Gadolinium Deposition in the Brain: Current Updates

  • Gadolinium-based contrast agents (GBCAs) are commonly used for enhancement in MR imaging and have long been considered safe when administered at recommended doses. However, since the report that nephrogenic systemic fibrosis is linked to the use of GBCAs in subjects with severe renal diseases, accumulating evidence has suggested that GBCAs are not cleared entirely from our bodies; some GBCAs are deposited in our tissues, including the brain. GBCA deposition in the brain is mostly linked to the specific chelate structure of the GBCA: linear GBCAs were responsible for brain deposition in almost all reported studies. This review aimed to summarize the current knowledge about GBCA brain deposition and discuss its clinical implications.

  • The proximity of thoracic aortic aneurysm to the left subclavian artery (LSA) has made the coverage of LSA during thoracic endovascular aortic repair (TEVAR) be essential. Despite controversy concerning the safety of LSA coverage and the indications for LSA revascularizations, the cerebral hemodynamic change after LSA coverage has not been demonstrated. We prospectively examined two patients who would undergo TEVAR with LSA coverage by using 2D cine phase contrast MR imaging. After LSA coverage, the left subclavian steal was properly compensated by the increased flow volumes of both carotid arteries and right vertebral artery, which is the major collateral supply. The total brain supply after TEVAR did not lessen, which showed good correlation with uneventful clinical outcome. Therefore, 2D phase contrast MR imaging can be recommended as a useful technique to evaluate the hemodynamic change of the LSA coverage during TEVAR and to triage the candidate for LSA revascularization.

Potential Explosion Risk Comparison between SMR and DMR Liquefaction Processes at Conceptual Design Stage of FLNG (FLNG개념설계 단계에서 SMR 및 DMR 액화공정의 잠재적 폭발위험도 비교)

  • An FLNG (floating liquefied natural gas) or LNG FPSO (floating production, storage and offloading) unit is a notable offshore unit with the increasing demand for LNG. The liquefaction process on an FLNG unit is the most important process because it determines the economic feasibility, but would be a hazard source because of the large quantity of hydrocarbons. While a high efficiency process such as C3MR has been preferred for onshore liquefaction processes, a relatively simple process such as the SMR (single mixed refrigerant) or DMR (dual mixed refrigerant) liquefaction process has been selected for offshore units because they require a more compact size, lighter weight, and higher safety due to their space limitation for facilities and long distance from shore. It is known that an SMR has the advantages of a simple configuration, small footprint, and lower risk. However, with an increased production rate, the inherent safety of SMR needs to be evaluated because of its small train capacity. In this study, the potential explosion risks of the SMR and DMR liquefaction processes were evaluated at the conceptual design stage. The results showed that an SMR has a lower overpressure than a DMR at the same frequency, only with a small production capacity of 0.9 MTPA. With increased capacity, the overpressure of the SMR was higher than that of the DMR. The increased number of trains increased the frequency in spite of the small amount of equipment per train. This showed that the inherent risk of an SMR is not always lower than that of a DMR, and an additional risk management strategy is recommended when an SMR is selected as the concept for an FLNG liquefaction process compared to the DMR liquefaction process.

The Dosimetric Effect on Real PTV and OARs at Various Image Fusion Protocol for Pituitary Adenomas (뇌하수체 종양의 방사선 수술 시 영상 융합 프로토콜이 실제 PTV와 OAR 선량에 미치는 영향)

  • The purpose of this study is to verify the dosimetric effect on real PTV (planning target volume) coverage and safety of OARs (organs at risk) at various image fusion protocol-based radiosurgery plan for pituitary adenomas. Real PTV coverage and its variation was acquired and maximum dose and the volume absorbing above threshold dose were also measured for verifying the safety of optic pathway and brainstem. The protocol that can reduce superior-inferior uncertainty by using both axial and coronal MR (magnetic resonance) image sets shows relatively lower values than that of case using only axial image sets. As a result, the image fusion protocol with both axial and coronal image sets can be beneficial to generate OAR-weighted radiosurgery plan.

Development and Evaluation of Traffic Conflict Criteria at an intersection (교차로 교통상충기준 개발 및 평가에 관한 연구)

  • For many rears, traffic accident statistics are the most direct measure of safety for a signalized intersection. However it takes more than 2 or 3 yearn to collect certain accident data for adequate sample sizes. And the accident data itself is unreliable because of the difference between accident data recorded and accident that is actually occurred. Therefore, it is rather difficult to evaluate safety for a intersection by using accident data. For these reasons, traffic conflict technique(TCT) was developed as a buick and accurate counter-measure of safety for a intersection. However, the collected conflict data is not always reliable because there is absence of clear criteria for conflict. This study developed objective and accurate conflict criteria, which is shown below based on traffic engineering theory. Frist, the rear-end conflict is regarded, when the following vehicle takes evasive maneuver against the first vehicle within a certain distance, according to car-following theory. Second, lane-change conflict is regarded when the following vehicle takes evasive maneuver against first vehicle which is changing its lane within the minimum stopping distance of the following vehicle. Third, cross and opposing-left turn conflicts are regarded when the vehicle which receives green sign takes evasive maneuver against the vehicle which lost its right-of-way crossing a intersection. As a result of correlation analysis between conflict and accident, it is verified that the suggested conflict criteria in this study ave applicable. And it is proven that estimating safety evaluation for a intersection with conflict data is possible, according to the regression analysis preformed between accident and conflict, EPDO accident and conflict. Adopting the conflict criteria suggested in this study would be both quick and accurate method for diagnosing safety and operational deficiencies and for evaluation improvements at intersections. Further research is required to refine the suggested conflict criteria to extend its application. In addition, it is necessary to develope other types of conflict criteria, not included in this study, in later study.

The Effectiveness of Facilitation Education for Call Quality of Medical Representative in Pharmaceutical Industry (제약회사 영업사원의 면담 품질(Call Quality) 향상을 위한 퍼실리테이션(Facilitation) 교육법의 효과)

  • Objectives: The purpose of this study is to figure out that Facilitation education can affect significant improvement in sales staff's understanding of precise pharmacokinetics, benefits, safety, and side effects, etc. and eventually lead to increase in call after taking courses. Methods: Data utilized in this study was collected from 413 sales staffs who completed Facilitation course for 5 months. This study used statistical methodologies, paired t-test, exploratory factor analysis, and logistic regression model in order to identify change in Call and after Facilitation courses. Results: The result shows that there are statistically significant increases in CALL quality after Facilitation courses based on the result of pared t-test. Moreover, Facilitation education is more effective in average time of one-time detail than average number of visits per day and average number of doctors per day from the result of logistic regression. Conclusions: In order for MR in pharmaceutical company to improve CALL quality, the education for precise pharmacokinetics, benefits, safety, and side effects is necessary. In addition, various professional training required for detail, including disease education, Selling Skills education, and literacy education are essential. Therefore, Facilitation education would be desirable choice in terms of pharmaceutical marketing strategy.

A Study on the Optimization of Offsite Consequence Analysis by Plume Segmentation and Multi-Threading (플룸분할 및 멀티스레딩을 통한 소외사고영향 분석시간 최적화 연구)

  • A variety of input parameters are taken into consideration while performing a Level 3 PSA. Some parameters related to plume segments, spatial grids, and particle size distribution have flexible input formats. Fine modeling performed by splitting a number of segments or grids may enhance the accuracy of analysis but is time-consuming. Analysis speed is highly important because a considerably large number of calculations is required to handle Level 2 PSA scenarios for a single-unit or multi-unit Level 3 PSA. This study developed a sensitivity analysis supporting interface called MACCSsense to compare the results of the trials of plume segmentation with the results of the base case to determine its impact (in terms of time and accuracy) and to support the development of a modeling approach, which saves calculation time and improves accuracy. MACCSense is an automation tool that uses a large amount of plume segmentation analysis results obtained from MUST Converter and Mr. Manager developed by KAERI to generate a sensitivity report that includes impact (time and accuracy) by comparing them with the base-case result. In this study, various plume segmentation approaches were investigated, and both the accuracy and speed of offsite consequence analysis were evaluated using MACCS as a consequence analysis tool. A simultaneous evaluation revealed that execution time can be reduced using multi-threading. In addition, this study can serve as a framework for the development of a modeling strategy for plume segmentation in order to perform accurate and fast offsite consequence analyses.

Mixed Reality System for Virtual Chemistry Lab

  • This paper presents a new method for improving teaching aids with the design and development of a chemistry education platform - the Mixed Reality (MR) Chemistry Lab. Our system provides a new educational experience in which students can simulate a chemistry experiment in a virtual lab and interact with objects using Oculus Helmet-Mounted Displays (HMD) and hand controller devices. The proposed system aims to familiarize students with experimental procedures and safety knowledge before conducting actual experiments. Moreover, students will be able to observe microscopic atomic structure models in three dimensions. Our research also includes a five-part, quantitative evaluation system user tests perform using a quantitative questionnaire consisting of five items, including hardware equipment, immersion, education, interaction sense of control, and degree of difficulty. The evaluation results confirm that this system will be helpful to the educational experience of conducting chemistry experiments with scientific evaluation methods, and the proposed system is also expected to have a broad range of applications in many other subjects.

  • A novel pneumatic artificial muscle actuator (PAM actuator), which has achieved increased popularity to provide the advantages such as high strength and high power/weight ratio, low cost, compactness, ease of maintenance, cleanliness, readily available and cheap power source, inherent safety and mobility assistance to humans performing tasks, has been regarded during the recent decades as an interesting alternative to hydraulic and electric actuators. In order to realize satisfactory control performance, a variable damper Magneto Rheological Brake (MRB), Is equipped to the Joint of the manipulator. Superb mixture of conventional PID controller and a phase plane switching control method brings us a novel controller. This proposed controller is appropriate for a kind of plants with nonlinearity, uncertainties and disturbances. The experiments were carried out in practical PAM manipulator and the effectiveness of the proposed control algorithm was demonstrated through experiments, which had proved that the stability of the manipulator can be improved greatly in a high gain control by using MRB with phase plane switching control method and without regard for the changes of external inertia loads.

A Study on the Functional Importance Determination Methodology for Components in Nuclear Power Plants (원전 기기의 기능적중요도결정 방법론에 대한 연구)

  • In around 2000, the U.S. NPPs have developed the various advanced engineering processes based on the INPO AP-913(Equipment Reliability Process Description) and showed the high performance in availability. With these benchmarking cases, the Korean NPPs have introduced the advanced engineering technology since 2005. The first step of the advanced engineering is to analyze and determine component importance for all components of a plant. This process is called Functional Importance Determination(FID). These results are basically utilized to determine the priority with limited resources in various areas. However, because the consistency of FID results is insufficient despite applying the same criteria in the existing operating NPPs, the degree of application is low. Therefore, this paper presents the improved methodology for FID interfacing system functions of Maintenance Rule Program and results of Single Point Vulnerability(SPV). This improved methodology is expected to contribute to enhance the reliability of FID data.
