intermedius. In all patients, the post-operative course was uneventful.

  • Diabetic nephropathy (DN) is a common complication and the leading cause of end-stage renal disease (ESRD) in diabetic patients. The occurrence of non-diabetic renal disease (NDRD) in diabetic patients has been increasingly recognized in recent years. Generally, renal injuries in DN are deemed difficult to reverse, whereas some NDRDs are often treatable and even remittable. Thus, the diagnosis of NDRD in patients with diabetes mellitus (DM) via a kidney biopsy would be significant for its prognosis and therapeutic strategy. According to recent studies, the most common NDRD is IgA nephropathy in type 2 diabetic patients, and some cases of minimal change disease and membranous glomerulonephritis have been reported in Korea. However, membranoproliferative glomerulonephritis (MPGN) is an uncommon condition in diabetic patients. To our knowledge, there has been no case yet of MPGN, except in a child with type 1 DM. We present an unusual case of a 27-year-old woman who had type 2 DM with MPGN, as confirmed via a kidney biopsy.

3-D Finite Element Analysis of Acetabular Reconstruction of THR (인공고관절 전치환술에 있어서 비구 재건 술에 관한 3차원 유한요소해석)

  • Using a 3-D finite element method (FEM), the biomechanical characteristics of a threaded truncated acetabular component and a porous coated hemispherical acetabular component were studied. The Von-Mises stress/strain patterns in the acetabulum reconstructed with these two different types of cementless acetabular cups were investigated. The geometry and dimensions of human hemi-pelvis used in the present shape modeling for finite element analysis were scanned with a 3-D laser scanner(TDS-9000, Cyberware, USA). The scanned data was numerically handled with a shape modelling software 'Pro-Engineer'. Using 19836, 16853 tetrahedral elements, respectively, the stress and displacement field of the acetabulum reconstructed with the two different types of the acetabular components were computed. While the hemi-sphere component was found to show a relatively similar stress/strain patterns to those in the normal hip, the results with the threaded cup showed a considerably different patterns from those in the normal condition. Several regions in cancellous bone near the threads and the edge of the truncated cup was found to be overstressed, especially in the superior-lateral part of the acetabulum. It was postulated that the excessive reaming-out of subchondral bone layer when the truncated cup was used can cause the presence of these overstressed regions of cancellous bone. This theoretical prediction for the implanted acetabulum appeared to consistent with the pathological observation of proximal/medial migration of the threaded truncated acetabular prostheses in the previous publications.

A Study on the Effect of Disparity-based Asymmetrical Filtering on the Binocular Stereoscopic Video (양안식 스테레오 비디오에 대한 변이 기반 비대칭 필터링의 효과에 관한 연구)

  • Current binocular stereoscopic displays cause visual discomfort when objects with large disparities are present in the scene. One solution for improving visual comfort is synthetic depth-of-field processing, which simulates the characteristics of a human visual system. With this technique, visual comfort is improved by blurring portions of the background and/or foreground in the scene. However, this technique has the drawback of degrading overall image quality because the blurring is typically applied to both left and right images. To alleviate the visual discomfort, we propose a novel disparity-based asymmetrical filtering technique. Proposed technique applies the filtering to the image of one eye only, and controls the blur level according to the disparity information between stereoscopic images. We investigate the effects of this technique on stereoscopic video by measuring visual comfort and apparent sharpness. Our results indicate that disparity-based asymmetrical filtering can improve visual comfort of stereoscopic video while it maintains apparent sharpness if unfixated regions with large disparities are blurred under the appropriate filtering condition.

  • As contemporary building construction type is getting higher and deeper, construction equipment usage is getting more, and related fatal accidents are on an increasing trend. Because of this, a method was drawn which could grasp the present state of construction equipment management and manage safety of the equipment more easily for accident prevention by choosing 2 kinds of Construction equipment which cause safety accident frequently among the equipment mainly used in construction site. This study suggested a method about construction equipment safety management using "smart phone" base which could be used in safety management for construction equipment by whomever in construction site. After attachment of QR code included safety checklist, It became possible that site managers could check more efficiently by scanning with their smart phone when they inspect equipment. Moreover, by the construction interested who didn't know what and how they have to inspect could point out unsafe condition in the early stage of equipment entering or take unsafe one out of the site by using new smart phone safety checking system is installed, it became possible that critical accident caused by construction equipment was prevented in advance.

  • Real-time video stabilization technology is used in correction for the camera vibrations of the hand-held camera by hand or fixed camera by external condition. This paper is about the counterplan to cope with the vibration of the movie generated by the large external cause relatively. we use the movie stabilization parameters with the phase correlation method based the DFT to get the displacements of the current frame to the reference frame. we use the kalman filter for the efficient and stable searching works on the phase correlation map and present the proper conditions for the real-time processing through the experiments. We propose the measure to evaluate the capability of the video stabilizer which is the standard deviation of the brightness of the center block. and compare the capability for the video sequences randomly shifted and the jittered video sequences obtained from camera.

Impacts of Managing Water in a Closed Basin: A Study of the Walker River Basin, Nevada, USA

  • Throughout much of the world, many ecological problems have arisen in watersheds where a significant portion of stream flows are diverted to support agriculture production. Within endorheic watersheds (watersheds whose terminus is a terminal lake) these problems are magnified due to the cumulative effect that reduced stream flows have on the condition of the lake at the stream's terminus. Within an endorheic watershed, any diversion of stream flows will cause an imbalance in the terminal lake's water balance, causing the lake to transition to a new equilibrium level that has a smaller volume and surface area. However, the total mass of Total Dissolved Solids within the lake will continue to grow; resulting in a significant increase in the lake's TDS concentration over time. The ecological consequences of increased TDS concentrations can be as limited as the intermittent disruption of productive fisheries, or as drastic as a complete collapse of a lake's ecosystem. A watershed where increasing TDS concentrations have reached critical levels is the Walker Lake watershed, located on the eastern slope of the central Sierra Nevada range in Nevada, USA. The watershed has an area of 10,400 sq. km, with average annual headwater flows and stream flow diversions of 376 million $m^3/yr$ and 370 million $m^3/yr$, respectively. These diversions have resulted in the volume of Walker Lake decreasing from 11.1 billion m3 in 1882 to less than 2.0 billion $m^3$ at the present time. The resulting rise in TDS concentration has been from 2,560 mg/l in 1882 to nearly 15,000 mg/l at the current time. Changes in water management practices over the last century, as well as climate change, have contributed to this problem in varying degrees. These changes include the construction of reservoirs in the 1920s, the pumpage of shallow groundwater for irrigation in the 1960s and the implementation of high efficiency agricultural practices in the 1980s. This paper will examine the impacts that each of these actions, along with changes in the region's climate, has had on stream flow in the Walker River, and ultimately the TDS concentration in Walker Lake.
