사업장 보건관리 사업의 형태별 수행성과 분석 -비용편익 분석을 중심으로- (Performance of Occupational Health Services by Type of Service : Cost Benefit Analysis)

  • Occupational health services in Korea have been operated as dual types : one is operated by occupational health care manager and the other is health care agency without their own personnel. The performance of occupational health service should be different due to the variety of characteristics of health care manager and workplace, qualification of health care manager. This study is to analyze performance of occupational health care services with a particular consideration of job performance shape and efficiency, based on comparing those two types of health care management to show on the basic data for the settlement of more qualitative health care management system at workplace. For this study, total 391 places in Seoul and Inchon city area ; 154 places (39.4%) managed by designated health care manager and 237 places (60.6%) by the agency with their commission are selected as research samples. Tools for data collection are questionnares that have been investigated during the period of 20 September 1993-20 December 1993. Those data are compared with percentiles, mean, standard deviation and B/C ratio using SPSS PC program. Conclusions observed from the tests and each comparison could be summerized as follows : 1. Occupational health care have been accomplished at workplaces with designated people than with agencies people, and coverage rate of the occupational health care services has differences, due to management types. The reason of these results is due to visit only one or two times monthly by the agencies, while their own health care manager obsess, at the workplaces all the times. 2. Most of the expense for environmental control of all health care services expenditures shows that there is almost no fundamental improvement because more expenses are needed for procuring personal protective equipment and measuring work environment instead of environmental improvement. 3. It is investigated how much the cost of occupational health care services needs per worker, and calculated how much the cost needs per service hour per worker. The results from this show that the cost of occupational health services at workplaces with their own managers used less than the cost of health care agencies, eventually the former gives better services with less cost than the latter. 4. Benefit/Cost ratio is also produced by total benefit/total cost. The result from the above way reads 4.57 as a whole, while their own manager having workplaces reads 4.82 and the agencies do l.56. Even if their own manager performing workplaces spent more cost, this system produces more benefit than the agencies management. 5. The B/C ratio for medical organization such as local clinic, health care center and pharmacy shows more than or equal to at the workplaces controlled by the agencies. It is inferred that benefit would be much less than the cost used, with so being inefficient. 6. It is assumed that the efficiency ratio of health education is equal to reduction rate of workers medical organization visit. Estimated reduction rate 5%, 10%, 15%, show that the efficiency ratio of health education have an effect on producing benefits. It is estimated that more benefit can be produced if more qualitative education will be provided for enhancing health care efficiency. 7. Results of this study cannot be generalized because there are large scale of deviation in case of workplaces with less than 300 full time workers, but B/C ratio reads 2.69 as a whole and 3.25 at workplaces with their own health care manager are higher than 1.63 at the workplaces manged by the agencies. Finally, all the benefit concerning health care services could not be quantified, measured and shown on the value of money. This is a reason that a considerable part of benefits are so underestimated. This is also thought that measurement tools should be developed for measuring benefits of health care services with a comprehensive quantification. in the future. It is also expected that efficiency of occupational health care services should be investigated using cost-effectiveness analysis.

서울지역 산업장 근로자의 산업보건관리에 대한 지식, 태도, 실천 연구 (A Study of Workers' Knowledge, Attitude, and Practice of the Occupational Health Services in Industries Located in Seoul)

  • This research is undertaken for the purpose of providing basic data to improve the occupational health service for future. 415 workers of 46 firms in Seoul are included in this data points. Results from this analyzing are the following. 1) The level of knowledge, attitude and practice of workers on the occupational health is moderate. 2) The cognition of necessity about special health examination gains the highest point(3.94), but the understanding of the occupational health educational program gains the lowest point (.85) in the knowledge region. 3) In the attitude region, the highest point is marked by the workers' prudence of expending time on medical examination(3.45). The lowest one by the workers' feeling of satisfaction of the follow-up service after medical examination(1.79). 4) The workers' participation in medical examination is the most active(3.15), and that in occupational health education in the least active(.86) in the practical region.

소규모사업장 보건관리지원사업에 대한 평가연구 (An Evaluative Study of a Subsidiary Program in Small Scale Industries)

  • This study was performed to evaluate the subsidiary program on occupational health services in small scale industries. The survey of occupational health services was undertaken in 346 industries from July 1 to August 31. 1994. The major results were as follows ; 1. The subsidiary program on occupational health services The evaluation of working environments were conducted in 305 industries, $42.3\%$ of the industries had the harzardous agents exeeding TLV. Special medical examinations were done in 310 industries. the results were detected $C(33.3\%).\;D_1(0.8\%), D_2(5.4\%)$. Health management were done in 341 industries, the average visits for a industry were 3.8 times. The activities of health management were included to check of the workplace. health education, health counselling, etc. 2. The recognition of the employees and employers on occupational health services $91.5\%$ of employers and $87.0\%$ of workers felt healthy themselves. But $39.7\%$ of employers and $21.2\%$ of workers were under the stress from their jobs or home affairs. The workers perceived more harmful for the hazardous agents than the employers. $32.8\%$ of employers and $42.9\%$ of workers perceived that employer's efforts were the most important things to protect for the hazardous agents. The result of the evaluation of working environments were understood by $57.3\%$ of employers and $24.5\%$ of workers. The notification of medical examinations were recognized $83.9\%$ of employers and $50.3\%$ of workers. Many employers and workers felt the necessities of health management in small scale industries. The priorities of health manegement were different between employers and workers. Employers' priorities were as follows ; follow up care of medical examination$(97.3\%)$, health counselling$(93.7\%)$, planning of health$(93.5\%)$, health education of occupational diseases$(93.2\%)$, health education of chronic diseases$(93.5\%)$, etc. Workers' priorities were as follows ; follow up care$(93.4\%)$, health counselling$(94.3\%)$, health education of occupational diseases$(93.0\%)$. first aids$(92.0\%)$. etc.

Work Ability Index: Psychometric Testing in Aeronautical Industry Workers

  • Background: The Work Ability Index (WAI) is an instrument that measures work ability. The wide dispersion of the WAI internationally has led to its adaptation for use in different countries. This study aimed to evaluate the psychometric properties of the Spanish version of the WAI. Methods: A methodological design was used over an opportunistic sample of 233 workers in the aeronautical industry in Spain. Reliability was evaluated through internal consistency. Factorial validity, known groups, and convergent validity were tested. Results: The Cronbach's alpha and item-total correlation indicated an adequate internal consistency. The confirmatory factor analysis, performed to evaluate the factorial validity, found adequate fit indices for a two-factor solution with a high correlation between the factors. Factor 1, "Subjectively estimated work ability and resources", was composed of 3 subscales and factor 2, "Ill-health-related", of 2 subscales. Subscales 4 and 6 had loading in both factors. Workers under 45 years of age obtained higher significant scores than older ones. Convergent validity was also evidenced since WAI was highly correlated with self-assessment of health status. Conclusions: The Spanish version of the WAI has shown evidence of reliability and validity in this study, supporting its use in individual and collective health surveillance by occupational health professionals. The factorial solution that was found has previously been reported in another international context. However, further research is needed to resolve the discrepancies detected in the role of some subscales between other national and international studies.

서비스직 근로자의 고객응대업무와 프리젠티즘: 상사 지지의 조절효과 (Exploring Interaction between Interactive Services and Presenteeism of Korean Service Workers: The Moderating Effects of Supervisor Support)

  • Purpose: This study delves into the relationship between interactive services and presenteeism among Korean service workers, aiming to discern the moderating effects of supervisor support on the relationship. Methods: Utilizing secondary data from the sixth Korean Working Conditions Survey, involving 7,628 service workers, this study conducted logistic moderated regression analysis to scrutinize the moderating effects of supervisor support on the relationship between interactive services and presenteeism. Results: Approximately 10% of workers experienced presenteeism in the previous 12 months. Females, low-educated, elderly, those working long hours, and low-wage workers were more likely to perceive presenteeism. Notably, a statistically significant U-shaped curvilinear relationship was observed between interactive service duration and presenteeism. The results also underscore the moderating role of supervisory support in shaping the relationship between interactive services and presenteeism. Conclusion: The study highlights the significance of supervisor interest and support in promoting workers' health, thereby not only improving the wellbeing of individual workers but also fostering organizational productivity by reducing presenteeism.

항공기 객실승무원의 감정노동, 건강증진생활양식과 건강증진 프로그램 요구도와의 관계 (The Relationships among Needs for Health Promotion Programs according to Emotional Labor and Heathy Lifestyle of Flight Attendants)

  • Purpose: The purpose of this study was to figure out emotional labor, healthy lifestyle, needs for health services of flight attendants and the relationships among needs for health services according to various characteristics of flight attendants. Methods: The participants of this study were 140 flight attendants who work on major and low cost airlines. Data were collected through a questionnaire from 2017 .Jan.~2017. Feb. Data were subsequently analyzed using the SPSS 21 Program. Results: The score of emotional labor was 3.76 surface acting was 3.75 and deep acting was 3.77. The average of healthy lifestyle was 2.41, and the highest was 3.13 of personal relationship and the lowest was 1.91 of eating habits. There is a positive correlation between emotional labor-deep acting and healthy lifestyle. Flight attendants demand health services for physical exercise, stress and emotional labor management, healthy eating habits, emergency treatment, prevention of fatigue, cancer screening, sexual harassment prevention, sex education and etc. Conclusion: These results show that flight attendants need to various interventions for improving healthy lifestyle and reducing emotional labor. It is necessary to develop customized health promotion program suited to their job and general characteristics.

임상간호사의 일과 삶의 균형과 간호서비스 질의 관계에서 일과 삶의 균형 조직문화가 미치는 매개효과 (Mediating Effect of Organizational Culture Supporting Work-Life Balance on the Relationship between Work-Life Balance and Quality of Nursing Service in Clinical Nurses)

  • Purpose: This study aimed to confirm the mediating effect of work-life balance organizational culture in the relationship between work-life balance and quality of nursing service for clinical nurses. Methods: Participants included 224 nurses from four hospitals located in D city. Data was collected from 3 to 21 August 2020 using a self-report questionnaires. Descriptive statistics, t-test, one-way ANOVA, Scheffé test, and Pearson's correlation coefficient were used for analysis. The mediating effect was assessed by Baron and Kenny's three-step hierarchical regression analysis and bootstrapping methods. Results: There were significant relationships between work-life balance and work-life balance organizational culture (r=.45, p<.001), work-life balance and quality of nursing service (r=.18, p=.005), and work-life balance organizational culture and quality of nursing service (r=.34, p<.001). Also, work-life balance organizational culture (β=.32, p<.001) showed a total mediating effect on the relationship between work-life balance and quality of nursing service. Conclusion: The development and provision of programs that can improve the work-life balance and establish an organizational culture that supports it will help improve the quality of nursing services.

소규모 사업장 보건관리 산업간호사의 업무서식과 기록행태 (Type and Contents of Health Records and Recording Tendencies of Nurses in Small-Scale-Enterprises)

  • This study was carried out to investigate the types of health documents for nurses, the content of informations in the documents, and writing behaviors of occupational health nurses. Health documents were collected from 7 nurses who were working in 7 group occupational health agencies (GOHA) located at Seoul and Inchon area in Korea. The collected health documents written in January to June 1999 were analyzed, and revealed the following results. 1) The occupational health nurses were using 9 to 18 different types of health documents. The contents of the documents were considered quite similar to each other with slightly different headings and items to record. Among different types of health documents. Health Management Card for Workplace', Nursing performance sheet and Workplace environmental checklist were in common among nurses and were used for content analysis. 2) The 'Health Management Card for Workplace' was the only formal sheet of small-scale-enterprises (SSE) for health management, in which health and safety related information was recorded. The information on nursing services were recorded on the Nursing performance sheet, which has slightly different names on each type with similar contents. The Workplace environment checklist was for the information on general work environment management and mainly status of workplace hygiene. This checklist is to be used by or with nurses among the 3 types health professional team such as doctor, hygienist, and nurse, but it seemed not being used frequently by nurses. 3) Analysis on recording tendencies of nurses revealed that the writing styles of occupational health nurses were associated with 'memo' using a few number of words and short sentences. The amount of information by this kind of recording style was considered not enough for health management situation. The possible reasons for nurses to use this writing style might be insufficient time for recording and improper designed format of health documents. Because nurses working in SSE spend more time on the roads to visit workplaces, nurses may not found enough time for recording properly within their working time. In addition, the health records were designed to focus on the frequency of nurse's performance in certain types of work rather than on the method they used to deal with health problems. In conclusion, this study suggests that some steps are necessary to develop health documents and recording system which is appropriate to occupational health nurses. The educational need for nurses on appropriate recording behavior is also recommended.

산재 환자의 의료서비스 만족도에 영향을 미치는 요인 (A Study on Contributing Factors of Industrially Injured Patients' Satisfaction)

  • Purpose: The purpose of this study was to measure patient satisfaction among injured workers and determine the factors that affect satisfaction of the hospital services. Methods: The subjects of this study were 231 injured workers hospitalized in I WC Hospital and D WC Hospital located in one of the major cities in Korea, all of whom agreed to participate in this research. The questionnaires were selected as literature suggested for explaining satisfaction of the hospital services, general characteristics of injured workers, characteristics of the hospital services. The data were analyzed with descriptive statistics, t-test, ANOVA, multiple regression analysis by SPSS/WIN 18.0. Results: The patient satisfaction level is at 3.2 point, which is lower than general patient satisfaction in previous study. There were significant differences in satisfaction scores, depending on the gender, degree of medical treatments, the specialty of medical team, staff's kindness, medical treatment process, the cleanliness of hospital and the convenience of facilities. It was found that the patient satisfaction among injured workers is affected by the characteristics of the hospital services. Conclusion: It is necessary to promote medical facilities, specialization of doctors and nurses, staff's kindness, medical treatment process and hospital cleanliness for developing Workers' Compensation hospital services.

Influence of Shiftwork on Greek Nursing Personnel

  • Background: The aim of this study was to investigate the burden experienced by nursing personnel working irregular shifts in Greece and to conduct the first test of a Greek version of the Standard Shiftwork Index (SSI). Methods: A cross-sectional survey was carried out. The SSI was completed by 365 nurses and nursing assistants working shifts, including nights. Results: Female nursing personnel and those suffering from a chronic disease were most affected by working rotating shifts as they had elevated scores on the majority of the SSI scales, such as sleep, chronic fatigue, digestive and cardiovascular problems, general health questionnaire, cognitive and somatic anxiety, shift time satisfaction, engagement and disengagement strategies, languidity, flexibility, and neurotisicm. Nurses with longer working experience and those with family responsibilities also scored higher on some of the SSI scales, such as the sleep, shift time satisfaction, social and domestic disruption, disengagement strategies, morningness, and languidity scales. Conclusion: Shiftwork affects female nurses, those with chronic disease, older age, and domestic responsibilities more severely. Therefore management should take these factors into account when designing work schedules to alleviate the burden caused by shiftwork.
