The Effect of Mobilization With Movements Applied Sacro-Iliac Joint on Gait of Stroke Patient (천장관절에 적용한 움직임을 동반한 관절가동술이 뇌졸중 환자의 보행에 미치는 효과)

  • Background: To determine effects of mobilization with movements (MWM) application on stroke patient with hypomobility on sacroiliac joint. Methods: The subject was 47years old male who have left hemiplegia because of right intra-cerebral hemorrhage. The subject was have hypomobility on sacroiliac joint. MWM using during 4weeks and using Berg balance scale (BBS), Timed up and go (TUG), 10meter walking (10MW) test for evaluation. Results: The results of this study were summarized below; BBS score for evaluating balance ability was increased, and TUG time, 10MW test time was decrease. Conclusion: We consider that MWM application on stroke patient with hypomobility on sacroiliac joint is not only regain mobility on sacroiliac joint but also increase in balance ability and walking speed.

Effects of MWM for Improving Ankle Dysfunction on Pain, Neck Disability, and Craniovertebral Angle in Patients with Neck Pain Accompanied by Forward Head Posture (발목 기능장애 개선을 위한 멀리건 관절가동술이 전방머리자세를 동반한 목통증 환자의 통증과 경부장애지수, 두개척추각에 미치는 영향)

  • Background: This study aimed to investigate the effect of mobilization with movement (MWM) applied to the ankle joint, on the craniovertebral angle (CVA), pressure pain threshold, and neck disability index (NDI) in asymptomatic adults with a forward head posture (FHP). Methods: A total of 32 subjects with FHP were assigned to either the MWM group (N=16) or the cranio-cervical flexion exercise (CCFE) group (n=16). The CVA, pressure pain threshold and NDI were measured before and 4 weeks after the intervention. Results: A significant improvement in the CVA was observed in the MWM group (p<.05), whereas no significant changes (p>.05) were observed in the CCFE group. Both groups showed significant differences in the pressure pain threshold and NDI before and after the intervention (p<.05). Conclusion: The results of the study suggest that MWM applied to the ankle joint can effectively improve the CVA, pressure pain threshold, and NDI of adults with a forward head posture. Based on this study, the ankle MWM technique for dorsiflexion can be used as an objective research method for additional studies targeting FHP patients in the future.

The effect of stabilization exercise and movement with mobilization on flexibility and pain of patients with acute low back pain (관절 가동술과 안정화 운동이 급성 요통 환자의 유연성과 통증에 미치는 영향)

  • Purpose : The purpose of this study was to investigate effects of movement with mobilization(MWM) and stabilization exercise on pain and range of motion of patients with acute low back pain. Methods : The subjects were consisted of 24 patients who had non specific acute low back pain. All subjects randomly assigned to the MWM group and the stabilization exercise group. The MWM group received sustained natural apophyseal glides(SNAGs) with modality treatment and stabilization exercise group received stabilization exercise with modality treatment. The remodified schober test(RST) was used to measure forward flexion and lateral flexion range of motion of lumbar segment. Visual Analogue Scale(VAS) was used to measure subjective pain level of the patients. The Oswestry Low Back Pain Disability Scale was used to measure functional disability level of the patients. Results : The lumbar flexion range of motion of MWM group was significantly increased compared with stabilization exercise group(p<.01). The range of motion of lumbar segment of MWM group was significantly decreased compared with stabilization exercise group(p<.01). The left lateral flexion range of motion of lumbar of MWM group was significantly decreased compared with stabilization exercise group(p<.05). The right lateral flexion range of motion of lumbar of MWM group was significantly decreased compared with stabilization exercise group(p<.05). The VAS of both MWM group and stabilization exercise group was significantly decreased(p<.001). The Oswestry Low Back Pain Disability Scale of stabilization exercise group was significantly increased compared with mobilization group(p<.001). Conclusion : In the result of this study, mobilization with movement and stabilization exercise had significant difference on pain and flexibility of patients with acute low back pain.

  • Background: This study aimed to investigate the effects of joint mobilization with movement (MWM) on pain, ankle joint range of motion, balance, and gait in patients who underwent total knee arthroplasty (TKA) and exhibited ankle joint inversion. Methods: We divided 35 patients who had undergone TKA into experimental and control groups. The intervention involved a 40-min session three times a week over 4 weeks. The control group received general physical therapy, ankle pumping exercises, Q-setting exercises, knee joint range of motion exercises, and gravity-controlled gait training. In addition to these treatments, the experimental group received MWM to evaluate knee pain, ankle joint angle, balance, and gait pre- and post-intervention. Results: Both groups experienced a decrease in pain levels, with no significant difference between the groups. Both groups also showed a significant decrease in ankle joint inversion angle, with notable differences between them. Additionally, both groups significantly increased their mean dorsiflexion angles and balance, with a significant difference observed between the groups. Although walking decreased in both groups, there was no significant difference observed between them. Conclusion: The group that received MWM treatment showed significant effects on inversion angle, dorsiflexion angle, balance, and gait. These results provide valuable insights into the potential benefits of MWM as a post-TKA intervention, while highlighting the importance of long-term follow-up studies on post-TKA lower limb alignment interventions.

Short-term Effects of Kaltenborn-Evjenth Functional Glide on Pain, ROM, and Function in Patients with Breast Cancer

  • Objective: The purpose of this study was to investigate the effect of Kaltenborn-Evjenth concept functional glide (KEFG) on shoulder pain, range of motion (ROM), upper extremity dysfunction in breast cancer patients. Design: Cross-sectional study. Methods: In this study, 42 subjects were randomly assigned and distributed to 21 subjects in the KEFG group and 21 subjects in the Mulligan technique (MWM) group. Results: After the application of joint mobilization, pain decreased significantly in both groups, and there was no significant difference between the two groups. The ROM of the joint was significantly increased in both groups, and there was no significant difference between the two groups. The SPADI pain was significantly decreased in both groups, and the disability and total components were significant only in the KEFG group, and there was no significant difference between the two groups. Conclusions: KEFG and MWM were found to be effective in improving shoulder pain, ROM and upper extremity dysfunction in breast cancer patients. KEFG is thought to be an intervention that can yield positive outcomes among breast cancer patients with upper extremity dysfunction.

Challenges of Medical Waste Treatment in Fiji (피지국에서의 의료폐기물 처리현황과 문제점)

  • Medical waste is any kind of waste that contains infectious material and recommended not to be transferred for infection control. As a means of disposal, incineration has better points than dumping or landfill in the quantity reduction, odorless and nonhazardous. However, open burning and incineration of health care wastes under bad circumstances, can result in the emission of environmental pollutants to air. A burial of biological waste brings pollution of soil and water. Most of sub divisional hospitals in Fiji transfer their medical wastes to divisional hospitals for incineration. In 2011, 62,518 kg of medical waste was incinerated in the three divisional hospitals. However, some medical wastes are considered as general waste and burnt or sent to landfill site, some are buried on site in some sub-divisional hospitals. In this regards, urgent education is necessary for awareness promotion to relevant personnel in medical waste treatment. On site incineration using small scale incinerator is more recommended than transportation of medical wastes treatment in Fiji. Moreover, remotely controllable and fixable small scale of incinerator is more desirable in sub-divisional hospitals. It is recommended that Fiji government to set up a legal framework for medical waste management (MWM), to develop specific guidelines for MWM, to set up a training system for MWM to ensure that all relevant personnel are trained, to develop a monitoring and supervision system for MWM, to clarify the future financing of MWM activities, and to improve the MWM infrastructure.

The Study on the Effects of Maitland Mobilization and MWM for Range of Motion, Static and Dynamic Balance, Plantar Pressure, Gait Ability Performed on the Ankle Joint of Hemiplegic Patients (편마비 환자의 발목관절에 시행된 메이틀랜드 관절가동술과 움직임을 동반한 관절가동술이 관절가동범위, 정적 및 동적 균형, 족저압, 보행 능력에 미치는 영향에 관한 연구)

  • Background: This study aimed to investigate the effect of maitland mobilization and mobilization with movement (MWM) applied to the ankle joint on ankle dorsiflexion range of motion (ROM), static and dynamic balance, plantar pressure, and gait ability. Methods: A total of 24 individuals were assigned to either the maitland mobilization group (n=12) or the MWM group (n=12). Dorsiflexion ROM, static and dynamic balance, plantar pressure, 10-meter walk test were measured before and 4 weeks after the intervention. Results: Both groups showed a significant difference in the evaluation after 4 weeks as the dorsiflexion ROM of the ankle increased (p<.05), and the sway length and sway area indicating static balance decreased (p<.05). Furthermore, the movement area showing dynamic balance showed a significant increase (p<.05), the plantar pressure difference between both feet significantly decreased (p<.05). In the 10-meter walk test, there was a significant difference as the time decreased (p<.05). However, no significant difference between the two groups was observed (p>.05). Conclusion: According to the results of this study where, maitland mobilization and MWM were applied to the ankle joint of hemiplegic patients for 4 weeks, we found no difference between the two groups. However, each technique was found to be effective for dorsiflexion ROM, static and dynamic balance, plantar pressure, and gait.

Monitoring the Rate of Frozen Denaturation of Bovine Myosin by Competitive Indirect ELISA Method (Competitive Indirect ELISA를 이용한 Bovine Myosin의 동결 변성도 측정)

  • This study shows the application of Ci-ELISA method for monitoring the denaturation of myosin by the frozen treatment in order to differentiate thawed beef from chilled. Hanwoo M.Semitendinosus (n=25) was treated under the two different frozen process as follows; simple frozen treatment (Exp-1) at 4 different temperatures, -10, -20, -50 and $-80^{\circ}C$, respectively, and repeated thawing-refreezing treatment (Exp-2) stored at 4 different temperatures, -10, -20, -50 and $-80^{\circ}C$, respectively. Antibodies (Abs) were produced from rabbits immunized with myosin whole molecule (MWM) isolated from beef round, heavy meromyosin S-1 (S-1) and light meromyosin (LMM) prepared by digestion of MWM. Each immunoglobulin G (IgG) was separated from antiserum. At 6 month storage, IA of anti-MWM IgG for myosin was decreased to 32.67, 32. 23, 51.52 and 34.27% in Exp-1 and to 14.82, 15.61, 25.3 and 23.7% in Exp-2 at -10, -20, -50 and $-80^{\circ}C$, respectively (P<0.05). In Exp-1, the reactivities of anti-LMM IgG were decreased to 25.12, 21.42, 49.05 and 28.96%, and those of Exp-2 were to 11.88, 9.56, 20.63 and 12.64% at -10, -20, -50 and $-80^{\circ}C$, respectively, at 6 times thawing (P<0.05). Conclusively, myosin was denaturated by freezing treatment and LMM or myosin rod part might have suffered from more extreme demage than HMM S-1, and samples at $-50^{\circ}C$ were slightly injured less than others by freezing treatment.

  • Background: The purpose of this study was to investigate the effects on balance and Jump performance in soccer player with functional ankle instability of Movement with mobilization (MWM) and taping. Methods: In 30 male college soccer player with functional ankle instability subjects of this study randomization, fibular reposition taping (FRT) group (n=10), kinesio taping (KT) group (n=10), control group (n=10) that included in the MWM and taping was classified group.Before and after intervention, measured in surface area ellipse and countermovement jump with arm swing. Results: Showed a significant balance and jump performance from the FRT group and KT group compared to the control group. Showed a significant improvement in balance from the FRT group compared to the KT group. Conclusions: MMW and taping showed the increased balance and Jump performance in soccer player with functional ankle instability.

Effect of Conditioned Medium of Human Endothelial Cell Line(tHUE-2 cell) on In Vitro Development of Mouse 1-cell Embryos In Vitro Fertilized (체내 수정된 Mouse 1-세포배의 체외발생에 미치는 혈관내피세포주(tHUE-2세포) 배양액의 영향)

  • Culture medium (ASF-301) of tHUE-2 cell, human endothelial cell line, and culture medium of these cells (conditioned medium : CM) which affect embryonic development of in vivo fertilized 1-cell embryos of mouse were examined. Two-cell stage block of mouse embryos was overicomed in ASF-301 and CM without EDTA, which usually added in basic medium (modified Whitten Medium: MWM, control) to overcome the 2-cell stage block. The developmental rates of embryos to the blastocyst stage were significantly increased in MWM containing 12.5% of growth factors added to ASF-301 (10mg/ $\ell$ transferrin, 1mg/$\ell$ insulin, 0.01mg/$\ell$ EGF) than those of 100% addition and control, 78.0% vs 20.8 and 52.3%, respectively (P<0.05), but the growth factors was not affected the hatching rate of blastocyst. Using ASF-301 or CM which was not treated, embryonic development into the blastocyst and hatched blastocyst stages were not affected. However, proportions of embryonic development into the blastocyst and hatched blastocyst stages were significantly higher in dilution (ASF-301 1:10; CM 1:3~1:6) than those in control (P,0.05). In ASF-301 dialyzed M.W.<10000 dialysis membrane, the developmental rate upto the hatched blastocyst stage was significantly increased, compared to ASF-301 which was not dialyzed (P<0.05), and hatching rate of blastocyst of these group was singnificantly increased than those in MWM (P<0.05). Compared to CM which was not dialyzed, however, in dialyzed CM was significantly decreased, compared to untreated CM (P<0.05), especially any hatched blastocyst was not appeared. As a result of these experiments indicated that a kind or porper treatment such as a dilution of complex synthetic cell culture medium and conditioned medium, and that a optimal concentration of growth factors are usuful for embryo cultrue in vitro.
