Yakson vs. GHT Therapy Effects on Growth and Physical Response of Preterm Infants and on Maternal Attachment (약손요법이 미숙아의 성장 및 생리적 반응과 미숙아 어머니의 애착에 미치는 효과 - GHT[Gentle Human Touch]요법과 비교하여 -)

  • Purpose: This study is aimed to confirm the effects of Yakson therapy on the growth and physical response of preform infants, and maternal attachment to them compared with GHT therapy. Method: The design of this study is nonequivalent control group with repeated measuring by quasi experimental study. The subjects are preterm infants in 26 - 34 gestational age hospitalized in the NICU of 4 university hospitals with an experimental group of 15 and a control group of 14. Yakson therapy consists of three phases: laying a hand, caressing by hand, and laying a hand again taking 5 minutes for each phase. Result: As a result of administering Yakson therapy to preform infants; the average weight gain of the Yakson group was higher than that of the GHT group, but there is no significant difference between groups. The oxygen saturation and maternal attachment difference between the Yakson and the GHT group were not significant. Significant differences in the average daily increase of oral intake and apical pulse rate were observed between the Yakson group and GHT group. Conclusion: These data suggested that Yakson therapy may be an effective nursing intervention which can facilitate growth and physical response of preform infants.

Effect of Berberis tinctoria leaf (Berberidaceae) extract on antidiabetic, antihyperlipidemic and antioxidant status in streptozotocin induced diabetes in rats

  • The present study was carried out to investigate the antidiabetic and antioxidant effect of methanolic extract of Berberis tinctoria leaves (MEBT), in streptozotocin induced diabetic rats. Oral administration of MEBT extract (150 mg/kg and 300 mg/kg) for a period of 14 days. Blood glucose levels, body weight food and liquid intake were measured on every $5^{th}$ day over a period of 14 days. In diabetic rats, MEBT at the dose of 150 mg/kg and 300 mg/kg body weight resulted in significant reduction in blood glucose levels. The study was further investigated to determine antioxidant and antihyperlipidemic potential of MEBT in streptozotocin (STZ) induced diabetic rats. These results suggest that the MEBT possess antidiabetic activity and is able to ameliorate biochemical damages in STZ induced diabetic rats and the results were found to be in a dose dependent manner.

Anti-diabetic Effect of the Exopolysaccharides (EPS) Produced from Cordyceps sinensis on ob/ob Mice (제 2형 당뇨쥐에서 동충하초로부터 생산된 세포외 다당류의 항당뇨 효과)

  • Anti-diabetic effect of the exopolysaccharides (EPS) produced from submerged mycelial culture of Cordyceps sinensis (Cs) was studiedin a type II diabetic animal model (C57BL/6J ob/ob). This study was designed to determine whether Cs-EPS improves clinical symptoms of type II diabetes in ob/ob mice. After Cs-EPS treatment at doses of 200 mg/kg body weight, the fasting blood glucose levels decreased by 47% after 7 weeks compared with those of the control mice. According to the oral glucose tolerance test, the glucose levels recovered its baseline after 120 min in Cs-EPS-treated mice, although the blood glucose levels increased significantly after 30 min. On the other hand, the control group (not-treated) did not recovered its initial level of glucose after 120 min. Furthermore, food intake, body weight, total plasma cholesterol and triglyceride concentrations in ob/ob mice treated with Cs-EPS were significantly decreased, compared with those in control ob/ob mice. Cs-EPS treatment increased significantly the plasma insulin level and the expression of leptin mRNA in adipose tissue of Cs-EPS-treated ob/ob mice. From these results, it is demonstrated that Cs-EPS could be effective for regulating normal blood glucose levels by increasing the amounts of plasma insulin and leptin expression in ob/ob mice, indicating that this compound could be a candidate material as a dietary supplement to control hyperglycemia in patients suffering from type II diabetes.

Feasibility and Safety of Totally Laparoscopic Radical Gastrectomy for Advanced Gastric Cancer: Comparison with Early Gastric Cancer

  • Purpose: Totally laparoscopic gastrectomy (TLG) for advanced gastric cancer (AGC) is a technically and oncologically challenging procedure for surgeons. This study aimed to compare the oncologic feasibility and technical safety of TLG for AGC versus early gastric cancer (EGC). Materials and Methods: Between 2011 and 2016, 535 patients (EGC, 375; AGC, 160) underwent curative TLG for gastric cancer. Clinicopathologic characteristics and surgical outcomes of both patient groups were analyzed and compared. Results: Patients with AGC required a longer operation time and experienced more intraoperative blood loss than those with EGC did. However, patients from both the AGC and EGC groups demonstrated similar short-term surgical outcomes such as postoperative morbidity (14.4% vs. 13.3%, P=0.626), mortality (0% vs. 0.5%, P=0.879), time-to-first oral intake (2.7 days for both groups, P=0.830), and postoperative hospital stay (10.2 days vs. 10.1 days, P=0.886). D2 lymph node dissection could be achieved in the AGC group (95%), with an adequate number of lymph nodes being dissected ($36.0{\pm}14.9$). In the AGC group, the 3-year overall and disease-free survival rates were 80.5% and 73.7%, respectively. Conclusions: TLG is as safe and effective for AGC as it is for EGC.

A Case of Acute Pancreatitis in a Neuroblastoma Patient after Retinoic Acid Therapy (신경모세포종 환아에서 레티노익산 치료 중 발생한 급성 췌장염 1례)

  • Retinoic acid has been used successfully as a differentiating agent in acute promyelocytic leukemia and neuroblastoma. However, some adverse effects have been recognized, such as headaches, dry skin and retinoic acid syndrome, a life threatening acute cardiorespiratory disorder. Acute pancreatitis with hyperlipidemia has rarely been reported. We experienced a case of acute pancreatitis with hyperlipidemia in a neuroblastoma patient after retinoic acid therapy for 21 months. Although the patient was ordered nothing by mouth and total parenteral nutrition was administrated, she died of disseminated intravascular coagulopathy and pulmonary hemorrhage, possibly because of oral intake during her recovery period.

  • A 47-day-old male infant presented with fever, poor oral intake, irritability, and right-sided bluish buccal swelling. Contrast-enhanced computed tomography of the neck showed a round mass lesion of about $2.0{\times}1.5cm$ that suggested abscess formation in the right masticator space. Ultrasound-guided extraoral aspiration of the abscess at the right masseter muscle was successful. Staphylococcus aureus was identified in the culture from the aspirated pus and blood. Appropriate antibiotics were given and the patient recovered. The patient underwent follow-up ultrasonography that showed an improved state of the previously observed right masseter muscle swelling at about 1 month after hospital discharge. A masticator space abscess usually originates from an odontogenic infection in adults. We report a case of masticator space abscess in a 47-day-old infant in whom septicemia without odontogenic infection was suspected.

Effects of Rhus verniciflua Stokes (RVS) Extract on Diet-induced Obesity in C57BL/6 mouse (옻나무 추출물이 식이에 의한 비만유도 C57BL/6 mouse에 미치는 영향)

  • We investigated the effects of Rhus verniciflua Stokes (RVS) on high fat diet- induced obesity in C57BL/6 that received ad libitum exposure to a high fat diet for 8 weeks. Male C57BL/6 mice weighing $20.1{\pm}0.95\;g$ were divided into four groups and the body weight and food intake were weighed for 8 weeks. After the administration of RVS extract, the liver, subcutaneous and retroperitoneal adipose tissue in the mice were weighed and common blood count in whole blood and biochemical parameters in plasma were measured. It was founded that body weight increase was significantly suppressed by the oral administration of RVS extract in group with high fat diet. Weight of subcutaneous adipose tissue and concentration of triglyceride in plasma from mice which were administrated RVS extract were lower than obesity control group. These results suggest that RVS extract may effect on inhibition of diet-induced obesity.

  • We performed hand-assisted laparoscopic surgery for a 67-year-old male with a 5.6 cm sized abdominal aortic aneurysm. To the best of our knowledge, this is the first report in Korea. After an initial hand dissection of the abdominal aorta under laparoscopy, we performed proximal anastomosis and distal abdominal. aorta suture ligation through a 6 cm abdominal incision. Distal anastomosis was done at the bilateral common femoral arteries. He resumed his oral intake 6 hours after the surgery and discharged at the $4^{th}$ postoperative day.

Effect on the Inhibition of Pancreatic Lipase and Lipid Metabolism of Zanthoxylum piperitum Extracts (산초 추출물의 췌장 리파아제 저해 및 지질대사에 미치는 영향)

  • Obesity is the most common nutritional disorder in the developed world and has become a global epidemic in recent years. In this study, Zanthoxylum planispinum extracts (ZPE) were evaluated on the effect on inhibition of pancreatic lipase and lipid metabolism by oral treatment for 2 months in high-fed diet obesity-induced Balb/c mice. The ZPE showed pancreatic lipase inhibitory activity with $IC_{50}$ of $0.3{\mu}g/ml$. No significant difference in feed intake was observed among the groups. The high-fat diet-treated Z. planispinum extracts groups (HFD+ZPE, 100mg/kg) significantly decreased body weight compared to the high-fat diet vehicle groups (HFD, p<0.05). The high-fat diet-treated Xenical$^{(R)}$ groups (HFD+Xenical, n=10, 30mg/kg) also showed a significant reduction of body weight compared to HFD (p<0.05). Biochemical parameters (triglyceride, total cholesterol, and high-density lipoprotein cholesterol) in HFD plus ZPE diet groups were significantly lower than those of the HFD groups (p<0.05). These results indicated that ZPE more effectively suppressed the effects of HFD on body fat gain with the inhibitory effect on pancreatic lipase.

Characterization of the Pathogenesis Mechanism after Pseudomonas aeruginosa Infection through Food Consumption Using Chick Embryo Model

  • This study introduced a chick embryos’ infection model to elucidate the pathogenesis mechanism of Pseudomonas aeruginosa, which causes serious diseases in human after ingestion of P. aeruginosa-contaminated animal originated foods. The embryonic chick model is able to give a rapid and relatively inexpensive method to assess bacterial pathogenicity compared to embryos of other vertebrates. Embryos were infected with P. aeruginosa and elastase-deficient P. aeruginosa. After infection with P. aeruginosa cells, total bacterial cell numbers and gelatinase activities in the embryos were compared. Thereafter, precartilage condensation and chondrogenesis were assessed by peanut agglutinin (PNA) binding on day 3 and by Alcian blue staining for sulfated proteoglycans on day 5, respectively. P. aeruginosa significantly increased in embryos, resulting in abnormal limb development, whereas P. aeruginosa defective in elastase activity partly impaired proliferation. In addition, P. aeruginosa-infected chick embryos significantly stimulated the production of matrix metalloproteinases. Several analyses showed that elevated proteases suppressed the proliferation and survival of chondrogenic cells. The results show that this infection model was a useful assay to determine the virulence mechanism of P. aeruginosa in human after intake of microbiologically contaminated foods.
