Bone Metastasis in Gastric Cancer Patients

  • Purpose: Bone metastasis from stomach cancer occurs only rarely and it is known to have a very poor prognosis. This study examined the clinical characteristics and prognosis of patients who were diagnosed with stomach cancer and bone metastasis. Materials and Methods: The subjects were 19 patients who were diagnosed with stomach cancer at Hanyang University Medical Center from June 1992 to August 2010 and they also had bone metastasis. The survival rate according to many clinicopathologic factors was retrospectively analyzed. Results: 11 patients out of 18 patients (61%) who received an operation were in stage IV and the most common bone metastasis location was the spine. Bone scintigraphy was mostly used for diagnosing bone metastasis and PET-CT and magnetic resonance imaging were used singly or together. The serum alkaline phosphatase at the time of diagnosis had increased in 12 cases and there were clinical symptoms (bone pain) in 16 cases. Treatment was given to 14 cases and it was mostly radiotherapy. There were 2 cases of discovering bone metastasis at the time of diagnosing stomach cancer. The interval after operation to the time of diagnosing bone metastasis for the 18 cases that received a stomach cancer operation was on average $14.9{\pm}17.3$ months and the period until death after the diagnosis of bone metastasis was on average $3.8{\pm}2.6$ months. As a result of univariate survival rate analysis, the group that was treated for bone metastasis had a significantly better survival period when the bone metastasis was singular rather than multiple, as compared to the non-treatment group, yet both factors were not independent prognosis factors on multivariate survival analysis. Conclusions: An examination to confirm the status of bone metastasis when conducting a radio-tracer test after the initial diagnosis and also after an operation is needed for stomach cancer patients, and bone scintigraphy is the most helpfully modality. Making the diagnosis at the early stage and suitable treatments are expected to enhance the survival rate and improve the quality of life even for the patients with bone metastasis.

  • Metastasis of gastric adenocarcinoma to the prostate gland is extremely rare. Herein, we report a case of gastric adenocarcinoma in a 56-year-old man with prostatic metastasis diagnosed through the analysis of biopsy specimens from representative lesions in the stomach and prostate gland. Immunohistochemistry of the prostatic tissue showed positive staining for cytokeratin 7 and negative staining for prostate-specific antigen (PSA), whereas the serum PSA level was normal, confirming the diagnosis of prostatic metastasis from carcinoma of the stomach.

  • 1. Objectives We introduced a case of postoperation stomach cancer patient with metastasis to peritoneum and colon who suffered from repetitive diarrhea and abdominal pain. 2. Methods Stomach cancet is the most common cancer in Korea. Eventhough the 5-year survival rate of stomach cancer is increasing recently, metastasized stomach cancer does not show good prognosis, especially when the surgical operation is not applicable. After partial resection of stomach, most of patients experienced gastrointestinal disorders like dyspepsia, reduced meal size, abdominal discomfort, loose stool, and constipation. 3. Results and Conclusions Taeumjowi-tang, a Sasang Constitutional Medicine, improved the diarrhea and abdominal pain.

Function of hepatocyte growth factor in gastric cancer proliferation and invasion

  • Cancer incidence has been increasing steadily and is the leading cause of mortality worldwide. Gastric cancer is still most common malignancy in Korea. Cancer initiation and progression are multistep processes involving various growth factors and their ligands. Among these growth factors, we have studied hepatocyte growth factor (HGF), which is associated with cell proliferation and invasion, leading to cancer and metastasis, especially in gastric cancer. We explored the intercellular communication between HGF and other surface membrane receptors in gastric cancer cell lines. Using complimentary deoxyribonucleic acid microarray technology, we found new genes associated with HGF in the stomach cancer cell lines, NUGC-3 and MKN-28, and identified their function within the HGF pathway. The HGF/N-methyl-N'-nitroso-guanidine human osteosarcoma transforming gene (c-MET) axis interacts with several molecules including E-cadherin, urokinase plasminogen activator, KiSS-1, Jun B, and lipocalin-2. This pathway may affect cell invasion and metastasis or cell apoptosis and is therefore associated with tumorigenesis and metastasis in gastric cancer.

Hepatic extraskeletal osteosarcoma with systemic metastasis in a dog

  • A ten-year-old dog was presented with pancreatitis and increased hepatic enzymes. On necropsy a large firm mass was observed in the liver extending to the gall bladder. Smaller masses with similar texture were found in multiple organs including lung, stomach, pancreas, lymph nodes, omentum, and mesentery. Neoplastic cells were spindle shaped with prominent osteoid production and occasional trabeculae of woven bone. Tumor cell emboli were observed in the blood vessels and lymphatics of the omentum and stomach. Hepatic osteosarcoma with systemic metastasis is very rare and may serve to broaden the diagnostic spectrum of hepatic and pancreatic diseases in dogs.

간 전이를 동반한 위의 간양 선암 1예 (Hepatoid Adenocarcinoma of the Stomach with Liver Metastasis)

  • 위암의 한 아형인 간양 선암은 조직학적으로 간세포암종과 형태학적으로 유사하며 알파태아단백 생산을 특징으로 한다. 간양 선암의 또 하나의 특징은 간과 림프절로의 조기 전이를 함으로써 불량한 예후를 갖는다는 것이다. 저자들은 간전이를 동반한 위의 간양 선암 1예를 보고하고자 한다. 소화불량을 주소로 내원한 52세 남자 환자는 위내시경 검사상 Borrmann 제2형 병변이 위암이 의심되었다. 환자의 혈청 알파태아단백은 123 ng/ml로 증가되어 있었다. 근치적 위아전절제술과 간우엽절제술을 시행하였고 조직학적 소견상 위의 병변은 점막하층에 국한된 간양 선암이었으며 간의 병변은 위로부터 전이된 선암이었다. 결국 환자는 위암 4기(T1N1M1)로분류되었다. 위의 간양 선암은 조기병변에서도 불량한 예후군으로 분류될 수 있다.

위암에서 새로운 제8판 AJCC 병기 분류의 임상적, 조직 병리학적 시사점 (Clinicopathologic Implication of New AJCC 8th Staging Classification in the Stomach Cancer)

  • Stomach cancer is the fifth most common malignancy in the world. The incidence of stomach cancer is declining worldwide, however, gastric cancer still remains the third most common cause of cancer death. The tumor, node, and metastasis (TNM) staging system has been frequently used as a method for cancer staging system and the most important reference in cancer treatment. In 2016, the classification of gastric cancer TNM staging was revised in the 8th American Joint Committee on Cancer (AJCC) edition. There are several modifications in stomach cancer staging in this edition compared to the 7th edition. First, the anatomical boundary between esophagus and stomach has been revised, therefore the definition of stomach cancer and esophageal cancer has refined. Second, N3 is separated into N3a and N3b in pathological classification. Patients with N3a and N3b revealed distinct prognosis in stomach cancer, and these results brought changes in pathological staging. Several large retrospective studies were conducted to compare staging between the 7th and 8th AJCC editions including prognostic value, stage grouping homogeneity, discriminatory ability, and monotonicity of gradients globally. The main objective of this review is to evaluate the clinical and pathological implications of AJCC 8th staging classification in the stomach cancer.

Endoscopic diagnosis of gastric metastases from malignant melanoma: systematic review

  • Background/Aims: Metastases of malignant melanoma (MM) are rare and associated with poor prognosis. The objective of this study was to analyze the clinical and endoscopic characteristics of gastric metastases of MM by systematically reviewing cases and case series involving patients diagnosed using upper gastrointestinal endoscopy. Methods: The PubMed and LILACS databases were searched. Reports containing individual patient data were included. Outcomes such as clinical data, endoscopic findings, treatments, and survival were analyzed. Results: A total of 88 studies with individual data from 113 patients with gastric metastases of MM were included. The primary sites of MM were the skin (62%), eyes (10%), and mucous membranes (6%). Most patients (56%) had multiple metastases in the stomach, located predominantly in the gastric body (approximately 80%). The overall survival rate at 2 years was 4%. There was a significant reduction in the survival of patients with multiple gastric metastases compared to that of patients with single metastasis (hazard ratio, 0.459; 95% confidence interval, 0.235-0.895; p=0.022). Conclusions: Gastric metastases of MM have a poor prognosis, especially in patients with multiple implants in the stomach. Additional studies are needed to verify whether ocular and mucosal melanomas are associated with a higher risk of gastric metastases than that of cutaneous melanomas.

Intrahepatic Splenosis Mimicking Liver Metastasis in a Patient with Gastric Cancer

  • A 54 year old man was referred to our hospital with gastric cancer. The patient had a history of splenectomy and a left nephrectomy as a result of a traffic accident 15 years earlier. The endoscopic findings were advanced gastric cancer at the lower body of the stomach. Abdominal ultrasonography (USG) and magnetic resonance imaging demonstrated a metastatic nodule in the S2 segment of the liver. Eventually, the clinical stage was determined to be cT2cN1cM1 and a radical distal gastrectomy, lateral segmentectomy of the liver were performed. The histopathology findings confirmed the diagnosis of intrahepatic splenosis, omental splenosis. Hepatic splenosis is not rare in patients with a history of splenic trauma or splenectomy. Nevertheless, this is the first report describing a patient with gastric cancer and intrahepatic splenosis that was misinterpreted as a liver metastatic nodule. Intra-operative USG guided fine needle aspiration should be considered to avoid unnecessary liver resections in patients with a suspicious hepatic metastasis.
