Limited Open Repair Technique of Achilles Tendon Ruptures - Operative Technique and Early Results - (최소 절개 봉합술을 이용한 아킬레스건 파열의 치료 - 수술 방법 및 초기 결과 -)

  • Purpose: To investigate the early results of limited open repair technique of Achilles tendon ruptures, and to describe the surgical technique. Materials and Methods: From October 2004 to February 2005, a total of 10 patients with Achilles tendon rupture underwent limited open repair. The average age of the patients was 39.3 years, and the average follow-up period was 9 months. The causes of injury were sports injuries in 8 cases, and slip down in 2. The mean interval between the injury and the operation was 9 days. The clinical results were assessed by patient's satisfaction, incision length, hospitalization, the ankle-hindfoot scale of American Foot and Ankle Society (AOFAS), and complications. Results: Of 10 patients, 8 were very satisfied, and the remaining 2 were satisfied. The mean incision length was 2.0 cm, and the mean hospitalization was 2 days. The mean AOFAS score was 97 points, and there was no complications such as infection, rerupture, or nerve injury. All patients returned to work at approximately 2 months, and resumed light exercise such as jogging at approximately 3 months. Conclusion: Limited open repair technique of Achilles tendon ruptures is provided for better cosmetic results, high patient's satisfaction, and functionally successful results without postoperative complications.

Comparison of Toluene Diisocyanate Concentrations Collected with Different Sampling Methods by Work Process (시료채취 방법에 따른 작업 공정별 Toluene diisocyanates 포집농도 비교)

  • Objectives: The aim of this study is to present an appropriate sampling method for individual exposure assessment based on a comparison of toluene diisocyanate (TDI) concentrations collected through different sampling methods by work process type. Methods: Two plants handling TDIs in the Incheon area were selected. The samplings were taken during respective processes of spray painting, drying, grinding, and foaming, in which the production of TDIs took on different forms. For the sampling methods for airborne TDIs, open-face cassette holder, modified 2-piece cassette holder, and impinger were used, and the sampling was performed simultaneously at the same locations. Results: The comparison of TDI collection concentrations by each process depending on the sampling method showed high concentrations in the order of the impinger, modified 2-piece cassette holder, and open-face cassette holder for spray painting and foaming. In all processes with the exception of drying, TDI collection concentrations were higher for sampling done with the modified 2-piece cassette holder than with the open-face cassette holder. Conclusions: Based on these results, the modified 2-piece cassette holder was found to be a more appropriate sampling method than the open-face cassette holder when taking individual samples of TDIs from spray painting, grinding, and foaming processes. In particular, for individual exposure assessment of the spray painting process, which features comparatively high collection concentrations compared to the other processes, the use of a modified 2-piece cassette holder is considered appropriate.

INTRAORAL OPEN REDUCTION OF MANDIBULAR SUBCONDYLAR FRACTURES USING KIRSCHNER WIRE (Kirschner wire를 사용한 과두하 골절의 구강내 접근법)

  • The treatment of mandibular subcondylar fractures is a matter of controversy. The majority of mandibular subcondylar fracture are treated by closed reduction, but the displaced or dislocated mandibular subcondylar fractures may be treated by open reduction. The characteristics of open reduction are the anatomical reduction, the functional restoration, the rapid function, the maintenance of vertical ramus dimension, the better appearance and the less resultant TMJ problem etc. When an open reduction is considered, the wire, miniplate, lag screw and Kirschner wire are available with internal fixation. Of these, Kirschner wire is a simple method relatively and correct positioning of the wire achieves rigid fixation. But many open reduction methods for mandibular subcondylar fractures require extraoral approach. The extraoral approach has some problems, the facial scar and the risk of facial nerve injury. On the other hand, the intraoral approach eliminates the potency of the facial scar and the facial nerve injury, but is difficult to access the operation site. Since the intraoral approach was first described by Silverman (1925), the intraoral approach to the mandibular condyle has been developed with modifications. The purpose of this article is to describe the intraoral technique with the Kirschner wire on mandibular subcondylar fractures. Conclusion : The intraoral reduction with Kirschner wire on mandubular subcondylar fractures avoids the facial scar and facial nerve injury and is simple method to the extraoral approach. And it has minimal morbidity and better esthetics.

Early Definitive Closure of an Open Abdomen by Using Porcine Dermal Collagen Graft: A Case Report (외상환자의 손상통제 수술 후 돼지진피아교질 이식편을 이용한 조기 완전 폐복의 사례보고)

  • Purpose: The open abdomen is now the standard of care in various clinical situations, especially it is used to treat abdominal compartment syndrome. Many techniques have been reported for closure after an open abdomen, but most take a long time for complete definitive closure and are associated with various problems. We describe a technique using biologic mesh that can achieve early definitive closure after an open abdomen. Methods: A 45-year-old man presented to the emergency room with a painful hip and painful lower extremities after a fall from 80 feet. Radiologic examination revealed multiple fractures of the pelvis and low extremities. Abdominal compartment syndrome caused by a retroperitoneal hematoma developed during the orthopedic surgery. We performed exploration immediately and closed abdomen temporarily. A peritoneal graft of porcine dermal collagen with anterior myofascial approximation of the rectus abdominis muscles and sliding skin flap was performed three days after the previous surgery. Results: There were no complications related to the wound. The patient was transferred to the Department of Orthopedic Surgery seven days after the initial surgery. Conclusion: Early definitive closure using porcine dermal collagen is a feasible method that can reduce the length of hospitalization and the number of operations for an open abdomen.

Early Outcomes of Endoscopic Vein Harvesting during the Initial Learning Period

  • Background: The endoscopic vein harvesting (EVH) method has been used in coronary artery bypass surgery in many countries. We started using the EVH method recently, and investigated the results during the early learning period. Methods: Between March 2012 and June 2014, 75 patients (31 patients in the EVH method group, and 44 patients in the open method group) who underwent isolated first-time coronary artery bypass grafting using vein grafts were retrospectively analyzed with respect to the early outcomes including graft patency and risk factors for leg wound complications. For assessing the patency of vein graft, we performed coronary computed tomography angiography during the immediate postoperative period and 6 months later. Results: Mean harvesting time of endoscopic method was about 15 minutes. Patency rate during the immediate operative period and the 6-month patency rate were similar between the two groups (postoperative period: EVH 100% vs. open method 94.4%, p=0.493; at 6 months: EVH 93.3% vs. open method 90.9%, p=0.791). Leg wound complications occurred more frequently in the open method group (EVH 3.2% vs. open method 13.6%, p=0.127). According to the analysis, age was an independent risk factor for leg wound complications. Conclusion: EVH is a feasible method even for beginners and can be performed satisfactorily during their learning period.

How to Pose an Open Problem? : Two Cases of Posing an Open-ended Problem by Reorganizing Given Closed Problems (개방형 문제를 어떻게 만들 것인가?: 두 개의 개방형 문제 제작 사례를 중심으로)

  • Open problems can provide experiences for students to yield originative and various products in their level, because it is open with respect to its departure situation, goal situation, or solving method. Teachers need to pose and utilize open problems in forms of solution-finding or proving problems. For this we first have to specify which resource and method to use by concrete examples. In this article, we exemplify a method and procedure of posing an open problem by the two cases in which we pose open problems by reorganizing given closed problems. And we analyze students' responses for the two posed open problems. On the basis of these, we reflect implications for mathematical education of open problems.

Cache Architecture Design for the Performance Improvement of OpenRISC Core (OpenRISC 코어의 성능향상을 위한 캐쉬 구조 설계)

  • As the recent performance of microprocessor is improving quickly, the necessity of cache is growing because of the increase of the access time of main memory. Every block of direct-mapped cache maps to one cache line. Although the mapping rule is simple, if different blocks map to one cache line, the miss ratio will be higher than the set-associative cache due to conflicts. In this paper, for the improvement of the direct-mapped cache of OpenRISC, 4-way set-associative cache is proposed. Four blocks of the main memory of the proposed cache map to one cache line so that the miss ratio is less than the direct-mapped cache. Pseudo-LRU Policy, which is one of the Line Replacement Policies, is used for decreasing the number of bits that store LRU value. The OpenRISC core including the 4-way set-associative cache was verified with FPGA emulation. As the result of performance measurement using test program, the performance of the OpenRISC core including the 4-way set-associative cache is higher than the previous one by 50% and the decrease of miss ratio is more than 15%.

  • This study extracts the plan that is suitable to open education through the assessment and analysis of the open education environment. For this, the questionnaire survey of teachers at the elementary school in Kyengsang-bukdo was used. The results of this study can be summarized as follows; 1) In the estimation of satisfaction on the classroom environment for open education, although the score of "Comfortableness" is high, the scores of "Variety", "Convenience", and "Silence" are low in satisfaction. 2) The image on the classroom environment is highly regarded in general in "Stability", "Nobleness" and "Activity". However, it is necessary to devise some methods to improve the image on "Stability", when removing the wall between classrooms and corridors. 3) To improve the satisfaction and image associated with classroom environment, it is important to maintain suitable size of school and density of classroom. 4) The factors of satisfaction and those of image are correlated each other. Thus, to enhance the total image on the open class, it is necessary to improve the whole classroom environment.

Parallel String Matching and Optimization Using OpenCL on FPGA (FPGA 상에서 OpenCL을 이용한 병렬 문자열 매칭 구현과 최적화 방향)

  • In this paper, we propose a parallel optimization method of Aho-Corasick (AC) algorithm and Parallel Failureless Aho-Corasick (PFAC) algorithm using Open Computing Language (OpenCL) on Field Programmable Gate Array (FPGA). The low throughput of string matching engine causes the performance degradation of network process. Recently, many researchers have studied the string matching engine using parallel computing. FPGA's vendors offer a parallel computing platform using OpenCL. In this paper, we apply the AC and PFAC algorithm on DE1-SoC board with Cyclone V FPGA, where the optimization that considers FPGA architecture is performed. Experiments are performed considering global id, local id, local memory, and loop unrolling optimizations using PFAC algorithm. The performance improvement using loop unrolling is 129 times greater than AC algorithm that not adopt loop unrolling. The performance improvements using loop unrolling are 1.1, 0.2, and 1.5 times greater than those using global id, local id, and local memory optimizations mentioned above.

A Study on Analyzing Solution Spaces of Open-ended Tasks in Elementary Mathematics (초등 수학 개방형 과제의 해법 공간 분석 연구)

  • The purpose of this study is to develop a framework for analyzing the solution spaces of open-ended task and to explore their usefulness and applicability based on the analysis of solution spaces constructed by students. Based on literature reviews and previous studies, researchers developed a framework for analyzing solution spaces (OMR-framework) organized into subspaces of outcome spaces, method spaces, representation spaces which could be used in structurally analyzing students' solutions of open-ended tasks. In our research, we developed open-ended tasks which had various outcomes and methods that could be solved by using the concepts of factors and multiples and assigned the tasks to 181 elementary school fifth and sixth graders. As a result of analyzing the student's solution spaces by applying the OMR-framework, it was possible to systematically analyze the characteristics of students' understanding of the concept of factors and multiples and their approach to reversible and constructive thinking. In addition to formal mathematical representations, various informal representations constructed by students were also analyzed. It was revealed that each space(outcome, method, and representation) had a unique set of characteristics, but were closely interconnected to each other in the process. In conclusion, it can be said that method of analyzing solution spaces of open-ended tasks of this study are useful for systemizing and analyzing the solution spaces and are applicable to the analysis of the solutions of open-ended tasks.
