Hyperprogressive disease (HPD) is a distinct pattern of progression characterized by acceleration of tumor growth after treatment with anti-PD-1/PD-L1 Abs. However, the immunological characteristics have not been fully elucidated in patients with HPD. We prospectively recruited patients with metastatic non-small cell lung cancer treated with anti-PD-1/PD-L1 Abs between April 2015 and April 2018, and collected peripheral blood before treatment and 7-days post-treatment. HPD was defined as ≥2-fold increase in both tumor growth kinetics and tumor growth rate between pre-treatment and post-treatment. Peripheral blood mononuclear cells were analyzed by multi-color flow cytometry to phenotype the immune cells. Of 115 patients, 19 (16.5%) developed HPD, 52 experienced durable clinical benefit (DCB; partial response or stable disease ≥6 months), and 44 experienced non-hyperprogressive progression (NHPD). Patients with HPD had significantly lower progression-free survival (p<0.001) and overall survival (p<0.001). When peripheral blood immune cells were examined, the pre-treatment frequency of CD39+ cells among CD8+ T cells was significantly higher in patients with HPD compared to those with NHPD, although it showed borderline significance to predict HPD. Other parameters regarding regulatory T cells or myeloid derived suppressor cells did not significantly differ among patient groups. Our findings suggest high pre-treatment frequency of CD39+CD8+ T cells might be a characteristic of HPD. Further investigations in a larger cohort are needed to confirm our results and better delineate the immune landscape of HPD.

The role of peripheral vestibular receptors in acute hypotension was investigated in anesthetized rats. Acute hypotension was induced by either intravenous infusion of sodium nitroprusside (SNP) or by experimental hemorrhage, and electrical activity and expression of cFos-like immunoreactive (cFL) protein were measured in the medial vestibular nuclei (MVN). Blood pressure decreased proportionately to the does of intravenous SNP and to the volume of the hemorrhage. Blood pressure decreased 10, 30, 50% for the 5, 10, $15{\mu}g/kg$ SNP injection, respectively, and also decreased 30 and 50% after 1- and 2-ml blood loss, respectively, due to hemorrhage. In animals with intact labyrinths, acute hypotension induced by either intravenous infusion of SNP or hemorrhage produced different electrical activities with three different patterns in type I and II neurons of MVN. The responses of type I neurons showed excitatory in 2/3 of recorded neurons and inhibitory or no change in 1/3 of neurons, while the responses of type II neurons showed inhibitory in 2/3 of recorded neurons and excitatory or no change in 1/3 of neurons. In unilateral labyrinthectomized animals, 2/3 of type I neurons ipsilateral to the lesion showed an inhibitory response, and 2/3 of contralateral type I neurons showed an excitatory response after the induction of acute hypotension. The response patterns of type II neurons were opposite from those of the type I neurons. After 30% decrease in blood pressure, cFL protein expressed in the bilateral vestibular nuclei of control animals with intact labyrinths. Expression of cFL protein increased significantly proportionately to the reduction of blood pressure. The unilateral labyrinthectomized animals with acute hypotension produced expression of cFL neurons in contralateral vestibular nuclei to the lesion side, but not in ipsilateral vestibular nuclei. However, cFL protein was not expressed in bilateral vestibular nuclei after acute hypotension in bilateral labyrinthectomized animals. These results suggest that the peripheral vestibular receptors might play a significant role in controlling blood pressure following acute hypotension via activation of type I neurons and inhibition of type II neurons in the vestibular nuclei.

The purpose of this study was to evaluate the thermoregulatory responses to postures under different environmental conditions and to obtain the basal information for standard clothing weight, indoor climates, and working condition. Two adult female (22.5yrs, 46kg) were participated in this study. The experimental conditions were divided into three groups ; 1) comfort($27{\pm}1^{\circ}C$, $60{\pm}10%$), 2) hot($34{\pm}1^{\circ}C$, $60{\pm}10%$), and 3) cold($21{\pm}1^{\circ}C$, $50{\pm}10%$) condition. The postures performed were as follows; standing, sitting on the chair, sitting on the floor, and supine on the floor. At each condition, subjective sensations, 12 points skin temperature, rectal temperature, total and local sweat rate, pulse rates, blood pressure, skin blood flow rate were measured. The results were as follows : 1. Rectal temperature was high significant among groups in order of supine, sitting on the floor, sitting on the chair, standing posture(p<0.01). 2. Skin temperature was high in part of contact with the surface of the floor or wall and the effect of posture was greater in peripheral temperature than torso temperature. Sitting on the chair and sitting on the floor posture showed higher peripheral temperature than standing and supine posture. And peripheral temperature was lower in supine posture than any other postures. 3. Total and local sweat rate were decreased in order of standing, sitting on the chair, sitting on the floor, supine posture. 4. Pulse rate and disastolic blood pressure were higher in standing posture than supine posture, and there was significant difference between two postures(p<0.001). 5. Blood flow rate of thigh was high in sitting on the chair and sitting on the floor posture and low in standing posture. Blood flow rate of leg was low in standing posture significantly(p<0.01). 6. In comfort and hot condition, temperature sensation and comfort sensation were higher in standing posture and lower in supine posture than any other postures. In cold condition, temperature sensation was lower and comfort sensation was higher in standing and supine posture than any other postures. And supine posture was appeared positive in hot condition and negative in cold condition. From this study, we confirmed the effects of posture on human thermoregulatory responses. Results indicate that even under same conditions and clothing weight, the insulation of clothing will be different to postures.
Hemorrhage shock occupies high rate in trauma patient's mortality and blood pressure is the variance that judges early diagnosis and the effect of remedy. Systolic blood pressure is related to pulse transit time(PTT). PTT means the time that is required to flow from the heart to peripheral artery. PTT is influenced from the length, cross section and stiffness of the blood vessels. It is hard to evaluate the correlation between systolic blood pressure and PTT because they are variable in human body. In this paper, we evaluated the correlation between the systolic blood pressure and PTT in normal and hemorrhage states using standardized rat. PTT is defined as the time differences between the R peak and the peak of pulse wave. The analyzed time differences of ECG and blood pressure are analyzed every 5minutes for 30 seconds when there is before and after bleeding. Before bleeding, systolic blood pressure and PTT are steadily preserved but when the bleeding comes started, systolic blood pressure is declined. However PTT was increased and decreased. Under the circumstance that the standardized rat is controlled by age, the length of the blood vessels, and any disease, it shows that PTT measurement using systolic blood pressure of bleeding is impossible.
Yoga is an ancient Indian system of life, encompassing various practices including practices for self-discipline and also for regulating the health states of the individual, being practiced for thousands of years. The present study aims at understanding the effect of two high frequency breathing practices over autonomic nervous system. Forty healthy male volunteers of age $21{\pm}2$ years with $9{\pm}3$ months of Yoga practice experience were recruited. The two high frequency Yoga breathing practices, kapalabhati (KB) and bhastrika (BH) were given as interventions randomly on either of the two days to minimise laboratory bias. They were assessed before and immediately after the interventions for heart rate, respiratory rate, heart rate variability (HRV), blood pressure and peripheral oxygen saturation. There was a significant increase in heart rate (p<0.01; p<0.001), systolic blood pressure (p<0.01; p<0.001), NN50 (p<0.01; p<0.001) component of HRV for both KB and BH groups respectively. There was a significant reduction in respiratory rate in both the groups (p<0.001, and p<0.05, BH and KB respectively) immediately following intervention. A significant increase in LF component of HRV and reduction in Diastolic blood pressure and high frequency (HF)component following KB was also observed (p<0.05, for all comparisons). The Mean peripheral oxygen saturation remained unaltered in both the groups (p>0.05).The results suggest that high frequency yoga breathing practices induce physiological arousal immediately as evidenced by increased blood pressure and heart rate. The sympathetic arousal was more following KB session as evidenced by an increased diastolic blood pressure, LF power and a decrease in HF power of HRV as compared to the BH session.

Background: Cushing syndrome is characterized by glucose intolerance, cardiovascular disease, and an enhanced systemic inflammatory response caused by chronic exposure to excess cortisol. Eosinopenia is frequently observed in patients with adrenal Cushing syndrome, but the relationship between the eosinophil count in peripheral blood and indicators of glucose level in patients with adrenal Cushing syndrome has not been determined. Methods: A retrospective study was undertaken of the clinical and laboratory findings of 40 patients diagnosed with adrenal Cushing syndrome at Chungnam National University Hospital from January 2006 to December 2016. Clinical characteristics, complete blood cell counts with white blood cell differential, measures of their endocrine function, description of imaging studies, and pathologic findings were obtained from their medical records. Results: Eosinophil composition and count were restored by surgical treatment of all of the patients with adrenal Cushing disease. The eosinophil count was inversely correlated with serum and urine cortisol, glycated hemoglobin, and inflammatory markers in the patients with adrenal Cushing syndrome. Conclusion: Smaller eosinophil populations in patients with adrenal Cushing syndrome tend to be correlated with higher levels of blood sugar and glycated hemoglobin. This study suggests that peripheral blood eosinophil composition or count may be associated with serum glucose levels in patients with adrenal Cushing syndrome.
Hemangioma is the most common benign tumor of the liver, with a prevalence estimated as high as 7%. Tc-99m red blood cell (RBC) hepatic blood pool scan with single photon omission computed tomography (SPECT) imaging is extremely useful for the confirmation or exclusion of hepatic hemangiomas. The classic finding of absent or decreased perfusion and increased blood pooling ("perfusion/blood pool mismatch") is the key diagnostic element in the diagnosis of hemangiomas. The combination of early arterial flow and delayed blood pooling ("perfusion/blood pool match") is shown uncommonly. In giant hemangioma, filling with radioactivity appears first in the periphery, with progressive central fill-in on sequential RBC blood pool scan. However, the reverse filling pattern, which begins first in the center with progressive peripheral filling, is also rarely seen. Studies with false-positive blood pooling have been reported infrequently in nonhemangiomas, including hemangiosarcoma, hepatocellular carcinoma, hepatic adenoma, and metastatic carcinomas (adenocarcinoma of the colon, small cell carcinoma of the lung, neruroendocrine carcinoma). False-negative results have been also reported rarely except for small hemagniomas that are below the limits of spatial resolution of gamma camera.
