의사의 환자중심 커뮤니케이션이 환자참여에 미치는 영향 : 신뢰의 조절효과를 중심으로 (Effect of Patient-centered Communication of Doctor on Patient Participation : Focusing on Moderating Effect of Trust)

  • 본 연구는 의사의 환자중심 커뮤니케이션(촉진요인, 금지요인)이 환자 참여(행동적 참여, 감정적 참여, 정보적 참여)에 미치는 영향을 분석하기 위한 것이다. 구체적인 연구목적으로는 첫째, 환자중심 커뮤니케이션이 환자참여에 미치는 영향, 둘째, 환자중심의 커뮤니케이션과 환자참여간의 관계에서 신뢰의 조절효과 확인이며, 이를 위하여 국내 종합병원의 환자 301명을 대상으로 SPSS 19.0을 통계패키지를 사용하여 요인분석과 조절회귀분석을 수행하였다. 연구결과, 커뮤니케이션 촉진요인은 행동적 참여, 감정적 참여, 정보적 참여 모두에 유의한 정(+)의 영향을 미치는 것으로 확인되었으나, 금지요인은 영향을 미치지 않는 것으로 나타났다. 또한 신뢰의 조절효과는 촉진요인과 감정적 참여간, 촉진요인과 정보적 참여간에서 발견되었으며, 신뢰의 수준이 높은 집단에서 촉진요인은 감정적 참여와 정보적 참여에 더 큰 영향을 미치는 것으로 확인되었다. 마지막으로 이러한 연구결과를 토대로 환자중심의 커뮤니케이션과 성공적인 의료서비스 제공에 필요한 이론적이고 실무적인 시사점을 제공하였다.

진료환경이 환자참여에 미치는 영향: 부정적 감정의 조절효과를 중심으로 (Effect of Treatment Setting of the Medical Services on the Patient Participation : Focusing on Moderating Effect of Negative Emotion)

  • 본 연구는 의료서비스 제공 과정에서의 진료환경이 환자참여에 미치는 영향과 이들 간의 관계에서 부정적 감정의 조절효과를 분석하고자 하였다. 이러한 연구목적 달성을 위하여 국내 C 지역의 병의원 환자 중 진료를 마친 320명을 대상으로 수집한 자료를 SPSS 19.0을 이용하여 단계적 회귀분석을 수행하였다. 실증분석 결과는 다음과 같다. 진료환경의 청결성은 행동적 참여에, 환자의 연령이 높을수록 감정적 참여에, 진료환경의 제3자의 존재는 감정과 정보적 참여에 긍정적 영향을 미치는 것으로 확인하였다. 환자의 부정적 감정은 감정적 참여와 정보적 참여에 부정적 영향을 미쳤고, 진료환경의 시간압박은 환자의 정보적 참여수준에 부정적 영향을 미쳤다. 둘째, 조절효과로는 진료환경의 청결성과 감정적 참여 간에 부정적 감정은 긍정적인 영향을 미쳤다. 마지막으로 이러한 연구결과를 중심으로 의료서비스의 진료환경과 부정적 감정을 고려하여 환자참여를 향상시키기 위한 방법 등 이론적, 실무적 시사점을 제시하였다.

간호사의 전문직관, 정치참여가 대상자 옹호에 미치는 영향 (Influence of Nursing Professional Value and Political Participation of Nurses on Patient Advocacy)

  • Purpose: This study aimed to identify the level of patient advocacy and to determine the influence of nursing professional value and political participation of nurses on patient advocacy theory. Methods: This study used a cross-sectional descriptive study design. Data were collected from 232 nurses from a tertiary hospital, three general hospitals, and a public health center in two districts of a metropolitan using self-reporting questionnaires. Data were analyzed by t-test, One-way ANOVA, Pearson's correlation coefficient, and multiple linear regression using SPSS/ WIN 21.0. Results: The mean score of patient advocacy was 4.64±0.55 out of 6, nursing professional value was 3.60±0.49 out of 5, and political participation was 2.21±0.71 out of 5. Nursing professional value (r=.37, p<.001) and political participation (r=.23, p<.001) showed a significant positive relationship with patient advocacy. Factors influencing patient advocacy were identified as nursing professional value (β=.28, p<.001) and political participation (β=.15, p=.014). Conclusion: Based on the findings that nursing professional value and political participation are key factors of patient advocacy, educational strategies and endeavors as level of professional organization are recommended for enhancing patient advocacy.

  • 본 연구의 주된 목적은 의사의 환자중심 커뮤니케이션(경청요인, 배려요인, 금지요인)이 환자만족과 치료성과에 미치는 영향, 그리고 의사의 환자중심 커뮤니케이션과 환자만족, 치료성과 간 환자참여(행동적 참여, 감정적 참여, 정보적 참여)의 매개효과 여부를 검증하는데 있다. 이러한 연구목적 달성을 위해 C지역의 종합병원에서 진료를 한 339명의 외래환자를 중심으로 실증분석을 수행하였다. 연구결과 첫째, 경청요인과 배려요인은 환자만족과 치료성과에 유의한 정(+)의 영향을 보였으나 금지요인은 그렇지 않았다. 둘째, 경청요인과 배려요인은 감정적 요인과 정보적 요인에 유의한 정(+)의 관계를 보였으나, 금지요인은 환자참여의 3개 요인에 유의한 영향을 보이지 않았다. 셋째, 의사의 환자중심 커뮤니케이션과 환자만족, 치료성과 간의 관계에서 환자참여의 부분 매개효과가 존재하는 것을 확인하였다. 마지막으로 이러한 연구결과를 중심으로 환자만족과 치료성과를 높이기 위한 방안들을 제시하였다.

Does Community Participation Work? : A Study on the Impact of Health Care Reform in Chile

  • Community participation in health has been praised as a new way of improving health inequality in developing countries for many decades. This paper is an attempt to evaluate community participation programs in health focusing on two intercultural health hospitals in IX Region of Chile. After exploring the process of program building and its impact on the quality of service, this paper concludes that a community participation program with stronger participation resulted in higher patient satisfaction. The author expects such finding to contribute to more comprehensive understanding of the impact of participation in health programs.

환자안전에 대한 환자의 이해: 초점집단토의 결과를 중심으로 (Patient Understanding of Patient Safety: Based on Results from Focus Group Discussion)

  • Purpose: To examine the degree of recognition regarding the concept of patient safety, as perceived by the patient, using a focus group discussion. Methods: A focus group discussion was conducted with a patient group comprising seven patients. Results: When the participants heard the term "patient safety" they seemed to understand it to be related to the hospital environment or satisfaction with the overall hospitalization experience. The participants emphasized communication between the medical staff and the patients in relation to the explanation of treatments, as well as the provision of information regarding prevention, experience, and the treatment of incidents with patient safety. They agreed on the need for indicators reported by patients. However, they emphasized that additional items and a questionnaire method that considers the patients' point of view are needed. Conclusion: It is necessary to establish and implement various strategies that can raise the awareness of patient safety using patient safety indicators and increase participation in patient safety activities.

신규간호사의 직무분석을 통한 간호학생의 실무수행 능력수준 결정 (Determination the nursing student's clinical competency based on new nurses' job analysis)

  • The purpose of this study was to examine frequency of job performance, level of urgency, index of importance experienced by 138 new nurses who were working at 6 hospitals that had over 400 bed in Incheon and Bucheon. The data were analyzed using a SPSS program for descriptive statistics [numbers of job performance, level of urgency, index of importance(job performance+1/4 level of urgency)]. Some of the most frequent job performance were application of aseptic technique, application of principle of infection control, and medication (IV, PO, IM, supply fluid and electrolyte). Some of the most uncommon job performance were activity as a lecturer in nursing department, activity of arrangement of community resources, and participation in multidisciplinary conference. Some of the highest level of urgency were use of aseptic technique, CPR, application of principle of infection control, defibrillation for dysrhythmia, blood transfusion, observation of patient's status in procedure (operation), v/s check, intervention for improvement of respiratory function and medication of antihypertensives. Some of the lowest level of urgency were activity as a lecturer in nursing department, preceptor's activity, supervisor and delegation of job to nurse aids, attendance at nursing conference, activity of arrangement of community resources, participation in clinical teaching practice, participation in multidisciplinary conference, and delegation of patient care. Those were indirect patient care. The order was physiologic integration(60.99%), safety and effective nursing environment, psychosocial care, and health maintenance and improvement. The most importance item in maintenance of physiologic integration was medication. Some of the highest index of importance were aseptic technique and application of principle of infection control, v/s check, observation of patient's status in operation and medication. Some of the lowest index of importance were activity as a lecturer in nursing department, preceptor's activity, activity of arrangement of community resources, participation in multidisciplinary conference, nursing conference, participation in clinical teaching, and delegation of job to nurse aids.

응급실 환자의 간호 요구도 및 만족도 분석 (A study of Nursing care need and the patient's satisfaction level of the nursing care in the emergency department)

  • Purpose : The purpose of this study was to analyze the nursing care needs which can properly cope with patients' needs in emergency room, and grasp the patients' satisfaction level which can assess the care quality. It was aimed to check the level of today's care and provide basic data for care performance and care standard. Method : Data were collected from 192 patients in general hospitals in Daegu. The results are as followed. Results : First, the result of analysis conducted on the degree of overall patients' need for nursing care revealed that the need for nursing information (4.21+57) was the highest, followed by the order of information of patient's condition (4.05+53), emotional support (3.98+47), and nursing participation (3.65+54). Secondly, our study revealed that there was a significant negative correlat interrelation between care need and care satisfaction (r=.516, p<.01), indicating the higher the overall emergency room patients' nursing care need, treatment and nursing information, patient's condition, emotional support and nursing participation were, the lower the actual feeling of their satisfaction turned out. Conclusion : The study shows that treatment and nursing related information is the most highly needed and the patients' satisfaction level relies on actual care participation.

의사와 간호사의 환자안전교육 경험에 관한 포커스 그룹 연구 (Focus Group Study on Health Care Professionals' Experience of Patient Safety Education)

  • Purpose:This study aims to understand and explore the subjective experiences of patient safety education among health care professionals in developing a patient safety curriculum in South Korea. Methods: A qualitative descriptive study was conducted through two focus group interviews in the period October-December 2018. Eleven participants who underwent patient safety education participated in each session. All interviews were recorded and transcribed as spoken, and qualitative content analysis was used to identify categories of discussion depicting participants' subjective experience with patient safety education. Results: A total of three categories and seven themes were identified out of 77 units of analysis. Topics were identified in the dimensions of a patient safety curriculum, as follows: (1) activities for patient safety; (2) principle of patient safety (five rights, ethics, patient participation) and patient participation; (3) leadership, teamwork, and communication; and (4) reporting and learning system for patient safety events. In the dimension of methods, (5) case and evidence-based education and (6) multidisciplinary and small group teaching were identified. Finally, in the dimension of the system, (7) policies for patient safety education were identified. Conclusion: Our findings indicate that patient safety education is a significant area for health care professionals. Health care professionals suggested that a systematic patient safety curriculum would improve their knowledge and attitude toward patient safety. Moreover, it enables them to better construct a safety environment in a hospital.

간호대학생의 임상실습 시 환자안전관리 실천에 미치는 영향요인 (Factors Affecting Nursing Students' Practice of Patient Safety Management in Clinical Practicum)

  • Purpose: This study was done to assess nursing students' practice of patient safety management (PSM), identify factors affecting PSM and provide basic data to develop education programs to strengthen students' competencies for patient safety. Methods: In this descriptive research the practice of PSM by nursing students was examined and predictive factors were identified. Participants were junior and senior nursing students from 7 universities in 7 cities. Self-report questionnaires were used for data collection. Results: Significant positive correlations were found between knowledge of PSM, perception of the importance of PSM and practice of PSM. In multivariate analysis, women students, participation in patient safety education in school, knowledge of PSM, and practice of PSM predicted high perception of the importance of PSM. In multivariate analysis, senior year and participation in patient safety education in school predicted higher knowledge of PSM. In multivariate analysis, perception of the importance of PSM predicted high practice of PSM. Conclusion: In this study, knowledge was not found to directly affect PSM practice, but was found to affect the perception of the importance of PSM, a significant predictive variable. Thus, the importance of PSM should be strongly emphasized during education.
