The vascular surgery is the field that has developed in early 20 century and is progressing nowadays. Recent advance in surgical technique accompanying with excellent medical diagnosis and treatment, prompt angiographic usage, development of variable prosthetic material, and concomitant use of anti-coagulant have made remarkable results of vascular surgery. 83 cases of vascular surgery have been performed at Thoracic and Cardiovascular Surgery Department of Pusan National Unversity Hosaital since 1971 till 1990, for 20 years and their results are followed. Patient ductus arteriosus and Buerger`s disease were omited in this study. 1. The age distribution shows that the fifth and sixth decades are most frequently affected and mean age was 56.1 years old. Male to female ratio is 1: 2.32. 2. Among the 83 cases of all, number of occlusive vascular disease is 46 and that of aneurysmal disease is 33. 3. In clinical manifestation, most common symptom of occlusive disease is pulselessness and pain was next. Mass sensation is most commonly complained by patients of aneurysmal disease. 4. CT scan was more important in diagnosis of aneurysmal diseases and angiogram was more commonly used in occlusive diseases. 5. The common site of arterial occlusion was common iliac artery, femoral artery, aortic bifurcation, and external iliac artery, as its frequency rate. The most commonly affecting portion of aortic aneurysm was abdminal aorta, and descending thoracic aorta and femoral artery were next 6. Preoperative associated diseases were atherosclerosis[41 cases], hypertension[21 cases], valvular heart disease[11 cases], and diabetes mellitus[9 cases], etc, 7. Operative methods in ocllusive diseases were thrombectomy[36.9%], endarterectomy [10.9%], and bypass graft insertion[52.7%]. Among the bypass graft, Y-graft was used in 7 case, straight graft was used in 17 cases, and saphenous venous graft was used in 2 cases. 8. Postoperative complications were developed in 17 cases, and morbidity rate was 36. 9. Eleven patient were died within 1 month after operation, so operative mortality rate was 13.3%. 10. Duration of patency was beteween 7 and 58 months[average 27.5 months] in occlusive diseases and their 5-year patency rate was 56.3%. Duration of patency of aneurysmal disease was 20 months in aveage and their 5-year patency rate was 51.3%. 11. Patients of eleven cases of occlusive disease and two cases of aneurysmal disease required reoperation for variable reason. 12. 35 cases of patient have used anticoagulants: coumadin, ticlid, and persanthin-ASA combination.

Fabry disease is an X-linked disease caused by deficiency of the lysosomal enzyme alpha-galactosidase A. Affected males present anhydrosis, acroparesthesia and angiokeratoma, and subsequently cardiac, cerebral and renal complications are followed. Females and atypical variants show heterogeneous clinical symptoms. In 2001, two recombinant enzymes were approved for Fabry disease: agalsidase alpha and agalsidase beta. Since the introduction of enzyme replacement therapy (ERT), the number of long-term follow-up studies has been reported. Long-term ERT showed effectiveness on renal function in patients with chronic kidney disease, decrease or stabilization of left ventricular mass, and improvement of pain and quality of life. However, there were limited effects on cerebrovascular events and their mortality. Current literatures on the clinical effect of ERT have reported limited datain adult patients who have already advanced disease. Therefore, further study for pre-symptomatic patients and atypical variants is needed to verify the impact of ERT. This review summarized recent progresses in ERT and limitations of long-term effect of ERT in patients with Fabry disease.

When livestock facilities in Korea have been changed larger and denser, rearing conditions have been getting worse and the productivity of animal production have been decreased. Especially in the cold season, the minimized ventilation has generally been operated to save energy cost in Korea resulting in very poor environmental condition and high mortality. While the stability, suitability, and uniformity of the rearing condition are the most important for high productivity, the ventilation configuration is the most important to improve the rearing condition seasonally. But, it is so difficult to analyze the internal air flow and the environmental factors by conducting only field experiment because the weather condition is very unpredictable and unstable as well as the structural specification can not be easily changed by the researchers considering cost and labor. Accordingly, an aerodynamic computer simulation was adopted to this study to overcome the weakness of conducting field experiment and study the aerodynamic itself. It has been supposed that the airflow is the main mechanism of heat, mass, and momentum transfers. To make the simulation model accurately and actually, simplified pig models were also developed. The accuracy of the CFD simulation model was enhanced by 4.4 % of errors compared with the data collected from field experiments. In this paper, using the verified CFD model, the CFD computed internal rearing condition of the mechanically ventilated pig house were analyzed quantitatively as well as qualitatively. Later, this developed model will be computed time-dependently to effectively analyze the seasonal ventilation efficiency more practically and extensively with tracer gas decay theory.
We examined the efficacy of a bacterium for biocontrol of the root-knot nematode (RKN) Meloidogyne hapla in carrot (Daucus carota subsp. sativus) and tomato (Solanum lycopersicum). Among 542 bacterial isolates from various soils and plants, the highest nematode mortality was observed for treatments with isolate C1-7, which was identified as Bacillus cereus based on cultural and morphological characteristics, the Biolog program, and 16S rRNA sequencing analyses. The population density and the nematicidal activity of B. cereus C1-7 remained high until the end of culture in brain heart infusion broth, suggesting that it may have sustainable biocontrol potential. In pot experiments, the biocontrol efficacy of B. cereus C1-7 was high, showing complete inhibition of root gall or egg mass formation by RKN in carrot and tomato plants, and subsequently reducing RKN damage and suppressing nematode population growth, respectively. Light microscopy of RKN-infected carrot root tissues treated with C1-7 showed reduced formation of gall cells and fully developed giant cells, while extensive gall cells and fully mature giant cells with prominent cell wall ingrowths formed in the untreated control plants infected with RKNs. These histopathological characteristics may be the result of residual or systemic biocontrol activity of the bacterium, which may coincide with the biocontrol efficacies of nematodes in pots. These results suggest that B. cereus C1-7 can be used as a biocontrol agent for M. hapla.

Although mud shrimp (Upogebia Major) provides benefits on the tidal flat ecosystem, it is causing the mass mortality of Manila clam (Tapes philippinarum) and large economic loss to the aquaculture farmer due to spreading in the entire coastal clam farm area of the West Sea of Korea. In this study, a mud shrimp blocking box for cultivating clam was tested to prevent the mud shrimp from invading the habitat of the clam. To determine the effect of the blocking box, the growth characteristics of the clams under various culture conditions were investigated. The shell length, total weight, condition index, the presence of mud shrimp in the box, and the soil decay in the box were examined according to whether the clams were raised in the box or in natural state, whether the box is covered with a net or not and the gap size of the box. As a result, no significant differences in shell length, total weight, and condition index were found between the clam grown in the box and the natural state (P>0.05) without any evidence of invading of mud shrimp to the box. In addition, the soil inside the box was in good condition, not decayed. Therefore, using the mud shrimp blocking box, mud shrimp can be prevented from propagating to the habitat of the clam, thereby contributing to the normal production of the clam and thus to restoring the clam farming.
Yellow tail (Seriola quinqueradiata) infected by Pasteurella piscicida have been occurred to mass mortality without showing apparent surface lesions in cage culture farms. In this case, it is necessary to consider countermeasure by rapid diagnosis of infected fish. The purpose of the present study was to investigate usefulness of the direct fluorescent antibody technique(FAT) for rapid diagnosis of pseudotuberclosis of cultured yellowtail caused by P. piscicida. Antibody produced by inoculating rabbit with formalin killed pseudotuberclosis bacteria antigen(strain KNP-2). Immunoglobulin-G(IgG) was purified from antisera by DEAE-cellulose column chromatography and conjugate with fluorescein isothiocyanate. Fluorescein-labeled antisera was purified by sephadex G-25 gel column chromatography. The fluorescein/protein molar ratio of labeled antisera was determined as 8.8-9.5. Diagnosis of cultured yellowtail was examined in cage culture farms which located in Tongyung, kyungnam from July to October 1990. The causative bacteria of pseudotuberclosis could be detected within two hours after the specimens were transferred to the laboratory for FAT, and it showed that FAT could be adapted as a rapid and accurate diagnostic method of pseudotuberclosis in yellowtail.
An apicomplexan parasite, Perkinsus sp. was observed from the cultured baby clams, Ruditapes philippinarum, collected from the coast of Kochang and Taean (South Korea), where it caused seasonal mortality of clams. Several milky-white cysts were observed on the surface of gill and visceral mass of parasitised clams. The trophozoites of parasite had eccentric nucleus and proliferated by schizogony in gill, mantle, hepatopancrease and reproductive tissues, resulting in the formation of granuloma and the intensive infiltration of hemocytes in the tissues. During incubation in FTM, trophozoites increased in size, resulting in prezoosporangia which appeared as round black spheres when colored with Lugols iodine solution. The prevalence of Perkinsus sp. in clams was Kochang, 73.1%; Taean, 94.8% (during 9-mo. survey) and showed size-dependent infection. Hemacolor kit was useful to reduce time for diagnosis of the trophozoite of Perkinsus sp. that has been responsible of massive motalities in the clam.
The ark shell (or 'blood clam') Scapharca broughtonii is a filter-feeding bivalve that has red blood and inhabits waters approximately 10m in depth off the southern coast of South Korea. This study was part of a larger research project investigating the causes of death and restoration of shellfish resources, which are important aquaculture products in South Korea. We examined physiological responses related to survival, respiration, excretion, and amino acid changes as a result of changes in salinity. The 9-day median lethal salinity ($LS_{50}$) was 16.5 psu with confidence limits of 14.9-18.1 psu. At $25^{\circ}C$, the oxygen consumption and ammonia-nitrogen excretion rates were increased with decreases in salinity. Although the osmolality of individuals was acclimated within 2 h at 26.4 psu and 12 h at 19.8 psu, it took more than 5 days at a salinity of 13.2 psu, whereas no individuals acclimated and all died at a salinity of 6.6 psu. Of the amino acids present in the blood, taurine and alanine increased in response to decreased salinity. Tissues of the gill and the mid-gut gland were affected by decreasing salinity. These data will provide important fundamental information for examining the causes of mass mortality of shellfish in the summer.
Purpose: Minimally invasive gastrectomy (MIG), including laparoscopic distal subtotal gastrectomy (LDG) and robotic distal subtotal gastrectomy (RDG), is performed for gastric cancer, and requires a learning period. However, there are few reports regarding MIG by a beginner surgeon trained in MIG for gastric cancer during surgical residency and fellowship. The aim of this study was to report our initial experience with MIG, LDG, and RDG by a trained beginner surgeon. Materials and Methods: Between January 2014 and February 2015, a total of 36 patients (20 LDGs and 16 RDGs) underwent MIG by a beginner surgeon during the learning period, and 13 underwent open distal subtotal gastrectomy (ODG) by an experienced surgeon in Bundang CHA Medical Center. Demographic characteristics, operative findings, and short-term outcomes were evaluated for the groups. Results: MIG was safely performed without open conversion in all patients and there was no mortality in either group. There was no significant difference between the groups in demographic factors except for body mass index. There were significant differences in extent of lymph node dissection (LND) (D2 LND: ODG 8.3% vs. MIG 55.6%, P=0.004) and mean operative time (ODG 178.8 minutes vs. MIG 254.7 minutes, P<0.001). The serial changes in postoperative hemoglobin level (P=0.464) and white blood cell count (P=0.644) did not show significant differences between the groups. There were no significant differences in morbidity. Conclusions: This study showed that the operative and short-term outcomes of MIG for gastric cancer by a trained beginner surgeon were comparable with those of ODG performed by an experienced surgeon.
Thyroid carcinoma is relatively rare in children. Eight cases of thyroid carcinoma were among 18 patients operated upon for thyroid tumors at Pediatric Surgery, Asan Medical Center in 11 years' period between 1989 and 2000. Five patients were boys and three were girls. The age distribution ranged from 10 to 14 years with a median age of 11.6 years. We studied clinical presentations, diagnostic workup, pathology, treatment, and follow-up (recurrence, mortality, and survival). All patients presented with anterior neck mass but one with multiple cervical lymph nodes enlargement. Familial history of thyroid cancer was seen in one case. All patients had a cold nodule by 1-131 thyroid scan. Fine needle aspiration (FNA) biopsy was performed in all patients. The right lobe was involved in four patients and the left lobe in two. Two had both lobes involvement. Pathologically, six cases were papillary carcinoma, one was follicular carcinoma, and one insular carcinoma. Four patients underwent unilateral lobectomy and isthmectomy. Total thyroidectomy was performed in three patients. The insular carcinoma case was preoperatively diagnosed as follicular neoplasm by FNA. After confirmation of the pathology by initial right thyroid lobectomy, total thyroidectomy was subsequently done. Cervical lymph node metastases were presented in three cases (37.5%), and lung metastasis in two cases (25%). Three patients received postoperative I-131 ablation. After a median follow-up of 53 months, all patients were alive without evidence of recurrence. In conclusion, thyroid carcinoma in children is frequently associated with lymph nodes involvement and distant metastasis, however, the prognosis is relatively good.
