Comparison of the Effects of Ultrasound, Laser, Ultrasound-Laser Integration in Patients with Knee Degenerative Osteoarthritis (무릎관절 퇴행성 골관절염 환자에 대한 초음파, 레이저, 초음파-레이저 복합치료의 효과 비교)

  • Background: The number of old people with degenerative osteoarthritis one of the chronic disease, were constantly increased. Many researchers have made a great effort to verify the effects of ultrasound and laser therapy in degenerative osteoarthritis treatment. This study applied ultrasound-laser integration therapy on knee joint degenerative osteoarthritis and compared its effect with ultrasound and laser. Methods: We assigned 60 patients age 65 to 85 who had been diagnosed as knee joint degenerative osteoarthritis in G nursing home in Daejeon city. Randomization was done in blocks of three, holding twenty people per each group to receive either ultrasound, laser, ultrasound-laser integration therapy. This study carried out the experiment for 6 weeks to from April 17, 2010. We measured variables using visual analog scale (VAS) and pressure threshold meter (PTM) of the effects by before and after exercise. Results: It has been found that VAS was reduced and PTM was increased in all three groups. Compare with the other groups, Ultrasound-laser integration therapy group had lower VAS and higher PTM than ultrasound therapy and laser therapy group. Conclusions: These results lead us to the conclusion that ultrasound-laser integration have influenced the pain reduction of the knee degenerative osteoarthritis.

  • A three-wavelength $Pr^{3+}:YLF$ laser at 604 nm, 607 nm and 640 nm simultaneously output by Fabry-Perot (F-P) etalon has been obtained. A 444 nm blue laser diode is used for pumping the $Pr^{3+}:YLF$ crystal, and a 0.1 mm F-P etalon is inserted in the resonator to select wavelength. The theoretical model of three-wavelength $Pr^{3+}:YLF$ laser is established, by adjusting the tilt angle of the etalon, the transmittances of the different wavelengths can be controlled, and the threshold values can be made to equalize by controlling the loss among different wavelengths. In the experiment, when the tilt angle of etalon is $9^{\circ}$ and the optimized length of resonator is 48 mm, the total output power of 25 mW at the three-wavelength is achieved at incident pump power of 7.5 W.

Comparison of the effects of transcutaneous electrical nerve stimulation, low level laser, and placebo treatment on temporomandibular joint disorders: a single-blind randomized controlled trial

  • Objective: Physical therapy techniques are required for patients with temporomandibular joint disorder (TMD), but the effects of treatment have not been compared. Therefore, effects of transcutaneous electrical nerve stimulation (TENS) and low level laser (LLL), which are most commonly used interventions, were compared. Design: Randomized controlled trial. Methods: Thirty-six participants with pain in the temporomandibular joint were enrolled, and 12 participants were randomly assigned to either the TENS group, LLL group, or placebo group. Each intervention was performed for a total of 6 sessions for 2 weeks. For the evaluation of the participants, the mouth opening (MO), pressure pain threshold (PPT), and stress were measured at three time periods: baseline, post-test, and follow-up at 2 weeks. Results: Significant interaction between groups according to each evaluation point was found only in PPT-masseter (p<0.05). The evaluation time point at which a significant difference appeared was at the post-test and follow-up at 2 weeks time periods. As a result of the post-test, the LLL group showed a significant improvement compared to the TENS group (p<0.05), and at 2 weeks follow-up, the TENS group showed a significant improvement compared to the placebo group (p<0.05). Conclusions: In this study, an experiment was conducted to compare the treatment effects when TENS, LLL, and placebo were given to patients with TMD. In addition, by quantitatively presenting the effect size of each treatment, this study suggests clinical use of TENS and LLL treatment for TMD.

Growth of Nd:YAG single crystal by czochralski method and characteristics of laser generation (Czochralski 방법에 의한 Nd : YAG 단결정의 육성 및 레이저 출력특성)

  • Nd:YAG single crystal widely used as solid state laser was grown by Czochralski method. <111> single crystal with 0.9at% of $Nd^{3+}$ was grown from the Czochralski furnace with a automatic diameter control system. The vertical temperature gradient in the liquid was the major factor that influence the crystal quality, and the crystal diameter was controlled by the home made computer program. The crystal boule with $\phi$50mm$\times$ι100mm effective size was cut, polished, and antireflection coated. The optical evaluation such as absorption spectrum, fluorescence spectrum coincide with typical features of Nd:YAG single crystal. The laser rod was assembled into the CW laser generator with a Kr lamp. The maximum CW laser output was 70 W and the threshold power and efficiency was 1.3kW and 1.64% respectively.

  • To apply a square-shaped beam homogenizer to laser shock peening, it should be designed with a long working distance and by considering metal targets with various shapes and textures. For long working distances, a square-shaped beam homogenizer with a long depth of focus is required. In the range of working distance, the laser beam is required to have not only high efficiency but high uniformity, in other words, a good peening quality is guaranteed. In this study, we defined this range as the working distance for laser shock peening. We have simulated the effect of some parameters on the working distance. The parameters include the focal length of the condenser lens, pitch size of the array lens, and plasma threshold of the metal. The simulation was performed through numerical analysis by considering the diffraction effect.

  • We have successfully termed brown colored patterns inside of a transparent borosilicate glass generally known as BK7, laying the focus of near infrared Ti: sapphire femtosecond laser beam in the bulk BK7 glass. It is important to keep the laser power well below the damage threshold of BK7 in forming the color center. Thanks to the low laser power, there was no laser induced mechanical damage such as cracks or threads in the color formed area. From the absorbance spectrum and its gaussian fitting, we found the band gap of BK7, 4.21eV, and three absorption edges.

Impact of Booster Section Length on the Performance of Linear Cavity Brillouin-Erbium Fiber Laser

  • The impact of booster section length made from passive erbium-doped fiber (EDF) on the L-band multiwavelength Brillouin-Erbium fiber laser (MBEFL) is studied experimentally in this paper. The influence on the performance of MBEFL in term of number of generated Stokes lines, tuning range and lasing threshold were investigated. A comparison was made between MBEFL without a booster section and with booster sections of different lengths. Through comparative study and at fixed BP power and 100mW of 1480 pump power, longer passive EDF length of 5m exhibits the highest average number of Stokes lines of 23 with tuning range of 14nm. In contrast, shorter passive EDF length of 1m shows the highest tuning range of 17nm and an average number of 21 Stokes lines.

  • In this paper, we propose a signal processing board suitable for a semi-active laser tracking to detect an optical signal generated from the laser target designator by applying an analog trigger signal, the quadrant photodetector and a high speed ADC(analog-digital converter) sampling technique. We improved the stability by applying the averaging method to minimize the measurement error of a gaussian pulse. To evaluate the performances of the proposed methods, we implemented a prototype board and performed experiments. As a result, we implemented a frequency counter with an error 14.9ns in 50ms. PRF error code has a stability of less than 1.5% compared to the NATO standard. Applying the three point averaging method to ADC sampling, the stability of 28% in X-axis and 22% in Y-axis than one point sampling was improved.

  • We present the fabrication and measured performance of a wavelength tunable Butt coupled DBR-LD. An average coupling efficiency between active layer and passive waveguide layer was measured over 85%per facet, and the average threshold current was 21 ㎃ for the waveguide integrated DBR laser. High output power of Butt coupled DBR-LD was obtained over 25 ㎽. As high as 25 ㎽ of output power was achieved by the butt coupled method. The maximum wavelength tuning range is about 7.4 nm, and the side mode suppression ratio was more than 40 ㏈ using 1.3 ${\mu}{\textrm}{m}$ InGaAsP waveguide layer.

A clinical study of the power control of Nd : YAG laser for painless irradiation on intraoral soft tissues (구강내 연조직에 대한 무통적조사를 위한 Nd:YAG laser의 출력조절에 관한 임상적 연구)

  • Most dentists are very interested in laser therapy on the intraoral soft tissue lesions because they want to accomplish the analgesic and aseptic surgery with little or no bleeding. In order to determine the difference of pain threshold according to different gingival tissues with or without inflammation, 25 patients with inflammatory periodontal disease and 10 volunteers with good general and oral health were selected as the inflamed group and the normal group, respectively. Interdental papilla, marginal gingiva, attached gingiva, and alveolar mucosa were irradiated by the contact delivery($300{\mu]m$ fiber optic, for 5 seconds) of a pulsed Nd:YAG laser(EN.EL.EN06O, Italy). And the laser power was gradually increased from 0.5W by the increment of 0.1W. The highest laser power was recorded as the first painful power when the painful gesture was recognized at first. The difference of the first painful power of laser according to different gingival tissues with or without inflammation was statistically analyzed by paired t-test in MICROSTAT program. Following results were obtained: 1. In the comparison related with the inflammation, the first painful power was significantly lower in the inflamed group than in the normal group, regardless of interdental papilla and marginal gingiva(p<0.05). 2. In the comparison related with the tissue structure, the first painful. power was significantly lower in alveolar mucosa than in attached gingiva(p<0.05). The results suggest that, for the painless therapy by a pulsed-Nd:YAG laser irradiation, the laser surgery over 2.0W of power should be necessarily accomplished under the local anethesia, and the local anesthesia should be considered according to the degree of inflammation, the tissue structure, and the purpose of laser therapy.
