A Study on Dental Hygienists' Needs for Specialized Education in Health Promotion Projects (보건분야 치과위생사들의 건강증진사업을 위한 전문교육 필요도 조사)

  • The purpose of this study was to analyze dental hygienists' opinions on health promotion projects in public health and their needs for specialized education in the projects, so that it could help prepare specialized education program required for health promotion projects in the future. To meet the goals, total 364 dental hygienists working in public health were asked to join a questionnaire survey via e-mail in September 2007 for data analysis. As a result, this analysis came to the following conclusions: First, it was found that 29.9% of total respondents took in charge of health promotion projects, and 26.9% respondents considered it very necessary to be responsible for the health promotion projects(56.0% considered it necessary). Almost half respondents considered nonsmoking assistance most desirable task(48.4%) and foremost demanded(50.5%) out of all current health promotion projects, 39.8% respondents answered that they could carry on health promotion task, if assigned, after completing occupational training course. Second, it was found that 39.3% respondents acquired other licenses and qualifications than certificate of registered dental hygienist, and their medical licenses or qualifications included certificate of nursing assistant(18.7%) and certificate of social worker(11.8%) by category. Third, in terms of opinions on possible ways to facilitate health promotion projects, it was found that over half respondents considered it recommended to step up specialized education(56.9%) and prepare legal basis and administrative system(53.6%). Notably, 90.7% respondents considered it necessary to step up specialized education in health promotion. Fourth, in terms of opinions on participation in health promotion projects, it was found that almost half respondents(49.7%) considered it necessary and very positive for extending dental hygienists' works in the future. Moreover, in terms of reasons for sum positive answers, it was found that many respondents considered it helpful for appointment(41.8%) and contributing to capability development and job satisfaction(44.5%). In terms of opinions that sud, participation would be unnecessary, 29.7% respondents thought that nothing would be changed in their dental health projects even with reduced number of public health dentists, and 31.3% respondents thought that dental hygienists are professional manpower responsible only for dental health works. Finally, in terms of on-the-job training(OJT) related to health promotion, it was found that 92.9% respondents desired for OJT. In terms of experiences in OJT, 79.9% respondents answered that they never joined OJT course. In other words, only 20.1% respondents joined OJT courses for health promotion task, such as nonsmoking assistance(8.8%), health promotion FMTP training(2.2%), exercises(1.4%) and nutrition(0.6%).

Satisfaction with Health Care in North Korea: A Study of North Korean Refugees in China (중국내 북한이탈주민을 통해 본 북한의료이용 만족도)

  • The aim of the study was to examine levels of satisfaction with health care in North Korea and to identify factors associated with it using a convenience sample of North Korean refugees in China. Data from the 2004 Survey of Health Seeking Behavior of North Korean Households conducted by the Center for Refugee and Disaster Response, Johns Hopkins Bloomberg School of Public Health were used. The study subjects were 273 North Korean refugees whose length of stay in China was less than 3 months. Factor analysis was used to extract factor dimensions from the 12 satisfaction items. Bivariate (t test and ANOVA) and multiple regression analyses were used in examining factors associated with satisfaction with health care use in North Korea Overall, satisfaction level was low ($2.36{\pm}0.36$, score range: 1-5). Of the three-factor dimensions, physician skills scored the highest $(2.93{\pm}0.36)$, followed by drug availability $(2.51{\pm}0.07)$ and general cleanliness $(1.66{\pm}0.55)$. In the multiple regression analysis, having a usual source of care was significantly associated with patient satisfaction. Respondents who identified primary care (section) doctors as their usual source of care tended to be less satisfied than those with the city or county hospital as their usual source of care. County residents tended to report a lower degree of satisfaction with general cleanliness than city residents. Among socioeconomic characteristics, the number of household assets positively predicted satisfaction with drug availability. North Korean residents appear to be dissatisfied with their medical care. It may reflect some inadequacies in the North's universal health care system to meet the healthcare needs of its people.

Effect of a Death Preparing Education Program on Death Anxiety and Meaning of Life in Volunteers (죽음준비교육 프로그램이 자원봉사자의 죽음불안과 삶의 의미에 미치는 효과)

  • Purpose: The purpose of this study was to analyze the effect of death preparing education on death anxiety and meaning of life for volunteers. Methods: Data collection and intervention were carried out from January 11 to 25, 2009. The subjects included 60 volunteers in Jeonju city, and they were divided into two groups; 30 each of experimental group and control group. Death preparing education program contained 5 steps. Data were analyzed with t-test, $x^2$-test, and ANCOVA test with SPSS version 12.0. Results: The death anxiety scores in the experimental group were significantly lower than in the control group (F=4.01, P=0.046). The meaning of life scores in the experimental group were significantly higher than in the control group (F=6.32, P=0.015). Conclusion: The death preparing education program for volunteers was confirmed to be an effective intervention to lessen death anxiety and to improve the meaning of life. Therefore, I strongly recommend that this program should generously be applied to volunteers.

Models for Spiritual Care in Hospice and Palliative Care

  • Spirituality is an essential part of human beings. Spiritual care, designed to meet the spiritual needs of terminally ill patients and their families, is one of the most important aspects of hospice and palliative care (HPC). This study reviewed and analyzed literature utilizing the most commonly used Korean and international healthcare databases to identify care models that adequately address the spiritual needs of terminally ill patients and their families in practice. The results of this study show that spirituality is an intrinsic part of humans, meaning that people are holistic beings. The literature has provided ten evidence-based theories that can be used as models in HPC. Three of the models focus on how the spiritual care outcomes of viewing spiritual health, quality of life, and coping, are important outcomes. The remaining seven models focus on implementation of spiritual care. The "whole-person care model" addresses the multidisciplinary collaboration within HPC. The "existential functioning model" emphasizes the existential needs of human beings. The "open pluralism view" considers the cultural diversity and other types of diversity of care recipients. The "spiritual-relational view" and "framework of systemic organization" models focus on the relationship between hospital palliative care teams and terminally ill patients. The "principal components model" and "actioning spirituality and spiritual care in education and training model" explain the overall dynamics of the spiritual care process. Based on these models, continuous clinical research efforts are needed to establish an optimal spiritual care model for HPC.

The related factors of adolescent smoker's cancer preventive behaviors (흡연 청소년의 암 예방행위 관련요인 (일개지역 금연학교 참여자를 대상으로))

  • This study was conducted to identify the related factors of cancer preventive behaviors for adolescent smokers. Data were collected from September 1st 2012 to March 31st 2013 of 200 adolescent smokers who were participating in smoking cessation school at W hospital in B city. As a results, there were significant differences in cancer preventive behaviors by grade, religion, benefit, barrier, perceived health status, daily living, alcohol, exercise, nicotine dependency, first smoking age, smoking duration and non-smoking intention. Cancer preventive behaviors were weakly negative related to knowledge about cancer, And there is weakly positive correlation between cancer preventive behaviors and attitude about cancer. Meaningful variables which explain cancer preventive behaviors were daily living, barrier, grade, exercise, benefit, knowledge about cancer, smoking cessation intention in order. Total explanation power was 30.6% and daily living has most explanation power. In conclusion, It is necessary to encourage sound hobbies and exercise instead of smoking for adolescent smokers in their daily living. And health education programmes, based on the grade and smoking cessation intention of adolescent smokers, need to be developed to emphasize the benefit of cancer preventive behaviors, and to decrease barriers against those behaviors.

Effects of Personality Virtues Education Program Using Visual Media for College Students (영상매체를 활용한 전문대학생들의 인성덕목 교육프로그램 효과)

  • The purpose of this study was to evaluate the effects of personality virtue education program using visual media for college students. This program was run in parallel with "Educational Psychology" lecture from August 30th through December 4th, 2015. The subjects of study were 348 freshmen of nursing department in J college. The methodology of this study was nonequivalent pretest-posttest control group experimental design. The researcher carried out pretest for experimental group and control group both before program execution by KEDI Personality Inventory and repeated the test afterwards to measure the effects of the program. The data analysis was implemented by SPSS 22.0. The ANCOVA was used to verify the hypothesis. The result showed that all personality virtues(self-respect, integrity, consideration communication, responsibility, etiquette, self-control, honesty courage, wisdom, justice, citizenship) scores were improved significantly after the program. In conclusion, virtue-centered personality education can make students realize and internalize the value of virtues and have motivation and will to act, when they empathized with visual media on the various situation and learned repeatedly in integrated curriculum.

Subjective Health Status, Attitude toward Death and Spiritual Well-being of Nurses (간호사의 주관적 건강상태, 죽음에 대한 태도 및 영적 안녕)

  • The purpose of this study is to identify the level and correlation of subjective health status, attitude toward death and spiritual well-being of nurses. The subjects were 338 nurses in two university general hospitals located in Seoul and Gyeonggi area. Data were collected using a structuralize questionnaires regarding subjective health status, attitude toward death and spiritual well-being from March to May in 2013. Data were processed with SAS for Windows statistics program. For analysis, t-test, ANOVA, and Pearson's correlation coefficients were performed. Nurses had negative attitudes toward death according to age, marital status, years of employment and job title, while spiritual well-being was different according to age, marital status, religion, education, years of engagement in clinical works, department, and job title. The higher subjective health of nurses was, the more positive their attitudes toward death were. The higher subjective heath of nurses was and the more positive their attitudes toward death were, the higher their spiritual well-being was. This study identified the relationship between the attitudes toward death and spiritual well-being and the health status subjectively recognised by nurses, and it is meaningful in that this study prepared basic data for development of an education program for spiritual nursing care or terminal care.

  • Purpose: The purposes of this study were to identify the types of widow's bereavement experience and understand the nature of it's bereavement by using Q-methodological approach. Methods: Contents Q sample included 46 statements obtained from literatures and interviews with 5 widows. P sample consisted of 13 widows who bereaved within 2 years. The data were collected from October 2004 to December 2006 and analyzed using Quanal program. Results: Two types of widow's experience were found. Type 1 was characterized by loss suffering, and type 2 was characterized by acceptance and adaptation Conclusion: Widows were found to experience different types of bereavement. Therefore, bereavement care team should assess the types of suffering pain and provide appropriate care to the widows. Also, need to be developed programs to relieve or prevent suffering of bereavement.

Antioxidative Effects of Cornis fructus Extract on Chromium Trioxide Toxicity (Chromium Trioxide의 독성에 대한 산수유 추출물의 항산화 효과)

  • This study examined the antioxidative effects of Cornis fructus (CF) extract on cultured C6 glioma cells. For this purpose, cytotoxicity analysis of chromium trioxide ($CrO_3$) was performed and the protective effects of the CF extract on $CrO_3$-induced cytotoxicity was examined after the C6 glioma cells were cultured for 48 hours. The antioxidative effects, such as electron donating activity (EDA) and lactate dehydrogenase (LDH) activity were also analyzed. In this study, $CrO_3$ decreased the cell viability in a dose dependent manner. The $XTT_{50}$ value was determined to be $33{\mu}M$ after the cells were treated for 48 hours at a concentrations of $20{\sim}40{\mu}M$ $CrO_3$. The catalase (CAT) antioxidant increased significantly the cell viability that had been decreased by $CrO_3$-induced cytotoxicity. Regarding the protective effect of the CF extract, the cell viability of the CF extract was increased significantly compared to that of $CrO_3$ only. In addition, the CF extract showed antioxidative effects, such as EDA and an inhibitory effect on the LDH activity. These findings suggest that the cytotoxicity of $CrO_3$ is correlated with oxidative stress, and the CF extract effectively prevented $CrO_3$-induced cytotoxicity through the antioxidative effects. In conclusion, natural products, such as the CF extract may be a useful therapeutic agent for the prevention or treatment of toxicity induced by heavy metals via oxidative stress.

Effects of Camping Programs on Self-efficacy and Sick-role Behavioral Compliance in Juvenile Diabetes Mellitus Patient (소아 당뇨 환자의 캠프 프로그램이 자기효능과 환자 역할 행위이행에 미치는 영향)

  • 당뇨병은 고혈당을 조절 안할 경우에는 돌이킬 수 없는 신체적 합병증은 물론 정신적 손상까지 받지만, 고혈당은 노력만 하면 조절이 가능하며, 정상에 가까울 정도로 조절되면 정상인과 같은 삶을 영위할 수 있다. 그러나 보건의료 전문가의 인슐린 주사 요법, 식사 및 운동요법 등의 일방적인 처방만으로 성공적인 당조절을 기대하기에는 어려움이 있다. 그래서 당뇨병 환자에게 당뇨병이라는 것이 무엇이며, 어떻게 이 병을 스스로 조절할 수 있게 만드느냐가 더욱 중요한 것이다. 따라서 우리나라에서도 당뇨병에 대한 관심이 높아짐에 따라 각 지역과 의료기관마다 다양한 당뇨병 교육 프로그램이 실시되고 있다. 특히 소아 당뇨병 환자는 이러한 전통적인 당뇨병에 관한 교육을 통해서 당뇨병에 관한 지식은 많이 가지고 있지만, 당뇨병 자가 조절을 위한 환자 역할 행위(sick-role behavior)를 변화시키는데는 그다지 성공절이라 할 수 없다. Bandura(1977)는 자기 효능은 ‘할 수 있다는 자신감’으로 수행 성취, 대리 경험, 언어적 설득, 정서적 각성에 관한 정보에 의해 결정된다고 하였다. 따라서 이러한 정보들은 자기 효능을 증진시키기 위한 중재의 방향을 제시해 준다. 오늘날 이러한 정보를 활용한 중재 프로그램 중에서 가정과 학교를 떠나 자연 환경 속에서의 집단 활동을 통하여 사회 학습 경험을 하는 캠프 프로그램에 대한 관심이 높아지고 있다. 캠프에 참여한 당뇨병 아이들은 캠프 활동속에서 다른 동료 아이들이 자신이 갖고 있는 문제들을 성공적으로 해결해 나가는 것을 관찰하여 대리 경험하게 될 때 희망을 가지게 되며, 당뇨병을 가진 다른 동료들과 공통의 경험을 공유할 수 있는 기회를 갖게 되어 그들은 자신이 더 이상 혼자가 아니며 남과 ‘다르지’도 않다는 것을 깨닫게 되어 점차 자신감을 가지고 살아갈 수 있게 된다. 본 연구는 캠프 프로그램이 소아 당뇨병의 자기 효능을 증진시키고, 당뇨병 환자 역할 행위 이행 정도를 높여주는지를 규명해 봄으로써, 소아 당뇨병 환자를 위한 효과적인 간호 중재 방안을 제시하고자 비동등성 대조군 전후 실험 설계의 유사 실험 연구를 시도하였다. 1996년 8월 10일 부터 12월 12일까지 종합병원에서 추후 관리하고 있는 소아 당뇨병 환자중 선정 기준에 맞는 환자 41명을 연구대상으로 하였으며, 그 중 19명은 실험군으로 Bandura의 자기 효능 증진을 위한 정보원을 활용한 캠프 프로그램을 5박 6일간에 걸쳐 실시하였고, 다음 22명은 대조군으로 캠프 프로그램을 실시하지 않았다. 자료수집은 자기 효능 척도와 환자 행위 역할 이행 척도를 캠프 프로그램을 실시하기 전에 사전 조사를 하고 중재 후 4주째 사후 조사를 하였다. 수집된 자료는 SPSS/PC+로 Chi-square test, t-test, ANCOVA, Pearson correlation을 이용하여 분석하였으며, 그 결과는 다음과 같다. 캠프 프로그램은 소아 당뇨병 환자의 자기 효능을 증진시키고 환자 역할 행위 이행을 높여주는데 효과적 이었다. 소아 당뇨병 환자의 자기 효능은 환자 역할 행위 이행과 순 상관 관계가 있어, 자기 효능이 증진될수록 환자 역할 행위 이행 정도가 높아졌다.
