Development of the Home-Based Prenatal Care System via Information Superhighway (초고속 통신망을 이용한 재택산전간호관리 시스템 개발)

  • Due to the rapid socioeconomic development and the introduction of the national health insurance system the general population's need for health care and utilization of health care services have increased dramatically. As a result of this change. Korea is experiencing a shortage of health care facilities and health manpower, and this leads long wailing line at doctor's offices. One of the solutions of this problem could be home health care system for those who have minor health related problems. With this background, this study was conducted to look at the feasibility of a home-based prenatal care system using information superhighway and nursing informatics specialists. With the home-based prenatal care system, the pregnant woman checks her blood pressure, tests her urine for sugar and protein, and measures her body weight at home and sends the information to a hospital computer via the information networks such as public telephone line and information superhighway. Nursing informatics specialist at the hospital will go through each patient record and screen those who have abnormal values and notify them to see a doctor as soon as possible. Besides telemonitoring features, the proposed system will include tole-education capabilities for the patients so that patient can learn whatever they need to know ragarding the prenatal care via information networks. If this system develops and operates, patient can save time in terms of travel to and from the hospital and waiting time in the hospital. And the health care institute can utilize its resource more efficiently.

A Context-Aware Treatment Guidance System (상황인지를 이용한 진료 안내 시스템)

  • As the quality of the medical treatment service provided by large hospitals grow, the number of patients utilizing the facilities is increasing dramatically. Various studies such as order communication system and treatment guidance system are under their process in order to shorten the waiting time for patients. However, the existing methods assign the treatments in successive order without recognizing the situation of each treatment, therefore increasing a patient's standby time at a hospital. This paper proposes a context-aware treatment guidance system, which recognizes the previously undermined estimated waiting time of each treatment for a patient and recommends a treatment with shorter estimated sojourn time first. This context-aware treatment guidance system provides detailed information of treatment services based on the recommended order of treatments to a patient's smartphone. By utilizing the context-aware treatment guidance system introduced in this paper, patients can reduce their standby time at hospitals to the minimum while hospitals can efficiently service more patients at the same amount of time. The proposed context-aware treatment guidance system proves to be outstanding in treatment order recommendation through comparisons to previously used methods.

Dental Treatment under Midazolam and $N_2O-O_2$ Sedation for Disabled Patients before General Anesthesia (장애 환자의 Midazolam 근육주사와 $N_2O-O_2$ 흡입진정을 통한 전신마취 수술 전 치과치료)

  • General anesthesia has been developed in dentistry as a type of management technique to sedate patients who may be uncontrollable or require medical consideration. However, emergency treatment for controlling pain before general anesthesia could be required due to the time for medical evaluation for general anesthesia. There is a greater possibility of developing complications under extended waiting period. A disabled patient who needs dental treatment under general anesthesia underwent pretreatment with intramuscular injection of midazolam and $N_2O-O_2$ inhalation sedation in these cases report.

Research of operators and patients exposed to electromagnetic field in the hospital (병원에서의 환자, 의료진의 전자파 노출 실태 조사)

  • In this study, electromagnetic fields emitted from the various environment in the hospital were measured. Measurement spot was patients' head. To monitor how much magnetic fields are emitting from operation room, monitoring device was attached to 19 anesthesiologist and monitoring lasted 8 hours. We also took a measurement from various medical devices. Devices include ESWL, PET, MRI, CT, Gamma knife, X-ray, Angiogram, Echocardiogram, Upper GI and Linear Accelerator. Electromagnetic fields were measured from 10 spots from each of 5 patient waiting room. As a results, there were no places showing risk of high exposure. All the measurement values were below the reference levels for general public exposure to time varying electric and magnetic fields which is issued by ICNIRP.

  • The objective of this study was to find out the level of patient expectation and satisfaction on their services. The questionaire reports of 781 patients, which were structured with the question about the level expectation and satisfaction of the services. Major findings of this study were as follows 1) The percentage of patients satisfied with the services as a whole is 59.1 %,where more patients are satisfied with the attitude of therapists (87.3 %), the therapeutic-skill of therapists(84.1 %) and the adequateness of treatment (63.9 %) than the expenses of services (42.0 %) and waiting time(47.4 %). 2) Fourty two percents of patients responded that they expected to be recovered completely.

  • During hospital peak times of outpatients, an effective mechanism that informs the newly receipted patient information to corresponding medical departments is lacking in current hospital reception systems. Since every department repeatedly searches entire patient reception database in sequential manner to acquire its reception information, this is a significant performance degradation factor in hospital information system. To improve hospital reception system efficiency, we developed two web socket based systems, a primary key transmitting batch system and a reception information transmitting real time system. The former reduced database access time compared to sequential search system as well as kept search time low regardless of receipted patient number. The latter effectively reduced waiting list updating time in request/response patient reception system by eliminating database access.

The development of Scale of nursing malpractice in operating room (수술실 간호과오 측정도구 개발)

  • Purpose : The purpose of this study was to develop the Operating Room Nursing Malpractice Scale and to test reliability and validity of the instrument and to describe frequency of malpractice. Method : The subjects used to verify the Scale's reliability and validity were 179 nurses who working at 3 university hospital and 6 general hospital in Busan and Kyoung nam province from september 1 to October 10, 2002. The data was analyzed by the SPSS/ WIN 10.0 program. Result : The factor analysis classified a total 5 factors statistically, it's communality was 44.18%. Item content are as follows. The factor were 'malpractice in the patient's preparation', 'malpractice in nursing technique', 'malpractice in the management of the environment', 'malpractice of the patient's security', 'malpractice in the supervision of the patient'. The most frequent malpractice was 'The patient complained of waiting for a long time to go back from the operating room because of inadequate communication with another department'. Conclusion : The scale of operating room nursing malpractice was to prevent the severe nursing accident and to provide the basis of needs of educational program.

  • Low back Pain(LBP) is a common problem, and the resulting disability frequently contains nonorganic, psycho-logical and social elements that are difficult for the physical therapists to manage. The physical therapist-patient relationship is the most important factors in the management of low back pain as chronic disease. The purpose of this study was to evaluate and to identify factors that had influenced low, back pain patients satisfaction with physical therapist The subjects of this study were 223 out-patients(116 males and 107 females) who had been visited to physical therapy room of medical institutions in Seoul and Uijgngbu city. They were examined by the questionaire of Dimatteo and Hays which was amended to serve the purpose of this study by author. The collected data was analyzed by ANOVA according to the purpose of this study. There was no difference with statistic value in LBP patient's satisfaction according to occupation, sex, schooling, religion, marital status, medical security, duration of disease, the tine required, recurrence, but was a difference according to age, income, period of treatment, cost, diagnosis, waiting time, the number of physical therpy's sort, sex ane age of physical therapists (p<.05). LBP patients was satisfied with physical therapist's communicative behavior, but was dissatisfied with physical therapist's competence.

Patients Satisfaction with Nursing Care in the Emergency Department (응급실 이용현황과 환자의 간호만족도 조사연구)

  • The purpose of this study was to describe patients satisfaction with nursing care in the emergency department and factors influencing satisfaction rates. 1. The mean score of patient satisfaction with nursing care in the emergency departments was 2.70. The mean scores of patient satisfaction with different aspects of nursing care services were 3.05 for nursing services for psychological safety: 2.67 for technical competence: 2.49 for information giving: and 1.35 for discharge teaching. 2. Patient satisfaction with nursing care services provided. according to the general characteristics of the patients. was revealed as being significantly high for the groups of patients made up of those who were male. those over 51years of age. married subjects. those with an education level of high school or below level. housewives. Buddhists. and those in the middle economic level. Patient satisfaction with nursing care according to their service utilization in the emergency departments revealed that satisfaction was significantly high for patients who were in internal medicine. neurology or psychiatry. those admitted for the first time. those for whom the distance from the department to home was less than 30 minutes. patients who visited between monday and Friday and between 8AM and 4PM. patients who did not have to wait to be seen and those who stayed in the department less than 24 hours. 3. The correlation between the patients' general characteristics. nursing service utilization of the patients in the department and their satisfaction with nursing care showed that patients with longer waiting time had lower satisfactory scores for nursing service. 4. The registered nurses working in the emergency department reported that the main reasons for low satisfaction rates included 'over worked nurses' and 'too many patients' in the emergency department. In conclusion. the results of this study suggest the necessity of developing relevant nursing interventions for discharge teaching to increase patient satisfaction with nursing care services in emergency departments: and the need for adequate support from hospital administrators to improve patients' service utilization.

Recipient Management before Lung Transplantation

  • Lung transplantation is considered a viable treatment option for patients with end-stage lung disease. Recent decades have seen a gradual increase in the number of lung transplantation patients worldwide, and in South Korea, the case number has increased at least 3-fold during the last decade. Furthermore, the waiting list time is becoming longer, and more elderly patients (>65 years) are undergoing lung transplantation; that is, the patients placed on the waiting list are older and sicker than in the past. Hence, proper management during the pre-transplantation period, as well as careful selection of candidates, is a key factor for transplant success and patient survival. Although referring and transplant centers should address many issues, the main areas of focus should be the timing of referral, nutrition, pulmonary rehabilitation, critical care (including mechanical ventilation and extracorporeal membrane oxygenation), psychological support, and the management of preexisting comorbid conditions (coronary artery disease, diabetes mellitus, gastroesophageal reflux disease, osteoporosis, malignancy, viral infections, and chronic infections). In this context, the present article reviews and summarizes the pre-transplantation management strategies for adult patients listed for lung transplantation.
