  • The purpose of this study was to construct database for a drug safety information website to serve as an access point of up-to-date resources for a wide variety of drug-safety information helping patients and healthcare professionals make well-informed decisions about medication use. All the contents developed were confirmed by the council of advisors who were the experts in drug safety. The detailed contents of database on frequently prescribed drug including 9 NSAIDs, 19 antibiotics, 24 cardiovascular, 21 metabolic, 14 respiratory, 20 digestive, 22 hormonal, 10 genitourinary, 10 anti-allergic, 27 antifungal/antiviral, and 71 neuropsychiatric agents were developed based on the approved drug labeling of the Korean FDA. A separately searchable database of drug-specific safety information for patients and health professionals was constructed for users in need of different depth of knowledge on using medications safely. The safety information on highly prevalent chronic diseases and drug classes was also developed. Finally the most recent global drug safety news was provided. The consumer directed information was developed in layman's terms as means of proving user-friendly information. The creation of this type of website is part of the Korean FDA's ongoing initiative to address and promote the safe use of medications for the public.

  • Background: Health information should be accurate and of high quality. While search websites are widely used to obtain health information, search results can vary depending on the availability of information in each language and the algorithms of search websites. We compared the search results and their reliability on popular search websites in Korea and the United States. Methods: Using the most popular search websites in Korea (A) and the US (B), with B divided into KR and US domains, we evaluated the search results for three diseases (hypertension, diabetes, osteoporosis) and two medications (Lipitor, Norvasc). We compared the proportion of paid ads and website categories, as well as quality and accuracy using a validated scoring tool. Results: Search website A had a high proportion of paid ads (40.3%), compared to B KR (4.3%) and B US (0%). B US had the highest proportion of reliable websites (58.1%), followed by B KR (52.9%) and A (14.6%). B US had the highest median quality score (25) while A had the lowest (14). Additionally, B US had the highest accuracy (95%), followed by B KR (87.7%) and A (52.2%). Conclusions: Our data suggest that health information provided by popular search website in Korea is inaccurate and of low quality compared with that provided by its counterpart in the US. Users should prioritize credible websites, be cautious of ads, and seek professional advice. In addition, joint efforts from the government, search websites, and healthcare professionals are needed to provide accurate online health information.

  • The elderly is a target group for health functional foods (HFFs). We surveyed 825 Korean elderly (71.4- years-of-age) for HFF use and significant variables for HFF use including demographic characteristics, health related variables, and nutritional beliefs. The prevalence of HFF use was 48.7%, and among all types of HFF, red ginseng products were eaten most frequently, followed by ginseng products, nutritional supplements, and mushroom products. The mean age of users of HFFs was lower than nonusers (p<.01). HFFs were eaten more frequently by subjects from families living with a spouse or unmarried children (p<.01) and those with higher levels of monthly pocket money (p<.001). HFFs were also eaten more frequently by subjects or their spouses with a higher education level (p<.001). The social activity participation ratio (p<.01) of subjects and the HFF usage ratio by the spouse (p<.001) were higher in users of HFF than in nonusers. Dietary assessment scores were not different between the two groups, and users had a more positive view concerning the potential health benefits of HFFs than did nonusers (p<.001). Most users of HFF ate HFFs "when they were healthy" (52.2%) and experienced some effect through HFF use (45.8%). Most users of HFFs received information on HFFs through their families or relatives (38.1%) and most purchased HFFs at the pharmacy or oriental medicine clinic (44.8%). HFF users preferred multivitamin- and Ca-supplements most among the vitamin and mineral supplements belonged to HFFs. The above results show that HFF use is widely spread among subjects, and that use of HFF is influenced by various factors. Thus, practical guidelines for HFF use should be provided for the elderly through elderly focused-nutrition education based upon significant variables and consumption behaviors for their HFF use of present study.

  • Various types of health functional foods (HFF) are popularly used among middle-aged. We surveyed 946 middle-aged in Korea for HFF use and significant variables for their HFF use including demographic characteristics, health related variables and nutritional beliefs. The use prevalence of HFF was 58.9%, and among all types of HFF, red ginseng products were taken most frequently, followed by nutritional supplements, ginseng products > mushroom products. HFF was taken more frequently in subjects from high monthly family income (p < .001), and in those from families with a high socioeconomic status (p < .001) in comparison to each corresponding group. Mean age of users was higher than nonusers (p < .001). Self concern about health (p < .001) and usage ratio of HFF by family (p < .001) were higher in users than in nonusers of HFF. Total dietary assessment score was not different in two groups, and users had a more positive view concerning the potential health benefit of HFF than nonusers (p < .001). Most users of HFF took HFF when they were healthy (52.8%) and experienced a little effect through the HFF use (51.2%). Most users of HFF obtained the information on HFF through family and relatives (30.5%) and most of them purchased HFF at pharmacy or oriental medicine clinics (44.5%). HFF users preferred multivitamin and Ca-supplements most among the vitamin mineral supplements belonged to HFF. Given the widespread use of HFF and inappropriate usage of HFF among subjects, the proper use of these products for the contribution to their optimal nutrition and overall health is a subject that should be formed through practical nutrition education considering the various variables affecting their HFF use.

  • Various types of functional foods for health (FFH) are popularly used among adolescents. We surveyed 858 students, $15.0 \pm$ 1.6 (14-18) years of age, attending general junior or senior high schools in Korea for FFH use and significant variables for their FFH use including demographic characteristics, food consumption frequency and nutritional beliefs. The use prevalence of FFH was $47.8\%$, and among all types of FFH, nutritional supplements were taken most frequently, followed by red ginseng products > enzyme containing products > plant extract-fermented products. FFH use was higher in older aged-group, subjects lived in small or middle city, and those from families with a high socioeconomic status in comparison to each corresponding group. Health concern on offsprings by parents and usage ratio of FFH by family was higher in users than in nonusers of FFH. Total score of dietary assessment was higher in users than in nonusers of FFH, and users had a more positive view concerning the potential health benefit of FFH than did nonusers. Most users of FFH took FFH when they were healthy ($50.7\%$) , and they did not feel special effects through the FFH use ($57.1\%$) . Most users of FFH got the information on FFH through family and relatives ($61.5\%$) , and most of them purchased FFH at pharmacy or oriental medicine clinics ($55.1\%$) . FFH users preferred vitamin C-and Ca-supplements most among the vitamin mineral supplements belonged to FFH. Given the widespread use of FFH by adolescents, the reasonable use of these products for the contribution to their overall health and well-being is a subject that is emphasized in nutrition education for them. • Background: Diabetes is associated with cancer risk in the aging population. Observational studies have indicated the beneficial effects of metformin against breast cancer, making studies on the anticancer potential of antidiabetic drugs worthwhile. This study investigated cancer incidence in patients on antidiabetic monotherapy. Methods: Using National Health Insurance Service data (2002-2013), a retrospective cohort study that included type 2 diabetes mellitus (T2DM) patients was conducted. Study subjects were enrolled if they were ${\geq}30$ years old, on monotherapy for diabetes, and cancer-free. They were followed up for cancer occurrence or death, until December 31st, 2013. A Cox proportional hazard model analysis was conducted between metformin and sulfonylurea (including meglitinide) users, to determine cancer risk, with adjustment for age, gender, comorbidity index, dyslipidemia, hypertension, and T2DM duration. Results: The number of antidiabetic monotherapy-treated T2DM patients without a history of cancer was 9,554 (metformin, n = 5,825; sulfonylurea, n = 3,225; others, n = 504). During the follow-up period (mean, 2.04; IQR, 3.18 years), the cancer incidence rate was 5.48/100 and 5.45/100 patient-years for metformin and sulfonylurea, respectively. The hazard ratio (HR) for risk of cancer incidence in the metformin group was 0.74 (95% confidence interval [CI], 0.66-0.83; p < 0.0001), compared with sulfonylurea. Additionally, the HRs for risks of lung, liver, and stomach cancer were respectively 0.46 (95% CI, 0.31-0.66; p < 0.0001), 0.41 (95% CI, 0.31-0.54; p < 0.0001), and 0.51 (95% CI, 0.35-0.73; p = 0.0003). Conclusion: Antidiabetic therapy with metformin reduces cancer risk by 26%, specifically for lung, liver, and stomach cancer.

  • Objective: Codeine may result in death or respiratory depression in children, particularly who are rapid metabolizer of CYP2D6, therefore it should be used cautiously among children under 12 years of age. This study was to investigate the prescribing pattern of codeine among children according to the age group, prescribed diagnosis, type of medical service and medical specialties. Method: We used Korea Health Insurance Review and Assessment Service-National Patient Sample (HIRA-NPS) database. Study subjects included inpatients or outpatients, who were prescribed codeine between January, 1, 2011 and December, 31, 2011. Contraindicated use of codeine was defined as the use of codeine at least one times under aged 12. Age groups were subclassified according to the <2 years, 2-4 years, 5-8 years, and 9-11 years. Frequently prescribed diagnosis (ICD-10), type of medical service, and medical specialties were also described among codeine users under aged 12. Results: Codeine users were 6,411 inpatients (9,958 prescriptions), and 3,397 outpatients (6,258 prescriptions), respectively. Codeine prescription under 12 years of age were 2.1% (210 prescriptions) among inpatients, and 12.3% (776 prescriptions) among outpatients (p-value<0.05). Outpatient prescriptions of codeine under 12 aged were issued mostly from primary care clinics and frequent diagnosis were unspecified bronchopneumonia (51.6%), and vasomotor rhinitis (23.7%). Conclusion: This study found prescribing of codeine under 12 aged is common in outpatient and primary clinics. Nationwide and community-based efforts should be needed to reduce inappropriate prescribing among children.

  • Objectives: The purpose of this study was to investigate the association between fracture risk and proton pump inhibitor (PPI) use to establish evidence for defining high-risk groups of fracture among PPI users. Methods: A case-control study was performed using the National Health Insurance Sample Cohort Database from January 2002 to December 2013. The cases included all incidences of major fractures identified from January 2011 to December 2013, and up to four controls were matched to each case by age, gender, osteoporosis, and Charlson comorbidity index. Conditional logistic regression was used to calculate the adjusted odds ratio (aOR) and associated 95% confidence interval (CI). Results: Overall, 14,295 cases were identified, and 63,435 controls were matched to the cases. The aOR of fractures related to the use of PPIs was 1.06 (95% CI: 1.01-1.11). There was a statistically significant association between fracture and PPI use within 3 months of the last dose, and a trend of increasing fracture risk with increasing cumulative PPI dose. The risk of fracture was significantly higher in patients who took PPIs for more than 1 year during the 2-year observation period. Conclusion: Patients who have been using PPIs for more than 1 year should be warned about the risk of fracture during or at least 3 months after discontinuing the PPI.
