Opioids profoundly inhibit evoked discharges of spinal nociceptive neurons, thereby inhibiting the transmission of pain. Intrathecal administration of opioids using implantable continuous infusion systems is an effective method of pain relief when other treatments have failed, as well as for patients with adequate analgesia on high dose therapy that produces unacceptable side effects. We report two cases of intrathecal pump implantation performed in patients suffering from intractable chronic pain. A test dose of 3 mg morphine was injected into the epidural space. No side effects were noted and patients experienced considerable pain relief. Implantation was performed one day after the test. The initial intrathecal morphine delivery dose was half of the equivalent dose of daily oral intake opioids and the infusion rate was increased gradually under close observation for opioid side effects. Two days post-implantation, both patients were discharged without any complications.
Lactic acid bacteria (LABs) have been proposed as a potential oral allergy-therapeutic means of modulating immune phenotype expression in vivo, via promoting or reducing cytokine production. This study investigated the ability of LABs to suppress allergic response via modulating cytokine production in mice splenocytes. BALB/c mice were intraperitoneally primed with ovalbumin together with alum adjuvant to invoke antigen-specific Th1/Th2 cytokine-secreting cell populations in splenocytes. Spleen cells from mice fed with Lactobacillus confusus PL9001 (KCCM-10245), L. fermentum PL9005 (KCCM-10250), L. plantarum PL9011 (KCCM-10358), and Bifidobacterium infantis PL9506 (KCCM-10406) suppressed the levels of Th2 cell cytokines such as IL-4 and IL-5 during antigen sensitization. In addition, all mice fed with LABs induced secretion of Th1 cell cytokines such as IL-2 in splenocytes. These results suggested that LABs are anti-allergic agents, in view of their Th1/anti-Th2 immunoregulation.
Purpose: The etiology of acute gastroenteritis (AGE) has changed since the introduction of the rotavirus vaccination. The aim of this study was to clarify which common pathogens, both bacterial and viral, are currently causing AGE in infants. Methods: Infants with acute diarrhea were enrolled. We tested for 10 bacterial pathogens and five viral pathogens in stool specimens collected from infants with AGE. The clinical symptoms such as vomiting, mucoid or bloody diarrhea, dehydration, irritability, and poor oral intake were recorded, and laboratory data such as white blood cell count and C-reactive protein were collected. The clinical and laboratory data for the cases with bacterial pathogens and the cases with viral pathogens were compared. Results: Of 41 total infants, 21 (51.2%) were positive for at least one pathogen. Seventeen cases (41.5%) were positive for bacterial pathogens and seven cases (17.1%) were positive for viral pathogens. Staphylococcus aureus (13 cases, 31.7%) and Clostridium perfringens (four cases, 9.8%) were common bacterial pathogens. Norovirus (five cases, 12.2%) was the most common viral pathogen. Fever and respiratory symptoms were common in the isolated viral infection group (p=0.023 and 0.044, respectively), whereas other clinical and laboratory data were indistinguishable between the groups. Conclusion: In our study, S. aureus (41.5%) and norovirus (12.2%) were the most common bacterial and viral pathogens, respectively, among infants with AGE.
X-linked adrenoleukodystrophy (ALD) is a uncommon metabolic disorder which derived by peroxismal ${\beta}$-oxidation and elevation of serum very long chain fatty acid (VLCFA). VLCFA is mainly accumulated in the myelin of the central nervous system and adrenal cortex, by which the expressed symptoms of this disease are mainly neurologic and endocrinologic (such as adrenal insufficiency). The mutations in the ABCD1 gene causes X-linked ALD, nevertheless its phenotypes and genotypes are poorly coordinated. We report the case of a 12-year-old boy with X-linked ALD who developed vomiting, fatigue and poor oral intake. Severe dehydration and hyponatremia were found in initial physical examination and laboratory test, but his motor/sensory nerve function and mental status were completely normal. We diagnosed ALD with diffuse high-intensity signal in both parietotemporal cerebellar white matter in brain MRI and elevated serum VLCFA. Later, we confirmed a novel c.1635-1G>A (IVS6-1G>A) mutations of the ABCD1 gene. With the discrepancy between its phenotypes and genotypes, various phenotypes could be seen in X-ALD patient. Careful examination and further studies for these patients will be needed.
췌장 손상은 소아에서는 흔하지 않으며 임상 증상과 이학적 소견이 뚜렷하지 않아 진단이 어려운 경우가 많은데 손상 정도와 위치에 대한 정확한 평가를 바탕으로 치료가 지연되지 않도록 하여 합병증 발생을 막도록 해야 할 것이다. 저자들은 8세 남아에서 가성 낭종, 늑막 삼출액을 동반한 췌장 절단 및 췌관 손상을 진단하고 경피적 배액술을 통하여 효과적으로 치료하였기에 문헌 고찰과 함께 보고하는 바이다.
Obesity occur from the imbalance between energy intake and energy expenditure. Obesity is a complex chronic disease that is suggested to cause other metabolic disorders such as type 2 diabetes, hyperlipidemia, hypertension, and arteriosclerosis. In this study, our purpose is to investigate the anti-hyperglycemic and anti-obesitic effects of Maydis stigma water extract in 3T3-L1 adipocytes and db/db mice. Maydis stigma water extract at dose of 100 and 500 ${\mu}g/ml$ slowly inhibited cell viability as compared to that of control in mature adipocytes. Also, the additions of 50 and 250 ${\mu}g/ml$ of Maydis stigma water extract significantly inhibited the lipid accumulations and CCAAT/enhancer-binding protein(C/EBP) ${\alpha}$ and peroxisome proliferator-activated receptor(PPAR) ${\gamma}$ expressions with dose-dependent manner in 3T3-L1 adipocytes. Maydis stigma water extract at 250, 500, and 1000 ${\mu}g/ml$ only showed the increasing pattern on lipolysis activity. The oral treatment of Maydis stigma water extract (100 or 400 mg/kg body weight) in db/db mice only showed tendency to decrease body weight, food efficiency ratio (FER), HbA1c, blood glucose, total cholesterol, triglyceride, and the adipocyte size of in db/db mice. However, Maydis stigma water extract increased the insulin level in a dose dependent manner. Thus these results indicate that Maydis stigma water extracxt inhibits adipogenesis through regulation of C/EBP${\alpha}$ and PPAR${\gamma}$ expressions in 3T3-L1 adipocytes and shows anti-hyperglycemic effect through increase of insulin secretion in db/db mice.
This study was conducted to investigate the potential embryo-fetal toxicity and toxicokinetics of the antimalarial agent artesunate (ARTS) in Sprague-Dawley rats. Pregnant rats were administered ARTS daily from gestational day 6~15 via oral gavage, at test doses of 0, 2, 4, or 8 mg/kg (22 females per group). The fetuses were examined for external, visceral, and skeletal abnormalities on gestational day 20. With regard to the dams, there were no deaths, treatment-related clinical signs, changes in body weight, or food intake in any of the treatment groups. There were no treatment-related gross findings at necropsy in any treatment group. In the 8 mg/kg group, there was a decrease in gravid uterine weight and in the weight of female fetuses. There was also an increase in fetal deaths (primarily late resorptions) and an increase in post-implantation losses (37%) at 8 mg/kg. An increase in the incidence of visceral and skeletal variations at 4 and 8 mg/kg was observed. These defects included minor changes in the appearance of the kidney and thymus, as well as absent ribs or thoracic vertebrae. Toxicokinetics were assessed in a parallel study, using 4 mated females per group. Using liquid chromatography-mass spectrometry (LC-MS) analysis, the concentration of ARTS and its metabolite dihydroartemisinin (DHA) were quantified in plasma from rats on gestational days 5, 6, 10, and 15. Amniotic fluid was assayed for ARTS and DHA on gestational day 15. There was evidence of rapid conversion of ARTS to the metabolite DHA in maternal plasma, since ARTS could not be consistently detected in plasma at the three doses tested. ARTS and DHA were not detected in amniotic fluid at gestational day 15, indicating limited placental transfer of the two agents. The embryo-fetal no-observable-adverse-effect level (NOAEL) of the test item was considered to be 8 mg/kg/day for dams, and 2 mg/kg/day for embryo-fetal development.
It has been known for a long time that Palmiwon water extract can be prescribed for the treatment of deficiency of Yang in oriental medicine. But the exact mechanism of it has not been Known yet. In the present experiments, it was attempted to explain the effect of Palmiwon water extract on the serum Hormone level in rabbits and the antifatigue in mice. Palmiwon water extract oral intake showed a increase of serum hormone level in rabbits and the effects of the antifatigue in mice. The results obtained were as follows. 1. The administration of this medicine was observed to cause continuously a significant increase of the testosterone content in the serum from the 7th day to the 21th day. 2. The estradiol centent in the serum, as compared with the control experiment, was seen to increase significantly by the administration of this medicine from the 7th day to the 21th day. 3. The LH and FSH content in the serum, as compared with the control experiment, indicated a tendency toward an increase by the administration of this medicine from the 7th day to the 21 th day, but with no significant difference. 4. In the effect to Palniwon on the antifatigue in mice, swimming time was prolonged in both sample A, B groups than control group, and significance revealed in both sample A, B groups. These data suggest that Palmiwon water extract can increase serum hormone level and that it can have therapeutic effectiveness to the antifatigue.
Objectives : This paper is designed to help the public prevent their future dental caries, based on the studies of xylitol about it's efficacy and effectiveness, and its appropriate usage. Methods : This study of xylitol on the basis of existing research papers and literature is not only to learn about the mechanism of what xylitol acts, but also to search the effectiveness of incompleteness of xylitol-containing foods, bacterial infection between xylitol and mother-baby, and the side-effects of xylitol. Results : 1. Xylitol's continuous intake of teeth effects the decrease of calculus amount, cleanness of teeth, as well as the remineralization process. The xylitol effect varies depending on the weight of xylitol, which has no effect on root caries. 2. Mouth gargle solution is higher resistant against acids; recalcification is significantly reduced; and remineralization effect represents. 3. It is reported that regular consumption of xylitol by mothers just during two years after birth has significantly decreased the spread possibility of mutans streptococcei. 4. The reduction of dental caries by xylitol is happened by its restraining the growth of bacteria resource caries, with their presence in amount of uncontrolled growth. 5. When a small amount of eating xylitol is absorbed slowly into gastrointestinal tract, being followed by glucose metabolism in the liver, while its excessive amount of eating can cause gastrointestinal problems. Conclusion : Dental hygienists should be required to provide dental information to the public on the basis of academic knowledge. In situation that one is not yet sure of xylitol on solid and multifaceted researches, there must be needed more accurate and detailed reviews on the side-effects of xylitol and the overall impact on the oral environments.
Abeliophyllum distichum Nakai is a deciduous shrub of a flowering plant in Oleaceae. It is an important plant resource and consists of only one species in the entire world. A. distichum Nakai is well known an edible, medicinal herb in its habitat districts, but the toxicological evaluation for the safe use of its extract is still insufficient. The study characterized the toxicity of an Abeliophyllum distichum Nakai ethanol extract in Sprague-Dawley (SD) rats and determined the safe dosage levels in a 13 weeks toxicity study. Abeliophyllum distichum Nakai ethanol extract was orally administered once daily for 2 weeks at 0, 500, 1,000 and 2,000 mg/kg/day to male and female SD rats. while recording the clinical signs of toxicity, body weight, food intake/consumption, eye test and urine analysis. Only the total protein frequency in the urine of male SD rats (p<0.05), the right ovary of the 500 mg/kg group (p<0.01) and the right adrenal gland of the 1,000 mg/kg group (p<0.05) in the female rats showed statistically significant changes. But no toxic effects were noted from repeated-dose administration of the Abeliophyllum distichum Nakai ethanol extract in the SD rats during the observation period. The post-mortem examinations showed no test substance-mediated changes. The hematological analysis and clinical blood chemistry data demonstrated no toxic effects from repeated-dose administration of Abeliophyllum distichum Nakai ethanol extract in the SD rats during the observation period. Based on these results, this data suggests that a dose of 1,000 mg/kg/day is a highest treatment to administer when conducting a further 13 weeks toxicity study.
