  • The purpose of this study is to identify the smoking patterns and their relevant factors of the public officials who are placed to guide people's way of thinking as conservative middle-income group and thereupon, provide for basic data which can be used to reduce the diseases and death rate caused by smoking in terms of preventive medicine. For this purpose, 170 public officials working for the government agencies located in seoul were surveyed for Jan. 10-Jan. 20, 1995. The results of survey can be analyzed/summarized as follows; 1. In terms of daily smoking frequencies, 50 officials $(29.4\%)$ are driven by stress to smoke. Among these offcials, 250nes $(50.0\%)$ smoke 20-30 times a day. Meanwhile, 22 officials $(12.9\%)$ answered that they smoke for pleasure and comfort, and among them 6 officials $(21.4\%)$ smoke 20-30 times a day. 26 officials $(15.3\%)$ are found to be nicotineaddicted physically or psychologically, and to 10 of them $(35.7\%)$ smoke 20-30 times a day. On the other hand, 10 officials $(5.9\%)$ have habit of smoking and 4 of them $(40.0\%)$ smoke 20-30 times a day. 2. Out of 50 officials $(24.9\%)$ who began smoking on impulse, 44 actually stopped smoking, and the rest 6 officials did not even implement their plan. In view of successful cigarette-stopping, only 8 officials $(15.4\%)$ out of 44 impulse-driven smokers succeeded in stopping cigarettes, while the rest 36 failed all. Meanwhile, out of those 84 officials $(64.6\%)$. Who smoked for reduced stress, 28 ones succeeded, while the rest 56 ones failed. Out of 20 officials who were nicotine-addicted but attempted to stop smoking, only 2 smokers succeeded and the rest 18 were all failures. 3. Factors involving smoking and Their patterns 1) By age group (30's, 40's and 50's), the smoking patterns can be grouped into stress-removing, nocotine-addictive and habitual ones, respectively. 2) By acadeic background, our officials smoke on impulse or due to nocotine addition. 3) The length of career is co-related with nicotine addition. 4) The ownership of house is related with smoking for comfort or less stress. 5) The number of children is related with impulse-driven smoking or smoking for comfort.

  • Background: Hardcore smoking is represented by a subset of daily smokers with high nicotine dependence, inability to quit and unwillingness to quit. Estimating the related burden could help us in identifying a high risk population prone to tobacco induced diseases and improve cessation planning for them. This study assessed the prevalence and associated factors of hardcore smoking in three South-East Asian countries and discussed its implication for smoking cessation intervention in this region. Materials and Methods: Global Adult Tobacco Survey (GATS) data of India, Bangladesh and Thailand were analyzed to quantify the hardcore smoking prevalence in the region. On the basis of review, an operational definition of hardcore smoking was adopted that includes (1) current daily smoker, (2) no quit attempt in the past 12 months of survey or last quit attempt of less than 24 hours duration, (3) no intention to quit in next 12 months or not interested in quitting, (4) time to first smoke within 30 minutes of waking up, and (5) knowledge of smoking hazards. Logistic regression analysis was carried out using hardcore smoking status as response variable and gender, type of residence, occupation, education, wealth index and age-group as possible predictors. Results: There were 31.3 million hardcore smokers in the three Asian countries. The adult prevalence of hardcore smoking in these countries ranges between 3.1% in India to 6% in Thailand. These hardcore smokers constitute 18.3-29.7% of daily smokers. The logistic regression model indicated that age, gender, occupation and wealth index are the major predictors of hardcore smoking with varied influence across countries. Conclusions: Presence of a higher number of hardcore smoking populations in Asia is a major public health challenge for tobacco control and cancer prevention. There is need of intensive cessation interventions with due consideration of contextual predictors.

  • Objectives : The study examined the correlation the between smoking and the actual smoking condition, knowledge, attitude and practice on the dental health of korean soldiers. Methods: The data was collected from a questionnaire given to 301 soldiers at two separate units in Kangwon-DO. Results : Soldiers dependance on nicotine according to the smoking frequence showed positive correlation(p<0.001). The respondents who had bad breath and bleeding gum had smoked for a relative correlation with how long they had smoked. Among the content of the dental health care program that respondents wished to undergo, dental whitening was the most attractive topic(37.9%) and decayed teeth was the most important part in dental health care(25.2%). The correlation coefficient between the dependancy on nicotine the less effort they made to improve their dental health(p<.05). Conclusion : The study suggest that antismoking campaigns should be include in military dental health care programs. Furthermore, the development of dental health care programs considering the needs of each military unit is basic for practice on dental health care between Korean soldiers.

  • Objectives: The purpose of the study is to investigate the oral health-related quality of life (OHIP-14) according to smoking in the male workers in Gunsan. Methods: A self-reported questionnaire was completed by 460 male workers in Gunsan from April 13 to 30, 2015. Among 460 workers, smokers were 205 and nonsmokers were 255. The questionnaire included three questions of general characteristics of the subjects, four questions of subjective oral health, fourteen questions of OHIP, ten questions of sobriety test (AUDIT), and eight questions of smoking (Fagerstrom Test for Nicotine Dependence, FTND). Data were analyzed by frequency analysis, t-test, ANOVA, Pearson's correlation, and multiple regression analysis using IBM SPSS(Statistical Package for the Social Science) for Windows 20.0 program. Results: In the nonsmokers, there was a negative correlation between the alcohol consumption, dental health condition, tooth pain, gum bleeding, and halitosis. The higher OHIP in the smokers had the negative correlation with nicotine dependence, alcohol consumption, tooth pain, gum bleeding, and halitosis. In order to improve OHIP, smokers are encouraged to manage gum bleeding, and non-smokers to halitosis. Conclusions: In order to improve OHIP, cessation of smoking and alcohol consumption is very important and it can lead to improve the quality of life in the workers.

  • Background : The purpose of this study was to analyze the smoking habits in patients with lung cancer and identify any difference of prevalence according to histologic types of lung cancer. Methods : The data were calculated by total amounts of tar and nicotine inhaled during the whole lifetime according to variation of smoking habit. This study was to investigated any difference of prevalence in lung cancer according to smoking habits. The subjects comprised 150 lung cancer cases and 300 hospital control cases that were matched by age and sex. Smoking habits during the whole lifetime were surveyed by standardized questionnaire. Odds ratios were estimated by unconditional logistic regression analysis. Results : There were 104 male and 34 female lung cancer cases. By histologic type, there were 53 cases of squamous cell carcinoma, 67 of adenocarcinoma and 30 of small cell lung carcinoma. The differences between lung cancer cases and controls according to smoking habits were total duration of smoking, total pack years of smoking and number of cigarettes smoked per day during the previous two years. The odds ratio were higher in Kreyberg I, but not in Kreyberg II, for the longer duration of smoking, the greater total pack years of cigarettes consumed, the more cigarettes smoked per day during the previous two years, the longer duration on non-filter smoking, the earlier life cases who began to smoke, and the higher amounts of calculated total tar and nicotine inhaled over the whole lifetime. When we added grade of inhalation to calculation of amounts of tar and nicotine inhaled over the lifetime, the odds ratios of total inhalation amounts of tar and nicotine were as high as those the without them. Conclusions : This study reconfirmed that smoking habits were strongly associated with lung cancer and that there were different associations between smoking habits and histologic types of lung cancer. In particular, calculations of total tar and nicotine amounts inhaled over the whole lifetime were calculated for the first time in trials from lung cancer epidemiologic studies.

  • We investigated the lung function change and quality of life in smokers after taking herbal medicine. The patients were divided into two groups(smoking and non-smoking). We measured lung function change by VRI and quality of life by BDI and Fatigue Scale Score before and after herbal medicine intake. And we examined changes of nicotine dependence, total smoking amount, tobacco flavor in smoking group. The VRI energy bar could not represent the change of the lung function in smoking group and non-smoking group. And there was no change of lung function by VRI in two group patients before and after herb medication. But after taking herb medicine, the patients who have abnormal sound in VRI were improved. The herbal medicine does not affect to the amount of smoking, tobacco flavor, nicotine dependence in Smoking patients. Smoking group were higher than non-smokers in BDI score and Fatigue Scale score. Smoking group were more improved than the non-smokers in BDI and Fatigue Scale Score after taking herbal medicine.

  • Objectives: Smoking cessation decreases morbidity and mortality due to chronic obstructive pulmonary disease (COPD). Pharmacotherapy for smoking cessation is highly effective. However, the optimal prescription rate of smoking cessation medications among smokers with COPD has not been systemically studied. The purpose of this study was to estimate the national prescription rates of smoking cessation medications among smokers with COPD and to examine any disparities therein. Methods: We conducted a retrospective study using National Ambulatory Medical Care Survey data from 2007 to 2012. We estimated the national prescription rate for any smoking cessation medication (varenicline, bupropion, and nicotine replacement therapy) each year. Multiple survey logistic regression was performed to characterize the effects of demographic variables and comorbidities on prescriptions. Results: The average prescription rate of any smoking cessation medication over 5 years was 3.64%. The prescription rate declined each year, except for a slight increase in 2012: 9.91% in 2007, 4.47% in 2008, 2.42% in 2009, 1.88% in 2010, 1.46% in 2011, and 3.67% in 2012. Hispanic race and depression were associated with higher prescription rates (odds ratio [OR], 5.15; 95% confidence interval [CI], 1.59 to 16.67 and OR, 2.64; 95% CI, 1.26 to 5.51, respectively). There were no significant differences according to insurance, location of the physician, or other comorbidities. The high OR among Hispanic population and those with depression was driven by the high prescription rate of bupropion. Conclusions: The prescription rate of smoking cessation medications among smokers with COPD remained low throughout the study period. Further studies are necessary to identify barriers and to develop strategies to overcome them.

  • Purpose: Smoking temptation is communicated frequently among people related to smoking cessation and has specific meanings within the context of smoking. However, the concept of smoking temptation has not been well articulated in the literatures. This study was conducted to clarify and to conceptualize the phenomena of smoking temptation. Method: The Hybrid Model of concept development was applied to develop a concept of smoking temptation, which included a field study carried out in Seoul, South Korea using in-depth interviews with 5 adult smokers. Results: The concept of smoking temptation emerged as a complex phenomenon having meanings in several different dimensions which encompassed several attributes. In addition, the conceptual structure of smoking temptation centered around five dimensions: Response to conditioned stimuli, Distancing from harsh reality, Nicotine dependency, Habitual routine, and Lack of control. Conclusions: Smoking temptation is a concept that needs to be treated in a specified individual way and it is possible to enrich the meanings and methods to manage smoking temptation in nursing smoking cessation interventions that its application may have positive impacts on patients' well being.
