건강한 폐경이행 구조모형 (Structural Equation Modeling on Healthy Menopausal Transition)

  • Purpose: This study was designed to construct and test structural equation modeling on healthy menopausal transition in middle-aged women in order to identify variables affecting healthy menopausal transition. Methods: Participants, 276 women, 45 to 60 years of age, with menopausal symptom score higher than 5 on the Korean version of Menopause Rating Scale, were recruited in three cities and one county of Gyeongnam Province. Research data were collected via questionnaires and analysed using SPSS version 18.0 and AMOS version 20.0. Results: After confirmatory factor analysis, one of the observed variables was excluded due to relatively low factor loading. The model fit indices for the hypothetical model were suitable for the recommended level: GFI=.93, CFI=.92, RMSEA=.05. Self-efficacy, self-differentiation, and menopausal symptoms explained 67.7% of variance in menopausal transition, and self-differentiation was the most influential factor for menopausal transition. Self efficacy and menopausal symptoms explained 9.6% of variance in menopausal management, although "menopausal symptoms" was not significant. Conclusion: These results suggest that nursing interventions to improve self-differentiation, self efficacy, menopausal management and decrease menopausal symptoms are critical for healthy menopausal transition in middle-aged women. Continued development of a variety of community-based nursing interventions to facilitate healthy menopausal transition is suggested.

유방암 환자의 항암화학요법 관련 인지기능 변화와 삶의 질의 구조모형 (A Structural Model for Chemotherapy Related Cognitive Impairment and Quality of Life in Breast Cancer Patients)

  • Purpose: This study aimed to develop and test a structural model for chemotherapy-related cognitive impairment of breast cancer patients based on a literature review and Hess and Insel's chemotherapy-related cognitive change model. Methods: The Participants consisted of 250 patients who were ${\geq}19$ years of age. The assessment tools included the Menopause Rating Scale, Symptom Experience Scale, Hospital Anxiety and Depression Scale, Everyday Cognition, and Functional Assessment of Cancer Therapy-Breast Cancer. Data were analyzed using the SPSS 21.0 and AMOS 21.0 programs. Results: The modified model was a good fit for the data. The model fit indices were ${\chi}^2=423.18$ (p<.001), ${\chi}^2/df=3.38$, CFI=.91, NFI=.91, TLI=.89, SRMR=.05, RMSEA=.09, and AIC=515.18. Chemotherapy-related cognitive impairment was directly influenced by menopausal symptoms (${\beta}=.38$, p=.002), depression and anxiety (${\beta}=.25$, p=.002), and symptom experiences (${\beta}=.19$, p=.012). These predictors explained 47.7% of the variance in chemotherapy-related cognitive impairment. Depression and anxiety mediated the relations among menopausal symptoms, symptom experiences, and with chemotherapy related cognitive impairment. Depression and anxiety (${\beta}=-.51$, p=.001), symptom experiences (${\beta}=-.27$, p=.001), menopausal symptoms (${\beta}=-.22$, p=.008), and chemotherapy-related cognitive impairment (${\beta}=-.15$, p=.024) had direct effects on the quality of life and these variables explained 91.3%. Conclusion: These results suggest that chemotherapy-related toxicity is highly associated with cognitive decline and quality of life in women with breast cancer. Depression and anxiety increased vulnerability to cognitive impairment after chemotherapy. Nursing intervention is needed to relieve chemotherapy-related toxicity and psychological factor as well as cognitive decline for quality of life in patients undergoing chemotherapy.

  • Objectives: The purpose of this study is to report on the efficacy of Korean medical treatment for treating menopausal vasomotor symptoms (heat flashes and sleep disorders). Methods: We treated a menopausal female patient with heat flashes and sleep disorders using Korean medical treatments (herbal medicines, acupuncture, pharmacopuncture, moxibustion, and chuna). To evaluate the results of this treatment, we used Kupperman's Index and the Numerical Rating Scale to measure heat flashes and total sleeping time, respectively. The patient's general health status was evaluated using the European Quality of Life 5 Dimensions Scale. Results: We observed improvements in Kupperman's Index, the Numerical Rating Scale, the patient's total sleeping time, and the European Quality of Life 5 Dimensions Scale following the treatments. Conclusions: This study suggests that Korean medicine may be an effective treatment for menopausal vasomotor symptoms (heat flashes and sleep disorders).

폐경 전후 여성의 핵심감정척도 단축형을 활용한 정서적 특성 연구 (Evaluation of Emotional Characteristics of Premenopausal and Postmenopausal Women Using the Core Seven Emotions Inventory-Short Form)

  • Objectives: The study evaluated middle-aged women using the Core Seven Emotions Inventory-short form (CSEI-s) followed by a correlation analysis with Menopause Rating Scale (MRS). Methods: In this study, the medical records of 47 women who visited W University Oriental Medicine Hospital and completed the CSEI-s and MRS were evaluated. A total of 47 women were analyzed using SPSS (Statistical Package for the Social Science, IBM, United States of America, Version 23.0). Frequency analysis, descriptive analysis, independent t-test, one-way ANOVA, Scheffe test, and correlation analysis were conducted. Results: 1. Approximately one-third of all subjects were more than cautious expressing joy and fright. 2. According to their educational status, high-school graduates thought they were superior to those with a bachelor's degree or above. 3. Anger was expressed with caution in the group with severe menopausal symptoms. 4. Anger, thought, depression, sorrow, fear, and fright showed a positive correlation with MRS scores. 5. Among premenopausal and postmenopausal women, fright was associated with menopausal symptoms and response by the autonomous nervous system. Conclusions: The results suggest that CSEI-s can be used to measure the psychological symptoms of middle-aged women.

폐경 전후 여성의 사상체질과 폐경상태에 따른 폐경증상 (Menopausal Symptoms according to Sasang Constitution and Menopausal Status among Perimenopause Women)

  • Purpose: The purpose of this study was to address the difference menopausal symptoms according to sasang constitution and menopausal status among perimenopausal women. Methods: Data for this cross-sectional study was collected by administering questionnaires that elicited general information. The menopause rating scale (MRS) and Questionnaire for the Sasang Constitution Classification II (QSCC II) was given to 284 perimenopausal women who met the eligibility criteria and agreed to participate in the study. Results: The study subjects were composed of 27.5% so-yangin, 24.6% so-eumin, 18.7% tae-eumin, and 29.6% undefined category. The total MRS and all subscales were significantly higher for postmenopausal women in comparison to premenopausal women. Especially, so-eumin and tae-eumin displayed significantly higher scores in urogenital symptoms. The MRS score in postmenopausal women who belonged to the group of tae-eumin and so-eumin was significantly higher than those for premenopausal women. However, so-yangin displayed higher scores in psychological and urogenital symptoms than tae-eumin for premenopausal women. So-eumin presented significantly higher scores in urogenital symptoms in comparison to the undefined category for postmenopausal women. Conclusion: These findings suggest that sasang constitution could be an important factor in understanding the woman's menopausal symptoms and identify the best treatment.

『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 준거하여 한약 투여 후 호전된 갱년기 증후군 1례 임상 보고 (A Case Report of Menopausal Syndrome treated by Herbal medicine based on Shanghanlun provisions)

  • Objective : This case report aims to demonstrate the effect of Herbal medicine on menopausal syndrome. Methods : A 54 years woman suffered from menopausal syndrome with hot flush, insomnia and dyspepsia. According to Diagnostic system based on Shanghanlun provisions, the patient was diagnosed with Taeyang-byung and treated by Sosiho-tang decoction at first. But the next time the patient visited, she was treated by Oryeong-san decoction. The result of administrations were evaluated by Kupperman's index(KI), Menopause rating scale(MRS). Results : After administration of Sosiho-tang decoction for 35 days, KI was decreased from 50 to 19, MRS was decreased from 9 to 4. But her chronic dyspepsia wasn't getting better. So the KMD prescribed Oryeong-san decoction. After administration of Oryeong-san decoction for 14 days, KI was decreased from 19 to 0, MRS was decreased from 4 to 1. And her chronic dyspepsia was functionally improved. Conclusions : The patient completely recovered from menopausal syndrome and dyspepsia as treated by Sosiho-tang decoction and Oryeong-san decoction according to Diagnostic system based on Shanghanlun provisions.

중년여성에서의 혈관운동증상과 우울, 스트레스, 삶의 질과의 상관성 (Associations between Vasomotor Symptoms and Depression, Stress and Quality of Life in Midlife Women)

  • 연구목적 본 연구의 목적은 폐경주위기, 폐경 후기 여성에서 혈관운동증상과 인구통계학적 변인들과 건강행태, 내과적 질환, 우울, 스트레스, 불안, 폐경에 대한 태도 및 삶의 질과의 관계를 규명하는 것이다. 방 법 한국건강관리협회에 건강검진을 위해 방문한 중년 여성을 대상으로 단면조사연구를 실시하였다. 대상자는 혈관운동증상과 관련 요인에 대한 자가보고 설문지를 작성하였다. 혈관운동증상과 요소들간의 상관관계가 분석되었다. 통계 분석에는 SPSS를 사용하였다. 결 과 폐경주위기 및 폐경후기 여성의 혈관운동증상의 발생률은 63.9%로 나타났다. 혈관운동증상 유무에 따른 유의한 차이를 보인 변인들은 BDI, BEPSI-K, 임상적으로 유의한 우울증상을 보이는 군의 비율($BDI{\geq}16$), 폐경증상척도, Attitude toward menopause, WHOQOL_BREF의 4개의 부척도(신체적 건강, 심리적, 사회적 관계, 환경), 그리고 PMS/PMDD의 과거력이었다. 이 중 다중 회귀분석에서 유의한 상관성을 보인 요인은 BDI, BEPSI-K, 폐경증상척도, WHOBREF 부척도 중 심리적 척도였다. 결 론 폐경기의 혈관운동증상은 심리적 요인 및 정신사회적 요인들과 상관관계를 보이는 것을 확인하였다. 특히 우울증상간의 밀접한 상관관계를 확인할 수 있었으며, 혈관운동증상을 보이는 중년여성군에서 우울증에 대한 선별검사가 필요할 것이다. 추후 명확한 인과관계를 파악하기 위해 임상군을 대상으로 보다 구조화된 진단을 통해 혈관운동증상과 우울증간의 전향적 연구가 필요할 것이다.

火病을 동반한 갱년기 환자 치험 5례 (5 Cases of Climacteric Syndrome Patients with Hwa-Byung)

  • Objectives: The purpose of this study is to evaluate the effects of Korean traditional treatments for Climacteric Syndrome Patients with Hwa-Byung. Methods: From February 2015 to May 2015, We treated 5 Cases Patients with Korean traditional treatments for a month. We measured treatment effects by Menopause Rating Scale (MRS), Beck's Depression Inventory (BDI), Visual Analogue Scale (VAS) and Digital Infrared Thermal Imaging (DITI). Results: After treatment, First, the scores of MRS, BDI were decreased. Second, the symptoms such as hot flush, pantalgia, palpitation, insomnia, fatigue were significantly improved. Also, the difference of 膻中 (CV17, Danjung) and 關元 (CV4, Guanyuan) temperatures were significantly decreased after treatment. Conclusions: This study suggests that Korean traditional treatments such as Gamicheonglijagam-hwan, acupuncture, moxibustion are effective on Climacteric Syndrome Patients with Hwa-Byung.

갱년기 여성의 안면홍조 증상에 대한 국내 한의학 연구 동향 분석 (A Review on Domestic Study Trends on Hot Flush in Menopausal Women with Traditional Korean Medicine)

  • Objectives: The purpose of this study is to analyse the domestic trends of Traditional Korean Medicine on Hot flush in menopausal women. Methods: We searched six databases and selected relevant papers according to the criteria. Results: 12 papers included case report, literature review, randomized controlled clinical trials (RCTs), Non-RCTs, and retrospective chart analysis study design. 1. The daily diary, Kupperman's Index, Menopause Rating Scale, Menopause-specific Quality of Life Questionnaire, Digital Infrared Thermographic Imaging were used as a measure of the symptoms of hot flush. 2. There were significant correlations between Thermographic images, Pulse energy, Lifestyle Habits, Low frequency power/High frequency power (LF/HF) ratio and certain items of Menopausal Symptoms Measurement. 3. The case report using Er-Xian decoction and Hwangryunhaedoktang-Gamibang showed significant effect. 4. Domestic RCT studies using herbal or acupuncture treatments showed a significant value of 3.4 on average in the quality assessment conducted in the study. However, the number of domestic studies included was five, which is not enough compared to foreign countries. Conclusions: This study examined domestic research trends on hot flushes of menopausal women and found that various evaluation tools and diagnostic methods were applied. It is necessary to conduct various RCT studies in Korea, where the number of studies is insufficient compared to overseas.

Low dietary calcium is associated with self-rated depression in middle-aged Korean women

  • It is thought that calcium (Ca) and magnesium (Mg) may be related to mental disorders such as depression; however, there have been few studies investigating the association between Ca and Mg nutrition status with depression in middle-aged female adults. Study subjects in this study included 105 women between the ages of 41 and 57 years. The subjects were divided into three groups according to the Zung Self-rating Depression Scale (SDS) score: Group I (SDS score < 33 percentile; n = 32), Group II (33 percentile ${\leq}$ SDS score < 67 percentile; n = 37), and Group III (67 percentile ${\leq}$ SDS score; n = 36). Anthropometric measurements, dietary intake survey using 3-day dietary records, SDS questionnaire and measurement of serum Ca and Mg were obtained and analyzed. No differences were observed in Ca, plant Ca, and Mg intake among the three groups. However, animal Ca intake by Group III was 141.3 mg, which was significantly lower than 207.6 mg by Group I and 198.3 mg by Group II (P = 0.0345). There were no significant differences in serum levels of Ca, and Mg among the three groups. Correlation analysis indicated that the SDS score had negative correlations with Ca intake (r = -0.2927, P<0.01) and animal Ca (r = -0.3411, P<0.001) after adjusting for age, menopause and energy intake. In conclusion, dietary Ca and animal Ca had negative associations with SDS score among middle-aged Korean female adults. Additional analysis of factors related to the association of calcium and magnesium nutritional status and depression is necessary.
