Testing for Self-care Ability Measurement in an Aged Health Center - A Delphi Technique - (일개 노인보건센터에서의 자가관리능력 측정도구 검증 - 델파이기법을 중심으로 -)

  • Purpose: To find out whether the self-care ability measurement used for visiting health care provide an appropriate standard to select service methods using a Delphi survey and to offer basic data that can improve the measurement. Method: A Delphi survey was done with 39 people consisting of theoretical professionals, field managers, and practitioners engaged in visiting health care. Result: It was found that items and weighted points in the currently used self-care ability measurement needed adjustment. Therefore, a modified evaluation standard was presented by domains, items and detailed items. Finally, the self-care ability measurement is composed of 4 domains, 11 items and 19 detailed items. Conclusion: This study is significant in that it presented an evaluation standard applicable directly to business practice by supplementing problems in the existing self-care ability measurement. This study suggests the need to develop various measurements for elderly households based on community.

Selecting Common Items for Linking the Oswestry Low Back Pain Questionnaire and a Short Form of Self-Reported Activity Measure for Low Back Pain

  • To develop an effective and efficient measurement system for tracking changes of functional status across two measures, it is essential to integrate information and communicate scores across two measures. The lack of communication between two measures leads to score incompatibility. A potential solution would be the development of a crosswalk table between those measures. Prior to creating a crosswalk table, selecting common items between two measures is critical. By using the Oswestry low back pain disability questionnaire (Oswestry) and a short form measuring disability resulting from low back pain, item level statistics as well as differential item functioning (DIF) using the Rasch measurement were investigated. Eighty-two participants with known group validity were recruited. Based on the application of the Rasch measurement model, item difficulties across the two measures were logically and hierarchically ordered. Ceiling effects for both measures were detected, which were not be able to be effectively measured with the two measures. The DIF analysis across the two measures confirmed that five paired items were found to have DIF and five common items were selected for common items. Although five paired items function differently across the Oswestry and the short form, all items of both measures were well targeted study participants. The common items selected by the Rasch measurement model may be effective when creating a crosswalk table between the Oswestry and the short form.

  • Purpose: The purpose of this study is to investigate various prior studies on artificial intelligence and to examine the concept and characteristics of various prior studies of existing service quality. And this paper is to study the concept and characteristics of artificial intelligence services and propose suitable quality measurement items. Methods: The research method of this paper is to examine previous research related to existing artificial intelligence and to analyze characteristics related to service quality. Results: This paper examines the concept and characteristics of artificial intelligence service in a new era by examining previous studies related to artificial intelligence and derives quality measurement items. Conclusion: In the future, it is necessary to verify the validity of the quality measurement items of artificial intelligence service. Therefore, it is necessary to elicit and verify the main quality measurement items through the investigation of the expert group.

Development of Measurement of Stress for Female Marriage Immigrants in Korea (여성결혼이민자의 스트레스 측정도구 개발)

  • Purpose: This study was conducted in order to develop and test a measurement for assessment of stress of female marriage immigrants in Korea. Methods: Forty four preliminary items were initially developed based on literature review and focus group interviews. Those items were evaluated by experts for content validity, resulting in six factors and 26 items. The 26 items were translated into Chinese, Vietnamese, and English by professional translators and were reviewed by native speakers of each language who are fluent in Korean. For testing validity and reliability, data were collected from 323 female marriage immigrants residing in five regions in Korea. Results: As a result of item analysis, 25 items were selected. Factor analysis yielded 21 items in four factors, including 1) household economic 2) parenting and discrimination 3) cultural and 4) emotional stressors, explaining 61.3% of the total variance of stress of female marriage immigrants in Korea. The Cronbach's alpha reliability coefficient was .903 for the overall instrument and .692-.892 for four factors. Conclusion: Measurement of stress for female marriage immigrants in Korea has high validity and reliability. Therefore, this measurement may be utilized for systematic assessment of stress and for identification of areas of support for female marriage immigrants in Korea.

A basic research for evaluation of a Home Care Nursing Delivery System (가정간호 서비스 질 평가를 위한 도구개발연구)

  • The purpose of this study was to develop a basic framework and criteria for evaluation of quality care provided to patients with the attributes of disease in the home care nursing field, and to provide measurement tools for home health care in the future. The study design was a developmental study for evaluation of hospital-based HCN(home care nursing) in Korea. The study process was as follows: a home care nursing study team of College of Nursing. Yonsei University reviewed the nursing records of 47 patients who were enrolled at Yonsei University Medical Center Home Care Center in March, 1995. Twenty-five patients were insured at that time, were selected from 47 patients receiving home care service for study feasibility with six disease groups; Caesarean Section (C/S), simple nephrectomy, Liver cirrhosis(LC), chronic obstructive pulmonary disease(COPD), Lung cancer or cerebrovascular accident(CVA). In this study, the following items were selected : First step : Preliminary study 1. Criteria and items were selected on the basis of related literature on each disease area. 2. Items were identified by home care nurses. 3. A physician in charge reviewed the criteria and content of selected items. 4. Items were revised through preliminary study offered to both HCN patients and discharged patients from the home care center. Second step : Pretest 1. To verify the content of the items, a pretest was conducted with 18 patients of which there were three patients in each of the six selected disease groups. Third step : Test of reliability and validity of tools 1. Using the collected data from 25 patients with either cis, Simple nephrectomy, LC, COPD, Lung cancer, or CVA. the final items were revised through a panel discussion among experts in medical care who were researchers, doctors, or nurses. 2. Reliability and validity of the completed tool were verified with both inpatients and HCN patients in each of field for researches. The study results are as follows: 1. Standard for discharge with HCN referral The referral standard for home care, which included criteria for discharge with HCN referral and criteria leaving the hospital were established. These were developed through content analysis from the results of an open-ended questionnaire to related doctors concerning characteristic for discharge with HCN referral for each of the disease groups. The final criteria was decided by discussion among the researchers. 2. Instrument for measurement of health statusPatient health status was measured pre and post home care by direct observation and interview with an open-ended questionnaire which consisted of 61 items based on Gorden's nursing diagnosis classification. These included seven items on health knowledge and health management, eight items on nutrition and metabolism, three items on elimination, five items on activity and exercise, seven items on perception and cognition, three items on sleep and rest, three items on self-perception, three items on role and interpersonal relations, five items on sexuality and reproduction, five items on coping and stress, four items on value and religion, three items on family. and three items on facilities and environment. 3. Instrument for measurement of self-care The instrument for self-care measurement was classified with scales according to the attributes of the disease. Each scale measured understanding level and practice level by a Yes or No scale. Understanding level was measured by interview but practice level was measured by both observation and interview. Items for self-care measurement included 14 for patients with a CVA, five for women who had a cis, ten for patients with lung cancer, 12 for patients with COPD, five for patients with a simple nephrectomy, and 11 for patients with LC. 4. Record for follow-up management This included (1) OPD visit sheet, (2) ER visit form, (3) complications problem form, (4) readmission sheet. and (5) visit note for others medical centers which included visit date, reason for visit, patient name, caregivers, sex, age, time and cost required for visit, and traffic expenses, that is, there were open-end items that investigated OPD visits, emergency room visits, the problem and solution of complications, readmissions and visits to other medical institution to measure health problems and expenditures during the follow up period. 5. Instrument to measure patients satisfaction The satisfaction measurement instrument by Reisseer(1975) was referred to for the development of a tool to measure patient home care satisfaction. The instrument was an open-ended questionnaire which consisted of 11 domains; treatment, nursing care, information, time consumption, accessibility, rapidity, treatment skill, service relevance, attitude, satisfaction factors, dissatisfaction factors, overall satisfaction about nursing care, and others. In conclusion, Five evaluation instruments were developed for home care nursing. These were (1)standard for discharge with HCN referral. (2)instrument for measurement of health status, (3)instrument for measurement of self-care. (4)record for follow-up management, and (5)instrument to measure patient satisfaction. Also, the five instruments can be used to evaluate the effectiveness of the service to assure quality. Further research is needed to increase the reliability and validity of instrument through a community-based HCN evaluation.

Analysis of Difference between Direct Measurement and 3-D Automatic Measurement According to Classification of Side Figure of Elderly Women (고령 여성의 측면체형 분류에 따른 직접측정치와 3차원 자동측정치간의 차이 분석)

  • This study analyzes differences between the results of 3D direct measurements and automated measurements for Korean elderly females according to age groups, side somatotype, and BMI groups. This study compares the measurement differences of the direct and the 3D automated measurements for women between the ages of 70 to 85, according to age group, BMI group, and side somatotype. A comparison of the results of the direct measurement and the 3D automated measurements for elderly women show that a meaningful discrepancy exists for 29 items out of 33 items. Furthermore, the results of comparing the average error tolerance recommended by ISO20685 shows that 30 items out of 33 items exceeded ISO recommendations. The results of the automated measurement program shows a higher degree of accuracy for straight postures; however, this unsuitable for postures of elderly women with a changed somatotype. The analysis results of the measurement difference indicate the suitability of the automatic measurement programs is found to be high for stood postures, while problems seem to exist on several items along with an automated program is not appropriately used due to posture and part of body changes for elderly women. Therefore, it is recommended to develop an algorithm, that reflects the body changes of elderly women first and then upgrade the automated program equipped with a measurement size method. It is hoped that the study results can be utilized as base data for improving the automated measurement program.

  • The upper-body characteristics of the right-handed and the left-handed are examined and compared through 1-dimensional measurement and 2-dimensional measurement. The results were as follows: 1. According to 1-dimensional measurement results, the clearest differences were seen in girth items rather than height or length items. In particular, hand girth, lower arm girth, and upper arm girth were bigger on the left side in the left-handed group and the right side in the right-handed group. This is thought to be relevant to the arm muscles frequently used. 2. According to 2-dimensional measurement results, significant differences appeared in the females of the left-handed group: shoulder point among width items. In the right-handed group, some differences of significance were shown in females (shoulder point, rear armpit point, and B.P. point among width items) and males (front neck point among extreme vertical distance items). In summary, the body type characteristics of the left-handed and the right-handed reveal great differences in the items related to arm length and arm girth. In other words, the sleeve and shoulder patterns production based on the right-handed don't rightly reflect the somatotype characteristics of the left-handed. In addition, in the cases of athletes, laborers, and the disabled who frequently use particular muscles or parts of the body, this sort of research in the patterns is consistently needed in order to reflect the size differences in girth items in upper body.

A Study on the Contents of Fundamental Nursing Practice (기본간호학 실습교육현황과 실습내용 개선을 위한 조사 연구)

  • The purpose of this study was to provide fundamental data for determining contents of Fundamental Nursing Practice and developing desirable bedside nursing techniques for clinical nursing areas. Subjects for this study were 86 nurses who were employees of two university hospitals located in Seoul and a district area. Data were collected by questionnaires. Items of Fundamental Nursing Practice were classified into 72 items according to the result from content analysis of 9 textbooks of Fundamental Nursing. The results are as follows : 1) Items which above 80% of respondents practiced during the school inside practice or clinical nursing practice course were axillary temperature measurement, radial pulse measurement, respiratory rate measurement, application of oral hygiene, hand-washing technique, application of hot and cold bags, intramuscular injection technique, open bed-making, soap enema method, application of nelaton catheterization and oral and nasal suction methods. 2) Above 90% of respondents replied that all items except temperature measurement and bed-making were requisite contents for Fundamental Nursing Practice. Above 10% of respondents replied oral and rectal temperature measurement and bed-making were unnecessary content. 3) Above 90% of respondents replied that operating methods of all items except isolation technique, admission and discharge procedures, and retention enema in the Fundamental Nursing Practice course and clinical situation were consistent. The main reasons that respondents did not apply methods which they learned in the Fundamental Nursing Practice course to the clinical situation were 'insufficient time', 'colleagues were using different methods', 'insufficient supply of instuments' or 'inappropriate appliances'.

Upper Body Measurement of Men using 3D Body Scanner -Compared to Anthropometry- (3차원 바디 스캐너를 이용한 남성 상반신 인체측정 -직접측정과의 비교-)

  • Three dimensional body scan technology is being targeted for utilization in the apparel industry. The purpose of this study was to test reliability of the body scan data targeting from 20 to 24 year old men by comparing 3DM, 3D body-scanning semi-auto measurement extraction method, Scanworx, 3D body-scanning auto measurement extraction method, and traditional anthropometric method. We found significant differences in 9 out of 25 items in upper body measurements using 3DM and 16 out of 25 items using Scanworx. In the range of difference value of scan measures, it showed 1 item in the absolute value of more than 40mm between two measuring methods, 3 items in 20 up to 40mm, and less than 20mm in other items. Overall, in height items, the numerical value of traditional measure was higher and in girth, width, depth items, 3D scan measure was higher. We found out that reliability of 3D measurements taken from whole body scans was different according to scanners, scanning softwares, programs, and subjects.

A Study on the Character for the Foot and Calf Growth of Primary School Children (초등학생의 발과 하퇴부 성장특성 연구)

  • For the purpose of investigating the factor of foot and calf growth of primary-school children who are fast-growing during this period a group of the 1st graders of primary-school had been the subject of this measurement in 1995 and there after they became the subject again in 1997, 1999 when they were the 3th, 5th graders measurement was carried out in 30 items including status and weight and marthin-type measure and foot-print were used for this measurement as the result, stature is correlated with height items and length items of foot and calf weight is correlated with width and circumference items of foot and calf angle of the first toe showed the highest growth like 3rd graders, heel circumference had rapidly grown rather than in the part of foot circumference in the factor analysis df foot measurement of 10-year, the first, second factors, appeared the size of shoes, in case of 12-year the first factor with status appeared the size of shoes.
