Accuracy of Self-Checked Fecal Occult Blood Testing for Colorectal Cancer in Thai Patients

  • Purpose: Colorectal cancer (CRC) screening with fecal occult blood testing (FOBT) has been associated with a reduction in CRC incidence and CRC-related mortality. However, a conventional FOBT requires stool collection and handling, which may be inconvenient for participants. The EZ-Detect$^{TM}$ (Siam Pharmaceutical Thailand) is a FDA-approved chromogen-substrate based FOBT which is basically a self-checked FOBT (no stool handling required). This study aimed to evaluate the accuracy of EZ-Detect for CRC detection. Methods: This prospective study was conducted in the Faculty of Medicine, Siriraj Hospital, Bangkok, Thailand between November 2013 and May 2014. Some 96 patients with histologically-proven CRC and 101 patients with normal colonoscopic findings were invited to perform self-checked FOBT according to the manufacturer's instructions. Results were compared with endoscopic and pathologic findings. Sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) for CRC detection were calculated. Results: The present study revealed the sensitivity, specificity, PPV and NPV of this self-checked FOBT for CRC detection to be 41% (95% CI: 31-51), 97% (95% CI: 92-99), 93% (95% CI: 81-98) and 63% (95% CI: 55-70), respectively. The overall accuracy of the self-checked FOBT for identifying CRC was 70%. The sensitivity for CRC detection based on 7th AJCC staging was 29% for stage I, 32% for stage II and 50% for stage III/IV (P=0.19). The sensitivity was 33% for proximal colon and 42% for distal colon and rectal cancer (P=0.76). Notably, none of nine infiltrative lesions gave a positive FOBT. Conclusions: The self-checked FOBT had an acceptable accuracy of CRC detection except for infiltrative tumors. This home-administrated or 'DIY' do-it-yourself FOBT could be considered as one non-invasive and convenient tool for CRC screening.

  • Kasabach-Merritt syndrome (KMS) consists of thrombocytopenia, microangiopathic hemolytic anemia, and localized consumption coagulopathy that develops within vascular hemangioma. This syndrome may also be associated with occult hemangiomas located at various sites. Tc-99m RBC scintigraphy and SPECT have proven to be reliable for confirming or excluding hemangioma. Total body blood pool imaging study during the scintigraphy also provides a means of screening for occult lesions. The authors report the case of a 29-year-old man who presented with a giant hepatic hemangioma complicated by KMS, and underwent Tc-99m RBC scintigraphy and SPECT including a total body blood pool imaging study.

  • Purpose: Porcine proliferative enteropathy (PPE), caused by the obligate intracellular bacterium Lawsonia intracellularis, is a widely distributed disease throughout the world causing substantial economic loss. In order to diagnose PPE rapidly, the rapid kit was developed and tested. Methods: In this study, a rapid kit was developed to screen the PPE rapidly at the pig farm. Also, occult blood test with fecal occult blood (FOB) kit was done for detecting the blood in pig feces which might be the evident of hemorrhagic PPE. For developing the kit, we tested fecal samples of PPE infected pigs diagnosed by polymerase chain reaction (PCR) method. Results: With the developed rapid kit, Lawsonia intracellularis was detected in high density emulsion of ileum. On the other hand, the test result of detecting Lawsonia in feces showed too high non-specific response. In addition, nevertheless the FOB test result showed that blood evident could be founded in pig feces, the diagnosing result was not fit to PCR test result, which shows blood in pig feces could be from not only hemorrhagic PPE but also many reasons. Conclusions: To deal with the PPE effectively, it will be better for farmers to screen the PPE in earlier stage with easy and rapid diagnosing tool on farm. This study found out that the rapid kit could detect the Lawsonia intracellularis and hemoglobin in pig feces. However, the non-specific response to negative samples of PPE was too high to use at a pig farm. Further research is needed for lowering the non-specific response with the rapid kit.

Noninvasive molecular biomarkers for the detection of colorectal cancer

  • Colorectal cancer (CRC) is the third most common malignancy in the world. Because CRC develops slowly from removable precancerous lesions, detection of the disease at an early stage during regular health examinations can reduce both the incidence and mortality of the disease. Although sigmoidoscopy offers significant improvements in the detection rate of CRC, its diagnostic value is limited by its high costs and inconvenience. Therefore, there is a compelling need for the identification of noninvasive biomarkers that can enable earlier detection of CRC. Accordingly, many validation studies have been conducted to evaluate genetic, epigenetic or protein markers that can be detected in the stool or in serum. Currently, the fecal-occult blood test is the most widely used method of screening for CRC. However, advances in genomics and proteomics combined with developments in other relevant fields will lead to the discovery of novel non invasive biomarkers whose usefulness will be tested in larger validation studies. Here, non-invasive molecular biomarkers that are currently used in clinical settings and have the potential for use as CRC biomarkers are discussed.

  • 대장암 검진 권고안 개정위원회는 지금까지 발표된 대장암검진 관련 국내외 임상지침 및 최신 문헌 검색을 통한 체계적 근거 평가를 통해 근거중심의 대장암 검진 개정 권고안을 제시하였다. 또한 우선 권고되는 면역화학 분변잠혈검사법의 질관리 필요성과 대장내시경검사의 이득과 위해를 평가할 수 있는 정량화된 자료의 필요성, 국내 실정에 맞는 질관리 지표의 개발, 좀 더 다양한 위험인자를 고려하는 개별적인 접근법의 필요성 등을 지적하였다.

Clinical Significance of Inflammatory Biomarkers in Acute Pediatric Diarrhea

  • Purpose: The aim of this study was to evaluate the clinical significance of inflammatory biomarkers in acute infectious diarrhea among children. Methods: Clinical parameters including fever, bacterial and viral etiology based on stool culture and multiplex polymerase chain reaction, and nine biomarkers including C-reactive protein (CRP), erythrocyte sedimentation rate (ESR) and leukocytes in blood and calprotectin, lactoferrin, myeloperoxidase, polymorphonuclear elastase, leukocytes, and occult blood in feces were evaluated in children who were hospitalized due to acute diarrhea without underlying disease. Results: A total of 62 patients were included. Among these patients, 33 had fever, 18 showed bacterial infections, and 40 patients were infected with 43 viruses. Of all the biomarkers, CRP was significantly correlated with fever (p<0.001). CRP, ESR, calprotectin, lactoferrin, myeloperoxidase, fecal leukocytes, and occult blood were significantly associated with infection with bacterial pathogens (p<0.001, p=0.04, p=0.03, p=0.003, p=0.02, p=0.03, p=0.002, respectively). The combination of CRP and fecal lactoferrin at their best cut-off values (13.7 mg/L and $22.8{\mu}g/mL$, respectively) yielded a sensitivity of 72.2%, and a specificity of 95.5% for bacterial etiology compared with their individual use. Conclusion: Blood CRP is a useful diagnostic marker for both fever and bacterial etiology in acute pediatric diarrhea. The combination of CRP and fecal lactoferrin yields better diagnostic capability for bacterial etiology than their use alone for acute diarrhea in children without underlying gastrointestinal disease.

오래된 혈흔 시료에 대한 혈청학적 식별법의 비교 연구 (Comparative study of serological detection methods on old bloodstain samples)

  • 혈액은 범죄현장에서 가장 흔하게 볼 수 있는 체액 중 하나이며 용의자를 확인하고 사건 현장을 재구성하는데 중요한 역할을 한다. 혈액의 혈청학적 식별법은 법과학 분야에서 널리 사용되고 있지만, 오래된 혈흔에 대한 혈청학적 식별법의 비교에 관한 연구는 거의 수행되지 않았다. 본 연구의 목적은 오래된 혈흔의 다양한 식별법을 비교하고 그 유효성에 대한 정보를 제공하는 것이다. 본 연구에서는 Tetramethylbenzidine, $Bluestar^{(R)}$, Leucomalachite Green, Kastle-Meyer의 네 가지 혈액 예비시험과 Fecal Occult Blood (FOB)와 Rapid Stain $Identification^{(TM)}-Blood$ ($RSID^{TM}-Blood$) 두 가지 혈액 확증시험을 사용하여 비교연구를 수행하였다. 혈흔 시료는 최소 7년부터 최대 30년동안 실온에서 거즈에 보관된 부검 혈액을 사용하였다. 실험 결과 네 가지 예비 실험에서는 30년까지 경과된 시료를 포함하여 모든 시료에서 양성반응으로 관찰되었다. 반면에 FOB와 $RSID^{TM}-Blood$는 17년 이상 경과된 일부 시료(1988년부터 2001년)에서 음성이 나타나 오래된 혈흔 시료의 경우 확증시험에서 음성반응이 나타날 수 있음을 확인하였다. 이러한 연구 결과는 오래된 증거물에 대한 혈청학적 감정에서 적절한 혈액 식별법 선정에 도움이 될 수 있을 것으로 기대한다. 더불어 법과학적인 감정분야에서 활용할 때 유용한 배경지식이 되리라 사료된다.

교정치료 4단계 동안의 구강 내 환경의 변화 (Changes in the oral environment during four stages of orthodontic treatment)

  • 교정치료 중 구강 내 환경 변화를 파악하기 위하여 교정치료 시작 후 24개월까지의 임상적 구강위생지수 및 타액, 구강 내 미생물의 변화양상을 평가하였다. 30명의 환자를 대상으로 하여, 임상적 구강위생지수(DMFS [decayed, missing and filled surfaces], O'Leary's plaque index, plaque pH), 타액의 변화(비자극/자극시의 타액량, 완충능(buffer capacity), pH, 잠혈(occult blood)류량, 구강 내 미생물(Streptococcus mutans, Lactobacillus) 변화를 파악하였다. 추가적으로 설문지를 이용하여 구강위생관리 및 식습관을 평가하였다. 분산분석, 회귀분석 및 Spearman의 상관분석을 통해 데이터를 분석하였다. 교정치료 도중 DMFS 값은 유의성있게 증가한 반면, plaque index는 감소하였고, plaque pH는 산성을 나타내었다 (p = 0.23). 비자극 시 타액량은 치료 중 유의성있게 변화하였으며 (p = 0.13), 자극 시의 타액량은 여성에서 장치 부착 이후 증가하는 양상을 보였다. Buffer capacity는 남성에서 치료 도중 감소되었으며, 타액의 pH는 유지되었다. 구강 내 미생물 및 잠혈류량은 고위험 수준까지 증가하였으며 성별의 차이는 나타나지 않았다 (p > 0.05). 치료 전 plaque와 구강위생용품의 사용 (r = 0.429; p = 0.018), 최종 DMFS와 비자극 시의 타액량 (r = -0.372; p = 0.043) 간에 뚜렷한 상관관계가 확인되었다. 교정장치의 부착으로 인해 구강 내 미생물은 증가하고, plaque pH는 산성화되어 치주조직의 손상이 일어나게 되었다. 또한 Buffer capacity는 변하지만, 치료 기간중의 타액의 pH는 유지되었다.

대장암 검진 행동 촉진을 위한 메시지 전략 (Message Strategy to Promote Colorectal Cancer Screening Behavior)

  • 본 연구는 대장암 검진 정보 콘텐츠에 대한 효과적, 설득적 메시지 유형을 확인하기 위한 목적으로 시도된 비동등성 대조군 사후설계의 유사실험 연구이다. 연구대상자는 일개 산업장 근로자 176명이었으며, 메시지 프레이밍(이득, 손실 및 일반메시지)에 따라 분류된 대상자에게 차별적으로 중재프로그램을 시행하였고, 실험중재 후 대장암 검진 관련 건강신념 및 iFOBT(immunoassay Fecal Occult Blood Test) kit 수거률을 확인하였다. 자료분석은 SPSS 21.0 program을 이용하여 $x^2$ test 및 one-way ANOVA 통계방법을 활용하여 분석하였다. 연구 결과 대장암 검진 건강신념은 손실메시지군에서 지각된 민감성 및 심각성이 높았으며, 이득메시지군에서 지각된 유익성이 높았다. iFOBT 수거률은 손실메시지군에서 높은 것으로 나타났다. 이는 대장암 검진을 주제로 하는 암정보 콘텐츠를 구성 하는데 유용한 이론적 근거가 될 것으로 기대되며, 메시지 유형에 따른 실질적 효과를 사전에 예상하고, 제시함으로써 실무적 도움 역시 제공할 수 있을 것이라고 생각된다.
