Benign Clear Cell ("Sugar") Tumor of the Lung -A Case Report- (폐양성 투명세포(당)종양 -1례 보고-)

  • We experienced a case of benign clear cell tumor of the lung. It is a rare and very unusual pulmonary neoplasm. Only 40 cases have been reported in the foreign literatures, but this is the 2nd case report in Korea. A 43 year old man who revealed a coin lesion in the chest X-ray at the health screening, underwent resection of the right lower lobe Pathologically the tumor ce ls have a clear cytoplasm due to abundant glycogen in the histochemical and electron microscopical studies.

  • Primary clear cell adenocarcinoma of uterine cervix is rare and cytomorphology in the vaginal smear have not been previously described in Korean literatures. The cytologic characteristics of clear cell adenocarcinoma of the uterine cervix include : malignant cells with abundant, finely vacuolated cytoplasm ; hobnail appearance, and distinctive basement membrane-like hyaline materials within cellular aggregates. A 36-year-old woman presented with vaginal bleeding. Cytologic examination of vaginal smear and histopathologic examination of a radical hysterectomy specimen allowed the diagnosis of hemorrhagic tumor in the uterine cervix as a clear cell adenocarcinoma. Cytologic findings were very characteristic. The tumor cells had abundant, pale, finely vacuolated cytoplasm with indistinct cytoplasmic membrane. The nuclei were round to oval with finely dispersed chromatin. Extracellular basement membrane-like hyaline substance, which stained a light green color in Papanicolaou's preparation, was frequently observed within the cancer cell clusters.

  • Journal of the Korean Society of Clothing and Textiles Vol. 21, Nt. 5 (1997) p. 865∼879 The purpose of the study is to identify a historical change about the women's underwear and trousers this nation of ours until an ancient times to modern ages. Because the record of the women's trousers exist scarcely anything, the study refers to several documents, for example, the literatures of the Chinese and the Chosun Dynasty, the Koryu's fresco, the Tou of Shinra, the painting of the rock face, the archaeological dresses and genre pictures, and The folkways methods through the residents is used to collect and to arrange of an enlightened age and modern times. Originally, the women's trousers appeared for the nomadism and the hunting in the northern distric and the northeastern provinces. Male and female, old and young wear the trousers which are the clothes native to Korea. The trousers which are originally narrow trousers are influnced by Chinese wide trousers. The trousers at the time of Samsuk hand down until Korea. The Skirt on the trousers is nothing but a courtesy. But gradually, it is distinguished only the trousers, and then, there are exchanged an underwear which not only the protection angainst the cold but also the beautiful. Therefore, the form of skirt is diminished, and the underwear is simplified as the increasing status and the activity of woman in the age of civilization. In a way, the reform trousers and Three-kingdom era trousers appeared and the Dansogok and Sogok disappeared at present. There are confirmed in my investigation of folk customs.

  • There are many approaches in surgery of posterior hypopharyngeal wall cancer according to location, extent, and invasion depth of primary cancer. And many reconstruction methods have been used in reconstruction of surgical defect remaining after wide resection of primary cancer. Posterior hypopharyngeal wall cancer is relatively rare, so its surgical experiences are fewer than those of pyriform sinus cancer and there have been few reports of surgical approaches and reconstruction methods of posterior hypopharyngeal wall cancer. Recently, we experienced a case of posterior hypopharyngeal wall cancer reconstructed with longus colli flap and skin graft after failure of radial forearm free flap in a 72-year -old man and report it with the review of the literatures.

  • Mucosal melanoma of the head and neck is a rare and usually lethal disease. Primary laryngeal malignant melanoma(LMM) are exceedingly rare tumors that morphologically are readily confused with more common types of laryngeal cancer. Treatment of choice for LMM is complete surgical excision and elective lymph node dissection is usually not recommended. The use of radiation or chemotherapy is generally thought to have no effect on local or distant disease and currently used as adjuvant therapy. The prognosis is extremely poor. We have experienced a 61 year old male patient with symptoms of foreign body and lump sense in throat. A dark pigmented polypoid mass was found on the right aryepiglottic folds with normal mobility of vocal cord. Total laryngectomy was performed under the diagnosis of malignant melanoma. Bone scan revealed multiple bony metastasis on ribs and lumbar vertebrae after 5 months of operation. There have been no evidence of recurrence at primary area. The patient died after 8 months of operation.

  • The meningiomas are slowly growing tumors arising from meningoepithelial cells and they generally occur in the intracranial space. Tumor extension to the extracranial structures occurred in about 20 percent of intracranial meningiomas. Meningiomas extending to the cervical area are uncommom. Beacuse these patients complain of localized symptoms, sometimes those tumors have been operated as cervical tumors. In this report, we incidentally found a meningioma in the left parapharyngeal space in a 63-year-old female, who was confirmed by histopathologic diagnosis to have meningioma. We report herewith this case with the review of literatures.

  • Meigs' syndome consists of an ovarian fibroma, or fibroma-like benign tumor, associated with fluid in both abdomen and chest. Characteristically, the ascites and hydrothorax disappeared and do not recurred following removal of the ovarian tumor. The authors presented a case of Meigs' syndrome that was cured by surgical removal of ovarian tumor and resulted in normal pregnancy outcome in 27-year-old pregnant woman with a brief review of literatures.

A Case of Bradycardia-Dependent Complete Atrioventricular(A-V) Block (서맥 의존성 완전 방실차단 1례)

  • Induction of A-V block by tachycardia is a well-known phenomenon. But there are few case reports of bradycardia-dependent A-V block. We report a case of bradycardia-dependent A-V block with review of literatures. This patient was a 52-year-old fe male who complained of dizziness and anterior chest discomfort. Electrocardiographic recording demonstrated complete A-V block. Monitor electrocardiographic recordings during sitting position and after atropine administration demonstrated decrease of degree of block from complete A-V block to first degree A-V block. The occurrence of complete A-V block for bradycardia during supine position suggests a phase 4-dependent block. After a permanent ventricular pacemaker was implanted, the patient recovered and was with out symptoms during 12 months follow up.

  • Scapulothoracic dissociation is a rare injury caused by blunt force or massive traction to the anterolateral shoulder girdle with disruption of scapulothoracic articulation. This injury is characterized by massive soft-tissue swelling of the shoulder with intact skin, lateralization of damaged scapular. It frequently is accompanied by various musculoskeletal injuries and neurovascular injuries. 33-year-old man sustained a scapulothoracic dissociation without fracture and Vascular injuries during arresting. We report diagnostic method, clinical finding and non-operative management with literatures.

Atypical Intramuscular Myxoma of the Lumbosacral Paraspinal Muscle : The First Case Report in Asian

  • Intramuscular myxoma (IM) is a benign neoplasm of mesenchymal origin. We report a rare case of IM which was located in the lumbosacral paraspinal muscles. A 62-year-old female patient presented with progressive low back pain for 2 months, and the radiologic findings showed a large mass ($4.0{\times}3.5{\times}6.5cm$) in the right lumbosacral paraspinal area. Total resection of the tumor was performed and the symptom was nearly resolved after surgery. Although the immuno-histopathological analysis was consistent with IM, there were some different findings from typical pathological characteristics of IM in this case. Firstly, the symptomatic change of the mass took relatively short time (less than 3 months), and this change was accompanied by partial calcification inside the mass. Moreover, iatrogenic interruption of paravertebral muscle by the other previous operation might be the promoting factor of the fibrous dysplasia, which can explain the pathogenesis of IM. To our knowledge, this is the eighth case of the lumbar paraspinal myxoma reported in the literatures and the first case in Asian population.
