First report of cross-species transmission of deer hepatitis E virus to a guanaco in Korea

  • The hepatitis E virus (HEV) is a leading causative agent of acute hepatitis in humans. Zoonotic HEV strains have been isolated from several animal species, including pigs. New HEV variants have been recently isolated from camels in the Middle East. In the present study, fecal samples from fallow deer, formosan deer, alpaca, and guanaco were analyzed for the detection of HEV. One HEV strain was detected from guanaco, a species of camelids. The nucleotide sequence of guanaco HEV was identical to those of deer HEV-3 strains, which implied the cross-species transmission of HEV-3 from deer to guanaco.

  • We hereby observe four co-infection cases of pandemic influenza H1N1 and Mycobacterium tuberculosis with various clinical presentations. It may be prudent to consider M. tuberculosis co-infections when patients with pandemic influenza reveal unusual clinical features that do not improve despite appropriate treatments against the influenza, especially in Korea, in the endemic areas of M. tuberculosis.

  • Several animal species including pigs are directly involved in the zoonotic transmission of the hepatitis E virus (HEV) to humans. This study was conducted to detect HEV in bovine and porcine raw livers by nested reverse transcription-polymerase chain reaction. Zoonotic HEV strains were identified in 1.0 and 3.0% of the tested bovine and porcine livers, respectively. HEV-4 was detected in the bovine livers, but both HEV-3 and HEV-4 were identified in the porcine livers. These results indicate that zoonotic transmission of HEV may occur via consumption of raw or undercooked livers of pigs and cattle.

A Case on Streptococcal Pneumonia Associated with Leptomeningitis, Osteomyelitis and Epidural Abscess in a Patient with AIDS

  • Patients with acquired immunodeficiency syndrome (AIDS) are at higher risks of bacterial pneumonia than the general population, and the pathogen is the most commonly involved Streptococcus pneumoniae. We hereby report a case of pneumococcal pneumonia associated with leptomeningitis, osteomyelitis and epidural abscess in a patient with AIDS. He is being successfully treated with ampicillin/sulbactam and clindamycin. And because the pneumococcal infection is usually associated with morbidity and mortality rates in the setting of AIDS, we should consider for pneumococcal vaccinations among the AIDS populations.

  • Hepatitis E virus (HEV) can infect not only human but also several animals. This study has been conducted to evaluate the comprehensive anti-HEV seroprevalence in zoo animals in Korea. Anti-HEV antibodies were identified in 14 of 64 zoo animal species. HEV antibodies were detected for the first time in Eurasian Lynx, Setland Pony, Fallow Deer, Ezo Sika, Formosa Deer, East Wapitis, Barasingha, Corriedale, American Bison, Guanacos, Reticulated Giraffe, and Saanen. These results indicate that the several zoo animal species were exposed to HEV.

  • The hepatitis A virus (HAV) induces severe acute liver injury and is adapted to human and monkey cell lines but not other cells. In this study, the HAV was inoculated into porcine kidney (PK-15) cells to determine its infectivity in porcine cells. The growth pattern of the HAV in PK-15 cells was compared with its growth pattern in fetal rhesus kidney (FRhK-4) cells. The growth of HAV was less efficient in PK-15 cells. In conclusion, HAV replication was verified in PK-15 cells for the first time. Further investigations will be needed to identify the HAV-restrictive mechanisms in PK-15 cells.

  • 리메렐라 아나티페스티퍼 감염증은 오리와 거위에서 섬유소성 심막염, 간주위염증, 기낭염, 건락성난관염, 뇌막염을 특징으로 하는 급성 또는 만성 패혈증이다. 이 균은 혈청형 또는 분리주별로 병원성에 큰 차이가 나타나는 것으로 알려져 있다. 그럼에도 불구하고 지금까지 이러한 다양한 병원성과 그 이유에 대한 연구는 거의 이루어지지 않았다. 본 연구에서는 리메렐라 아나티페스티퍼의 병원성과 serum resistance 상관성을 구명하였다. 우리는 다양한 분리원으로부터 확보한 130주의 균주를 대상으로 serum resistance 특성을 분석하였다. 건강한 오리 인후두에서 분리된 균주들은 혈청에 대한 감수성이 높은 반면에 전신감염을 일으킨 균주들은 강한 serum resistance를 보였다. 또한 우리는 이러한 혈청의 살균효과가 혈청내 보체 성분에 의해 유도됨을 확인하였다. 강한 serum resistance를 유발하는 세균의 표면 유전자와의 관련성을 조사한 결과, 외막 단백질의 AS87_09335, AS87_00480, AS87_05195 유전자가 serum resistance와 관련 있음을 알 수 있었다. 본 연구 결과로 serum resistance 특성이 리메렐라 아나티페스티퍼의 병원성 결정 요소 중 하나라는 것을 확인하였다.

Evaluation of immunocontraceptive vaccine composed of gonadotropin-releasing hormone conjugated with granulocyte-macrophage colony-stimulating factor in male rats

  • Surgical castration performed to reduce male-associated problems results in pain and microbial infections in male animals. Therefore, immunocontraception, which is mediated by the animal's own antibodies against reproductive hormones, has been recommended as an alternative to surgical castration when considering the animal's welfare. In this study, a new immunocontraceptive vaccine composed of six tandem copies of gonadotropin-releasing hormone (GnRH) fused to rat granulocyte-macrophage colony-stimulating factor (GM-CSF) was developed, and its efficacy was evaluated in male rats. Three different doses (10, 50, and $100{\mu}g$) of recombinant GM-CSF-GnRH protein were injected three times at intervals of two weeks into male rats. The rats vaccinated with three doses of GM-CSF-GnRH produced a significantly higher level of antibodies against GnRH than that in the negative control rats. Severe atrophy of gonads was observed in rats vaccinated with three doses of GM-CSF-GnRH but not in the negative control rats. The results reveal that the new GnRH vaccine conjugated with rat GM-CSF induces efficient immunocontraception in male rats. This formulation of the immunocontraceptive vaccine would be applicable to both domestic and pet male animals.

Infectious Causes of Eosinophilic Meningitis in Korean Patients: A Single-Institution Retrospective Chart Review from 2004 to 2018

  • Eosinophilic meningitis is defined as the presence of more than 10 eosinophils per µl in the cerebrospinal fluid (CSF), or eosinophils accounting for more than 10% of CSF leukocytes in patients with acute meningitis. Parasites are the most common cause of eosinophilic meningitis worldwide, but there is limited research on patients in Korea. Patients diagnosed with eosinophilic meningitis between January 2004 and June 2018 at a tertiary hospital in Seoul, Korea were retrospectively reviewed. The etiology and clinical characteristics of each patient were identified. Of the 22 patients included in the study, 11 (50%) had parasitic causes, of whom 8 (36%) were diagnosed as neurocysticercosis and 3 (14%) as Toxocara meningitis. Four (18%) patients were diagnosed with fungal meningitis, and underlying immunodeficiency was found in 2 of these patients. The etiology of another 4 (18%) patients was suspected to be tuberculosis, which is endemic in Korea. Viral and bacterial meningitis were relatively rare causes of eosinophilic meningitis, accounting for 2 (9%) and 1 (5%) patients, respectively. One patient with neurocysticercosis and 1 patient with fungal meningitis died, and 5 (23%) had neurologic sequelae. Parasite infections, especially neurocysticercosis and toxocariasis, were the most common cause of eosinophilic meningitis in Korean patients. Fungal meningitis, while relatively rare, is often aggressive and must be considered when searching for the cause of eosinophilic meningitis.

  • The main purpose of this thesis is the Korean version of Gwang On Yeok Ron(廣瘟疫論). Gwang On Yeok Ron is a specialized book about infectious diseases(瘟疫). The book was compiled by Dae Cheon jang(戴天章) in 1722 who was a medical man in the Qing dynasty. The book consists of four volumes and an appendix. "Gwang On Yeok Ron Vol. I(廣瘟疫論卷之一)" is for the demonstration of colds(傷寒) and infectious diseases through a patient's odor(氣), skin tone(色), tongue(舌), mental state(神) and pulse(脈). Also, it describes the complications(兼證, 夾證) of infectious diseases. "Gwang On Yeok Ron Vol. II(廣瘟疫論卷之二)" describes the external symptoms(表證) of infectious diseases and the remedies. "Gwang On Yeok Ron Vol. III(廣瘟疫論卷之三)" describes the internal symptoms(裏證) of infectious diseases and the remedies. "Gwang On Yeok Ron Vol. IV(廣瘟疫論卷之四)" presents principles for the remedy of infectious diseases, and describes the sequelae(遺證) after the recovery of infectious diseases. "Appendix of Gwang On Yeok Ron(廣瘟疫論方目)" presents 83 prescriptions for the remedy of infectious diseases.
