Optimal timing for salvage surgery after definitive radiotherapy in hypopharyngeal cancer

  • Purpose: Use of radiotherapy combined with chemotherapy is increasing in hypopharyngeal cancer. However, many show residual tumor after radiotherapy. Timing for treatment evaluation and salvage therapy is essential. However, optimal timing for salvage surgery has not been suggested. In this study, we tried to evaluate optimal timing for salvage surgery. Methods and Materials: Patients who were diagnosed with hypopharyngeal squamous cell carcinoma between 2006 and 2015 were retrospectively analyzed. All patients received definitive radiotherapy with or without chemotherapy. Response of all treated patients were analyzed at 1, 3, and 6 months after radiotherapy. Any patients with progression before 6 months were excluded. Results: A total of 54 patients were analyzed. Complete remission (CR) rates at 1 month (CR1), 3 months (CR3) and 6 months (CR6) were 66.7%, 81.5%, and 90.7%, respectively. Non-CR at 1 month (NCR1), 3 months (NCR3), and 6 months (NCR6) showed poor locoregional recurrence-free survival rates (1-year rates of 63.7%, 66.7%, and 0.0%, respectively) compared to CR1, CR3, and CR6 (1-year rates 94.3%, 88.0%, and 91.5%, respectively). Particularly significant differences were seen between CR6 and NCR6 (p < 0.001). Of 10 patients with NCR3, 5 showed CR at 6 months (NCR3/CR6). There was no statistical difference in locoregional recurrence-free survival between CR3 and NCR3/CR6 group (p = 0.990). Conclusion: Our data suggest half of patients who did not show CR at 3 months eventually achieved CR at 6 months. Waiting until 6 months after radiotherapy may be appropriate for avoiding additional salvage therapy.

Development of Selectable Vector Plasmid in Bacteriophage P2-P4 System and Its Stability (박테리오파지 P2-P4 시스템을 위한 벡터 플라스미드 개발과 안정성)

  • While bacteriophage P2-P4 system is very useful experimental tool for the study of viral capsid assembly, there is no useful plasmid vector for the DNA manipulation in bacteriophage P2-P4 system. In this study, a new vector plasmid, P4 ash8 (sid71) kmr, was constructed by swapping the non-essential region of P4 DNA for kanamycin resistance(kmr) gene cassette of plasmid pUC4-K. P4 ash8 sid71 was starting material for the construction, since it tends to be maintained as a plasmid in the absence of the helper phage. The total size of this chimera was designed to be packaged into P4 or P2 size heads with induction by P2 infection. The conversion of plasmid P4 ash8 (sid71) kmr to bacteriophage was proved by burst size determination experiment and CsCl buoyant equilibrium density gradient experiment. Integrase destructed P4 derivative, P4 ash8 sid71 kmr intS, was able to be constructed easily by in vitro DNA manipulation of P4 ash8 sid71 kmr. The plasmid stability experiment with P4 ash8 sid71 kmr if/tS showed that the integrase of P4 affects the stable maintenance of plasmid P4 state.

  • Most of the diagnostic methods currently used for the detection of neoplastic masses provide indirect evidence. To obtain greater specificity in the interpretation of neoplasias by in vivo methods, the immunological approach appears to be most promising. Two problems that interfered with progress in this field were the lack of tumor specific antigen and the lack of well-defined and reproducible antibodies. To improve the sensitivity and specificity of radioimmunoscintigraphy as a technique for tumor localization, the use of monoclonal antibodies, fragments of antibodies and single photon emission computerized tomography (SPECT) are reasonable. The obvious advantages of monoclonal antibodies are their homogeneity, their specificity for the immunizing antigen and the reaction with a single determinant-thus no large immunecomplexes with antigen are formed. Monoclonal antibody technique has recently provided an opportunity to reevaluate the role of nuclear medicine for the diagnosis of malignant diseases by using the immunological approach. Out first results by means of radioimmunoscintigraphy of CEA and CA 19-9 producing tumors using a cocktail of fragments F $(ab')_2$, of mocolonal antibodies to CA 19-9 and CEA labeled with $^{131}I$ (IMACIS-1) are reported. The aims of this investigation was to evaluate the role of immunoscintigraphy in patients with colorectal and other cancers for diagnosis of local recurrences and metastasis. This report contains results of the first 8 colorectal and pancreas cancer patients with the elevation of the level of serum CEA and/or CA 19-9. IMACIS-1 was injected intravenously during 30 minutes in 100 ml saline solution after skin test. Planar scintigrams were recorded 3, 5 and 7 days after the injection of the IMACIS-1. Anterior, lateral and posterior views of the liver as well as anterior and posterior views of the pelvis were obtained in each patients as an $^{131}I-antibody$ image. We were able to localize exactly the malignant process with the double-nuclide double-compound $^{99m}Tc\;^{131}I$ (Tc+l) scintigrams. In Tc & I double-nuclide scintigraphy, computer subtraction display provided more clear localization of the tumor. We compared the results of radioimmunoscintigraphy with CT, ultrasonograms, conventional scintigrams. The results were as follows: 1) The sensitivity and specificity of radioimmunoscintigraphy using the fragments $F(ab')_2$ of the cocktails of CEA and CA 19-9 monoclonal antibodies were 80% and 100% respectively. 2) Tumor detection rate was not proportionated to the level of serum tumor markets. 3) Second tracer technique was essential for tumor localization as an anatomic landmark using double-nuclide scintigraphy. 4) A slow infusion of the antibodies was necessary to prevent the formation of large immune complexes. 5) Tumor/non-tumor radioactivity was most elevated at 7 days delayed imaging. 6) Using planar scintigraphic technique of $^{131}I$ labeled monoclonal antibodies are possible for imaging most of the tumors.

  • Onion has been reported to contain various organosulfur compounds which have antibiotic and anticarcinogenic properties and flavonoid like quercetin which is a valuable natural source of antioxidants. Carbohydrates in onion constitute about 80% of dry matter, and the major non-structural carbohydrate of onion bulb is fructo-oligosaccharides, well known as fructan, followed by glucose, fructose, and sucrose. The sugar concentration is associated with dormancy and storage life of onion, occurring as decrease in glucose, fructose and fructan, particularly towards the end of storage. Forced air pre-drying for 15-20 days at room temperature is an essential procedure to reduce freezing injury and sprouting, then onion bulbs can be stored at $0^{\circ}C$ for 6 months to control sprouting and decay. Bacterial soft rot caused by $Erwinia$ and $Pseudomonas$ is the main postharvest disease when the bulbs are infected with the bacteria and stored at room temperature. Browning in sliced onion is due to oxidation of phenolic compounds by polyphenol oxidase and it can be inhibited by citric acid treatment, packing with nitrogen gas, and polyethylene film.

Performance Analysis of Siding Window based Stream High Utility Pattern Mining Methods (슬라이딩 윈도우 기반의 스트림 하이 유틸리티 패턴 마이닝 기법 성능분석)

  • Recently, huge stream data have been generated in real time from various applications such as wireless sensor networks, Internet of Things services, and social network services. For this reason, to develop an efficient method have become one of significant issues in order to discover useful information from such data by processing and analyzing them and employing the information for better decision making. Since stream data are generated continuously and rapidly, there is a need to deal with them through the minimum access. In addition, an appropriate method is required to analyze stream data in resource limited environments where fast processing with low power consumption is necessary. To address this issue, the sliding window model has been proposed and researched. Meanwhile, one of data mining techniques for finding meaningful information from huge data, pattern mining extracts such information in pattern forms. Frequency-based traditional pattern mining can process only binary databases and treats items in the databases with the same importance. As a result, frequent pattern mining has a disadvantage that cannot reflect characteristics of real databases although it has played an essential role in the data mining field. From this aspect, high utility pattern mining has suggested for discovering more meaningful information from non-binary databases with the consideration of the characteristics and relative importance of items. General high utility pattern mining methods for static databases, however, are not suitable for handling stream data. To address this issue, sliding window based high utility pattern mining has been proposed for finding significant information from stream data in resource limited environments by considering their characteristics and processing them efficiently. In this paper, we conduct various experiments with datasets for performance evaluation of sliding window based high utility pattern mining algorithms and analyze experimental results, through which we study their characteristics and direction of improvement.

  • Potassium Sorbate (PS) is a potassium salt version of sorbic acid, which has antimicrobial and fungistatic features in foods. Therefore, PS is used as a food preservative against fungi and mold. PS has been found to be non-toxic even when taken in large quantities given its trait to be broken down in the body into water and carbon dioxide. Gap Junctional Intercellular Communication (GJIC) is essential in the maintenance of tissue homeostasis during development and differentiation. This study was made of the effects of PS on GJIC in WB-F344 rat liver epithelial (WB) cells. We found dramatic decrease of cell viability in time- and dose-dependent manners when WB cells were treated with PS. The effect of PS on GJIC is strong inhibition, and it took place in parallel with a hyperphosphorylation of connexin 43 expression. The finding that PS interferes with gap junction functionality should be considered with respect to the mechanism of PS-induced hepatotoxicity.

A Study on Male High School Students' Smoking Patterns (일부 남자 고등학생의 흡연 실태에 관한 조사연구)

  • This study aims to investigate smoking patterns in high school student and to give student smoker effective information. The sample of 250 male highschool students out of two different schools in Tae-Jon was questioned from July 10th to 15th. 1995. In analyzing these date. the statistics shows the realities by means of number of students. The results are summarized into 17 items as follows. Regarding the level of smoking. 140 students out of 250 admit that they have ever smoked. 52.1% of smoking students say that the motivation of beginning smoking is mainly curiosity. The survey shows that 22.9% of smoking students feel very good when smoking. It also shows that 30.0% of smoking students began smoking in the first grade of high school. With regard to the volume of smoking per day. 41.4% of smoking students smoke variably. 42.1% drink when smoking. 15.0% spend more than W 70.000 a month. About the question who knows the fact of their smoking. 51.5% answer that their friends know the fact of their smoking. In regard to the resaltionship between smoking and school per-formance. 18.2% of non smoking students make poor grades as compared with 40% of smoking students. 9.3% of smoking students say that they are satisfied with the school life. but 35.7% of them are not satisfied. Regarding the attitude to smoking teachers. 35% of smoking students state that they are affected by them. 69.3% of smoking students say that they will stop smoking. while the remaining 30.7% say that they will keep smoking. The reason of 63.9% to stop smoking is that smoking is bad for the health. The reason of 46. 5% to keep smoking is the acquired habit of smoking. 97.2% know the fact that the major element of cigarettes is nicotine and it is very harmful to the health. 40.8% recognize the harmful effect of smoking by TV and radio programs. 97.2% know that smoking could cause lung cancer. From the above results. I propose as follows We should make specific plan to keep smoking by simple curiosity from being developed into habitual smoking. We should teach them how harmful smoking is and make them stop smoking by themselves. It is very essential for family members and teachers to give continuous interest since childhood. As the teacher affect the students very much, they should give up smoking first. The incidence of smoking should be identified in each of the middle and high schools. smoking prohibition programs relevant to each school should be developed and implemented. The local community should ban cigarette vending machines. Cigarettes should not be sold to adolescents. By setting every place where adolescents gather including schools nonsmoking area. we should decrease their impulse to smoke. then smoking opportunities. and harmful effects to them caused by passive smoking.

UREASE ACTIVITY OF STREPTOCOCCUS SALIVARIUS (Streptococcus salivarius의 요소분해효소 활성에 관한 연구)

  • Dental caries is induced by organic acids produced by oral bacteria. In order to prevent dental caries, therefore, it is essential to maintain neutral pH in the oral cavity. Urea plays a major role in oral pH homeostasis. Urea is hydrolyzed by bacterial ureases to ammonia, causing a pH elevation. Streptococcus salivarius has been shown to be a major contribution to oral ureolysis. Synthesis of urease by S. salivarius appears to be constituitive, but can be greatly enhanced by low pH. It is, therefore, conceivable that ureolytic activity of S. salivarius from a carious lesion is greater than that of the bacterium from a healthy tooth. In the present study, urease activity of S. salivarius isolates from dental plaque of carious lesions was compared with that of the isolates from plaques of the teeth and the dorsum of the tongue; 45 S. salivarius strains were isofated from carious lesions(>C2) of 21 individuals with dental caries and 30 strains from 10 individuals without dental caries. The results were as follows: 1. All the 21 individuals with dental caries harbored ureolytic S. salivarius whereas 3 of 13 individuals without dental caries harbored non-ureolytic strains of S. salivarius. 2. All the 45 S. saliuarius isolates from carious lesions showed urease activity. In contrast, of 30 isolates from individuals without dental caries, 17 isolates(56.7%) did not demonstrate urease activity, or if any, very little(<5${\mu}mol$/min/mg). 3. Urease activity of the isolates from carious lesions was greater than that of the isolates from individuals without dental caries : the urease activity ranged from 42 to $381{\mu}mol$/min/mg and from 0 to $208{\mu}mol$/min/mg, respectively. 4. At acid pH(5.5), the isolates which showed intermediate urease activity at pH 7.0 demonstrated even higher activity whereas the isolate with no or lower urease activity did not show any significant difference in their activity. However, the isolates with the greatest urease activity from both individuals with and without dental caries, exhibited a rather much lower urease activity at pH 5.5. The overall results suggest that isolates may have their own urease activity but the isolates exposed to chronic acidic environment of the carious lesion might elevate urease activity of S. salivarius, which in turn, might influence on survival of S. salivarius itself and other bacteria, establishing a new oral bacterial ecosystem.

Improvement of Contract Change Order System for the Fairness of Subcontracting in Public Construction Projects (공공건설공사의 하도급 거래 공정화를 위한 계약변경 제도개선 방향)

  • Subcontracting of construction is essential to carry out public construction projects. Subcontractor of the construction work shall enter into a contract with the original contractor without directly entering into a contract with the owner. Subcontracts are therefore greatly affected by the original contract with the owner. To protect subcontractors, the Fair Trade Act is enacted and the construction company's standard subcontracting contract is in operation. However, subcontracts are not properly reflected in the Government contract system, which deals with the relationships between the owner and the original contractor. In particular, the subcontractor may complain of difficulties at the public construction site as such procedures are not properly reflected in the construction work standard subcontract, although various procedures shall be carried out depending on the amount when change order occurs in subcontracts. Thus, the direction of improvement of subcontracting systems was proposed in the case of change order at public construction project sites as follows: First, the rights of subcontractors should be strengthened. Second, in order to resolve the information non-identity, subcontractors should have access to information related to subcontracts. Third, the status of subcontractor shall be guaranteed by reflecting the characteristics of the subcontract when the original contract for public works is concluded. Fourth, the dispute settlement method should be prioritized over litigation in order to fair subcontracting.

A Comparative Study of Contents on Children's Rights between South and North Korea through the UN Convention on the Rights of the Child (유엔아동권리협약 국가보고서를 통해 본 남북한 아동권리 내용 비교)

  • Using the United Nations Convention on the Rights of the Child(UNCRC) as a frame of reference, this study examined South and North Korean children's rights as reported in the UNCRC and the pattern of changes in the reports depending on the period in which the reports were written. For this purpose, we reviewed Article 1 of the Convention to see what age range the South and North Korean governments, respectively, specify to define a child. We also analyzed the method and attitude of how each Korean government acknowledges the rights of children in the four general principles of the Convention("non-discrimination", "the best interests of the child", "the right to life, survival, and development" and "respect for the views of the child"). The results showed that there was asymmetric content between South and North Korea in the four general principles of the Convention. Both South and North Korea lack a sense of respect for the views of children. There were also differences in the rights of children depending on the distinct characteristics of each Korea. In South Korea, it is important to guarantee the rights of multicultural children and provide a plan for child safety. On the other hand, North Korea has emphasized that they provide the necessities of life and free education to children. Both Koreas have continuously submitted their reports and showed improvement in understanding the rights of children in each report. We conclude that it is essential for South and North Korea to build common ground in their respective contents on children and to establish a foundation of normative standards in the path to prepare for reunification. Moreover, we also conclude that South and North Korea should work collaboratively to reach mutual consent to achieve these goals.
