영아의 상순에 발생한 유피낭종 (DERMOID CYST IN AN INFANTILE UPPER LIP)

  • 유피낭종(dermoid cyst)은 표피형태의 상피세포로 이루어진 낭종의 벽에 피부부속조직을 포함하는 발생학적 낭종이다. 눈썹 부근이나 구강저의 중심선에 호발하지만 혀, 입술, 협점막의 병소도 보고된 바 있다. 이 병소는 천천히 커지고 무통성이며 임파선비대는 동반하지 않는다. 내용물은 피지성, 화농성등이며 크기는 수 mm에서 12cm까지 다양하다. 조직학적 특징은 상피세포 이장과 낭종벽내의 땀샘, 피지샘, 모낭동의 피부부속조직의 존재이다. 유피낭종의 치료는 완전절제이다 완전한 절제시 재발의 가능성은 거의 없다. 본 증례의 2세 여자환아는 1년 전에 넘어진 이후 상순에 딱딱한 것이 생겼다는 것을 주소로 연세대학교 치과대학병윈 소아치과에 내원하였다. 임상 구강검사 결과 둥근 고무질감의 종괴가 상순에서 관찰되었다. 조직학적 검사를 위해 조직생검을 시행하였으며, 낭종의 벽에서 피지샘이 관찰되었다. 이에 상순에서는 드물게 발생하는 유피낭종으로 진단되어 보고하는 바이다.

매복견치의 치료: 증례보고 (TREATMENT OF IMPACTED CANINE : CASE REPORTS)

  • 상 하악 견치는 기능적 관점에서 뿐 아니라 심미적 관점에서도 매우 중요한 치아이다. 그러나 불행히도 견치의 매복은 종종 관찰되며 여러 임상적인 문제점을 유발한다. 상악 견치의 매복이 하악 견치보다 빈번하며, 주로 구개측에 매복되는 것으로 보고되고 있다. 하악의 경우 견치의 매복이 상악보다 드물게 나타나며, 특히 정중선을 지나 전위 매복되는 경우는 매우 드물다. 대부분의 환자에서 증상이 없어 방사선 사진으로 발견되는 경우가 많은데 그 원인은 불분명하고 다양하다. 매복 견치의 치료는 장애물의 존재 여부와 매복 위치와 방향, 맹출 가능한 공간의 유무, 치근의 형성 단계에 따라 맹출 장애 요인을 제거한 후의 주기적인 관찰 및 외과적 노출과 교정적 견인, 치아이식, 외과적 발치 등의 방법으로 이루어진다. 상악 우측 견치의 매복을 주소로 내원한 환아에서는 외과적 노출과 교정적 견인을 시행하였고, 하악 양측 견치의 매복을 주소로 내원한 환아의 경우 견치가 정중선을 지나 전위 매복되어 있어 교정적 견인이 어렵다고 판단되어 자가이식을 시행하였다.

혼합치열기의 miniscrew를 이용한 교정치료 (ORTHODONTIC TREATMENT WITH MINISCREWS IN MIXED DENTITION)

  • 교정치료 시 고정원은 항상 염두에 두면서 치료를 진행해야 하는 중요한 요소이다. 전통적인 고정원을 이용한 치료는 환자의 적극적인 협조를 필요로 하고 원하는 치아이동에 대한 반작용이 나타날 수 있는 등의 부작용을 가지고 있다. 그러므로 보다 견고한 구강 내 고정원이 요구되어져 왔으며 이를 위해 miniscrew가 임상에서 흔히 이용되고 있다. Miniscrew를 통한 고정원의 확보는 구외력 의존도 감소, 치료기간의 단축, 식립 후 즉시 교정력 적용, 환자의 협조도 불필요, 식립의 간편성, 저렴한 비용 등의 장점을 가지고 있다. 그러나 성장기 환자는 성인에 비해 골질이 좋지 않아 miniscrew의 성공률이 성인에 비해 낮다. 그러나 일단 고정에 성공하면 많은 교정치료 시 매우 유용하게 이용할 수 있다. 본 증례는 혼합치열기 환자를 miniscrew를 이용하여 정중이개 및 정중선 불일치, 매복치에 대한 양호한 교정치료를 하였기에 보고하는 바이다.

양산시 청소년의 부정교합 유병률 및 관련 자존감 조사 (The Prevalence of Malocclusion and Related Self-Esteem of Adolescents in Yangsan)

  • 본 연구는 청소년을 대상으로 부정교합 요소들의 유병률을 조사하고 각 요소와 자존감의 연관성을 평가할 목적으로 시행되었다. 양산시 소재 3개 중학교에 재학 중인 1,380명을 대상으로 부정교합 요소별 유병률과 자존감 상태를 조사 분석하여 다음과 같은 결과를 얻었다. Angle 분류에 의한 제1대구치 관계는 I, II 및 III급 교합이 각각 69.0%, 19.4%, 10.6%로 나타났다. 수평피개는 정상 86.9%, 반대교합 5.6%, 큰 수평피개 6.6%, 극심히 큰 수평피개 0.8%로 나타났다. 수직피개는 정상 94.1%, 과개교합 4.7%, 개방교합 1.2%였다. 정중선 불일치는 0~1 mm는 98.2%, 2~4 mm는 1.4%, 5 mm 이상은 0.4%였다. 총생은 상, 하악에 총생이 있는 경우가 각각 9.6%, 14.1%, 양악 모두의 경우가 24.1%였으며, 치간공극은 상, 하악에만 있는 경우가 각각 3.0%, 2.4%, 양악 모두의 경우는 1.7%였다. 부정교합 요소별 자존감의 차이는 여자의 경우, 반대교합과 개방교합 그룹에서 자존감 지수가 낮은 값을 보였다(p < 0.05).

자동화 프로그램을 이용한 아동의 전체두개강내용적 평가 (Total Intracranial Volume Measurement for Children by Using an Automatized Program)

  • Objectives Total intracranial volume (TIV) is a major nuisance of neuroimaging research for interindividual differences of brain structure and function. Authors intended to prove the reliability of the atlas scaling factor (ASF) method for TIV estimation in FreeSurfer by comparing it with the results of manual tracing as reference method. Methods The TIVs of 26 normal children and 26 children with attention-deficit hyperactivity disorder (ADHD) were obtained by using FreeSurfer reconstruction and manual tracing with T1-weighted images. Manual tracing performed in every 10th slice of MRI dataset from midline of sagittal plane by one researcher who was blinded from clinical data. Another reseacher performed manual tracing independently for randomly selected 20 dataset to verify interrater reliability. Results The interrater reliability was excellent (intraclass coefficient = 0.91, p < 7.1e-07). There were no significant differences of age and gender distribution between normal and ADHD groups. No significant differences were found between TIVs from ASF method and manual tracing. Strong correlation between TIVs from 2 different methods were shown (r = 0.90, p < 2.2e-16). Conclusions The ASF method for TIV estimation by using FreeSurfer showed good agreement with the reference method. We can use the TIV from ASF method for correction in analysis of structural and functional neuroimaging studies with not only elderly subjects but also children, even with ADHD.

Extent of Hyperostotic Bone Resection in Convexity Meningioma to Achieve Pathologically Free Margins

  • Objective : Hyperostosis in meningiomas can be present in 4.5% to 44% of cases. Radical resection should include aggressive removal of invaded bone. It is not clear however to what extent bone removal should be carried to achieve pathologically free margins, especially that in many cases, there is a T2 hyperintense signal that extends beyond the hyperostotic bone. In this study we try to investigate the perimeter of tumour cells outside the visible nidus of hyperostotic bone and to what extent they are present outside this nidus. This would serve as an initial step for setting guidelines on dealing with hyperostosis in meningioma surgery. Methods : This is a prospective case series that included 14 patients with convexity meningiomas and hyperostosis during the period from March 2017 to August 2018 in two university hospitals. Patients demographics, clinical, imaging characteristics, intraoperative and postoperative data were collected and analysed. In all cases, all visible abnormal bone was excised bearing in mind to also include the hyperintense diploe in magnetic resonance imaging (MRI) T2 weighted images after careful preoperative assessment. To examine bony tumour invasion, five marked bone biopsies were taken from the craniotomy flap for histopathological examinations. These include one from the centre of hyperostotic nidus and the other four from the corners at a 2-cm distance from the margin of the nidus. Results : Our study included five males (35.7%) and nine females (64.3%) with a mean age of 43.75 years (33-55). Tumor site was parietal in seven cases (50%), fronto-parietal in three cases (21.4%), parieto-occipital in two cases (14.2%), frontal region in one case and bicoronal (midline) in one case. Tumour pathology revealed a World Health Organization (WHO) grade I in seven cases (50%), atypical meningioma (WHO II) in five cases (35.7%) and anaplastic meningioma (WHO III) in two cases (14.2%). In all grade I and II meningiomas, bone biopsies harvested from the nidus revealed infiltration with tumour cells while all other bone biopsies from the four corners (2 cm from nidus) were free. In cases of anaplastic meningiomas, all five biopsies were positive for tumour cells. Conclusion : Removal of the gross epicentre of hyperostotic bone with the surrounding 2 cm is adequate to ensure radical excision and free bone margins in grade I and II meningiomas. Hyperintense signal change in MRI T2 weighted images, even beyond visible hypersototic areas, doesn't necessarily represent tumour invasion.

Effects of L-carnitine, Selenium-enriched Yeast, Jujube Fruit and Hwangto (Red Clay) Supplementation on Performance and Carcass Measurements of Finishing Pigs

  • Fifty castrated crossbred ($Landrace{\times}Yorkshire$) pigs, weighing an average of $60.6{\pm}3.1kg$ were allotted to one of five treatments in a randomized block design to examine the effects of dietary inclusion of 0.1% L-carnitine (50 ppm carnitine), 0.1% selenium-enriched yeast (0.3 ppm selenium), 0.1% Jujube fruit or 0.1% Hwangto (Red clay) on pig performance and carcass quality. All diets were based on corn, wheat, soybean meal and wheat bran and were formulated to supply 13.8 MJ DE/kg. Dietary supplementation did not influence daily gain (p = 0.57), feed intake (p = 0.52), or feed conversion (p = 0.32). Digestibility of dry matter (p = 0.60), organic matter (p = 0.74), crude protein (p = 0.76), crude fibre (p = 0.70) and energy (p = 0.75) were also unaffected by inclusion of any of the additives. Tissue samples taken from the longissimus muscle showed that the levels of carnitine (p = 0.0001) and selenium (p = 0.0001) were significantly higher with dietary inclusion of carnitine or selenium-enriched yeast. Dietary treatment did not affect dressing percentage (p = 0.33), carcass lean yield (p = 0.99) or first, $10^{th}$ and last rib midline backfat depth (p = 0.45, 0.82 and 0.47, respectively). Dietary treatment also did not affect the percentages of tenderloin (p = 0.37), bacon (p = 0.36), fat and bone (p = 0.56), picnic shoulder (p = 0.25), skirt (p = 0.80), fresh ham (p = 0.31) or ribs (p = 0.79). However, pigs fed the diet containing Jujube fruit had a higher percentage of Boston butt than pigs fed the carnitine or selenium supplemented diets (p = 0.01). Pigs fed added Hwangto had a higher (p = 0.04) percentage of loin compared with pigs fed supplementary selenium or Jujube fruit. Loin muscle from pigs fed carnitine had a significantly lower Hunter colour value for L (whiteness, p = 0.004) and a higher value for $a^*$ (redness; p = 0.069). The overall results indicate that supplementation with L-carnitine and selenium-enriched yeast can produce pork containing higher levels of carnitine and selenium, which could provide health benefits for consumers of pork without detrimental effects on pig performance.

Positional symmetry of porion and external auditory meatus in facial asymmetry

  • Background: The porion (Po) is used to construct the Frankfort horizontal (FH) plane for cephalometrics, and the external auditory meatus (EAM) is to transfer and mount the dental model with facebow. The classical assumption is that EAM represents Po by the parallel positioning. However, we are sometimes questioning about the possible positional disparity between Po and EAM, when the occlusal cant or facial midline is different from our clinical understandings. The purpose of this study was to evaluate the positional parallelism of Po and EAM in facial asymmetries, and also to investigate their relationship with the maxillary occlusal cant. Methods: The 67 subjects were classified into three groups. Group I had normal subjects with facial symmetry ($1.05{\pm}0.52mm$ of average chin deviation) with minimal occlusal cant (<1.5 mm). Asymmetry group II-A had no maxillary occlusal cant (average $0.60{\pm}0.36$), while asymmetry group II-B had occlusal cant (average $3.72{\pm}1.47$). The distances of bilateral Po, EAM, and mesiobuccal cusp tips of the maxillary first molars (Mx) from the horizontal orbital plane (Orb) and the coronal plane were measured on the three-dimensional computed tomographic images. Their right and left side distance discrepancies were calculated and statistically compared. Results: EAM was located 10.3 mm below and 2.3 mm anterior to Po in group I. The vertical distances from Po to EAM of both sides were significantly different in group II-B (p=0.001), while other groups were not. Interside discrepancy of the vertical distances from EAM to Mx in group II-B also showed the significant differences, as compared with those from Po to Mx and from Orb to Mx. Conclusions: The subjects with facial asymmetry and prominent maxillary occlusal cant tend to have the symmetric position of Po but asymmetric EAM. Some caution or other measures will be helpful for them to be used during the clinical procedures.

청각자극에 따른 세타파와 알파파의 스펙트럼적 반응 (Spectral Perturbation of Theta and Alpha Wave for the Affective Auditory Stimuli)

  • 감정을 유발하는 청각자극에 따라 발생되는 EEG(Electroencephalographic) 스펙트럼 강도와 감정 반응 간의 상관관계는 중요한 변수이다. 약 20여 년 전 쾌감을 주는 청각자극에 대하여 측정된 신호의 강도가 전전두엽에서 비대칭적으로 반응을 보였다는 연구 결과가 발표되었다. 본 연구에서는 자극관련 스펙트럼적 반응(Event-related Spectral Perturbation: ERSP)이 전두엽에서 비대칭적으로 발생되는지 고찰하기 위하여 청각자극 실험을 수행하였다. 30명의 건강한 대학생 지원자들이 IADS(International Affective Digital Sounds) 실험에 참여하였다. 이 청각자극은 행복, 공포, 그리고 중립의 3단계로 분류되었으며, 데이터의 분석은 세타파(4-8Hz)와 알파파(8-13Hz)에서 이루어졌다. 알파대역의 ERSP 지도는 오른쪽 전두엽에서 행복감과 공포감의 자극에서 높은 신호의 강도를 보여주는 반면 중립성 자극에서는 왼쪽 전두엽에서 높은 신호가 발생되었다. 그러나 세타파는 행복감과 공포감의 자극에 따라 좌측 전두엽에서 높은 신호가 나타나는 반면, 중립적 자극에 따라 우측 전두엽에서 높은 신호를 보여주었다. 높은 쾌감성 자극(행복)은 상대적으로 우측 전두엽에서 높은 EEG 신호가 측정되었으며, 낮은 쾌감성 자극(중립과 공포)은 왼쪽 전두엽에서 측정되었다. 또한 가장 큰 세타파의 변화가 전두엽의 중앙에서 발생되었다. 알파파에서 행복한 청각자극에 가장 강한 신호가 전두엽 전체에서 관찰되었다. 이 결과들은 기존의 감정들과 일치되며, 세타파와 알파파가 감정처리에서 중요한 역할을 하고 있음을 시사한다.

전치 유도각의 인위적 증가에 의한 저작근과 하악 운동 양상의 변화에 대한 연구 (A STUDY FOR THE CHANGES OF THE MASTICATORY MUSCLES AND THE MANDIBULER MOVEMENT EFFECTED BY INTENTIONAL INCREASE OF ANTERIOR GUIDANCE ANGLE)

  • This study was performed to measure the changes of the mandibular movement and the masticatory muscular activities - anterior temporal and masseter muscle of both side - reflected by intentional increase of anterior guidance angie. For this study, 5 volunteers (3 males and 2 females with average age of 24.0) were selected. Each volunteer had Angle's classification I and did not have any missing tooth except third molar and any extensive restorations. Metallic guide plate was made at volunteer's working model fabricated by improved dental stone and cemented to the palatal surface of maxillary central incisor using resin cement(Panavia $21^{(R)}$) and then adjusted not to give any occlusal interferences at intercuspal position. The activity of masticatory muscles and the changes of mandibular movement were recorded by EMG and Sirognathograph in Biopak analysing system(Bioresearch Inc., Milwaukee, Wisconsin, USA). Measurement was done at before experiment, immediatley after placement, 1 week after placement, immediately after removal, and 1 week after removal. The results were as follows: 1. Moderate phonetic disturbance and mild headache were occured to 3 volunteers for 2 days after setting and 1 volunteer had positive reaction to percussion and slight midline diastema. But all of these clinical signs were diappeared 1 week after removal and the other volunteer did not have any special clinical sign. 2. In the EMG of the mandibular rest position, the mean value of anterior tempotal muscle was increased immediately after placement(p<0.01) and then decreased 1 week after placement(p<0.05) and increased 1 week after removal(p<0.05) but not recovered as before experiment. The mean value of masseter muscle was decreased during the experiment period. 3. In the EMG during mandibular protrusive movement, all muscular activity was decreased during the experiment period. Reduced activity was not recovered 1 week after removal(p<0.03). 4. During the habitual opening, anteroposterior movement of mandible was decreased immediately after placement(p<0.05) and then increased 1 week after placement but not statistically significant(p>0.1). Vertical movement was not shown significant difference during the experiment period(p>0.1). Lateral movement was decreased immediately after placement(p<0.05) and then increased 1 week after placement but not recovered as before experiment. The opening and closing velocity of mandible was shown minor changes but not statistically significant. 5. During the habitual opening, anteroposterior movement of mandible was decreased 1 week after placement(p<0.05) and then increased immediately after removal and recovered 1 week after removal as before experiment. Vertical movement was not shown significant changes. Lateral displacement of mandible was increased continuously and recovered 1 week after removal. Opening velocity was temporarily increased immediately after removal but recovered and closing velocity was not shown significant changes. 6. During the right side chewing, anteroposterior movement of mandible was increased immediately after removal but recovered and vertical movement was not shown statistically significant results. Lateral displacement and velocity of mandible were not shown significant results. 7. During the left side chewing, the changes of mandibular movement pattern were not shown statistically significant results.
