The Effect of Obstacle Height on Balance Control While Stepping Over an Obstacle From a Position of Quiet Stance in Older Adults (노인의 정적인 자세로부터 장애물 보행 시 장애물 높이의 변화가 평형감각에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effect of an obstacle height on the balance control of older adults while stepping over an obstacle from a position of quiet stance. Methods: Fifteen community-dwelling healthy older adults (mean age, $74.4\pm4.27$ yrs; age range, 67-82 yrs) volunteered to participate in this study. The subjects performed gait initiation (GI) and they stepped over obstacles of two different heights (10 cm and 18 cm) at a self-paced speed from a position of quiet stance. Their performance was assessed by recording the changes in the displacement of the COP in the anteroposterior (A-P) and mediolateral (M-L) directions using a force platform. Results: The M-L displacement of the COP significantly increased for an 18 cm obstacle height condition as compared to the GI and a 10 cm obstacle height condition (p<0.01). Furthermore, the M-L displacement of the COP for a 10 cm high obstacle was significantly greater for that for the GI (p<0.01). However, the mean of the A-P displacement of the COP was similar between the stepping conditions for the A-P displacement of the COP (p>0.05). Conclusion: This study suggests that the M-L COP displacement could be a better parameter to identify the dynamic balance control in older adults when negotiating obstacles.

A experimental study on the physiological effects of electrical stimulation treatment of serum myoglobin and aldolase in human body (인체에 적용한 전기자극이 Serum myoglobin과 Aldolase에 미치는 영향에 대한 생리학적 효과)

  • Prior studies have revealed that several stimulation to the muscle have released serum myoglobin into the blood vessel and increased aldolase activity. The present authors carried out a study which effect of electrical stimulation treatment (induced a isotonic wrist exerceise) on serum myoglobin(Mb) levels and aldolase(Al) activity were investigated in 6 healthy female. There were four groups of female: 1. no electrical stimulation control: 2. electrical stimulation 10min (EST10'); 3. electrical stimulation 20 min (EST20'); 4. electrical stimulation 30min (EST30'). Each groups is all the same one. Radioimmunoassay and Ultraviolet Spectrophotometry were performed to increased or decreased of serum myoglobin and aldolase. Serum myoglobin significantly increased in electrical stimulated groups[EST10' $(30.20{\pm}5.27ng/ml)$, EST20'$(31.65{\pm}3.96ng/ml)$, EST30'$(31.95{\pm}2.0ng/ml)$] to be compared with control group$(24.43{\pm}2.20ng/ml)$. Aldolase significantly increased in electrical stimulated groups [EST10' ($6.85{\pm}1.17$ Sigma U/mL), EST20'($6.70{\pm}1.46$ Sigma U/mL), EST30'($6.56{\pm}1.01$ Sigma U/mL)) to be compared with control group($5.03{\pm}1.86$ Sigma U/mL). The results of this study show that isotonic exercise result in electrical stimulation treatment increased serum myoglobin content and aldolase activity. In conclusion, our results support that stimulation release serum myoglobin and increase aldolase activity.

Prolonged low-dose cyclophosphamide treatment after pulse therapy attenuates lung injury in rats with paraquat intoxication

  • Background/Aims: This study tested the hypothesis that prolonged low-dose cyclophosphamide (CTX) treatment after pulse therapy attenuate paraquat (PQ)-induced lung injury in rats. Methods: PQ (25 mg/kg) was administered intraperitoneally to induce PQ-intoxicated rat model. The rats were randomly divided into four groups: control group (1 mL/day saline solution for 14 days), PQ group (1 mL/day saline solution for 14 days after PQ exposure), pulse group (15 mg/kg/day CTX in 1 mL of saline solution for 2 days and subsequent 1 mL/day saline solution for 12 days), and prolonged low-dose group (15 mg/kg/day CTX in 1 mL of saline solution for 2 days and subsequent 1.5 mg/kg/day CTX in 1 mL of saline solution for 12 days). A 14-day follow-up was conducted to determine the survival rat, and lung hydroxyproline (HYP), wet-to-dry weight ratios (W/Dc) and histopathological changes were evaluated. Results: Results showed similar survival rate (55% vs. 50%, p > 0.05) between prolonged low-dose and pulse groups. Lung W/Dc ($4.94{\pm}0.38$ vs. $5.47{\pm}0.28$, p < 0.01), HYP ($3.34{\pm}0.29{\mu}g/mg$ vs. $3.65{\pm}0.19{\mu}g/mg$, p < 0.001), and fibrosis score ($2.69{\pm}0.84$ vs. $3.13{\pm}0.63$, p < 0.05) were lower in prolonged low-dose group than those in the pulse group. Conclusions: These findings suggested prolonged low-dose CTX treatment after pulse therapy could attenuate PQ-induced lung injury in rats.

Comparison of breast volume change between oncoplastic breast-conserving surgery with radiation therapy and a simultaneous contralateral balancing procedure through the inverted-T scar technique

  • Background Reduction mammoplasty or mastopexy is performed as an additional balancing procedure in patients with large or ptotic breasts who undergo breast-conserving surgery (BCS). Radiation therapy on breasts that have undergone surgery may result in changes in the volume. This study presents a comparative analysis of patients who received post-BCS balancing procedures to determine whether volume changes were larger in breasts that received radiation therapy than on the contralateral side. Methods Thirty-six participants were selected among patients who received BCS using the inverted-T scar technique between September 2012 and July 2017, were followed up for 2 or more years, and had pre-radiation therapy computed tomography images and post-radiation therapy images taken between 12 and 18 months after completion. The average age of the participants was 53.5 years, their average body mass index was 26.62 kg/㎡. Results The pre- and post-radiation therapy volumes of the breasts receiving BCS were 666.08±147.48 mL and 649.33±130.35 mL, respectively. In the contralateral breasts, the volume before radiation therapy was 637.69±145.72 mL, which decreased to 628.14±166.41 mL after therapy. The volume ratio of the affected to the contralateral breasts was 1.05±0.10 before radiation therapy and 1.06±0.12 after radiation therapy. Conclusions The ratio of the volume between the two breasts immediately after surgery and at roughly 18 months postoperatively was not significantly different (P=0.98). For these reasons, we recommend a simultaneous single-stage balancing procedure as a reasonable option for patients who require radiation therapy after BCS without concerns regarding volume change.

  • The purpose of this study is to know the average of hip joint range of motion and difference according to the aging for the elderly. This study consisted of elder male(n=75) and elder female(n=109). The result of assessment and analysis in hip pint range of motion are as follows : 1) The average hip flexion(knee flexed) joint range of motion in 60-69(from sixty to sixty-nine)years old are $104.26^{\circ}$(Left-Male), $101.00^{\circ}$(Right-Male), $107.05^{\circ}$(Left-Female), $107.05^{\circ}$(Right-Female). 70-79years old are $104.59^{\circ}$(L-M), $102.05^{\circ}$(R-M), $105.73^{\circ}$(L-F), $108.75^{\circ}$(R-F). 80-89years old are $101.53^{\circ}$(L-M), $101.13^{\circ}$(R-M), $96.83^{\circ}$(L-F), $97.67^{\circ}$(R-F). There was significant difference in hip flexion(knee flexed) among female group(p<.01). The average hip flexion(knee extended) joint range of motion in 60-69(from sixty to sixty-nine)years old are $73.13^{\circ}$(Left-Male), $72.04^{\circ}$(Right-Male), $77.29^{\circ}$(Left-Female), $75.97^{\circ}$(Right-Female). 70-79years old are $74.95^{\circ}$(L-M), $72.19^{\circ}$(R-M), $76.73^{\circ}$(L-F), $76.65^{\circ}$(R-F). 80-89years old are $70.83^{\circ}$(L-M), $70.37^{\circ}$(R-M), $69.00^{\circ}$(L-F), $69.00^{\circ}$(R-F). There was significant difference in left hip flexion(knee extended) among female group(p<.05). 2) The average hip extension joint range of motion in 60-69years old are $13.09^{\circ}$(L-M), $12.78^{\circ}$(R-M), $10.97^{\circ}$(L-F), $10.68^{\circ}$(R-F). 70-79years old are $8.95^{\circ}$(L-M), $8.48^{\circ}$(R-M), $11.24^{\circ}$(L-F), $10.90^{\circ}$(R-F). 80-89 years old are $8.40^{\circ}$(L-M), $8.23^{\circ}$(R-M), $7.33^{\circ}$(L-F), $7.33^{\circ}$(R-F). There was significant difference in left(p<.01) and right(p<.05) hip extension among male group(p<.05). 3) The average hip abduction joint range of motion in 60-69 years old are $33.04^{\circ}$(L-M), $33.17^{\circ}$(R-M), $33.16^{\circ}$(L-F), $33.37^{\circ}$(R-F). 70-79 years old are $31.00^{\circ}$(L-M), $30.05^{\circ}$(R-M), $32.44^{\circ}$(L-F), $32.68^{\circ}$(R-F). 80-89 years old are $29.07^{\circ}$(L-M), $27.90^{\circ}$(R-M), $28.17^{\circ}$(L-F), $28.67^{\circ}$(R-F). There was no significant difference among group. 4) The average hip adduction pint range, of motion in 60-69years old are $29.57^{\circ}$(L-M), $29.35^{\circ}$(R-M), $31.87^{\circ}$(L-F), $31.89^{\circ}$(R-F). 70-79, years old are $27.41^{\circ}$(L-M), 27.00(R-M) $30.85^{\circ}$(L-F), $31.28^{\circ}$(R-F). 80-89 years old are $26.87^{\circ}$(L-M), $26.63^{\circ}$(R-M), $24.67^{\circ}$(L-F), $24.83^{\circ}$(R-F). There was significant difference in hip abduction among female group(p<01). 5) The average hip external rotation pint range of motion in 60-69years old are $32.26^{\circ}$(L-M), $31.17^{\circ}$(R-M), $33.53^{\circ}$(L-F), $34.42^{\circ}$(R-F). 70-79 years old are $31.64^{\circ}$(L-M), $28.62^{\circ}$(R-M) $31.29^{\circ}$(L-F), $31.45^{\circ}$(R-F). 80-89 years old are $26.40^{\circ}$(L-M), $26.07^{\circ}$(R-M), $24.77^{\circ}$(L-F), $24.27^{\circ}$(R-F). There was significant difference in left(male, female p<.01) and right(female p<.0l) hip external rotation among group. 6) The average hip internal rotation joint range of motion in 60-69years old are $30.30^{\circ}$(L-M), $28.13^{\circ}$(R-M), $34.27^{\circ}$(L-F), $36.03^{\circ}$(R-F). 70-79years old are $31.24^{\circ}$(L-M), $29.57^{\circ}$(R-M), $28.51^{\circ}$(L-F), $29.10^{\circ}$(R-F). 80-89 years old are $24.63^{\circ}$(L-M), $24.40^{\circ}$(R-M), $24.27^{\circ}$(L-F), $24.27^{\circ}$(R-F). There was significant difference in left(male p<.05, female p<.01) and right(female p<.01) hip internal rotation among group.

Effects of Laser Acupuncture Therapy According to the Wavelength at GB34·GB39 on Neuropathic Pain Rat Induced by Tibial and Sural Nerve Transection (양릉천(GB34)·현종(GB39)에 대한 파장대별 레이저침 시술이 동통에 미치는 영향)

  • Objectives : The purpose of this study is to examine the effects of laser acupuncture according to the wavelength(532 nm, 650 nm, 830 nm, 904 nm, and 1064 nm) at the acupoint of GB34 GB39 on neuropathic pain rat induced by tibial and sural nerve transection(TST). Methods : Neuropathic pain in rats was induced by tibial nerve and sural nerve transection. The rats were divided into the intact group, the TST control group, and the laser acupuncture therapy group. The laser acupuncture therapy groups were then divided into subgroups with 532 nm(L532), 650 nm(L650), 830 nm(L830), 904 nm(L904), and 1064 nm(L1064) laser acupuncture therapy. The acupoint of GB34 GB39 was selected, and laser acupuncture therapy was provided on both sides alternatively twice a week in a total of 6 sessions. Results : All the laser acupuncture groups showed a significant decrease in reaction time and force intensity. L532, L904, and L1064 groups showed a significant decrease in Bax, the L532 group showed a significant increase in Bcl-2, L532 and L1064 groups showed a significant decrease in the Bax/Bcl-2 ratio, and L532 and L650 groups showed a significant increase in mGluR5, as compared with the TST control group, among nerve tissue reaction. Conclusions : These results showed that laser acupuncture therapy at each of the wavelengths had some significance on neuropathic pain.

Maximum Value Calculation of High Dose Radioiodine Therapy Room (고용량 방사성옥소 치료 병실의 최대치 산출)

  • Purpose: According to increment of thyroid cancer recently, patients of high dose radioiodine therapy were accumulated. Taking into consideration the acceptance capability in the current facility, this study is to calculate the maximum value of high dose radioiodine therapy in patients for treatment. Materials and Methods: The amount and radioactivity of waste water discharged from high dose radioiodine therapy in patients admitted at present hospital as well as the radiation density of the air released into the atmosphere from the high dose radioiodine therapy ward were measured. When the calculated waste water's radiation and its density in the released air satisfies the standard (management standard for discharge into water supply 30 Bq/L, management standard for release into air 3 $Bq/m^3$) set by the Ministry of Education, Science and Technology, the maximum value of treatable high dose radioiodine therapy in patients was calculated. Results: When we calculated in a conservative view, the average density of radiation of waste water discharged from treating high dose radioiodine therapy one patient was 8 MBq/L and after 117 days of diminution in the water-purifier tank, it was 29.5 Bq/L. Also, the average density of radiation of waste water discharged from treating high dose radioiodine therapy two patients was 16 MBq/L and after 70 days of diminution in the water-purifier tank, it was 29.7 Bq/L. Under the same conditions, the density of radiation released into air through RI Ventilation Filter from the radioiodine therapy ward was 0.38 $Bq/m^3$. Conclusion: The maximum value of high dose radioiodine therapy in patients that can be treated within the acceptance capability was calculated and applied to the current facility, and if double rooms are managed by improving the ward structure, it would be possible to reduce the accumulated treatment waiting period for radioiodine therapy in patients.

Antimicrobial Effects of 5-Aminolevulinic Acid Mediated Photodynamic Therapy against Pathogenic Bacteria (병원성세균에 대한 5-Aminolevulinic Acid로 중재된 광역학치료의 항균효과)

  • This study evaluates the improved effect of photodynamic therapy (PDT) by subjecting pathogenic bacteria to a combination of 630 nm light-emitting diode (LED) and 5-aminolevulinic acid (ALA). Bacterial suspensions of 1.5×104 cells/mL were diluted and exposed to ALA concentrations of 10, 5, 2.5, 1.25, and 0.625 mg/mL, incubated for 30 minutes, followed by irradiation with 630 nm LED (18 J/cm2 ). The non-irradiated P. aeruginosa group and the group administered only LED light averaged 415 and 245 colonies, respectively. Conversely, the PDT group showed an average of 109, 225, 297, and 285 colonies at concentrations of 10, 5, 2.5, and 1.25 mg/mL of ALA. Evaluating the effect on E. faecalis revealed an average of 8,750 and 8,000 colonies in the group that did not receive the control photosensitizer and the group exposed to light alone, respectively. However, an average of 0, 2350, 4825, and 7475 colonies at concentrations of 5, 2.5, 1.25, and 0.625 mg/mL ALA were determined for the PDT groups. In conclusion, better inhibitory effects were observed for E. faecalis than for P. aeruginosa. Moreover, our results validate the possibility of improved PDT efficacy using a combination of ALA and 630 nm LED.

The Effect of Modified Reverse-Six Taping on the Balance and Gait Performance in Pes Planus - Pilot study (수정된 리버스 식스 테이핑 적용이 편평족의 균형과 보행에 미치는 영향 - 사전연구)

  • Background: The purpose of this study was to investigate the effect of modified reverse-six taping on the balance and gait performance in Pes Planus. Methods: A total of twelve subjects (4 males, 8 females) in Pes Planus participated in this study. Navicular drop test was used to evaluate Pes Planus. Balance performance(anterior-posterior; A-P, medial-lateral; M-L), overall) was evaluated using the Biodex balance system in two difference condition(no-taping, with reverse-six taping). Gait performance (cadence, velocity) was evaluated using GAITRite System in two difference condition (no-taping, with reverse-six taping). Results: There were significant improvements of A-P and overall in the balance performance after using reverse-six taping (p<.05). there was no significant improvements of M-L balance performance. and, there were no statistical difference of cadence and velocity in the gait performance after using reverse-six taping. Conclusions: This study found that modified reverse-six taping in Pes Planus was improve the balance performance. but dose not affect the gait performance.

HBV-DNA Suppression in a Chronic Hepatitis B Patient Treated with Western Medicine Plus Traditional Korean Medicine Combination Therapy (한.양방 협진에 의한 만성 B형 간염 환자에서의 HBV-DNA 감소 증례보고)

  • Here we describe a case of lamivudine and entecavir resistant chronic hepatitis B, treated with western medicine plus traditional korean medicine combination therapy. We administered Injinchunggan-Tang(茵蔯淸肝湯), a traditional Korean herb remedy, with entecavir to a 45-year-old chronic hepatitis B patient who didn't have achieved HBV-DNA suppression, in spite of 18 month lamivudine mono therapy and 14 month entecavir mono therapy. HBV-DNA decreased more than one thousandth from 98,100 IU/mL to 53 IU/mL just in 23 days, and resultingly it seems that the combination therapy could be very potent, at least to some chronic hepatitis B patients.
