Review of Clinical Studies for Herbal Medicine Treatment on Childhood Obesity - Focusing on Studies from the China Academic Journal (CAJ) - (중의학 데이터베이스 (CAJ)를 이용한 소아 비만의 한약치료에 대한 임상 연구 동향)

  • Objectives The purpose of this study was to analyze recent traditional Chinese medicine clinical studies for treatment of childhood obesity. Methods We analyzed 11 clinical studies from January, 2000 to March, 2018 about childhood obesity from the China Academic Journal (CAJ) and China National Knowledge Infrastructure (CNKI). All selected studies were Randomized Controlled Trials and analyzed in order of publication year, demographic information, treatment method, evaluation methods, results, adverse reactions and relapse after treatment discontinuation. Results The main Traditional Chinese Medicine (TCM) treatment was the herbal decoctions, granules and pills. In most studies, the total efficacy of the treatment group was reported to be higher than that of the control group. Adverse reactions were found in small number of studies. Frequently used medical herbs were Crataegi Fructus (山?), Atractylodis Rhizoma (蒼朮), Citri Pericarpium (陳皮), Poria (茯?), and Nelumbinis Folium (荷葉). Also, Water-draining and swelling-dispersing medicinal (利水退腫藥), Qi-regulating medicinal (理氣藥), Resolving dampness with aroma medicinal (芳香化濕藥), Blood-activating and stasis-dispelling medicinal (活血祛瘀藥), Qi-tonifying medicinal (補氣藥), and Digestant medicinal (消食藥) were commonly used. Conclusions This study showed that traditional Chinese medicine, especially herbal medicine treatment can be effective and safe option for treating childhood obesity. These research results can be utilized in other clinical studies as well as in treatment of childhood obesity. Additional well-designed randomized controlled trials and systematic reviews are need to confirm the conclusion.

Two Cases of Pseudo-Gynecomastia Treated with Wild Ginseng Pharmacopuncture (가성여성형유방에 산삼약침 시술 2예)

  • Wild ginseng pharmacopuncture was administered to 2 patients with pseudo-gynecomastia. Each patient received the treatment once a week, total 5 times. Outcome was measured by the comparison of photos which were taken before and after the treatment. One case showed significant difference in the breast shape and size after the treatment and the patient was satisfied with the result. However, the other case did not show noticeable change but subtle one, and satisfaction degree of the patient was also low. From the above cases, to figure out the effect of wild ginseng pharmacopuncture on pseudo-gynecomastia, further studies and reports on other affecting factors should be followed.

A Case Report of Patient with Acute Alcoholic Hepatitis and Renal Dysfunction (신기능 장애를 동반한 급성 알코올성 간염 환자 치험 1예)

  • The purpose of this study is to report the clinical effectiveness of Korean medicine, especially Galhwahajung-tang and Yijintang-gamibang on patient with acute alcoholic hepatitis and renal dysfunction. The patient in this case had suffered from pantalgia and dizziness with nausea. He was diagnosed as acute alcoholic hepatitis. Based on related symptoms and blood-test, we could diagnose acute alcoholic hepatitis. We treated him with Korean medicine, involving herbal decoction, acupuncture, moxibustion and cupping. We used visual analogue scale and blood-test for assessment. After 31 days of treatment, the pantalgia and dizziness with nausea were disappeared and liver function test was in the normal range. According to this study, Korean medicine, including Galhwahajung-tang and Yijintang-gamibang are an effective treatment for the cure of acute alcoholic hepatitis with renal dysfunction which occurs on crapulence.

  • Women with moderate weight loss have secondary or primary amenorrhea. The amenorrhea. which is usually reversible with weight gain, decreased exercise. or both, is due to hypothalamic dysfunction. But 30% of amenorrheic women with weight gain is irreversible. Recently we experienced one case of secondary amenorrhea with severe weight loss, as oriental medicine treatment secondary amenorrhea is cured, so report a medical treatment course and result.

Childhood Obesity Patients treated by Korean Medicine Treatment: A Case Series (한의 치료로 호전된 소아비만 환자 9례에 대한 임상증례군 보고)

  • Objectives This study was written to report Korean medicine treatment for obese children. Methods This study included obese children who visited the Department of Korean Pediatrics at the Daejeon Korean Medicine Hospital of Daejeon University from September 18, 2020 to March 30, 2023. Height and weight at the first visit and on the day of the last herbal medicine termination were measured, and changes before and after treatment were observed. Liver function tests were performed before and after the treatment. Results Of the nine obese children who received Korean medicine treatment, five were male (55.6%) and four, female (44.4%). In term of age, there were five 8-year-olds (55.6%), two 6-year-olds, and two 10-year-olds (22.2%); the patients' mean age was 8.00 ± 1.414 years. The treatment period was more than three months, and the average treatment period was 177.11 ± 106.013 days. After treatment with Korean medicine, the patients' heights increased, and the body mass index (BMI) percentile and obesity decreased. Liver levels were maintained within the normal range without abnormal liver function after herbal medicine treatment, and no adverse reactions were observed during Korean medicine treatment. As a result of the pattern identification analysis, gastrointestinal damp-heat type and yin deficiency with internal heat were reported at a high frequency. Conclusions Korean medical treatment can reduce the degree of obesity without hindering the growth of childhood obesity.

The Effect of Cultivated Wild Ginseng Extract on Preadipocyte Proliferation (산양산삼 추출물이 지방세포의 분화에 미치는 영향)

  • Objectives The purpose of this study is to investigate the effects of cultivated wild ginseng extract on primary cultured preadipocyte and adipocytes. Methods Diminish preadipocyte proliferation does primary role to reduce obesity. So, preadipocytes and adipocytes were performed on cell cultures with using Sprague-Dawley rats and treated with 0.01-1mg/ml cultivated wild ginseng extract. Result At all concentrations, cultivated wild ginseng extract wasn't show the suppress proliferation of preadipocytes significantly and failed to show effects on decomposition of adipocytes except high dosage. Conclusion Based on these findings, cultivated wild ginseng is not a suitable choice for the treatment of localized obesity.

  • Metabolic syndrome consists of several risk factors of metabolic origin like elevated blood pressure, abdominal obesity, elevated triglyceride concentrations, decreased HDL-cholesterol concentrations, and elevated fasting glucose. It appear to directly increase the development of atherosclerotic cardiovascular diseases which leading to cardiovascular diseases in later life. This 39 years female patient was diagnosed schizophrenia at the age of 17. but she didn't take reasonable treatments. during 20 years, she stayed in home and lived unsuited life with her family. As a result she gains heavy weight and centeral obesity, diabetic mellitus and hypertention, and developed cerevral vascular accident twice. This patient was treated with herb medication, acupuncture, western medication, psychotherapy primarily in focusing of pre-illness state(未病) as well as the stagnation of phelm. During 140 days of treatment from admission, before 2nd attack tbe patient showed improvement of Lt. hemiparesis but after second attack improved a little. And symptom of schizophrenia improved considerably. Also diet habit changed.

Trend of Pharmacopuncture Treatment on Obesity: Recent 10 Years (비만 치료에 대한 약침연구의 국내외 동향 분석: 최근 10년을 중심으로)

  • Objectives: The purpose of this study is to research domestic and foreign trend of pharmacopuncture treatment on obesity during recent 10 years. Methods: 5 Databases (Korean Studies Information Service System, Research Information Sharing Service, Oriental Medicine Advanced Searching Integrated System, Scopus, PubMed) were searched with keywords of ('pharmacopuncture', 'herbal acupuncture', 'aquapuncture', 'obesity') from 2012 to 2022. Results: 25 Articles were selected and analyzed. 15 articles (60%) were animal experimentations, 8 articles (32%) were case reports, 1 article (4%) was cell experimentation, and 1 article (4%) was clinical trial. In this study, 25 articles were analyzed by subject, acupoints, injections, metrics and results. Pharmacopuncture treatment for obesity is continuously being studied, and the anti-inflammatory effect as well as the effect of reducing obesity factors has been revealed. Conclusions: This study suggests the efficacy and future development of pharmacopuncture for obesity. The studies of the past decade have been concentrated on animal experiments, so many clinical trials and various studies on new complex pharmacopuncture for obesity are expected.

Review on predictors of dropout and weight loss maintenance in weight loss interventions (비만치료에 있어서 중도탈락과 감량 후 체중유지에 영향을 주는 인자들에 대한 고찰)

  • Objectives: Dropout and weight regain are common problems in most obesity treatments. The purpose of this study was to review previously published study results of the predictive factors associated with dropout during weight loss treatment and weight loss maintenance after successful weight loss. Methods: Authors searched for the articles related to dropout and weight loss maintenance, published from 2007 to 2016 found on Pubmed, Scopus, RISS, and KISS. A total of 19 articles were finally selected. From the study results, unchangeable and changeable predictors were extracted, and these predictors were examined according to dropout and weight loss maintenance categories. Results: The unchangeable predictors of dropout were younger age, lower education level and female, whereas the changeable predictors of dropout were lower initial weight loss, symptoms of depression and body dissatisfaction. The strongest factor for predicting the dropout was initial weight loss. The unchangeable predictors of weight loss maintenance were old age, male and family history of obesity, whereas the changeable predictors of weight loss maintenance were regular exercise, dietary restraint, self-weighing and low depressive symptoms. Initial weight loss, depressive symptoms, body image, dietary restraint, physical activity, weight loss expectation and social support were considered to be dominant factors for weight loss treatments. Conclusions: Our review results suggest that unchangeable and changeable predictors of dropout and weight loss maintenance should be carefully examined during treatments of obesity.

Recent Clinical Research Trends of Chuna Treatment in Obese Children through CAJ and Pubmed (CAJ와 Pubmed를 이용한 비만아동의 추나치료에 대한 최신임상연구 동향)

  • Objectives The purpose of this study is to obtain knowledge from clinical studies conducted in China to examine the effectiveness of Chuna treatment and its potential in childhood obesity. Methods 6 clinical studies up to March, 2018 about childhood obesity were reviewed using 'CAJ', Chinese Academic Journal from China National Knowledge Infrastructure (CNKI) and 'Pubmed'. Result 1. 6 eligible randomized control studies were selected based on our inclusion criteria. Then, the studies are analyzed by demographic information, treatment method, evaluation methods, and results. 2. Ordinary acupoints (正經穴) is the most common acupoint-applying method of Chuna therapy. The most frequently used acupoints were Zhongwan (CV12), Tianshu (ST25), Zusanli (ST36), Qihai (CV6), Fenglong (ST40) and Guanyuan (CV4). Conclusion In most of the studies, the overall effectiveness in the treatment group was better than that of the control group. The results of this study can be used for future studies of Chuna therapy in the child obesity.
