A Study on the Relationship between Digital Infrared Thermal Imaging (DITI), Kupperman’s Index (KI) and Menopause Rating Scale (MRS) of Postmenopausal Women (일개 한방병원에 내원한 갱년기 여성의 연령에 따른 적외선체열분포(DITI)와 Kupperman’s Index(KI), Menopause Rating Scale(MRS)간의 상관성 연구)

  • Objectives : To study correlation between Digital Infrared Thermal Imaging, Kupperman’s Index (KI) and Menopause Rating Scale (MRS) of Postmenopausal Women Methods : We analyzed 114 patients visiting Pohang Oriental Hospital from 1 st January 2014 to 30 th September 2015. The evaluating indexed of this trial were Age, DITI, KI, MRS. Temperature and different of temperature, such as In-dang (HN3), dan-Jung (CV17), gwan-won (CV4), hyup-back (LU4), no-gung (PC8), bock-to (ST31), and tae-chung (LR3). Results : Age was no correlation with DITI, KI, and MRS. There were positive correlation with KI and MRS significantly. DITI was not correlated with Age, KI and MRS, while ⊿T1 (LU4-PC8) was correlated with ⊿T2 (ST31-LR3) and ⊿T3 (CV17-CV4). Conclusions : There were non-significant correlation between DITI and Menopausal scale, but significant correlation in DITI themselves, Menopausal themselves. The More studies ar needed for the more accurate oriental medical diagnosis

Administration Progression and Adverse Event Report of Er-Xian Decoction for Menopausal Hot Flushes: Case Report and Review of Literature (폐경기 안면홍조 증상에 대한 한약처방 이선탕의 투여경과 및 이상반응 보고 : 증례보고 및 문헌고찰)

  • Objective: We report on the administration progression and adverse event of Er-Xian decoction for menopausal hot flushes. A hot flush is a rapid and intense heat-dissipation reaction that involves considerable sweating, enlargement of the peripheral blood vessels, and internal body heat. Er-Xian decoction is a prescription for treating menopausal hot flush symptoms in postmenopausal women. We report administration progression and adverse event of Er-Xian decoction for menopausal hot flush. Methods: We used the Menopause Rating Scale and Menopause-Specific Quality of Life questionnaire to evaluate the progression of hot flushes, and we conducted a literature review to determine the effective dosage of Er-Xian decoction. Result: In all cases, Facial hot flushes and evaluation variables were improved by the administration of EXD. However, in Case 1, a rise of liver function indexes which may be related to EXD administration was observed. Conclusion: Considering that menopausal flushing usually lasts for several years, The resluts are meaningful that the short-term administration of EXD led to improvement of symptoms. In order to further use EXD in the future, it will be necessary to conduct follow-up studies on the subject of safety verification, such as repeated dose toxicity studies.

Analysis of Metabolic Syndrome Risk Factors among the Menopausal Women in Her Fifties (50대 폐경 여성의 대사증후군 위험인자 분석)

  • Purpose: The purpose of this study was to assess the difference of metabolic syndrome (MS) risk factors according to the MS diagnosis in the age of fifties women. Methods: Forty-two subjects (non metabolic syndrome 30, metabolic syndrome 12) were recruited from the health promotion center of a tertiary care hospital in an urban city. MS was defined by third report of the national cholesterol education program (NCEP) expert panel on detection, evaluation, and treatment of high blood cholesterol in adults (Adult Treatment Panel III) (ATP III), and abdominal obesity was determined by Asia-Pacific criteria in waist circumference. Results: The prevalence of MS by definition of NCEP-ATP III and Asia-Pacific criteria in waist circumference was 28.6% in the age of fifties women. The educational level and the median HDL-cholesterol were significantly lower in the MS patient group than in the Non-MS group. The median of weight, triglyceride, and fasting glucose were significantly higher in the MS patient group than in the Non-MS group. Conclusion: These results indicate that the nursing care should be focused on weight, triglyceride, HDL-cholesterol, and fasting glucose of the MS patients in the fifties women.

A Case Report of Menopausal Syndrome treated by Herbal medicine based on Shanghanlun provisions (『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 준거하여 한약 투여 후 호전된 갱년기 증후군 1례 임상 보고)

  • Objective : This case report aims to demonstrate the effect of Herbal medicine on menopausal syndrome. Methods : A 54 years woman suffered from menopausal syndrome with hot flush, insomnia and dyspepsia. According to Diagnostic system based on Shanghanlun provisions, the patient was diagnosed with Taeyang-byung and treated by Sosiho-tang decoction at first. But the next time the patient visited, she was treated by Oryeong-san decoction. The result of administrations were evaluated by Kupperman's index(KI), Menopause rating scale(MRS). Results : After administration of Sosiho-tang decoction for 35 days, KI was decreased from 50 to 19, MRS was decreased from 9 to 4. But her chronic dyspepsia wasn't getting better. So the KMD prescribed Oryeong-san decoction. After administration of Oryeong-san decoction for 14 days, KI was decreased from 19 to 0, MRS was decreased from 4 to 1. And her chronic dyspepsia was functionally improved. Conclusions : The patient completely recovered from menopausal syndrome and dyspepsia as treated by Sosiho-tang decoction and Oryeong-san decoction according to Diagnostic system based on Shanghanlun provisions.

A Review of Acupuncture Treatment Methods for Urinary incontinence and Overactive bladder in Postmenopausal Women (폐경 후 여성 요실금과 과민성 방광의 침 치료법에 대한 고찰)

  • Objectives: The purpose of this study is to review the acupuncture treatment Urinary incontinence (UI) and Overactive bladder (OAB) in postmenopausal women. Methods: We searched articles in 8 search engines with keywords related to 'Overactive bladder', 'Urinary incontinence' and 'Menopause' in July 2022. Randomized Controlled Trials (RCT) that used acupuncture on Urinary incontinence (UI) and Overactive bladder (OAB) after menopause were included. Animal studies and non RCT data were excluded. Data on acupuncture treatment such as methods, site, duration, frequency, and period were analyzed. Also, network analysis between acupoints was conducted. Results: 15 articles were selected and analyzed. Studies were conducted using manual acupuncture, electroacupuncture, pharmacopuncture and fire acupuncture. Most studies used more than one acupoint, and there were 32 acupoints selected for acupuncture treatment for UI and OAB after menopause. The most commonly used acupoint was 中極 (CV3) (n=8). In terms of the correlation of acupoints, 太谿 (KI3) had the highest value of degree centrality at 0.75. The mean treatment time, number of treatments, and duration were 26.42±6.10 minutes, 18.71±9.09 times, and 6.87±4.77 weeks. Conclusions: The results of this study could be useful in establishing the evidence for performing standardized acupuncture treatment for Urinary incontinence and Overactive bladder in postmenopausal women.

A Retrospective Analysis on Outpatients with Climacteric Syndrome and Postmenopausal Syndrome in a Korean Medicine Hospital (일개 한방병원에 내원한 갱년기장애 및 폐경기후증후군 외래 환자에 대한 후향적 분석)

  • Objectives: This study aimed to identify the current use and investigate the clinical practice of Korean Medicine for climacteric syndrome and postmenopausal syndrome outpatients. Methods: We studied climactic syndrome and postmenopausal syndrome outpatients who visited Daejeon Korean Medicine Hospital of Daejeon University from January 1, 2019 to December 31, 2021 via an analysis of the medical records. Results: 130 patients were finally selected. Their average age was 51.91±4.57 years and the average number of visits was 7.51±11.40 times. Patients usually complained two or more addresses, and the most frequent symptoms were hot flushes. 126 patients took herbal medicines and 99 of them took herbal decoctions. 106 patients were treated by acupuncture. 58 patients were treated by pharmacopuncture and Hominis Placenta was used to 50 subjects. As for the improvement rate by symptoms, hot flushes were the highest at 65.5%. We compared the improvement rate of groups taking Guibi-tang-gami A and Guibi-tang-gami B, the common herbal decoction of Daejeon Korean Medicine Hospital of Daejeon University. In the group of Guibi-tang-gami A, the improvement rate of hot flushes was the highest and in the other, vaginal dryness was the highest. Urinary frequency of the group with Hominis Placenta pharmacopuncture showed more than three times higher improvement rate of the non-executive group. Conclusions: We analyzed general characteristics, clinical characteristics, types of Korean medicine treatment, improvement rates by symptoms of 130 outpatients diagnosed with climactic syndrome and postmenopausal syndrome. This study could be used as reference to provide practical data of outpatient treatment for Climacteric Syndrome and Postmenopausal Syndrome.

The Effect of a Dietary Companion on Metabolic Syndrome in Adult Women (성인 여성의 식사 동반자 여부가 대사증후군에 미치는 영향)

  • Purpose: This study aimed to compare the differences in the prevalence of metabolic syndrome between menopausal women and women of childbearing age and to determine the risk of metabolic syndrome among women in each group depending on whether they eat alone. Methods: Data of 1,813 women from the seventh Korea National Health and Nutrition Examination Survey (2016) were used. The collected data were analyzed using SPSS 20.0, and complex sample frequency analysis, descriptive statistics, complex sample cross analysis, complex sample general linear regression, and complex sample logistic regression analysis were performed. Results: According to the results of the study, there was no difference in the prevalence and risk of metabolic syndrome according to the presence of companions during meals between women of childbearing age and post-menopausal women, but there was a difference in health behavior. In other words, women of childbearing age who ate alone had a lot of experience of drinking, and menopausal women who ate alone did not tend to make any efforts to control their weight and did not perform aerobic exercise. In particular, the negative health behavior of menopausal women who ate alone increased the risk of prevalence of metabolic syndrome. Conclusion: The findings indicate that, for women who eat alone, interventions to prevent metabolic syndrome should be differentiated before and after menopause. Therefore, it is suggested to offer an educational program to prevent metabolic syndrome in women of childbearing age as well as provide regular assessments to diagnose metabolic syndrome and health behavior improvement programs for menopausal women.

Two Cases Report of Climacteric Syndrome were Measured by Kupperman's Index (Kupperman's index로 평가된 갱년기증후군 치험 2례)

  • Purpose: The purpose of this study is to report the effect of Traditional Korean Medicine(TKM) on climacteric syndrome. And we wanted that climateric symptoms were measured objectively and diagnosed definitely using climacteric index. Methods: Two patients complained climacteric symptoms. We treated them by TKM, and evaluated the progress of symptoms by Kupperman's index. And then We compared climacteric symptoms before treatment with one after treatment by Kupperman's index. Results: After TKM treatment, the climacteric symptoms were improved and mean score of K upperman's index was reduced (one reduces from 77 to 53, the other reduces from 65 to 25). Conculsion: TKM treatment is effective on climacteric syndrome. And Kupperman's index would be valid to measure for treatment's effect of climacteric syndrome.

A Report on the Results of Korean Medicine Health Promotion Project 'Vitality UP Health UP Class for Climacteric Women' in Boeun-gun in 2022 (2022년도 보은군 한의약 건강증진사업 '갱년기 여성 활력UP 건강UP 교실'에 대한 결과 보고)

  • Objectives: The purpose of this health promotion project for Korean medicine is to promote the health of climacteric women in Boeun-gun. Methods: During the project, pressure needle acupuncture treatment, herbal medicine treatment, meditation pore therapy, and health education were conducted. To evaluate the results, basic health surveys (body height, body composition test, blood pressure etc.), female hormone tests, Kupperman's index (KI), Menopause-specific quality of life questionnaire (MENQOL), and Perceived stress scales (PSS) were conducted. Results: No significant change was observed in the body composition test and blood pressure after the project. Follicle stimulating hormone (FSH) was significantly increased after the project, but Luteinizing Hormone (LH) and total estrogen levels were not significantly changed. KI and MENQOL scores significantly decreased after the project, and significantly decreased in 4 out of 11 items of KI and 3 out of 4 domains of MEMQOL. No significant change was observed in the PSS score after the project. Conclusions: From the results of this project, it can be seen that the Korean medicine health promotion project can help alleviate symptoms of climacteric syndrome and postmenopausal syndrome.

An Analysis of The Correlation between Breast-feeding, Bone Mineral Density and Metabolic Syndrome in Elderly Women (여성노인의 대사증후군과 모유수유, 골밀도와의 연관성 분석)

  • Women are reported to have increased risk of metabolic syndrome after menopause. Nevertheless, there is a lack of study on the convergent association between breast-feeding, bone mineral density(BMD) and metabolic syndrome due to women's childbirth. In this study, the data of 939 elderly women using raw data from the fifth Korean National Health and Nutrition Examination Survey(KNHANES V-1 and 2) in 2010 and 2011 were analyzed. The correlation between breast-feeding children, BMD and metabolic syndrome was analyzed by dividing them into three groups based on the number of breast-feeding children. As a result of the analysis, no specific association was found between risk factors of metabolic syndrome and BMD according to the increase in the number of breast-feeding children after adjustment for confounders. However, elderly women with a large number of breast-feeding children showed a significant association with more risk factors of metabolic syndrome. These findings can be used as a basic material for the prevention and diagnosis of metabolic syndrome and health care in elderly women.
