Implications of Liver-Directed Therapy for Postoperative Hepatic Metastasis from Esophageal Cancer

  • Background: Distant recurrence of esophageal cancer (EC), even after radical resection, is common, and the most frequent site of EC metastasis is the liver. However, a multidisciplinary treatment strategy for postoperative liver metastasis (LM) from EC has yet to be established; in particular, the role of liver-directed therapy (LDT) remains uncertain. We investigated the clinicopathological features and outcomes of patients undergoing post-esophagectomy LM with versus without LDT to explore its therapeutic implications. Methods: Among 624 consecutive patients undergoing R0/R1 esophagectomy for EC, 30 were identified in whom LM had developed as the initial recurrence. Their characteristics were retrospectively reviewed. Results: Six of the 30 subjects underwent LDT for metachronous LM. Five of those 6 also received systemic chemotherapy. A comparison between the 6 LDT and 24 non-LDT cases revealed no significant differences in major clinicopathological and operative factors, except for concurrent metastasis to extrahepatic organs (1/6 vs. 15/24, p=0.044). Twenty-nine of the 30 patients died during the study period, whereas 1 who had received multimodal treatment with LDT remained alive more than 200 months after multiple LM had been detected. Kaplan-Meier analysis for survival after LM demonstrated significantly prolonged survival in LDT cases compared to non-LDT cases treated with systemic chemotherapy alone (p=0.014). Even when the analysis was limited to patients without extrahepatic metastasis, this significant prognostic advantage of LDT was maintained (p=0.047). Conclusion: Multimodal treatment combined with LDT might be beneficial for patients with metachronous LM from EC and should therefore be considered a potential treatment option.

  • This paper proposes a computational model which can simulate peculiar language phenomena observed in human lexical decision task. The model is designed to mimic major language phenomena such as frequency effect, lexical status effect, word similarity, and semantic priming effect. The experimental results show that the propose model replicated the major language phenomena and performed similar performance with that of human in LDT.

The Neighborhood Effect in Korean Visual Word Recognition (한국어 시각단어재인에서 나타나는 이웃효과)

  • We investigated whether the first syllable plays an important role in lexical access in Korean visual word recognition. To do so, one lexical decision task (LDT) and two form primed LDT experiments examined the nature of the syllabic neighborhood effect. In Experiment 1, the syllabic neighborhood density and the syllabic neighborhood frequency was manipulated. The results showed that lexical decision latencies were only influenced by the syllabic neighborhood frequency. The purpose of experiment 2 was to confirm the results of experiment 1 with form-primed LDT task. The lexical decision latency was slower in form-related condition compared to form-unrelated condition. The effect of syllabic neighborhood density was significant only in form-related condition. This means that the first syllable plays an important role in the sub-lexical process. In Experiment 3, we conducted another form-primed LDT task manipulating the number of syllabic neighbors in words with higher frequency neighborhood. The interaction of syllabic neighborhood density and form relation was significant. This result confirmed that the words with higher frequency neighborhood are more inhibited by neighbors sharing the first syllable than words with no higher frequency neighborhood in the lexical level. These findings suggest that the first syllable is the unit of neighborhood and the unit of representation in sub-lexical representation is syllable in Korea.

Paired analysis of tumor mutation burden calculated by targeted deep sequencing panel and whole exome sequencing in non-small cell lung cancer

  • Owing to rapid advancements in NGS (next generation sequencing), genomic alteration is now considered an essential predictive biomarkers that impact the treatment decision in many cases of cancer. Among the various predictive biomarkers, tumor mutation burden (TMB) was identified by NGS and was considered to be useful in predicting a clinical response in cancer cases treated by immunotherapy. In this study, we directly compared the lab-developed-test (LDT) results by target sequencing panel, K-MASTER panel v3.0 and whole-exome sequencing (WES) to evaluate the concordance of TMB. As an initial step, the reference materials (n = 3) with known TMB status were used as an exploratory test. To validate and evaluate TMB, we used one hundred samples that were acquired from surgically resected tissues of non-small cell lung cancer (NSCLC) patients. The TMB of each sample was tested by using both LDT and WES methods, which extracted the DNA from samples at the same time. In addition, we evaluated the impact of capture region, which might lead to different values of TMB; the evaluation of capture region was based on the size of NGS and target sequencing panels. In this pilot study, TMB was evaluated by LDT and WES by using duplicated reference samples; the results of TMB showed high concordance rate (R2 = 0.887). This was also reflected in clinical samples (n = 100), which showed R2 of 0.71. The difference between the coding sequence ratio (3.49%) and the ratio of mutations (4.8%) indicated that the LDT panel identified a relatively higher number of mutations. It was feasible to calculate TMB with LDT panel, which can be useful in clinical practice. Furthermore, a customized approach must be developed for calculating TMB, which differs according to cancer types and specific clinical settings.

  • High Speed Rail Train Control System consists of CTC, IXL and ATC. IXL and ATC perform train control and command via interchanging relevant information between a signal room and CTC. However, it is proved that IXL and ATC are attributed to train delay error since those systems are highly sensitive to trackside conditions. Especially, network error on IXL blocks transmitting signal information to adjacent signal room so that its effects give rise to system overall problems. In order to figure out the measures for which minimizing the occurrence rate of train delay error due to HSR TCS, This paper is performed analysis on communication network structure, the length of SSI network roof and SSI-TFM distance by examining and analyzing the error cases related to IXL in a network aspect.

  • The purpose of this paper is to define WiFi LDT(Location Determination Technology) and Service available for mobile wireless network. This paper introduces positioning technology such as Basic Technology Element and QoS(Quality of Service) etc. of WPS(WiFi Positioning System) for mobile wireless network. The LDT and wireless positioning technology in order to determine the position of terminal when mobile based positioning service is provided, and by providing service with postioning technology, it will not only provide convenience to the users or subscribers but also contribute to the activation of LBS(Location Based Services) industries.

  • A coupled room system consists of adjacent rooms and apertures where the sound energy is exchanged between the two rooms. Acoustically, a coupled room system shows a non-exponential decay profile. Most of the related researches have been to analyze the acoustic properties of two-room coupled system so far whereas three-room coupled system were seldom studied. In this regard, this paper aims to analyse the distribution of sound pressure level, sound decay curve of three-room coupled system and sound energy flow between them by using the acoustic diffusion model and to further verify them through experiments. Firstly, the sound pressure level distribution and mean sound pressure level in the steady-state condition are analyzed at various frequencies and source locations. Good agreements are observed in both experiments and analysis results. Secondly, two double slope effect quantifiers of sound attenuation, LDT/EDT and LDT/T10 are compared at various frequencies and for different source locations. The result indicates that LDT/T10, less affected by the early reflection patterns than LDT/EDT, is more suitable to the analysis and experiments of a multi-slope sound decay curve. Lastly, the sound energy flow in each room is analyzed based on the acoustic diffusion model. After the early decay stage, the sound energy is observed to flow from the room with a long reverberation time to the room with a short one.

A Study of vibration Characteristics of Compressors with FEM model (유한요소 모델을 이용한 압축기의 진동특성에 관한 연구)

  • Today, as the demands for home appliances are increasing, the understanding of noise and vibration characteristics have become more important. It is hard to control its vibration and noise characteristics, because its mechanical structure is very complex. In this study a model of reciprocating compressor is developed. Spring, frame, and LDT are modeled as flexible body, and the other parts are modeled as rigid. FEM model of frame is simplified in order to save the simulation time. We validated the simple model by comparing their natural frequencies and mode shapes. Motor torque is applied to a rotor, and the piston is subjected to a gas pressure. The vibrational characteristics of compressor is analyzed with LS-DYNA. Its results are compared with the simulation results of rigid body frame. The effect of LDT is also studied by comparing the vibration of frame with the results of simulation with no LDT.

The Effect of Semantic Neighborhood Density in Korean Visual Word Recognition (한국어 시각단어재인에서 의미 이웃크기 효과)

  • The lexical decision task (LDT) commonly postulates the activation of semantic level. However, there are few studies for the feedback effect from semantic level. The purpose of the present study is to investigate whether the feedback effect from semantic level is facilitatory or inhibitory in Korean LDT. In Experiment 1, we manipulated the number of phonological syllable neighbors (PSN) and the number of semantic neighbors (SEN) orthogonally while orthographic syllable neighbor (OSN) is dense. In the results, the significant facilitatory effect was shown in words with many SEN. In Experiment 2, we examined same conditions as Experiment 1 but OSN was sparse. Although the similar lexical decision latency pattern was shown, there was no statistical significance. These results can be explained by the feedback activation from semantic level. If a target has many SENs and many PSNs, it receives more feedback activation from semantic level than a target with few SENs and PSNs.

  • This paper examined two hypotheses. Firstly, if the first syllable of word play an important role in visual word recognition, it may be the unit of word neighbor. Secondly, if the first syllable is the unit of lexical access, the neighborhood size effect and the neighborhood frequency effect would appear in a lexical decision task and a form primed lexical decision task. We conducted two experiments. Experiment 1 showed that words had large neighbors made a inhibitory effect in the LDT(lexical decision task). Experiment 2 showed the interaction between the neighborhood frequency effectand the word form similarity in the form primed LDT. We concluded that the first syllable in Korean words might be the unit of word neighborhood and play a central role in a lexical access.
