Effects of Fermented Lotus Extracts on the Differentiation in 3T3-L1 Preadipocytes (3T3-L1 전지방세포에서 연잎-연근 혼합 발효물의 지방세포 분화 억제 효과)

  • Objectives: This study was performed to evaluate the effects of fermented lotus extracts on the inhibition of differentiation in 3T3-L1 preadipocytes. Methods: Extracts of lotus leaf and lotus root were fermented using 4 different probiotics separately, including Lactobacillus plantarum, Lactobacillus rhamnosus, Bifidobacterium breve, and Bifidobacterium longum. Inhibition of preadipocyte differentiation was examined by Oil red O dye staining. Expressions of adipogenic transcription factors including CCAAT/enhancer binding proteins (C/$EBP{\alpha}$) and peroxisome proliferators-activated receptor ${\gamma}$ ($PPAR{\gamma}$) were analyzed by real time polymerase chain reaction and Western blotting analysis. Results: Fermented lotus extracts inhibited adipogenic transcription factors by inhibiting preadipocytes differentiation. All of the groups fermented by 4 kinds of probiotics showed reduction in Oil Red O dye staining. Bifidobacterium breve showed the most effective inhibition of C/$EBP{\alpha}$. Bifidobacterium breve and Bifidobacterium longum showed the best downregulation of $PPAR{\gamma}$ expressions compared with the control and the unfermented lotus group. Conclusions: Fermented lotus extracts showed significant effects on inhibition of preadipocyte differentiation in 3T3-L1 preadipocytes showing correlation with insulin sensitivity and lipid metabolism related with obesity.

A Case Report on Laryngopharyngeal Reflux Combined with Depressive and Anxiety Disorder (우울, 불안증상을 동반한 인후두역류증 환자 한방치험 1례)

  • Objective: This study was performed to report the effect of traditional Korean medicine as a treatment for laryngopharyngeal reflux (LPR) combined with depressive and anxiety disorder. Methods: We treated this patient with traditional Korean medicine and measured symptom severity using the reflux symptom index (RSI), Beck depression inventory (BDI), and Beck anxiety inventory (BAI). Results: After 3 weeks of treatment, most symptoms had decreased. The RSI score dropped from 21 to 8, BDI from 27 to 14, and BAI from 29 to 15. Conclusions: Traditional Korean medicine may be effective as a treatment for LPR combined with depressive and anxiety disorder, and a correlation may exist between LPR and psychological factors. However, more rigorous studies are required to identify exactly what treatment is most efficient for relieving LPR combined with depressive and anxiety disorder and whether LPR and psychological factors are clearly correlated.

The Anti-inflammatory Effect of Cheongyeolhawlhyeoltanggagyehyeoldeung (CYHHT) in cultured RAW264.7 cells and murine models of inflammation (RAW264.7세포주와 염증생쥐모델에서 항염증(抗炎症) 작용(作用)에 대한 청열활혈탕가계혈등(淸熱活血湯加鷄血藤)의 효과(效果))

  • Purpose : The Purpose of this research was to investigate the effects of Cheongyeolhawlhyeoltanggagyehyeoldeung (CYHHT) on anti-inflammatory effects. Methods : As for the parameters of inflammation, levels of several inflammatory cytokines and chemical mediators were determined in mouse lung fibroblast cells(mLFC) and RAW264.7 cells. Also, changes in pathological features by drug treatment were investigated in the in vivo edema-induced rats by carrageenin /arachidonic acid or in the colitis-induced mice by DSS treatment. Results : The cytotoxicity of CYHHT on mLFC and RAW264.7 cells was not observed at 100, 50, 10, and $1{\mu}g/ml$ of CYHHT treatments. $IL-1{\beta}$, IL-6 and NOS-IImRNA expression of RAW264.7 cells was inhibited by CYHHT treatments in a dose-dependent manner. CYHHT treatment of RAW264.7 cells inhibited $TNF-{\alpha}$ and COX-2 mRNA expression. CYHHT treatment of RAW264.7 cells significantly inhibited IL-6 and NO production. CYHHT treatment of RAW264.7 cells inhibited ROS production. CYHHT inhibited rat's paw edema induced by carrageenin or arachidonate treatment in all concentrations examined. The body weight and colon length of colitis-induced mice were recovered to a normal level by DSS treatment. Clinical disease levels were significantly improved compared to the control animals. CYHHT treatment of colitis-induced mice significantly increased hematological values such as WBC and RBC counts, Hgb and HCT levels, but decreased PLT values. CYHHT treatment of colitis-induced mice decreased IL-6 and $TNF-{\alpha}$ production significantly CYHHT treatment of colitis-induced mice significantly increased CD3+(T) cell counts. In contrast, CYHHT treatment decreased CD19+ B cell counts and CD3+/CD69+ significantly, and also decreased B/T ratio (%) though not significant. Conclusion : These results indicated that CYHHT could be used for treating diverse female diseases caused by the inflammation.

Anti-oxidant Effects of Samultang-Gami on MEF Cells (사물탕가미방(四物湯加味方)의 항산화 활성에 대한 실험적 연구)

  • Purpose: This experiment is designed to find out anti-oxidant effects of Samultang-Gami which was composed of Rehmanniae Radix(RR), Angelicae Gigantis Radix, Cnidii Rhizoma(CR), Paeoniae Radix(PR), Cortex Moutan Radicis, Hedyotis Diffusa(HD) and Caesalpinia Sappan on MEF cells. Methods: In vitro antioxidant effects were measured by MTT assay, DPPH assay, cell cycle analysis, AnnexinV-FITC/PI assay and DAPI staining using MEF cells treated with various concentrations of 70% ethanol extract of Samultang-Gami. Results: 1. In the scavenging for DPPH radical, the each treated groups of PR, CR and HD showed positive effects. RR and CR increased the viability of oxidative damaged MEF cells in a dose-dependent manner. 2. 70% ethanol extract of Samultang-Gami was shown best antioxidative effect in the concentration of $0.5mg/m{\ell}$. 3. The treatment of Samultang-Gami in oxidative damaged MEF cells didn't have any effect on cell cycle restoration. but it could lower late apoptosis rate a little and be observed the protection of nucleus. Conclusion: It can be concluded that Samultang-Gami, RR and CR have antioxidant effects on MEF cells.

Can Ambient Temperature Change Affect the Efficacy of Acupuncture on the Postmenopausal Hot Flash? (기온 변화는 폐경후 안면홍조의 침 치료 효과에 영향을 미치는가?)

  • Purpose: To find the correlation between ambient temperature change and the efficacy of acupuncture on the postmenopausal hot flash. Methods: 52 patients entered a randomized clinical trial which was to verify the efficacy of acupuncture on postmenopausal hot flash. Participants were treated with optimal/minimal acupuncture in the course of 13 weeks. 34 of 52 visited for the temperature-rising period(Group 1), while 18 of 52 for the temperature-falling period(Group 2), 100mm hot flash VAS and average temperature were taken 18 times from each person. Linear mixed effects model was used to find the correlation between temperature and hot flash VAS score(p-value<0.05). Group1/2, optimal/minimal acupuncture, age, age of menarche, height, weight, percent body fat, BMI, waist-hip ratio, past history of hysterectomy and HRT were also examined. Results: Period of treatment had influence on the hot flash VAS. As time went by, hot flash decreased(p<.0000). Participants who underwent minimal acupuncture showed higher hot flash VAS(p=0.0479) and as waist-hip ratio rose, hot flash increased(p<.0000). As the temperature rose, hot flash VAS score became lower(p=0.0143) and it was because 34 of 52 participants were treated for the temperature-rising period. Group 1/2 had no interrelation with hot flash(p=0.5741). Conclusion: Hot flash diminished as participants were treated with acupuncture, independently of whether the ambient temperature rises or falls.

A study on effects of Ligustri Lucidi Fructus, Ecliptae Herba and Yijihwan on antioxidant activity and blood pressure (여정자(女貞子), 한연초(旱蓮草) 및 이지환(二至丸)의 항산화활성 및 혈압에 미치는 영향에 관한 연구)

  • Purpose: Ligustri Lucidi Fructus and Ecliptae Herba has long been used for clinical therapy associated especially with menopausal symptoms in Korea. To provide a scientific rationale for such use, we have investigated the antioxidant and vasorelaxant effects of Ligustri Lucidi Fructus, Ecliptae Herba and its mixture. Methods: The antioxidant activity of the extracts from Ligustri Lucidi Fructus, Ecliptae Herba and its mixture were evaluated and compared with that of BHA (butylated hydroxyanisole), BHT (butylated hydroxytoluene), vitamin C and vitamin E, using the ${\alpha}$,${\alpha}-diphenyl-{\beta}-picrylhydrazyl$ (DPPH) radical scavenging method. Results: Antioxidant activity of all extracts using the DPPH radical scavenging method decreased in the order vitamin C>BHA>vitamin E>Ligustri Lucidi Fructus>Ligustri Lucidi Fructus:Ecliptae Herba(1:1)>Ecliptae Herba>BHT. The vasorelaxant effects of extracts were investigated on the vasomotor tone of the rat thoracic aorta in an organ bath. All of the extracts elicited along-term relaxing response in the endothelium-intact as well as endothelium-denuded rat aorta contracted with norepinephrine. This relaxant effect was abolished by Precontraction with 72 mM KCI. Thus, it is suggested that the mechanism of vasorelaxant effect of extracts not involve voltage-operated $Ca^{2+}$ channel blocking but receptor-mediated route. Conclusion: These antioxidant and vasorelaxant effecs of the extracts may contribute to the beneficial effects in postmenopausal women.

The effect of Kami-Saenghwatang and Kami-Bohertang to postpartum weight retention (가미생화탕과 가미보허탕의 연속 투여가 산후 체중정체와 체성분 변화에 미치는 영향)

  • Purpose: The purpose of this study was to investigate the relationships between the herbal medicines used for women in puerperium on postpartum care, Kami-Saenghwatang(SHT) and Kami-Bohertang(BHT) and postpartum body composition change. We also examined the influence of gestational age, the number of parity and the route of delivery for gestational weight gain and postpartum weight loss. Methods: 239 women followed by postpartum care center. Variety factor s were conformed and then checked body composition analysis. 8 weeks later, we followed up body composition of 36 women among the former checked. and additionally asked the body weight of another 42 former checked women by telephone call. Results: $Mean{\pm}S.D$(standard deviation) of pre-pregnancy weight is $55.22{\pm}8.98kg$, $Mean{\pm}S.D$ of gestational maximun weight is $69.26{\pm}9.77kg$, $Mean{\pm}S.D$ of weight loss in postpartum 8 weeks(means sub-tract postpartum 1 week weight from postpartum 8 week weight) is $57.86{\pm}8.60kg$. The number of parities had corrrelation with gestational weight gain. Age, number of parities and route of delive were not significantly correlated with postpartum weight loss during 8 weeks after delivery, and also it was not correlated with body water mass, body fat mass, body mass index and body weight on postpartum 8 weeks whether they treated with SHT and BHT. conclusion: It was not correlated with body water mass, body fat mass, body mass index and body weight on 8 weeks postpartum whether they treated with SHT and BHT.

A Study on Menstrual Symptoms and Pattern according to the Sasang Constitution in High School Girls in Pusan (여고생의 사상체질(四象體質)에 따른 월경양상에 관한 연구)

  • Purpose : The purpose of this study was to investigate menstrual symptoms and patterns of high school girls by Sasang constitution and relationship between such symptoms and Sasang constitution. Methods : For the study, QSCCII and 'M.B.Q' employed to analyze the data collected from 581 high school girls in Pusan from July 11 to July 20, 2006. Results : As for menstrual characteristics of the subjects by Sasang constitution Taeeumins start menarche is relatively early in contrast with Soeumins who experienced menarche relatively late(P=.012). This survey proves that menses last relatively long for Soeumin whereas Soyangins have relatively short period of menstruation(P=.043). Furthermore, Taeeumins account for majority of those who have much quantity of menstruation with 22.7% while Soeumins account for majority with 13.2%. From the analysis into menstrual symptoms of the subjects put under 5 categories of Sasang constitution, it became evident that Soeumins turned out to have most negative affect(11.8${\pm}$3.78) (P=.053). Soeumins scored the highest points in the response area of autonomic reactions(10.6${\pm}$3.24)(P=.014). In the total points of menstrual symptoms including the above 5 sub-categories, Soeumins recorded the highest scores(52.4${\pm}$12.5), followed by Soyangins(50.0${\pm}$12.2) and Taeeumins in order, showing that there is no statistically significant difference(F = 2.859, p = .058) among them although Soeumins seem to feel slightly more menstrual symptoms than Soyangins and Taeeumins. Conclusion : We hope these results could be helpful to diagnosis and treatment of mestrual diseases.

A Study on the Methodology of Acupuncture Clinical Trial on the Postmenopausal and Perimenopausal Hot Flashes (갱년기 안면홍조에 대한 침 임상시험 방법론 연구)

  • Purpose: In spite of many arguments on the result of WHI (Women's Health Initiative) study, no one can deny the necessity for researches on the alternative treatment to HRT (hormone replacement therapy). In this study, the author wanted to investigate the method of precedent acupuncture RCTs (randomized controlled trials) to make out the appropriate acupuncture study design on postmenopausal and perimenopausal hot flashes in Korea. Methods: Precedent studies were investigated using Pubmed search and key-words "acupuncture and hot flash", "acupuncture and menopause", "acupuncture and vasomotor", limited to RCT, from 2000 to 2008 April. Results: As a result. 10 studies were searched. In the hereafter studies, multi-center clinical trials which consist of population group of postmenopausal and perimenopausal women that would be pre-stratified and more than 50 patients per treatment arm seem adequate. Sham control study can make out the proper consequence because many people are get used to acupuncture in Korea. Flexible choice of acupoints addressed an individual's symptoms using standardized algorithm is recommended. Treatment consist of 4 weeks' observation, 11 acupuncture sessions during 7 weeks, follow-up of 3 months or more after treatment and hot flash score as a primary outcome measure seem appropriate. After all, higher level of description according to global standard must be obtained in the study report and publishing. Conclusion: The researchers should develope the methodology of acupuncture clinical trial on the postmenopausal and perimenopausal hot flashes.
