Effects of carboxymethyl chitosan fabric and low molecular weight heparin on reducing adhesion formation in the rat (Rat에서 carboxymethyl chitosan fabric과 low molecular weight heparin의 유착방지 효과)

  • This study was carried out to compare the efficacy of carboxymethyl chitosan fabric (CMCF) with that of the combination of low molecular weight heparin (LMWH) and CMCF for the prevention of postoperative adhesions in the rat. Adhesions were induced by suturing both the ileal serosa and peritoneum scraped until petechial bleeding occurred. CMCF were sutured between two surfaces, and LMWH were instilled intraperitoneally immediately before closure of the peritoneomuscular layer. The adhesions were blindly assessed 2 weeks later by using a tensiometer. The mean tensile strength(Newton) of formed adhesions was $2.59{\pm}0.85$ in control group, $2.10{\pm}0.75$ in the CMCF group and $1.53{\pm}0.44$ in the CMCF+LMWH group. The most favorable prevention against adhesion was achieved in the CMCF+LMWH group. Therefore, we could conclude that CMCF+LMWH were effective in prevention against postoperative adhesion in the rat.

  • Free tissue transplantation is commonly performed with the brilliant achievement in microsurgery and anticoagulants and antithrombotic agents have been prescribed in the procedures. However, there is no clean-cut indication as to which agents would be more effective in every steps and final consequences. Low molecular weight heparins inhibiting coagulation in plateletrich plasma and acting on the vascular endothelium have antithrombotic and fibrinolysis action. The experiment with rat groin free flap transplantation after 6-hour ischemia and injection of the low molecular weight heparin was performed and the results between the injection and non-injection group were analysed as follows, 1. Both of the 24-hour groups, vessel patency was not proportional to color change of the groin flap. 2. On the second day after anastomois, heparin-injection group showed intact intima, patent lumen without thrombus, and mild granulomatous inflammation around the suture material and control group with doubtful patency revealed intimal loss and thrombus formation. 3. On the 5th, 7th, and 9th postoperative day, heparin group was patent in anastomosis and showed acute inflammatory cells. 4. The 7th-week period, heparin-injection group showed intact flap color, patent lumen with intact intima and persistent foreign body granuloma.

Adhesion Prevention in the Dog with Sodium Carboxymethylcellulose and Low Molecular Weight Heparin (개에서 Sodium Carboxymethylcellulose와 Low Molecular Weight Heparin(Fraxiparine$^{(R)}$)의 유착방지효과)

  • This study was carried out to compare the efficacy of sodium carboxymethyl cellulose (SCMC) with the combination of low molecular weight heparin (LMWH) and SCMC for the prevention of postoperative adhesions in the dog. After laparotomy, abrasions were made on the surface of ileal serosa in 17 dogs. The animals were randomly divided into the control group (N = 5) which received no treatment, the SCMC-treated group (N = 6), and the SCMC & LMWH-treated group (N = 6). Hematologic values were monitored before laparotomy and 1, 4, 7, 10 and 14 days after laparotomy. The location and score of adhesion were assessed at second laparotomy 3 weeks later, Although the mean adhesion score both in the SCMC-treated group (7.17${\pm}$1.17) and in the SCMC & LMWH-treated group (4.50${\pm}$1.38) was found to be significantly lower than that in the control group (9.40${\pm}$0.89) (p<0.01 and p<0.01, respectively), more favorable adhesion prevention was achieved in the SCMC & LMWH-treated group in comparison with the SCMC-treated group without any hemor-rhagic complications (p<0.01). This study could conclude that SCMC & LMWH are highly effective in prevention against postoperative adhesion in the dog.

Synergistic Effects of Carboxymethylchitosan Fabric and Low Molecular Weight Heparin in Reducing Adhesion Formation in the Rat Uterine Horn Model (Rat에서 Carboxymethylchitosan Fabric과 Low Molecular Weight Heparin의 자궁각 유착 형성 방지 상승효과)

  • This study was performed to compare the efficacy of carboxymethylchitosan fabric (CMCF) with that of the combination of CMCF and low molecular weight heparin (LMWH) for the prevention of postoperative uterine adhesion in rats. Adhesions were induced by suturing both the uterine serosa and peritoneum abrased until petechial bleeding occurred. Fourteen days later, adhesions were evaluated clinically and histopathologically. The mean tensile strength was significantly decreased in the CMCF and CMCF+LMWH groups compared to that of control group, and the CMCF+LMWH group had the lowest tensile strength. The distance of adhesion site was highest in the CMCF group and slightly decreased in the CMCF+LMWH group comparing to that of control group. The inflammatory cell infiltration and neovascularization of the CMCF group were significantly lower than those of the control group. It was observed that the damage at intestinal serosa was significantly decreased in the CMCF+LMWH group comparing to that of control group. Therefore, it was concluded that CMCF and LMWH may be useful to prevent postoperative uterine adhesion in rats.

Oral Surgery using Low-molecular-weight Heparin in the Anticoagulated Patients (항응고제 복용 환자에서 저분자량 헤파린을 사용한 구강 내 소수술)

  • Anticoagulation therapy with warfarin sodium is used to reduce the risk of thromboembolic events in patients with valvular heart disease, prosthetic heart valve, recurrent myocardiac infarction, etc. To keep anticoagulation state and minimize bleeding risk, patients with high risk of thromboembolism have been usually hospitalized for heparinization before oral surgery like extraction. However, this protocol requires time and high expense because of the long period of hospitalization and this is why low-molecular-weight heparin (LMWH) therapy is receiving attention in medical field as well as dentistry. LMWH has several advantages over unfractionated heparin (UFH) including predictable anticoagulant response which makes coagulation monitoring unnecessary in most patients and longer half-life than heparin which enables the patients to give themselves a subcutaneous injection once or twice daily. These advantages of LMWH make patients get oral surgery on an outpatient basis so that they can save time and cost. This case report introduces the use of LMWH in dental surgery and suggests proper use of LMWH. Though LMWH bridging therapy is widely used most of the previous studies are observational studies. Therefore randomized controlled trials are necessary to evaluate the safety and efficacy of LMWH bridging therapy.

Does intrauterine injection of low-molecular-weight heparin improve the clinical pregnancy rate in intracytoplasmic sperm injection?

  • Objective: Heparin can modulate proteins, and influence processes involved in implantation and trophoblastic development. This study aimed to assess the improvement of clinical pregnancy and implantation rates after local intrauterine injection of low-molecular-weight heparin (LMWH) in patients undergoing intracytoplasmic sperm injection (ICSI). Methods: A randomised case/control design was followed in women scheduled for ICSI. The study arm was injected with intrauterine LMWH during mock embryo transfer immediately following the ovum pickup procedure, while the control arm was given an intrauterine injection with a similar volume of tissue culture media. Side effects, the clinical pregnancy rate, and the implantation rate were recorded. Results: The pregnancy rate was acceptable (33.9%) in the LMWH arm with no significant reported side effects, confirming the safety of the intervention. No statistically significant differences were found in the clinical pregnancy and implantation rates between both groups (p= 0.182 and p= 0.096, respectively). The odds ratio of being pregnant after intrauterine injection with LMWH compared to the control group was 0.572 (95% confidence interval [CI], 0.27-1.22), while the risk ratio was 0.717 (95% CI, 0.46-1.13; p= 0.146). No statistical significance was found between the two groups in other factors affecting implantation, such as day of transfer (p= 0.726), number of embryos transferred (p= 0.362), or embryo quality. Conclusion: Intrauterine injection of LMWH is a safe intervention, but the dose used in this study failed to improve the outcome of ICSI. Based on its safety, further research involving modification of the dosage and/or the timing of administration could result in improved ICSI success rates.

  • Lipoprotein lipase(LPL) is an acylglycerol hydrolase and is the extrahepatic enzyme responsible for the hydrolysis of triglyceride-rich plasma lipoproteins. LPL has been isolated from bovine milk by affinity chromatography on heparin-sepharose in 2M NaCl, 5mM barbital buffer, pH 7.4. Para-nitrophenyl butyrate(PNPB) was used as a substrate for the determination of LPL activity. Molecular weight of LPL was 55KD on 10% SDS-PAGE. When the effects of heparin on LPL activation were compared, LPL activity of heparin added group increased approximately 5 times higher than that of heparin non-added groups. These results indicated that heparin involved in the stabilization of LPL structure that led to increase enzyme activity. Furthermore, LPL activity increased about 4 times compared to the absence of heparin at various pH. LPL was stabilized when heparin was added either low or high salt concentrations. With the presence of heparin, NaCl concentration did not affect LPL activity at pH range 6∼9.

Anticoagulation Activities of Low Molecular Weight Sulfated Chitosan and Sulfated Sodium Alginate (저분자량의 황산화 키토산과 황산화 알진산 나트륨의 항응고성)

  • Sulfated chitosan and sulfated sodium alginate were synthesized by sulfating reaction of low molecular chitosan and low molecular sodium alginate with SO$_3$-pyridine complex. When the weight ratio of SO$_3$-pyridine complex to polysaccharide was 1:5, the degrees of sulfation were the highest at 2.75 and 2.53 respectively. The anticoagulation effect was the highest when the molecular weight was 8.0${\times}$10$^3$ Da, and the anticoagulation activity was the highest at 91% of that of heparin when sulfated chitosan and sulfated sodium alginate were mixed at a weight ratio of 1:1. The anticoagulation activity was highest at 84% of that of heparin in the active plastin trombo test (aPTT) when sulfated chitosan and sulfated sodium alginate were mixed at a weight ratio of 1:1.

Serious Bleeding Complication Due to the Use of Low-molecular-weight heparin to treat a Traumatic Patient with Acute Renal Failure (급성신부전이 발생한 중증 외상 환자에서 저분자량헤파린 투여 후 발생한 심각한 출혈 합병증)

  • Trauma is an important risk factor for a pulmonary thromboembolism, and anticoagulation is essential to prevent deep vein thrombosis (DVT) in patients with trauma. Low-molecular-weight heparin (LMWH) is excreted in the kidney; therefore, using LMWH in patients with renal insufficiency may increase the risk of bleeding complication. The following case describes a 55-year-old traffic accident victim who had massive bleeding and underwent a laparotomy for bleeding control. The patient had acute renal failure, and enoxaparin was administered for the prophylaxis of DVT. Although the patient suffered from serious complications such as pericardial hematoma, the patient recovered without sequellae and was discharged at day 84.
