A Study on Knowledge, Attitude and Behavior for Radiation Protection of Nurses : Focus on the Operating Rooms and Intensive Care Units (간호사의 방사선 방어 지식, 태도 및 행위에 관한 연구 : 수술실과 중환자실 중심으로)

  • The purpose of this study was to investigate the knowledge, attitudes and behaviors of radiation protection in operating room and intensive care unit nurses. The Participants for this study consisted of 240 nurses working in operating rooms and intensive care units of 7 general hospitals located in Busan, South Korea. The data were collected from Feb. 15 to Feb. 29, 2016. The study results were as follows. First, 72.9% of the nurses had never been educated for radiation safety. Second, the mean score of knowledge for radiation protection was 9.21. The knowledge score of radiation safety educated nurses is higher than uneducated of nurses(t=-5.67, p<.001). Third, The mean score of attitude towards radiation protection was 41.80. The attitude score of radiation safety educated nurses is lower than uneducated of nurses(t=0.02, p<.030). Fourth, the mean score of behavior for radiation protection was 32.57. The behavior score of radiation safety educated nurses is higher than uneducated of nurses(t=0.35, p=.001). Consequently, it was found that nurses had lack of the radiation safety education. Overall knowledge and behavior towards radiation protection was low. Therefore, it is necessary to raise awareness of radiation protection and to educate, manage, and improve processes to increasing compliance with protective measures against radiation.

A Study of the Neonatal Care in the hospitals located in Pusan (부산광역시 소재 병원의 신생아간호 실태조사)

  • This study is designed to produce basic data on the nursing intervention between infants and mothers after discharge from hospitals. This research is carried by investigating the nursing intervention during their after-delivery staying in the hospitals located in Pusan. The object of this study includes the neonatal wards of 51 hospitals. Data collection was carried from the 3rd of September, 2001 to the 4th of October in the same year. Tools for measurement using in this research is the nursing checklist designed by these researchers. The data were analyzed by using SPSS and the output was produced in frequency and percentage considering general characteristics and nursing activities. On the course of investigation, it was revealed that the average number of items checked are as follows; total beds in one hospital came up to 360.1, the beds in each newborn infant ward came up to 36.1, in each ward the numbers of incubators came up to 9.2 and intensive care unit to 1.7. In each ward 8.4 nurses and 2.9 nurse aids were working at that time. In each hospital, secure of air way, physical examination, eye care, umbilical cord care, maintenance of body temperature, identification of infant, nutritive condition, safety and danger management, infection management, mother-infantile interaction, medical assistant to doctor, medication and discharge education were comparatively well done. But, in case of emotional, social and cognitive aspect of nursing activities through interactions between newborn infants and nurses as substitutes are not reached to the adequate level. In the end, this study hereby suggests that further investigation on the device to facilitate the interaction between newborn infant and nurse as a nursing intervention of high quality.

Nurses' Perceptions toward Parent Participation in Pediatric Intensive Care Unit: A Content Analysis (소아중환자실 부모의 돌봄참여에 대한 간호사의 인식: 내용 분석 연구)

  • This paper describes nurses' perceptions toward parental participation in pediatric intensive care units (PICU). Qualitative data were collected from five nurses working at two PICUs in Seoul through individual in-depth interviews. The interviews were conducted from January to February 2016 and analyzed using traditional content analysis. Five categories were found for the parent participation in PICU: needs, attributes, benefits, barriers, and facilitating strategies for parent participation in PICU. Nurses acknowledged the necessities and benefits of parent participation, particularly in PICU, considering the parents' emotional burdens due to the critical health condition of their child as well as the limited visiting policy. The major barriers were a lack of knowledge and the skills of nurses to facilitate parent participation, nurses' heavy workloads, and lack of policies and guidelines supporting parent participation within the PICUs and hospitals. The participants indicated that organizational facilitating-strategies, such as education for nurses about meaning and skills of parent participation in PICU, raising awareness for nurses as well as parents, and ensuring professional staff dedicated to promoting parent participation, to be significant factors. Further study will be needed to develop nursing interventions to integrate parent participation in PICU care.

Development of a Nasogastric Tube Feeding Algorithm to Prevent Aspiration Pneumonia (흡인성 폐렴 예방을 위한 비위관 영양 알고리즘 개발)

  • Purpose: The purpose of this study was developing a nasogastric tube feeding algorithm to prevent aspiration pneumonia. Methods: The algorithm was developed through a methodological design. First, a pilot study was performed to determine the incidence of pneumonia. The second step was development of a preliminary algorithm through a literature review and collection of nurse opinions. The third step was to establish content validity using a panel of 12 experts. The fourth step was revision of the algorithm. Next, 20 intensive care unit nurses applied the revised algorithm for six months to their actual treatment, and the practical feasibility was verified after that. Results: In the patients for whom this algorithm was applied, no cases of pneumonia occurred. The algorithm that was developed by the present author was suitable for clinical application. Conclusion: The effect and practical feasibility of the algorithm was tested with a few patients in this study. The effect of this algorithm should be examined by applying it to more patients on an ongoing basis.

  • Purpose: This study was to investigate Job Stress and the Health status of ICU Nurses. Methods: Data was accumulated from 230 ICU nurses serving at least more than one year in 500 bed order hospitals during the period of three months from June 1 to August 31, 2009. Results: The average job stress was $3.14{\pm}.59$ points, which was relatively high, The average health status was $1.52{\pm}.19$ points, There was significant difference according to religion and subjective health status in Job Stress, There was significant difference according to age, religion and subjective health status in Health Status, There was significant difference according to satisfaction of work, satisfaction of personal relations, impulse to give up duty and in service education in Job Stress, There was significant difference according to satisfaction of work. satisfaction of personal relations, and expectant nursing job in Health Status, There was significantly negative relationship between job stress and health status. Conclusion: Job stress has the most important impact on health status of ICU nurses, Based on the finding, we could conclude that job stress management of ICU nurses should be required to improve health status.

Factors Influencing Professional Quality of Life in Intensive Care Unit Nurses of University Hospitals (간호·간병통합서비스병동 간호사의 직무스트레스가 소진에 미치는 영향: 감성지능의 조절 및 매개효과 )

  • Purpose : This study aims to identify job stress, emotional intelligence, and exhaustion of the nurses in comprehensive nursing service units and then verify the moderating and mediating effects of emotional intelligence on the relationship between job stress and exhaustion. Methods : Participants are 118 nurses working in two general hospitals. The collected data are analyzed using the SPSS WIN 27.0 and AMOS 18.0 programs by assessing frequency and percentage, mean and standard deviation, independent t-test and one-way ANOVA, Scheffé test, Pearson correlation coefficient, and hierarchical multiple regression analysis and bootstrapping. Results : Exhaustion has a statistically significant positive correlation with job stress (r=.40, p <.001) and a statistically significant negative correlation with emotional intelligence (r=-.26, p =.004). A partial mediating effect of emotional intelligence is found between job stress and exhaustion, however, there is no moderating effect. Conclusion : It is expected that to reduce the exhaustion of nurses in comprehensive nursing service units, it is necessary to assess job stress and emotional intelligence, and strengthen emotional intelligence along with job stress intervention.

A Study on Job Stress and the Coping of ICU Nurses (중환자실 간호사의 직무 스트레스와 대처에 관한 연구)

  • Purpose: This study investigated job stress and coping of ICU Nurses. Method: Data was accumulated from 206 ICU nurses serving at least more than one year in 500 bed order hospitals during the period of three months from June1, to August 30, 2006. Results: The average job stress was $2.96{\pm}.95$ points, which was relatively high. The average coping was $2.55{\pm}.23$ points. Conclusion: The extent of the job stress of ICU nurses was relatively high, and they received the heaviest stress from job circumstances. For the prevention of job stress, every effort is required to analyze the causes of stress caused by job circumstances and to pose an appropriate solution. Meanwhile, job stress, needs to be controlled using a solution for the central problem, and search for social support.

An Analysis on Clinical Education of Pediatric Nursing (아동간호학 임상실습교육 현황)

  • This study was conducted to analyse the current clinical education of pediatric nursing in baccalaurate nursing program, then to give basic data for enhancing the quality of future clinical education of pediatric nursing. Data were collected through self-reported questionnaire by mail from December 2001 to February 2002. The subjects were 29 schools of 50 baccalaurate nursing education programs. The data were analysed by double raters, researcher and assistant researcher. The results were summarized as follows: 1. Twenty-eight schools had the objectives of the clinical education of pediatric nursing, and 28 schools in pediatric ward, 23 schools in nursery, 22 schools in neonatal intensive care unit(NICU), 15 schools in objectives related to profession by clinical site. 2. Credits on clinical education of pediatric nursing were most 15 schools of 3 credits. 3. The clinical sites were mainly the hospital that sick children were admitted in. 4. The clinical teacher were 9 types including pediatric professor and field nurse. 5. On teacher's role, the professor instructed the case study and conference, and field nurse instructed the patient assignment and nursing procedures. 6. All of schools used explanation and conference as a method of clinical education, 1 or 2 schools used PBL or role play or field study. 7. On clinical education content, most of school included Apgar scoring system, physical examination in newborn assessment, respira- tion maintenance, temperature maintenance, infection prevention, nutrition, and bath in newborn care. 8. On clinical education content, most of school included care of incubator, phototheraty, infusion, gavage feeding and how to use the instruments in NICU. Eighteen schools included attachment promotion, and 20 schools case study. 9. On clinical education content, most of school included a checklist of nursing procedures, case study, assessment of growth and development in pediatric ward and other sites. 10.There were various evaluation types in scores, measuring items. In conclusion, the results of this study revealed that there were some discrepancy in the objectives and contents, clinical sites on hospital focused, teacher's role, and diversity of measurement items and ratings in clinical education of pediatric nursing. There is a need for a standardization of content, clinical site, and evaluation tool to improve a quality of clinical education of pediatric nursing based on this study.

Results of Applying a Ventilator Weaning Protocol Led by an Advanced Practice Nurse for Cardiac Surgery Patients (심장수술 환자를 위한 전문간호사 주도의 인공호흡기 이탈 프로토콜 적용 결과)

  • Purpose : This study aimed to assess the effectiveness of an advanced practice nurse (APN)-driven ventilator weaning protocol for patients undergoing cardiac surgeries. Methods : A retrospective analysis was conducted on 226 patients admitted to the intensive care unit (ICU) of a tertiary hospital between January and June 2020, following a cardiac surgery. Patients were divided into an APN protocol-applied group (experimental group, n=152) and a control group managed based on doctors' judgment (n=74). Ventilator weaning criteria and clinical outcomes, including duration of ventilation, length of ICU stay, and rate of reintubation, were compared between the two groups. Results : Patients in the control group were older and had a higher incidence of massive bleeding from chest tube drainage (>100 cc/hr) at baseline. The average duration of ventilation was significantly shorter in the experimental group compared to the control group (7.44 vs. 21.61 hours, p <.001). Furthermore, the mean length of ICU stay was shorter in the experimental group compared to the control group (47.96 vs. 77.97 hours, p <.001). There was no difference in the rate of reintubation between the two groups. Conclusion : These findings suggest that an APN-driven ventilator weaning protocol can improve clinical outcomes without significant complications. These results support the adoption of APN-driven mechanical ventilator weaning protocols in clinical practice.

The Association of Health Care Workers' Uniforms and Health Care-associated Infection: Systematic Review (병원근무자 유니폼에 의한 병원 내 감염에 대한 체계적 문헌고찰)

  • Purpose: To identify an associations between health care workers' uniforms and health care-associated infection. Methods: Electronic databases, including Ovid-Medline, the Cochrane Library, CINAHL, EMBASE, KMbase, and KoreaMed, were searched. The search terms included doctor, nurse, health care worker/staff/assistant, clothing, (white) gown, uniform, (neck)tie, and attire. Only papers published in English and Korean were included. Results: 16 studies were selected from 1,900 references screened. All of the studies were non-comparative studies except for one. Four were conducted with doctors, six with nurses, one with health care workers including physiotherapists and one for medical staff plus visitors in a neonatal intensive care unit. Doctors more frequently changed their uniforms than neckties; therefore, the degree of contamination was more serious in neckties. The cuff zone was more likely to be heavily contaminated than other areas of long-sleeve gowns. Coats become contaminated quickly once worn, and colony counts reached a similar level within the first few hours after wearing them. Wearing a plastic apron or protective clothing did not prevent the bacterial contamination of nurses' uniforms, and the best way to decrease the contamination was changing to newly laundered uniforms before starting every duty. Conclusion: Healthcare workers' uniforms are a potential source of health care-associated infection although there was no robust evidence. The government must establish standards for laundering of uniforms or a requirement for institutions to provide a laundering service for healthcare workers' uniforms.
