Questionnaire Survey on the Physical Environment of Child Care Centers for Children with Disability (장애아 보육시설의 물리적 환경에 대한 설문조사연구)

  • Eater Seal, the child care experts, observes that children with disability significantly benefit from receiving appropriated care along side their peers. Typically developing children serve as role models from which children with disabilities learn age-appropriated communications and social behavior. Therefore the children with disability can actively participate in well-designed child care settings. But unfortunately in Seoul, Korea, only 2% of children with disability are taken care of in child care centers. And even the disabled children at the child care centers are mostly mentally or emotionally disabled because the building, programs and services of the centers are restrictive to the children with severe physical disabilities. This study reviewed Korean domestic physical environment of child care centers for children with disabilities. Questionnaire survey was conducted to 103 centers located in Seoul by mail. Our survey revealed that they need more areas for nursing room and special rooms and the accessibility to buildings has to be improved. Also, there are no elevators in all buildings surveyed. Accessibility to toilet is noted as significant problem, especially wet floors in toilets. In a nursing room, a space for psychological rest and special education is needed for children with mental disability. Technical specifications on how to make buildings and facilities accessible for the children with disability should be developed. The goal of this study is to provide basic information to develop domestic design guidelines to ensure that the child care centers are safe, convenient, and usable for everyone possible.

The Effect of Care-workers Work Environment on Organizational Commitment: The Mediation Effects of Job Satisfaction (요양보호사의 직무환경이 조직몰입에 미치는 영향: 직무만족의 매개효과 검증)

  • The purpose of this study is to identify the effect of the job environment of nursing care workers on organizational commitment and to verify the mediating effect of job satisfaction based on this. Data collection was conducted on 340 nursing care workers, centering on elderly care facilities in Gyeonggi-do, and the uncollected questionnaires and abnormalities were removed and the final 267 were selected as the subjects of the study. Data analysis results were derived through SPSS WIN 2.0 and PROCESS Macro V3.5, and Hayes (2013)'s Macro Model Number 4 was applied to verify the mediating effect of job satisfaction. As a result of the analysis, the partial mediating effect of the job satisfaction was confirmed in the effect of the job environment of the caregiver on organizational commitment. Based on the analysis results, practical and policy suggestions for improving the job environment and increasing job satisfaction of nursing caregivers, and follow-up studies were proposed to overcome the limitations of this study.

Manpower Acquisition Strategies of Long-term Care: Focused on Nurses and Nurse Aides (노인장기요양인력 확보 방안: 간호사와 간호조무사 중심으로)

  • Purpose: The purpose of this study was to identify manpower acquisition strategies of nurse and nurse aide in long-term care Method: The study was proceed using following three stages: to explore workforce status and labor environment with database analysis, literature reviews, to explore the best practices, practical experiences with expert interviews, with focus group interview, to identify problems and present acquisition strategies Results: Acquisition strategies were increasing of pay, improvement of working condition, development of practical educational program, diversification of entry paths, establishing clear roles for nursing staffs, diversification of training program. Conclusion: We found that the acquisition strategies of nurse and nurse aide can be as an useful methods to increase manpower in long-term care for higher quality of services and care.

Development of Evaluation Indicators for Integrated Home Care (통합가정방문급여 평가지표 개발)

  • Purpose: The purpose of this study was to develop integrated evaluation indicators of home care services in the hope that the increasing group of long-term home care patients could receive quality care services. Methods: The development involves a methodological study on a development phase and a verification phase. The main survey at a verification stage was conducted by the staffs at 146 institutions who agreed to participate on this study. Results: The evaluation index for the integrated home visit care consisted of five categories and 57 indicators including Managing Institution (12), Environment and Safety (3), Right and Responsibility (7), Process of Care (31) and Results of Care (4). The criterion-related validity was verified in regard to the participation in the 2010 evaluation of long-term home-care institutions by the National Health Insurance Corporation. Conclusion: The evaluation index of the integrated home visiting care developed in this study is considered suitable to utilize as evaluating indicators in managing and evaluating the way of how institutions integrate and provide home visit care services as well as home nursing care services.

Predictors of Person-Centered Care among Nurses in Adult Intensive Care Units (성인 중환자실 간호사의 인간중심간호 수행과 영향요인)

  • Purpose: The purpose of this study was to examine the level of Person-centered Critical Care Nursing (PCCN) and the factors influencing PCCN for nurses in Intensive Care Units (ICU). Methods: This study was designed by cross-sectional descriptive correlational study. The participants included 147 ICU nurses in two general hospitals in Seoul, Korea. Demographic characteristics, PCCN, communication skills, professionalism, and work environment were measured. The collected data were analyzed using descriptive statistical analysis, independent t-test, One-way ANOVA, Pearson's correlation coefficient, and stepwise multiple linear regression with the SPSS/Win 25.0 program. Results: The average age of the participants was 29.6±4.7 years and the mean work experience in the ICU was 4.67±3.52 years. The level of PCCN was 3.70±0.41, which was moderate to high, and it significantly showed a positive correlation with therapeutic communication skills (r=.66, p<.001), global interpersonal communication competence (r=.42, p<.001), professionalism (r=.38, p<.001), and work environment (r=.16, p=.048). The factors influencing PCCN were identified as therapeutic communication skill and global interpersonal communication competence (Adj R2=.45, p<.001). Conclusion: The findings of this study were confirmed that the strategies to promote PCCN are necessary to enhance therapeutic communication skill and global interpersonal communication competence. In addition, they may be particularly meaningful in providing basic data for nursing education and future intervention development research to promote PCCN for the ICU nurses. For improving PCCN for healthcare providers in ICU, further studies should be conducted to develop education and intervention programs.

Job Analysis of the Staff Nurse in Cardiac Surgery Intensive Care Unit (심장외과 중환자실 일반간호사의 업무분석)

  • Purpose : This study was conducted to provide for a basic resource, which can be used to set up a efficient management system in Cardiac Surgery Intensive Care Units(CSICU). Method: 1) Questionnaires were administered and observation methods were used, to examine the nursing activities performed in the CSICU after having reviewed related literatures and a review by the experts. Thus, the nursing activities were designating 254 activities and classified into 28 categories. 2)The 22 nurses in the 2 CSICUs filled out questionnaires about nursing activities from 12 April, 2002 to 17 April, 2002. The frequency of the nursing activities in the 28 categories counted and new nursing activities added by directly observing 12 nurses by two trained research staffs for 4 day. 3)In terms of validity, the 264 nursing activities were analysed by the 25 experts. As a result, 231 nursing activities were found valid and remained as appropriate nursing activities to be used for the careful analysis of the nursing activities in CSICUs. Result: The 22 categories are as below: assessment, monitoring, respiration management, nutrition management, elimination/drainage management, mobility management, sanitation management, safety management, temperature management, specimens collection, preparation and assistance of treatment, skin/wound management, infection management, medication management, education/support, dying patient care, recording/keeping, supplies management, environment management, communications, evaluations, professional development Conclusion : The manifest job description of the staff nurse will contribute to improving the efficiency of the nursing activities and to reducing the role conflicts among the medical staffs.

Effects of General Hospital Nurses' Work Environment on Job Embeddedness and Burnout (종합병원의 간호업무환경이 간호사의 직무배태성, 소진에 미치는 영향)

  • Purpose: The aim of this study was to investigate and identify work environment, job embeddedness, and burnout among general hospital nurses in Korea. Methods: The participants were 563 clinical nurses working in 13 general hospitals across the country. Data were analyzed using SPSS and Microsoft Excel programs. Results: Mean scores were 2.62 for nurses' work environment, 2.97 for job embeddedness, and 3.61 for burnout. Nurses' work environment showed a positive correlation with job embeddedness (r=.70, p<.001), but a negative correlation with burnout (r=-.49, p<.001). Subcategories of nurses' work environment that predicted job embeddedness included satisfaction and happiness, hospital support for the work environment, patient care environment, satisfaction with work schedule, manager leadership, supportive environment for nurses' work, and computer problems. Subcategories of nurses' work environment that predicted burnout included satisfaction and happiness, violence within ward, hospital support for work environment, and patient care environment. Conclusion: Findings from this study indicate the need to evaluate and improve the work environment for nurses to increase job embeddedness and control burnout. Future studies should explore ways in which turnover intention can be decreased by changing nurses' work environment.

Conecpt Analysis on Woman Health (여성건강에 대한 개념분석 연구)

  • In order to help the theory development for "women health", and to apply to the women client care, the concept analysis of "women health" was done. According to the concept analysis method by Walker & Avant, the characters of concept "women health" were analyzed from literature. The results were as followings : first, it emphasizes reproductive focused health care(childbirth-oriented) and normal development process regarded as abnormal. Second, it is affected by cultual environment. Third, it is related with sex. Forth, it considers the human, feminine and maternal aspects. Fifth, it need woman-centered nursing approach to promote woman's quality of life. Sixth, it stresses not only wholistic human health promotion but also the harmony between human and environment. Seventh, it is needed to be considered woman's various experiences as important and it approaches phenomenogically and interdisciplinary. Eighth, it needed to understands person-situation interactional aspect. Ninth, it can promote the adaptation power to surrounding environment. Above results can elucidate the concept of "women health", and will be a basis of theory development of "women health" In practical nursing, these results will be a basis for the promotion of women health and for the increase of life quality.

Relationship between Hospital Nurses' Perceived Patient Safety Culture and Their Safety Care Activities (병원 간호사가 지각하는 환자안전문화와 안전간호활동과의 관계)

  • Purpose: This study was performed to measure hospital nurses' perceived patient safety culture and their safety care activities, and to investigate the relationship between these two factors. Method: This study was a cross-sectional survey. The participants were 301 nurses working at 4 general hospitals, and data collection was done from June 22 to June 30, 2009 by self-administrated questionnaires. Results: With a possible score of 5 points, the average score for nurses' perceived patient safety culture was 3.34, and for their safety care activities, 4.25. There were perceived differences in patient safety culture and safety care activities according to age, position, length of work experience and number of patient safety education sessions attended. All sub-factors in patient safety culture had a positive relationship with safety care activities. Factors influencing nurses' safety care activities were number of patient safety education sessions attended, hospital environment, and supervisor/manager. These factors explained 58.2% of the variance. Conclusion: The findings indicate that patient safety education is very important to improve nurses' safety care activity. So nursing supervisors/ managers should develop strategies encourage patient safety education, and make nurses' working environment safer.

Effect of Supervising Infectious Medical Waste and Eco-Friendly Health Care Service Orientation as Perception of the Environment Problem of Hospital Nurse (병원간호사의 환경문제인식이 감염성폐기물 관리 실천 및 친환경의료서비스 지향성에 미치는 영향)

  • Purpose. To examine the orientation of the eco-friendly Health care service on korean Hospital nurses Methods. Between October 2012 and November 2012, two hundred eighty five nurses (mean age, $25{\pm}17$ years; age range, 22~42yrears) were enrolled in this study. They performed a questionnaire which related to environmental problem, hospital infectious wastes manage and eco-friendly care service orientation. Results. Within the results, There were statistically significant correlation between "Hospital infectious wastes management" and "environmental problem awareness"(r=0.45, p<.001), "eco-friendly care service orientation"and "environmental problem awareness" (r=0.48, p<.001), and "eco-friendly Health care service orientaton" and "Hospital infectious wastes management"(r=0.50, p<.001). Conclusions. In current study, these results express that it is necessary to promote environmental problem awareness and hospital infectious wastes management practice for the "orientation of the eco-friendly Health care service on korean hospital nuurses.
