Implant assisted removable partial denture using a few remaining maxillary teeth and locator bar system: A case report (소수 잔존치를 가진 상악에 Bar와 Locator®를 이용한 임플란트 융합 국소의치 치료 증례)

  • Treatment with removable partial denture is effective for partially edentulous patients who are unable to obtain sufficient retention and stability for functional and esthetic restoration. There are several cases reporting the improvement of retention and stability of the partial denture using a small number of implants. However, there are limited studies on the implant-assisted removable partial denture using a small number of remaining teeth and the bar locator system. The bar locator system has an advantage in that it could compensate the angle of insertion of removable prosthesis on implant with inconsistent placement angle due to anatomical constraints compared to when using the locator only. This case report describes the patient with $Parkinson^{\circ}$Øs disease who was treated with the Locator bar system using two previously placed implants and two remaining teeth on maxilla. No additional implants could be placed because of the medical and economic condition of the patients, and the angle of one of two implants could not be matched with the direction of the removable partial denture insertion. Considering the angle of the implants, the patient was treated with implant-assisted RPD using the Locator bar system and had satisfactory results in the aspect of esthetics, masticatory function, and oral hygiene maintenance.

Three-Dimensional Kinematic Model of the Human Knee Joint during Gait

  • It is well known that the geometry of the articular surface plays a major role in the kinematic and kinetic analysis to understand human knee joint function during motion. The functionality of the knee joint cannot be accurately modeled without considering the effects of sliding and lolling motions. We Present a 3-D human knee joint model considering sliding and rotting motion and major ligaments. We employ more realistic articular geometry using two cam profiles obtained from the extrusion of the sagittal Plain view of the representative Computerized Tomography image of the knee joint compared to the previously reported model. Our model shows good agreement with the already reported experimental results on Prediction of the lines of force through the human joint during gait. The contact point between femur and tibia moves toward the Posterior direction as the knee undergoes flexion, reflecting the coupling of anterior and Posterior motion with flexion/extension. The anterior/posterior displacement of the contact Point on the tibia plateau during one gait cycle is about 16 mm. for the lateral condyle and 25 mm. for the medial condyle using the employed model Also. the femur motion on the tibia undergoes lateral/medial movement about 7 mm. and 10 mm. during one gait cycle for the lateral condyle and medial condyle. respectively. The developed computational model maybe Potentially employed to identify the joint degeneration.

Reconstruction of the Finger using Rerouting the Transverse Digital Palmar Arch in the Crushing or Avulsion Injured Finger (짓니긴 및 벗겨진 손상에서 가로손가락손바닥활을 이용한 손가락 재건)

  • Purpose: In the finger, there are three major palmar arches in the arterial system. The location of this arches are constant. The middle and distal transverse arches are consistently large (almost 1 mm) and may be used for arterial vessel repairs either proximally or distally, depending on the length and direction needed. This paper describes our experiences in reconstruction and replantation of the finger using rerouting the transverse digital palmar arch. Methods: 31 patients with injuries according to our classification were treated from March of 2005 to October of 2008. In this study the authors subdivided injuries into those with amputation distal to the insertion of the flexor digitorum profundus (Class I, 31 fingers); those with amputation distal to the insertion of the flexor digitorum superficialis (Class II, 4 fingers). Replantation was performed using the artery-only technique with neither vein nor nerve repair. Because the artery has been damaged, it is still possible to make a direct suture by transposing the arterial arch in an inverted Y to I arterial configuration or converting the arch. Venous drainage was provided by an external bleeding method with partial nail excision, medical leech, and repaired margin. Results: The success rate was 87% (n=27) in class I and 75% (n=4) in class II. The authors conclude that crushing and complete avulsion injuries & amputations are salvageable, with acceptable functional results in select patients, especially those with amputation distal to the insertion of the flexor digitorum superficialis. Conclusion: We performed replantation and reconstruction with only-arterial transposing anastomosis successfully, resulting in good recovery of aesthetic and functional outcome. Three major digital palmar arches, especially distal two branches, give us additional treatment options. In the finger replantation and reconstructive techniques using rerouting healthy the transverse digital palmar arch increase the survival rate of the finger.

Analysis of ZjWRKY3, ZjWRKY7 induced by multiple stress in Zoysia japonica (다양한 스트레스에 유도되는 들잔디 ZjWRKY3, ZjWRKY7의 분석)

  • Many crops including cereals, tuber crops, feeds, and turf grasses are often damaged by various environmental stresses such as drought, salt, cold, and high temperature, causing the reduction of their productivity. Plants are sessile and cannot escape from environmental stresses. Thus, plants evolve in the direction of overcoming the environmental stresses. Some plant genes such as ARF, ABI3, NAC, HSF, and WRKY are known to respond to environmental stresses as they transcriptionally regulate the stress response pathways. For example, the OsWRKY76 gene contributes to the enhanced resistance to low temperatures and pathogenic infections. The AtWRKY28 also plays a role in environmental stresses. Zoysiagrass (Zoysia japonica Steud.) is popularly grown for gardens and golf courses. However, the function of the WRKY gene, another environmental stress-related gene, is not known in zoysiagrass. In this study, the ZjWRKY3 and ZjWRKY7 genes with one shared WRKY domain have been isolated in zoysiagrass. The expression of these genes increased in response to low temperature, drought, and salt stresses. Furthermore, the infection of the brown patch-causing Rhozoctonia solani induced the expression of ZjWRKY3 and ZjWRKY7. The corresponding proteins bind to the W-box of the Zjchi promoter, possibly regulating their transcriptions. The researchers suggest that the ZjWRKY3 and ZjWRKY7 genes transcriptionally regulate abiotic and biotic stress related downstream genes.

  • The purpose of this paper is to suggest a direction for future studies based on the analysis of the articles published in the Korean Journal of Hospice and Palliative Care from 1998 to 2012. A total of 240 articles (51 reviews, 189 original) were examined in three five-year groups. Categories of analysis include authors' background (profession, region) and general characteristics and qualitative aspects of the original paper (participants, topic, study design, data analysis, ethical consideration, multidisciplinary approach, research funds and sample size estimation). While the journal publishes more of articles than before, it is mainly due to the increase in the number of review articles, not original articles. As for study topics, healthcare industry and physical symptoms were most frequently studied. The disparity in authors' regional background is fading, and more articles are published by nurses than before. Moreover, more studies are funded while fewer papers tend to adopt a multidisciplinary approach or focus on care givers. Also, in terms of a study design, the number of experimental and methodological studies has slightly increased. In the qualitative aspect, studies considered ethical issues and collected participation consent, and fewer studies reported an estimated sample size. In data analysis, post-adjustment comparison decreased, and new analytical methods are increasingly used. Our results indicate the need to conduct research with more extensive scientific data in various fields of hospice and palliative care.

  • The important items, which should be considered in Private Investigation Law, can include subjects, licenses, the scope of business, qualifying examinations, and supervisory and penal provisions. The subjects of Private Investigation Law should be permitted to be both natural persons and juridical persons in terms of providing various services, but should be permitted to be juridical persons and should be administered on a license system, even in order to ensure public interests. Concretely, the introduction scope of Private Investigation Law can be regulated to include the followings: that is, investigating the whereabouts identification of runaways and missing children, investigating the personal identification, habit, way of action, motivation, whereabouts identification, real child confirmation, association, transaction, reputation, and personality of specific persons or specific groups, investigating the whereabouts identification of missing persons, owners of government-vested properties or renounced properties, investigating the whereabouts of lost properties or stolen properties, investigating the causes of fire, character defamation, slander, damage, accident, physical disability, infringement on real estate or movable property, and investigating all sorts of accidents including traffic accidents, insurance accidents, and medical malpractices. In the qualifying examination, examinees' age should be restricted to be over age 25. The person, who is exempted from its primary examination, should be restricted to be the person, who has the career of over 20 years in related fields, in consideration of its equity with other certificates of qualification. In the supervisory institution, as the policy institution is the supervisory institution in many countries including France (the police) and Japan (public security committee), so the National Policy Agency should be the supervisory institution in consideration of management aspects. In the penal regulations, especially, we should clarify the management of personal information (personal information protection, personal information management), and so should prevent the infringement of people's basic rights, and then should ensure the public interest.

  • Sens of insecutity of the public and professionals about harmful effects of radiation is increasing in an accident at the Chernobyl and Fkushima nuclear power plant.Anxiety was amplified to lack of information about radiation majority of people. To target the middle and high school in the region of Daegu and Gyeongsangbuk-do, to investigate the radiation recognition of the next generation, it is intended to present a model of education for the safe use of radiation. The High School of the six metropolitan cities, city, town through the questionnaire and needs to be educational experience of radiation and use knowledge level of radiation, experience in daily life, understanding of man-made radiation and natural radiation, information channel on radiation, the radiation Distribute the total 800 parts of, to recover the 629 unit, was analyzed for 155 females 474 males. Many people 75.36% of the people, to 24.64% female subjects of this investigation, was constant, respectively from 13 to 18 years age. It is a large number and 30.37% of the respondents as "normal" level of knowledge of radiation, for the type of radiation, most knew. You have answer for risk experience of the medical radiation was higher, touching a lot of information via the broadcast medium in general, and the accuracy is low. I thought we wanted to be educated three or more twice a year, as an educator,about 71.37% and radiation-related understanding of knowledge and background in accordance with the diversification of information channels, the regional differences between urban and rural areas. But I considered the difference age (grade) for each is displayed, intended for junior high school students, the target surface and use the occurrence of radiation, high school students, the need for education about risk and application of radiation through this study.

STUDY ON MONITORING UNIT EFFICIENCY OF FLATTENING-FILTER FREE PHOTON BEAM IN ASSOCIATION WITH TUMOR SIZE AND LOCATION

  • To investigate monitoring unit (MU) efficiency and plan quality of volumetric modulated arc therapy (VMAT) using flattening-filter free (FFF) photon beam in association with target size and location. A virtual patient was generated in Eclipse$^{TM}$ (ver. A10, Varian Medical Systems, Palo Alto, USA) treatment planning system. The length of major and minor axis in axial view was 50 cm and 30 cm, respectively. Cylindrical-shaped targets were generated inside that patient at the center (symmetric target) and in the periphery (asymmetric target, 7.5 cm away from the center of the patient to the right direction) of the virtual patient. The longitudinal length was 10 cm and the diameters were 2, 5, 10 and 15 cm. Total 8 targets were generated. RapidArc$^{TM}$ plans using TrueBeam STx$^{TM}$ were generated for each target. Two full arcs were used and the axis of rotation of the gantry was set to be at the center of the virtual patient. Total MU, homogeneity index (HI), target mean dose, the value of gradient measure and body mean dose were calculated. In the case of symmetric targets, averaged total MU of FFF plan was 23% and 19% higher than that of flattening filter (FF) plan when using 6 MV and 10 MV photons, respectively. The difference of HI, target mean dose, gradient measure and body mean dose between FF and FFF was less than 0.04, 2.6%, 0.1 cm and 2.2%, respectively. For the asymmetric targets, total MU of FFF plan was 21% and 32% was higher than that of FF when using 6 MV and 10 MV photons, respectively. The homogeneity of the target was always worse when using FFF than using FF. The maximum difference of HI was 0.22. The target mean dose of FFF was 3.2% and 4.1% higher than that of FF for the 6 MV and 10 MV, respectively. The difference of gradient measure was less than 0.1 cm. The body mean dose was higher when using FFF than FF about 4.2% and 2.8% for the 6 MV and 10 MV, respectively. No significant differences between VMAT plans of FFF beam and FF beam were observed in terms of quality of treatment plan. The HI was higher when using FFF 10 MV photons for the asymmetric targets. The MU was increased noticeably when using FFF photon beams.

A Study on Health Care Activities of Some Industrial Nurses and their Related Factors in Kyungnam Area (경남지역 일부 산업간호사의 보건관리 업무 및 관련요인에 관한 연구)

  • The purpose of this study was to assess the performance of the role and function of some industrial nurses and to characterize the factors affecting the performance of their activities. Thus the results could be used to suggest the direction in the performance of industrials nurses' activities effectively. During a period from January 10 to March 31, 1994, the data were collected from 87 industrial nurses, who were working as health managers in the plants, in Ulsan city and the vicinity in Kyungnam province, using a structured questionnaire. The results were as follows : 1. The general characteristics of industrial nurses in this study were 82.8% being 30 years old or less, 60.9%, being not married, and 93.1% having eduction levels above junior college. 2. With respect to general work conditions, 94.3% were working in a separate room provided for health care division, 40.2% working under the safety and health department, and 98.9% working as common-level staffs. And 60.9% were working less than 44 hours a week, 70.1% had work experiences less than 5 years, and 50.6% had annual incomes ranging 10 to 14 million wons. 3. As work conditions related to health care activities, 49.4% performed the activities not related to health care as always or occasionally, and 87.4% answered that occupational physicians were appointed in their plant and among them, however, only 6.9% worked on full-time basis and 52.8% perform little activities as occupational physicians. For a decision related to health care activity, 69.0% discussed the problems with the supervisors, and 19.5% made decisions by themselves. 4. As for attitude and perception to their activities as health managers, 66.7% moderately recognized the importance of health manager in the workplace, with 63.2% being satisfied their wages and treatment from the company, 57.5% being satisfied with their job positions and 51.7% having positive attitudes as being health managers. 5. The degree of performance at least in one of health related activities were very high in activities such as general medical care(100%), general health examination(98.0%) and specific health examination(100%), and relatively high in health education(72%), new employee health examination(60.9%), document handling(79.3%) and activity for work environment(70.1%). However, the performance rate was very low in preparing protective equipment (20.8%). 6. The levels of activities related to health care were significantly high when making decisions by themselves, when occupational physicians not being full-time, and when satisfying their job positions, and, on the other hand, significantly decreased as work hours increased. 7. In addition to some kinds of periodic education asked by all of the nurses, 89.7% wanted a specialized licensing system for industrial nurse, and 97.4% wanted to apply for the license test. As a conclusion, it is suggested that industrial nurses should be given more authority and placed in more self-controlled system to perform health care and other activities more efficiently, and the role and function of the occupational physician should be clearly distinguished from that of the industrial nurse as a health manager to avoid an unnecessary overlapping.

  • The non pulsation blood pump is divided into axial flow and centrifugal style according to the direction of inlet and outlet flow. An axial flow blood pump can be made smaller than a centrifugal blood pump because centrifugal pump's rpm is fewer than axial flow pump. Hemolysis is an important factor for the development of an axial flow blood pump. It is difficult to identify the areas where hemolysis occurs. Evaluation of hemolysis both in in-vitro and in-vivo test requires a long-time and more expensive. Computational fluid dynamics(CFD) analysis enables the engineer to predict hemolysis on a computer which just can get not only amount of htmolysis but also location of hemolysis. It takes shorter time and less expensive than in-vitro test. The purpose of this study is to git Computational fluid dynamics in axial flow pump and to verify the accuracy of prediction by the possibility of design comparing CFD results with in-vitro experimental results. Also, wish to figure out the correction method that can bring improvement in shape of axial flow blood pump using CFD analysis.
