위.장관계 수술 환자간호의 질평가를 위한 도구개발 (Development of an evaluation tool of quality of nursing care for gastrointestinal surgery patient)

  • Background : Quality of professional nursing care is the most essential factor for survival and growth of nursing profession. Then, nursing professionals have responsibility for the evaluation of quality of professional nursing care. The purpose of this study was to develope an evaluation tool of nursing care for patients received gastrointestinal surgery with general anesthesia. This study was a primary work for the developement of a computer program for the evaluation of nursing care. Methods : This study was done through some consecutive steps. They were (1) Developement of items for the tool (2) Developement of an evaluation tool of nursing care quality for the G-I surgery patient (3) Test of reliability and validity of the tool. Two groups of experts and expert pannels who had much experience of the QA and the care of G-I surgery patients participated for developement of the items. 85 nursing records were used for the test of reliability and validity of the developed tool. The evaluation tools were developed with two types of scoring, norm-referenced tool and criterion-referenced tool. Results The system of items for tool was evaluation area evaluation item-indicator. There were 7evaluation areas which contained 32evaluation items which contained 7lindicators. Evaluation areas 1, 2, 3, 4 were for the evaluation of process and 5, 6, 7 were for the evaluation of outcome of nursing care for G-I surgery patient. For the test of interrator reliability, correlation coefficients of each scores of items and intragroup correlation coefficients were calculated. The average correlation coefficients between two rators were 0.65, 0.54 and the intragroup correlation coefficient were 0.99 and 1.00 by the types of scoring. The Cronbach alpha coefficients of the tools were 0.54 and 0.46 by the types of scoring. The average content validity index of the items was 0.95 from 4 pairs of experts. Because there were significant differences between some scores of quality of nursing care of 3 general hospitals regardless of the types of scoring, the tools could be thought to have some construct validity. And also, there were significant correlations between some scores of quality of nursing care and admission days and admission days after surgery regardless of the types of scoring, the tools could be thought to have predictive validity. Conclusion In this study, the evaluation tool of nursing care was developed for the very specified group of patient, G-I surgery patient. And the items were developed and tested by the experts of nursing practice. Because of these reasons, it was supposed that the tool could be used effectively in nursing pratice. And the procedures for the development and the test of the evaluation tool of nursing care in this study were supposed to be used for the developement of other tools.

교육환경 변화에 따른 기술교사의 변화몰입과 기술교육혁신 요구, 커뮤니케이션 및 자기효능감의 구조적 관계 분석 (The Structural relationship among Commitment to Change, Needs of Technology Education Innovation, Communication and Self-Efficacy of Technology Teachers)

  • 이 연구는 기술교사의 변화몰입과 기술교육혁신 요구, 커뮤니케이션 및 자기효능감의 구조적 관계를 구명하기 위한 것으로, 문헌연구와 조사연구에 의하여 수행되었다. 변인 간의 가설적 모형의 적합성을 검증하기 위해 카이자승, 근사치 오차평균제곱근(RMSEA), 적합지수(GFI), 조정적합지수(AGFI), 표준적합지수(NFI), 비교적합지수(CFI) 등을 활용하였다. 연구의 결과에 의하면, 첫째, 기술교육 혁신요구는 기술교사들 간 커뮤니케이션과 변화몰입에 직접적인 영향을 주는 것으로 나타났다. 둘째, 기술교사들 간 커뮤니케이션은 변화몰입에 직접적인 영향을 미치는 것으로 나타났다. 셋째, 자기효능감은 기술교사들의 변화몰입에 직접적으로 영향을 미치는 것으로 나타났다. 따라서, 기술교사들이 변화에 몰입하여 기술교육이 더욱 발전하고 활성화되기 위해서는 기술교사들 간에 활발한 커뮤니케이션이 이루어져야 하며, 무엇보다도 기술교사들이 기술교육의 현황을 분명하게 인식함으로써 기술교육 혁신의 필요성에 공감하도록 할 필요가 있다. 또한, 기술교사들의 자기효능감을 제고하기 위한 정책적인 노력이 있어야 할 것으로 판단된다. 기술교사들의 기술교육혁신 요구, 변화몰입, 커뮤니케이션, 자기효능감 등의 관계를 분석하기 위한 이 연구는 기술교육 분야에서는 처음으로 시도된 것으로 추후에 관련 연구가 이어져야 할 것이다. 특히, 기술교사들의 변화몰입에 영향을 미치는 체계적인 변인 도출, 기술교육 혁신을 성공적으로 이루어내기 위한 방안 등의 연구가 요구된다.

치과병·의원에 내원한 환자들의 행복지수에 영향을 미치는 요인 조사 (Investigation of Factor about Influet to Treatment to Happiness Index among Dental Patients)

  • 본 연구는 주관적 치주건강인식 수와 진료내용(임플란트, 교정진료, 일반진료)에 따른 행복지수를 파악하고, 구강보건측면의 영향요인을 파악함으로써 구강보건관리 프로그램을 개발하여 행복지수를 향상시킬 수 있는 기초자료를 제공하고자 한다. 부산.경남지역에 위치한 치과병 의원에 내원한 환자를 대상으로 2013년 3월부터 약 7개월간 총 400명을 대상으로 실시하여 불성실한 응답 16명을 제외하고 최종적으로 384명을 대상으로 하여 진료내용에 따른 행복지수는 단일문항(p<0.001), 5문항(p=0.037) 모두 교정치료를 받는 경우 높았고, 임플란트 진료를 받는 경우 낮았다. 주관적 치주건강인식 수에 따른 행복지수는 단일문항, 5문항 행복지수 모두 구강증상이 없는 경우 높았고(p<0.001), 주관적 건강상태와 주관적 구강건강 상태에서 매우 건강하다고 인식할수록 행복지수가 높았다(p<0.001). 단일문항 행복지수에 영향요인은 소득이 많을수록(${\beta}$=0.198), 주관적 건강상태가 좋을수록(${\beta}$=0.340) 행복지수가 높았으며, 당뇨질환이 있을수록(${\beta}$=-0.103), 치주질환수가 많을수록(${\beta}$=-0.204) 행복지수가 낮게 나타났다. 5문항 행복지수에 영향요인은 연령이 높을수록(${\beta}$=0.241), 소득이 많을수록(${\beta}$=0.198), 진료내용이 일반일 때(${\beta}$=0.107), 주관적 건강상태가 좋을수록(${\beta}$=0.275), 주관적 구강건강상태가 좋을수록(${\beta}$=0.132) 행복지수가 높았으며, 치주질환수가 많을수록(${\beta}$=-0.195) 행복지수가 낮았다. 병 의원에 내원한 환자를 대상으로 주관적 구강건강 인식을 향상하기 위한 구강건강프로그램이 마련되어야할 것이다.

산모와 배우자의 태교인식과 실천에 관한 연구 (A Study on the Cognition and Practice of the Delivered Woman and Her Husband for the Fetal Education)

  • The purpose of this study was to perform the fetal education effectively for the delivered woman and her husband. This study was conducted by the questionnaire survey on 199 delivered women and their 171 husbands at several hospitals located in Seoul and Kyonggi Province from February 28 to March 26, 2002. The contents of questionnaire included the purposes, the cognition and the practice of fetal education. The SAS program was used for the statistical analysis of the data. The character of subject was analyzed by the percentage. The difference between the cognition and practice of fetal education was analyzed by t-test, ANOVA, Wilcoxon rank sum test. The factor analysis affected on the practice of fetal education was adopted by Multiple regression. The results were as follows; 1. In the purpose of fetal education, the well- balanced emotion showed the highest mark. The cognition of fetal education(woman:$4.39{\pm}0.52$, husband:$3.88{\pm}0.70$) and the practice (woman:$3.88{\pm}0.60$, husband:$3.83{\pm}0.70$) showed the relatively high mark, but the score of cognition showed higher than that of practice. 2. In the comparison of the cognition between the delivered woman and the husband for the fetal education the item of the mental or health state would affect unborn baby, which occupied the highest mark in both woman and the husband(woman:$4.81{\pm}0.44$, husband:$4.81{\pm}0.50$). But they were prohibited to eat the deformed food, which showed the lowest mark(woman:$3.19{\pm}1.12$, husband:$3.21{\pm}1.29$). 3. In the comparison of the practice for the fetal education between the delivered woman and the husband, the practice for the healthy baby showed the highest mark in woman ($4.51{\pm}0.71$), which had a statistically significant difference(P=0.025), compared with that of the husband($4.13{\pm}0.99$). 4. In the comparison of cognition and practice for the fetal education, the general character was associated with the duration of marriage, the satisfaction with marriage and the support of husband on pregnancy. The mark was associated with the age of woman, the level of education and the first birth. 5. The significant factors influencing on the practice for the fetal education were connected with the cognition of fetal education, age, satisfaction with marriage, the support of husband on pregnancy, the type of family, the experience of delivery and the state of health during the period of pregnancy. etc. In conclusion, it is indicated to make effort for transforming and developing the traditional fetal education in accordance with the modern fetal education. And it is suggested that the fetal education might be recognized by all members of family, and the importance of husband's role for the fetal education should be informed as well as that of woman's.

경인 아라뱃길의 물환경 개선을 위한 오염원인 평가항목 및 지표 개발 (Developing Evaluation Index and Item for Water Environment Improvement of Gyeongin ARA Waterway)

  • 본 연구에서는 델파이 기법과 AHP분석을 이용하여 경인 아라뱃길의 수질에 영향을 미치는 다양한 인자를 설정하고 하천의 수질을 체계적이고 객관적으로 평가할 수 있는 평가 항목 및 지표를 개발하였다. 분석결과, 최상위 평가항목에서의 중요도는 물리 환경적 요인 28%, 관리적 요인 26%, 자연 고정적 요인 26%, 사회 문화적 요인이 20% 순으로 나타났다. 3계층 평가항목인 물리 환경적 요인의 내적 요인에서는 자정능력의 손실, 외적 요인에서는 귤현보 및 교량에 의한 오니토 퇴적이 중요도가 가장 높은 것으로 나타났으며, 관리적 요인의 시설요인에서는 축산 및 폐수처리시설의 영향, 정책/제도적 요인에서는 규제권역상의 문제로 나타났다. 또한 자연 고정적 요인의 수생태/점오염원에서는 굴포천의 오염수, 생활환경/비점오염원은 기타하천의 유입수로 나타났으며, 사회 문화적 요인에서 경제적 요인에서는 화물 및 여객운항, 외적 요인에서는 하수처리시설 부족에 따른 영향이 중요도가 가장 높은 것으로 나타났다. 이러한 결과들은 향후 경인 아라뱃길의 수질개선 사업의 우선순위를 산정할 수 있는 논리적 근거와 객관성을 마련하기 위하여 각각의 평가지표들을 측정할 수 있는 구체적인 방법론과 기법에 대한 연구가 지속되어야 할 것이다.

Psychometric Properties of the Persian Version of Satisfaction with Care EORTC-in-patsat32 Questionnaire among Iranian Cancer Patients

  • Background: Cancers impose an increasing burden on health of the populations and individuals, but little is known about cancer patient satisfaction with care. The aim of this study was to assess the psychometric properties of the Persian version of European Organisation for Research and Treatment of Cancer (EORTC) In-Patsat32, as a recently developed questionnaire to assess cancer patient satisfaction with care and information provided during hospital admission. Materials and Methods: Complying with EORTC protocols, the Persian version of Inpatsat32 was translated and piloted in a small group of patients, then applied to 380 cancer patients admitted to different oncology wards in Tehran. Validity (convergent, discriminant, and divergent) and reliability of the tool was assessed through using multitrait analysis, factor analysis, intraclass correlations, Chronbach's alpha and test-retest (on a sample of 70 patients). Results: Good acceptance and high sensitivity of the questionnaire with low floor and ceiling effects were recognized, indicating power of the instrument to detect differences between groups with heterogeneous levels of satisfaction. Multitrait scaling analyses supported the convergent validity of the majority of scales (correlation coefficient >0.4) and favorable discriminant validity (item own scale correlation >0.8). There was no correlation between In-patsat32 scales and the EORTC-C30, which measures different concepts, confirming divergent validity of the tool. Internal consistency for all domains was high (${\alpha}$ >0.70) except for the hospital access score and the test-retest reliability was excellent (r=0.86-0.96). There was a weak responsiveness to change except for nurses technical skills. Principle component analysis confirmed five domains with much improved internal consistency (${\alpha}$ >0.9). Conclusions: The Persian version of the EORTC-in-patsat32 module is a reliable and valid instrument to measure cancer patient satisfaction with care received during their hospitalization period and can be utilized in clinical cancer research.

기내 상품 유통에서 면세품 구색의 혁신: 운항거리와 승무원 이미지 효과 (Innovation in the Assortment of Goods: Effects on Consumer Attitude for In-Flight Duty Free Items)

  • Purpose - The goals of this study are the following. First, this study focused on customer satisfaction of in-flight service. Specifically, in-flight duty free items were considered because of their potential value related with the differentiated strategy of airline companies. Second, this study analyzed feasible strategies that would fence off the aversive attitudes of consumers toward innovation regarding in-flight duty free items. Third, this study strived to discover implicit routes related with the reactions of of consumers to innovation. Fourth, the construal level theory was applied to the context of in-flight service. Psychological distance is expected to promote acceptance of innovation for duty free items. Research design, data, and methodology - This study consisted of three experiments. All data were collected through the participation of university students. First, the experiment employed a 2×2 between-subject design. The first independent variable was temporal distance (long vs. short of navigation time). The second independent variable was innovativeness (innovative duty free items vs. typical items). Further, experiment 2 involved a 2×2 between-subject design. The first independent variable was social distance (typical vs. atypical stewardess image). The second was innovativeness that was based on a pattern similar to that of the prior experiment. The third experiment involved a 2×2×2 design. The first and second independent variables were temporal distance and item innovation, respectively, based on the method of experiment 1. The third independent variable was cognitive depletion (depletion vs. control condition). Results - Experiment 1 demonstrated that the innovation of duty free items would need to consider the journey time of the airline. Specifically, innovative items were preferred in case of a long journey; typical items, however, were liked in a short journey. Further, experiment 2 demonstrated that, in spite of a short journey, innovative items would be preferred if an atypical stewardess was serving. An atypical stewardess was linked with social distance, and the psychological effects would activate a creative and flexible mindset that would fit with innovative duty free items. The final experiment was accomplished for the examination of cognitive processing of psychological distance on innovation-acceptance. Specifically, if the effects were related with systematic processing, then cognitive effort would be needed. In contrast, if they were related with heuristic processing, then such efforts would not be required. The same pattern appeared under both cognitive depletion and control condition; therefore, the effects of psychological distance were implied to be heuristic processing. Conclusions - Managers need to consider the navigation time, stewardess concepts, and depletion of consumers as important factors for innovative strategy regarding in-flight service. Longer journeys are more successful for innovative trials. Further, a more atypical stewardess image is more successful for atypical service. Long navigation and unfamiliar stewardesses may activate creative and flexible thinking. Further, cognitive depletion of consumers is not a dominant factor of psychological distance effects, because the effects are not related with systematic processing, but with heuristic processing.

한국 성인 남녀에서 지방섭취제한 행동 변화단계에 따른 영양상태와 건강관련 행위에 대한 신념 비교 (Comparison of Nutritional Status and Beliefs on Health Behavior Regarding Stages of change in Dietary Fat Reduction among Korean Men and Women)

  • This study describes the application of the stages of change construct to fat intake by examining the associations of the stages of change with nutritional status and beliefs on health behavior. Data were obtained from apparently healthy 596 adults(326 females and 270 males) residing in large cities. Stages of change assessed by an algorithms based on 6 items were designed each subject into one of the 5 stages: precontemplation(PC), contemplation(CO), preparation(PR), action(AC) and maintenance(MA). Beliefs on health behavior were assessed by self efficacy as well as 4 belief scales from the Locus of Illness Control(LIC) developed using factor analysis such as internal disease cure and prevention and external disease cure and prevention. Energy and fat intakes were measured by a 39 item short form food frequency questionnaire. Regarding the 5 stages of change, MA stage comprised the largest group(37.9%), followed by Ac(30.7%), PC(11.4%), CO(10.4%) and PR(9.6%). Subjects who were females, older or healthier were more likely to belong to either AC or MA. Stage assignment of individuals was corroborated by their nutritional variables. Those in PC had the most energy and fat and those in MA ate the least for females. BMI was higher in PR than any other stages for both males and females. Those in PC were distinctive in that they were more externally oriented in terms of health control showing higher scores on external disease prevention(for males) and external disease cure(for females), and lower score on internal disease cure. On the other hand, those in MA received the highest scores on internal disease prevention and self efficacy, which suggested that they were more internally oriented. Canonical discriminant function analysis indicated that the 5 stages were importantly discriminated by BMI, self efficacy, internal disease prevention and external disease prevention for males and by fat intake, self efficacy and external disease cure variables for females. The results of our study confirm differences in stages of change in fat intake in terms of nutritional status and beliefs on heath behavior and indicate the need for taking these phases of change into account in nutrition advice. 해상교통공학적 고려 요소를 이용한 광양항의 장래교통량 예측에 대한 연구 (A Study on the Future Traffic Volume Estimation for Kwangyang Port Using The Consideration Factors of Marine Traffic Engineering)

  • 항만개발의 적정성 및 해상교통 환경평가를 위하여 대상항만의 현재의 입출항 교통량을 이용하여 장래의 교통량을 추정하고 있다. 이는 장래 교통량의 추정을 기초로 하여 항로의 혼잡도, 항로 폭의 결정, 각종 운영규정을 설정하기 때문에 상당히 중요한 요소로 반드시 고려되어야 할 요소이다. 장래 해상교통량 추정방법은 프레터 법칙, 경향 추세식을 이용한 방법 등이 있는데 이전 연구의 대부분은 교통량 추정요소는 그 항만의 입출항 척수를 기초로 장래교통량을 추정하고 있다. 그러나 항만 특성상 입출항 선박의 종류 및 크기가 상이하여 지금과 같이 입출항 척수라는 하나의 요소로 변화 추이를 이용한 장래 교통량 예측은 상당히 어렵다. 이 논문에서는 각 항만의 해상교통 구성 특성요소인 연안 외항선박 척수, 선박 크기별 입출항 척수, 각 선박 당 수송 물동량 등의 변화 추이를 이용하여 장래 교통량 추이를 조사하여 예측하고자 한다. 그리고 수학적으로 모델을 구하기 어려운 비선형 시스템이라 할지라도 입 출력 특성을 묘사할 수 있으며, 입력정보의 왜곡, 잡음 등에 강인한 특성을 가지고 있어서 최근에 비선형 동특성 시스템의 동정(Identification)에 응용되고 있는 신경회로망을 이용하여 장래교통량을 예측한 결과와 상호 비교하고자 한다.

일 대학병원 간호사의 직업만족도와 그에 관련된 요인 조사연구 (A Descriptive Study on Job satisfaction and its Related Factors for Clinical Nurses in Goneral Hospital)

  • Herzberg and his associates concluded from their findings that job satisfaction consisted on two independent dimension ; the first dimension was related to job satisfaction, the second to job dissatisfaction. According to the Herzberg theory, the satisfiers are related to the nature of the work itself and the rewards that flow directly from the performance of that work. The dissatisfaction factors are associated with the individual's relation to the context or environment in which he works. The purpose of this study was to investigate selected factors which result in job satisfaction / dissatisfaction of nurses employed in general hospital. In a study of this nature, it is important that the population be as homogenous as possible in order to reduce the effects of the different environments and backgrounds on job satisfaction. A job satisfaction questionnaire developed by Slabitt et als. was used for this study. It contains 45 statements and utilizes a Likert type scale of 5. Participants were asked to select response which were congruent with their perceptions of the item. It was decided to conduct the study in one general hospital in Seoul. A sample of 505 clinical nurses were selected to participate in this study. The results of this study were as follows ; 1. The overall mean score for the 45 five-point scales of job satisfaction items was 2.945, showing that the subjects of this study were neither satisfied nor dissatisfied. 2. To identify the specific job related factors that result in Job satisfaction / dissatisfaction, the 45 items of job satisfaction were divided into 6 areas and the mean scores and % of agreement were compared. The specific job related factor that resulted in job satisfaction was the job status and those that resulted in job dissatisfaction were salaries, task requirements and organization requirements. But the areas of autonomy and interaction did not belong to either side of the specific job related factors. 3. To identify the relationship between the job satisfaction and the subject's general characteristic, data was analyzed using the t-test and the Pearson correlation coefficient. It was found that the relationship between the job satisfaction and the request for rotation and intention to remain on the job were statistically significant at .05 level, but the relationship between the job satisfaction and age, work experience, and educational background were nor statistically significant at the .05 level.
