This study examined the factors affecting service utilization of elderly residential care among long term care services recipients during long term care insurance pilot project period in Korea. Help-seeking Behavior model developed by Andersen and Newman(1973) was used to analyze the factors affecting their utilization residential care service among 1,939 long term care services recipients. Frequency and Decision Tree Regression analysis on SPSS 13.0 used. Analyses show strong significant factor is service preference(predisposing factors), and then significant factors are enabling factors such as co-residence type, household income. According to this results, need factors such as cognition disorder, problem behavior, ADL and IADL disabilities are affecting utilization behavior of elderly residential care services. These findings provide implications and suggestions about how long term care service system would be settled in Korea. And these finding provide information about target-efficient long term care continuum system to policy makers and helping professionals.
Purpose - This study is to propose discriminative management strategies to long-term care facilities based on the empirical analysis after reviewing the effects of social support, perceived by long-term care facility employees, to service orientation. Research design, data, and Methodology - The research model designed social support, job stress, organizational commitment, and service orientation. The survey collected data from 453 customers in a long-term care facility in jeju. The SPSS 18.0 package was used for analysis. Results - First, social support for long-term care facility employees has a negative(-) effect to job stress. Test results, social support factors except appraisal support had a negative impact on job stress. Second, social support has a positive(+) effect to organizational commitment. Test results, informational support, tangible support and appraisal support had significant effects on organizational commitment. However, emotional support had a positive impact on affective commitment and normative commitment. Third, social support has a positive(+) effect to service orientation. Test results had a positive impact. Fourth, job stress has a negative(-) effect to organizational commitment. In the test results, employee's continuance commitment and normative commitment had significant negative effects in job stress. However, affective commitment had no significant impact. Fifth, job stress has a negative(-) effect to service orientation. Test results showed a negative impact. Conclusions - The study implies the following. First, that there should be a change in the social perception of long-term care facilities. 'Long-Term Care Insurance for The Elderly' was enacted to emphasize this responsibility for the elderly problems as a new system. Enactment of this Act was expected to improve the quality of life of the people by stabilizing the elderly life and reducing the burden of families. Therefore, long-term care facility system should be as efficient as possible for making plans for systematic and organizational support. Second, the efforts of facility managers to minimize job stress of employees is necessary. Accordingly, performing spontaneous work is required for a comfortable working environment and management. Third, the systematic education and training to employees for service oriented behavior of the facility will be required in the long term.
Purpose: The purpose of this study is to understand nurses' perception of visiting nursing services of long-term care insurance. Method: The descriptive survey study involved 188 nurses selected by the convenient sampling of visiting nurses who participated in professional education sessions. Results: Of the 188 participants, 149 (79.3%) were aware of long-term care insurance. Awareness of aspects of long-term care services was 78.7% for facility service, 77.7% for ordinary visiting care service, 85.1% for visiting nursing service and 77.7% for visiting bathing service. Concerning visiting nursing service provision, the majority of the study subjects considered nurse-aid not to be the appropriate route for delivery of services including nasogastric tube exchange, tracheostomy tube management and stitch removal. Conclusion: Continuous evaluation and research on the standards and requirements of the nursing workforce is needed to secure and maintain the high quality of visiting nursing services. Exhaustive studies concerning task division and workforce separation according to nursing services type and level of difficulty should be done to develop the appropriate job description for visiting nursing service staff.
The objective of running a long-term care institution is to provide services helpful for maintaining, supporting, and improving elderlies' optimum level of physical, mental, and psychosocial functioning. For the purpose of analyzing the current situations of institutional long term care facilities in Korea, 27 facilities were selected proportionately from each of the cities and provinces, out of the total 152 facilities. About 20% of those who were institutionalized during 25 August through 2 Qctober 1993, the 391 elderlies were chosen on a systematic random basis. The instrument of this study was developed by modifying the tools of CARE, MAI and PCTC. A multivariate approach of discriminant analysis and clustering technique were employed for this study. The Stiudy reveals that there is no clear differentiation of goals and functions among the longterm care institutions in Korea. Staffing patte군 of long-term care facilities shows a shortage of nurses, physical therapists, and dieticians. The linkage between acute care facilities and long-term care is weak, and administration of long-term care faciltiy is carried out by non-professionals. They are responsible for assessing health status before entering the facility, and evaluating elderlies' care. Therefore, it is not surprising to find that most of the facilities have accommodated agede regardless of their real needs and health status. Based upon findings of the analysis, this study has classified long-term care facilities into four types : Type I is to help elderlies maintain independence in daily living activities. Type II facilities have the objective of maintaining and improving the current level of elderlies' function. Type III is to maintain maximum independence of elderlies in activities of daily living. And Type IV is identified for the group of facilities designed to restore or improve functional abilities of elderlies. In conclusion, the following suggestions are made : the need for long-term care should be assessed by multidimensional measurement. Institutional long-term care facilities should be classified and developed in response to type of type of care and service need. Both acute and long-term care facilities should be linked together in order to support the evaluation of service operation and program development.

In this study, we analyze the relationships between long-term care workers' person-centeredness and job satisfaction, emotional labor, and service quality. Self-administered questionnaires were used to collect data from 460 long-term care workers in long-term care facilities. In the research model, person-centered care was verified using independent variables, with emotional labor (surface behavior, internal behavior) and job satisfaction applied as parameters, and service quality applied as a dependent variable. The results of this study are as follows. First, the fit of the model is good. Second, the implicit behavior of emotional labor is found to be an important factor affecting service quality. Also, emotional labor is an important mediator, improving the value of person-centered care and service quality. There is a positive correlation between internal behavior and service quality, and a negative correlation between surface behavior and service quality. Third, emotional labor (surface behavior, internal behavior) is the most influential variable in terms of service quality. The results of this study demonstrate the necessity to pay clinical and academic attention to person-centered care in terms of long-term care workers' emotional labor and service quality.
Purpose: This is a qualitative study to identify dimensions of long-term care hospital care quality that provide high-level medical services for long-term care patients in Korea. Methods: Service consumers and providers were interviewed, and collected data were analyzed into thesis, type and dimension. The focus group method was applied to two provider groups and individual interview was applied to two persons who had experienced a long-term care hospital. Results: The results of analyzing the consumers and providers was integrated into 8 dimensions: physical environment, staff, clinical care and nursing, multiplicity of activity program, atmosphere, interaction with family, nutrition, and quality improvement system. Conclusion: The dimensions of long-term care hospital care quality from this study can be used as a basis of quality indicators. Quantitative studies to test these dimensions are required for establishing quality management systems.
Purpose: The purpose of this study was to explore the reported good service experiences from the perspective of elderly residents of long-term care facilities. Methods: Of those residents who are 65 years old or older, 14 residents whose length of stay were one month or longer and scores of the K-Mini Mental State Examination were 15 or higher were interviewed. The interview data formed the basis for the empirical statements about the reported nature of patients' experiences as residents of long-term care facilities. These data were used in concept mapping. Results: Through multidimensional scaling analysis and hierarchical cluster analysis, 62 core statements, two dimensions, and six clusters of good service experiences were derived. The two dimensions were classified as 'care centered-participation centered services' and as 'physical-emotional services.' Six cluster themes emerged as good service experiences: 'safety of care and treatment', 'responsible and supportive staff', 'comfort of living environment', 'mental well-being', and 'respect and communication'. Conclusion: The result of the study provides information about what experiences are important to older adults with cognitive impairment. The concept map can be used to develop a patient experience index for the elderly residents of long-term care facilities.
Purpose: In Japan, the Long-Term Care Insurance Act has stipulated visiting rehabilitation since 2000. This study aimed to identify the actual conditions of visiting rehabilitation in Japan through a literature review of reports published by the Japanese government. Methods: This literature review was conducted on eight articles among various government reports on the topic of the actual conditions of visiting rehabilitation. These reports were published by the Ministry of Health, Labor and Welfare of Japan based on their own investigations or reports from an external agency entrusted with the task, and were issued between 2000 and 2021. Results: The characteristics of the visiting rehabilitation offices, their number, manpower allocation, the number of users, and their visits to each office were identified. Also, the characteristics of the users, number of users, age and required degree of long-term care, causative disease, and required medical care were identified. To evaluate the actual status of the visiting rehabilitation service, the service use time, frequency, period of use, intervention by doctors and the degree of such intervention, therapist's service content, visitors' address before the use of the service, reason, and timing of the service introduction, evaluation of the service effectiveness, combination of services and transfer destination after termination, and status after service termination were checked. Conclusion: Based on the Japanese experience where visiting rehabilitation was introduced and applied to long-term care insurance, it would be meaningful to review the factors that required benchmarking among the Japanese service models while designing a similar model in Korea.
This study examined the factors affecting forms of long-term care service use by elderly and the forms of use are classified facility care service, home care service, and unused. It is used data from the 2nd pilot program for the Long Term Care Insurance scheme and it is analysed 5,497 cases. Multi-nominal regression is used. According to the results, women use formal service more than man do, and wowen use facility care than home care. Those who eligible for National Basic Livelihood Security System(NBLSS) are shown to have higher use of formal care(especially facility care) than the middle income class, and the low income class than the middle income class has lower use of formal care. In addition, higher the family care is available, lower the taking part in the service. The big cities and mid sized cities than rural are used the formal service and moreover mid sized cities are used facility care than home care. Furthermore, the level of care need is determinants of service use and function of ADL, IADL, and abnormal behavior is also determinants of formal service(especially facility care). But nursing need and rehabilitation need are not determinants of formal service use. Based on the results, the recommendations are developed and implemented for the improvement the elderly long-term care insurance.

The demand for long-term care is continuously on the rise as number of elders among the population increases. Due to the rapid growing demand for long-term care in Korea, there have been discussions for the introduction of new long-term care system into Korean society. The purpose of this study in to analyze changes of elderly care facilities in Japan after the beginning of long-term care system in 2000. The functional and architectural changes of Japanese facilities were researched to be used as references for predicting changes in Korean facilities. In Japan, after the execution of long term care system, the alterative aspect of facilities is divided into some categories; in sum, the change of environment for long term care, the effort to spread specialized facilities in a whole community, and the tendency to complex a variety of function of facilities such as an facility for home care service, and the like.
