Emergency Room access by Population Density and distance of Daegu Metropolitan city (대구광역시의 인구밀집과 거리에 따른 응급실 접근성)

  • This study examined the accessibility of emergency rooms according to the population density and distance in Daegu Metropolitan City to help improve the quality and emergency medical accessibility problems in Daegu Metropolitan City. To observe the geographical requirements, the number of population in eup, myeon, dong, available emergency, and available emergency compared to the population were mapped through the S-GIS of Statistics Korea to visualize the data based on the 3km radius. To determine the difference in accessibility to emergency rooms for each district in Daegu Metropolitan City, the data were analyzed by ANOVA and Scheffe. According to the average number of emergencies available in Daegu Metropolitan City within a 3-kilometer radius were 5.7 in Jung-gu, 5.0 in Nam-gu, 1.6 in Buk-gu, 0.4 in Dong-gu, 2.4 in Seo-gu, 0.9 in Suseong-gu, 3.5 in Dalseo-gu, and 0.1 in Dalseong-gun; there was a statistically significant difference (p<.001). The available emergency within a 3km radius and available emergency per 1,000 people were concentrated in the center. Therefore, it may be necessary to institute an urban emergency medical network by establishing a point that serves as an intermediate hub to provide emergency medical care to citizens far from the center.

A simple and novel equation to estimate the degree of bleeding in haemorrhagic shock: mathematical derivation and preliminary in vivo validation

  • Determining blood loss [100% - RBV (%)] is challenging in the management of haemorrhagic shock. We derived an equation estimating RBV (%) via serial haematocrits (Hct1, Hct2) by fixing infused crystalloid fluid volume (N) as [0.015 × body weight (g)]. Then, we validated it in vivo. Mathematically, the following estimation equation was derived: RBV (%) = 24k / [(Hct1 / Hct2) -1]. For validation, non-ongoing haemorrhagic shock was induced in Sprague-Dawley rats by withdrawing 20.0%-60.0% of their total blood volume (TBV) in 5.0% intervals (n = 9). Hct1 was checked after 10 min and normal saline N cc was infused over 10 min. Hct2 was checked five minutes later. We applied a linear equation to explain RBV (%) with 1 / [(Hct1 / Hct2) -1]. Seven rats losing 30.0%-60.0% of their TBV suffered shock persistently. For them, RBV (%) was updated as 5.67 / [(Hct1 / Hct2) -1] + 32.8 (95% confidence interval [CI] of the slope: 3.14-8.21, p = 0.002, R2 = 0.87). On a Bland-Altman plot, the difference between the estimated and actual RBV was 0.00 ± 4.03%; the 95% CIs of the limits of agreements were included within the pre-determined criterion of validation (< 20%). For rats suffering from persistent, non-ongoing haemorrhagic shock, we derived and validated a simple equation estimating RBV (%). This enables the calculation of blood loss via information on serial haematocrits under a fixed N. Clinical validation is required before utilisation for emergency care of haemorrhagic shock.

  • 현대 사회에서 차량은 널리 보급되어 있으며, 일상생활에서 없어서는 안 될 필수품이 되어 있다. 그러나 자동차의 증가와 함께 차량화재 및 교통사고 등과 같은 문제점들도 함께 늘고 있는 실정이다. 2009년 기준, 1년 동안의 전체 화재발생건수 47,071건 가운데 차량 화재의 발생건수가 5,958건으로 전체의 12.6% 정도를 차지하였다. 특히 차량 내장재의 주 재질은 가연성을 지닌 열가소성 합성수지들로서 화재가 발생하였을 경우, 다량의 가연성 가스 및 독성가스를 방출하기 때문에 인명 및 재산 피해를 증가시키는 문제점을 갖고 있다. 화재 시 발생하는 독성가스는 짧은 시간에 주변으로 확산되어 인명피해에 치명적인 영향을 준다. 따라서 본 연구에서는 차량에 대한 화재위험성을 다각적으로 평가하기 위해서 연소생성가스에 주안점을 두고 연구를 수행하였다. 연구결과를 살펴보면, 차량내장재의 연기밀도는 624~172, 독성지수는 13.7~2.9인 것으로 나타났다.

A study on enhancing professionalism of civil anti-disaster organizations for public safety in Korea: with a focus on CAIND (한국 재난관리 체계에서 민간 방재조직의 전문성 제고 방안 - 지역자율방재단을 중심으로 -)

  • Purpose: This study explored a plan for improving the overall preventive activity of Korean Citizen-Corps-Active-in-Disaster (CAIND). Methods: The study used the documentary survey and the technical approach methods. This way, detailed information regarding civil anti-disaster organizations was found in scholarly monographs, specialty publications, and master's/doctoral dissertations. It further utilized the statistical yearbooks between 2015 and 2018 from the Korean Ministry of Public Administration, Security, and Fire Service, and the National Statistical Office, to discover practical problems through a statistical analysis. Volunteer activities being inherent, related issues were reviewed at the same time to for purposes of clarifying the characteristics of disaster prevention activity by CAIND. Results: The study provided four major suggestions for improvement. First, the quota management system of Korean CAIND considering the characteristics of rural areas should be supplemented. Second, the education and training systems of Korean CAIND should be established to satisfy regional conditions. Third, management members' readership competency in operating organizations, including that of the Korean CAIND captain in charge, should be strengthened. Fourth, the reward system for Korean CAIND activities should be improved. Conclusion: In the future, the results of this study are expected to be utilized as a basic data to develop Korean CAIND.

Association between Weekend Admission and In-hospital Mortality among Cardiovascular Patients in Korea

  • Background: Weekend admission is known for having association with increased mortality attributed by poor quality of care and severe patients. We investigated the association between hospital admission on weekends and the in-hospital mortality rates of patients with cardiovascular disease. Furthermore, we examined this association stratified by admission via emergency room or not. Methods: We analyzed claim data provided by the Health Insurance Review & Assessment in 2013. In total, 80,817 cardiovascular patients were included in this study, which treated in-hospital mortality (early and during total length of stay) as a dependent variable. A generalized linear mixed effects model was used. We conducted subgroup analyses stratified by admission via emergency room or not. Results: Patients who admitted on weekend showed higher in-hospital mortality both early (odds ratio [OR], 1.48; 95% confidence interval [CI], 1.23-1.78) and during total length of stay (OR, 1.17; 95% CI, 1.02-1.33) compared to those admitted on weekdays. Patients who were admitted to the hospital on a weekend by emergency room were more likely to experience early in-hospital mortality compared to those admitted on weekdays. Furthermore, we found that patients not admitted to the hospital through the emergency department were more likely to experience both early and total length of stay in-hospital mortality. Conclusion: Our study shows higher in-hospital mortality rates for cardiovascular patients admitted on weekends. Efforts to improve the quality of care on weekend are important to mitigate the 'weekend effect' and improve patient outcomes.

A Study on the Guardian's Experience about Tepid Massage of Children Who were Admitted to a Pediatric Emergency Room (소아응급실을 내원한 환아 보호자의 미온수 마사지 경험실태)

  • Purpose: The purpose of this study was to investigate the guardian's experience about tepid massage of children who visited Pediatric Emergency Room (PER). Methods: Data for this study was collected from August 1 to September 30, 2009 using questionnaires of 300 guardians who visited a PER. All data analyses were done using the SPSS 17.0 program and statistical data on frequency and percentage were obtained. $x^2$ test was done as well. Results: The results of this study were as follows: 73.7% of the guardians were doing the tepid massage, and 40% of them were doing the tepid massage when guardians thought that their children had a fever. 8.6% of them were incorrectly doing the tepid massage on their children such as using ice or alcohol. 90.5% of the children's condition changed when they had the tepid massage applied. Only 48% of the guardians got information about tepid massage, and 52.8% of the guardians got this information from the medical team. There were significant differences according to the education background about tepid massage (p<.001) and past hospital admissions history. There were differences according to whether their child had experience with febrile seizure(p=.085) and having more than one child at home (p=.085). Conclusion: This study revealed that the guardians were incorrectly doing the tepid massage on their children. The results of this study suggest that it is necessary to develop certain emergency nursing processes on accurate information and management strategies about tepid massage, and further studies on the effects of this kind education intervention are necessary.

The current status and legal review of advanced airway management implemented by 119 EMTs (소방 119구급대원에 의해 시행된 전문기도관리 현황 및 법적 고찰)

  • Purpose: This study aimed to analyze the data from public information disclosure about pre-hospital advanced airway management and identify the problem by considering domestic laws and guidelines. Methods: Data were collected between 2017 and 2018 and analyzed using SPSS 25.0. Then, the problems of the analysis results based on the relevant laws and practical guidelines were reviewed. Results: The review of domestic laws and practice guidelines revealed that ambulance nurses can implement supraglottic airway device only under the following three conditions: ① smart advanced life support pilot project area, ② trained to insert I-Gel, and ③ member of a special ambulance. In total, 21,574 cases of advanced airway management (endotracheal intubation: 2,428, I-Gel: 18,502, LMA : 499, KING AIRWAY: 144) were reported. In many cases, advanced airway management was performed by ambulance nurses who did not meet the above conditions, which was in violation of laws and guidelines. In addition, the prognosis of intubated patients was not followed up. Conclusion: The Korea National Fire Agency must stop all unlicensed medical practice by untrained, uneducated, and uncertified nurses and demand quality control programs for intubated patients.

Analysis of trunk angle and muscle activation during chest compression in 119 EMTs (가슴압박시 구급대원의 체간 각도와 근활성도 분석)

  • Purpose: We aimed to investigate trunk angle and muscle activation of the extremity and back to evaluate the effect of chest compression on work-related musculoskeletal disorders in 119 emergency medical technicians (EMTs). Methods: Eighteen 119 EMTs performed 2-minute chest compression without interruption on a cardiopulmonary resuscitation manikin, during which we measured changes in the trunk and shoulder joint angles, muscle activation (triceps brachii, biceps brachii, erector spinae, gluteus maximus, pectoralis major, rectus abdominis, and rectus femoris) and chest compression accuracy. Results: The decrease in trunk angle by trunk muscle activation was the highest in event 2, the major direction of chest compression. Both shoulder joint angles had no significant difference. Muscle activation of the triceps brachii (p < .01), biceps brachii (p < .05), rectus abdominis (p < .05) and rectus femoris (p < .01) significantly increased during the compression phase compared with the decompression phase, with the rectus femoris showing an increase of 19%. Muscle activation of the erector spinae significantly increased in the decompression phase compared with the compression phase (p < .01). Conclusion: 119 EMTs mainly use the triceps brachii, biceps brachii and pectoralis major muscles during chest compression.

Study on Automatic External Defibrillators deployed at General Supermarkets

  • Purpose: This study was to propose effective deployment of automatic external defibrillators (AEDs) installed at general supermarkets. Research design, data, and methodology : We conducted interview and data surveys on 72 large distributors in Seoul and Gyeonggi province in South Korea. The content of this survey was consisted of general status on the general supermarkets, AED deployment and management regarding public access defibrillation (PAD). GPower (v 3.1.9.4; Universität Kiel, Kiel, Germany) was also used to analyze statistical power. Radius and actual distance, radius and retrieval time were compared by t-test at α=.05. respectively. Results : Difference between the radius (102.7 meters) and the actual distance (187.8 meters) was 85.1m, it had statistically significant difference (p<.001). The actual distance was longer compared to the radius distance. Difference between the radius (114.1 seconds) and the retrieval time (208.7 seconds) was 94.6s, it had statistically significant difference (p<.001). The retrieval time took longer compared to the radius time as well. Conclusions : The finding shows that only 45.9% of the general supermarkets are satisfied with the actual AED coverage within 3 minutes. This needs to enhance AED deployment to reduce defibrillation time and AED management to boost application in South Korea.

Clay-based Management for Removal of Harmful Red Tides in Korea: A Multi-perspective Approach

  • Periodically, harmful algal blooms (HABs) have occurred, with impacts on various areas including public health, tourism, and aquatic ecosystems, especially aquacultured and caged fisheries. To prevent or manage invasions of HABs into fish farms on an emergency basis, many methods have been proposed. Frequently over the past 30 years in coastal countries, treatments of clay and clay mixed with polyaluminum chloride (PAC) and chitosan have been tested for HAB-removal effectiveness in both the laboratory and the field. In Korea, yellow loess clay (hwangto) has been dispersed using electrolytic clay dispensers, both to decrease the amount of yellow loess clay's usage in containers and enhance HAB-removal efficiency. However, this emergency method has limitations, among which is the requirement for more effective controlling agents for field applications. Thus, in this paper, we review technologies for clay-based red tides prevention and control and their limitations, and, further, introduce next-generation algicidal technologies for the emergency protection of fish farms.
