Nausea, vomiting and retching are universal symptoms that affect individuals' state of health and self-care activities of individuals. Accurate measurements of individual symptoms are required to gather more definitive data, and enhance understanding, planning, and implementation of self-care actions. Recently the Rhodes Index of Nausea, Vomiting, and Retching(INVR), a new format of the INV-2(the Rhodes Index of Nausea and Vomiting), was developed to measure the symptoms of nausea, vomiting and retching in an English speaking population. To determine the reliability and validity of the INVR, and the possibility of using the instrument in Korea, the Korean translation of the INVR and the INV-2 were administered to a convenient sample of 105 patients at two University Hospitals in Kwangju, Korea. The Cronbach's alpha to estimate the internal consistency of reliability for INVR was 0.844. Equivalent measures of reliability were conducted to determine the percentage of agreement and the Spearman rank correlation coefficients for responses on the two instruments. The percent agreement was 83% and the correlation coefficient was 0.906 over all. A significant differences between the INVR scores of the patients with and without nausea, vomiting, or retching were seen, which indicated a construct validity. The INVR was found to be more user friendly for the patient and the healthcare providers. As a result, it is suggested that the INVR can provide a scientific base for measuring the symptoms of nausea, vomiting, and retching for nurses to improve patients' care and quality of life.
Object : The purpose of this study is to report vomiting patient with medullary infarction, who improved by additional Kwan-Won herbal acupuncture. Method : We have treated the patient with Kwan-Won acupuncture and other Oriental Medical treatment. The effect was and evaluated by progress of symtoms and Mcgill Nausea Question(MNQ) that consists of Nausea Rating Index(NRI), Overall Nausea Index(ONI) and Visual Anlogue Scale(VAS). Result : After 6th times of treatment, nausea and vomiting was disappeared. In the score of MNQ, NRI score changed from 19 to 1, ORI score from 5 to 1 and VAS score from 10 to 0. Conclusion : These results suggest that Kwan-Won herbal acupuncture and other Oriental Medical treatment have good effect on severe vomiting. But further studies are required.
Purpose: The aim of this study was to identify the effects of auricular acupressure on nausea, vomiting and serum serotonin level in gynecological cancer patients with chemotherapy. Methods: The research was a randomized control group pretest-posttest design. Participants were 26 patients for the experimental group, and 26 patients for the control group. Auricular acupressure in Shen men, Occiput, Stomach and Sympathetic points was administered to those in the experimental group. Data were analyzed using ${\chi}^2$-test, t-test, ANCOVA, and repeated measures of ANOVA. Results: The experimental group had reported significantly lower mean nausea NRS (Number Rating Scale), INVR (Index of Nausea, Vomiting, and Retching), and serum serotonin level than those in the control group. Conclusion: Results of the study show auricular acupressure is an effective nursing intervention for reducing chemotherapy-induced nausea, vomiting and serum serotonin level of gynecological cancer patients.
Objectives: We carried out an observational study of the effects of Nei Guan (P6) and Gong Sun (SP4) acupuncture for the treatment of CINV, preparing for further randomized controlled trial study. This is a case series to explore the changes in the incidence of CINV by acupuncture. Methods: Patients reported a Rhodex index indicating the severity of nausea and vomiting and loss of appetite, before acupuncture, after acupuncture and 1-week follow-up examination. Twelve patients with CINV participated in this study. We included patients receiving chemotherapeutic agents that might induce moderate or severe nausea and vomiting. We conducted 5 treatment sessions with P6 and SP4 acupuncture over the course of 5 days. Results: The median Rhodex score were decreased in 5 patients over time, while we observed loss of appetite of 6 patients were improved over time. During the study, the mean of Rhodex of total patients revealed a decrease of Day 1, Day 7, Day 14; 0.74, 0.68, 0.38, respectively, while the mean of loss of appetite showed a decrease of Day 1, Day 7, Day 14; 5.25, 4.08, 4.00, respectively. Conclusions: The study gives a preliminary data that manual acupuncture of P6 and SP4 may reduce the severity of CINV and loss of appetite, and justifies further study.
Purpose: The purpose of this study was to determine the changes on Index of Nausea, Vomiting, & Retching (INVR) during a cycle of chemotherapy, Methods: Forty-three patients hospitalized for chemotherapy at C University Hospital during a period of 5 days from March to May, 2003 were examined, Scores of INVR were measured once a day, Anxiety, anorexia, fatigue, and sleep satisfaction were measured before chemotherapy, Data was analyzed by repeated measures of ANOVA, Results: The score of INVR increased over time during the days of hospitalization and showed a peak on the third day, The score was significantly higher on the third and consecutive cycles than on the first and second cycle, The score was significantly higher in patients in their forties and fifties rather than in their sixties, The score was higher in women than in men, and also increased as the sleep satisfaction decreased, Conclusion: These results suggested that specific interventions for relief of nausea & vomiting were needed in middle age, women, the third chemotherapy cycle, and the third day after chemotherapy.
Purpose: The purpose of this study was to compare the effects of ondansetron combined with dexamethasone on Post-Operative Nausea and Vomiting (PONV) and pain with ondansetron alone in patients with laparoscopy assisted vaginal hysterectomy under general anesthesia. Methods: Data were collected from April 1 through September 30, 2005 using a double blind method. Ondansetron 4 mg and dexamethasone 10 mg were administered to the experimental group (25 patients), and ondansetron 4 mg only to the control group (25 patients). The medications were administered through an intravenous line at the beginning peritoneum suture. PONV by Index of Nausea Vomiting and Retching (INVR), nausea by Visual Analogue Scale (VAS), and pain (VAS) were assessed at postoperative 1 hr, 3 hr, 6 hr, 24 hr, and 48 hr. Data were analyzed using repeated measures ANOVA, and Bonferroni methods. Results: The experimental group that received ondansetron combined with dexamethasone had less PONV (p=.048), and nausea (p=.012) than control group that received ondansetron alone. However, there was no difference in pain (p=.557) between the patients in the two groups. Conclusion: We conclude that the administration of ondansetron combined with dexamethasone is more effective than the administration of ondansetron alone to reduce PONV in patients with laparoscopic hysterectomy.
Purpose: To determine the effects of hand massage on nausea and vomiting, and anxiety in children with Iymphocytic leukemia receiving high dose chemotherapy. Method: The children were assigned to an experimental group(15) or a control group(15). All of the children were diagnosed with acute Iymphocytic leukemia and admitted for high dose chemotherapy at C University Medical Center in Seoul. The hand massage was performed for 10 minutes twice a day over three days. To measure the effects of hand massage, the Index of Nausea and Vomiting by Rhodes et al. and the State-Trait Anxiety Inventory for children by Spielberger were used. The level of anxiety was measured by systolic blood pressure, diastolic blood pressure, and pulse rate. Results: The score for nausea and vomiting decreased in the experimental group. State anxiety for the experimental group was significantly more positive than for the control group at the 2nd measurement. There was a significant difference in systolic blood pressure between the two groups. The level of diastolic blood pressure in the two groups decreased significantly over time. Conclusion: Hand massage could be effective in decreasing nausea and vomiting, state anxiety, pulse rate and blood pressure in children with acute leukemia receiving high dose chemotherapy.
Purpose: Patient-controlled epidural analgesia (PCEA) is an effective method for controlling post surgical pain. However, it is associated with adverse drug reactions such as nausea and vomiting. In this, study we tested the effects of Nei-Guan (P6) acupressure on PCEA-associated nausea, vomiting, and pain in geriatric patients after total knee replacement arthroplasty (TKRA). Methods: Ninety-nine patients who had TKRA for the first time were randomly assigned to either, experiment group (n=50) or control groups (n=49). All patients received PCEA immediately after surgery, but acupressure on Nei-Guan (P6) point was applied to experiment group only. Ten minutes of finger acupressure on Nei-Guan (P6) acupressure point was applied twice with 15 minute-interval in the experiment group. The incidence of nausea, vomiting, and retching as well as pain intensity were assessed at 12 and 24 hours after surgery. Nausea, and vomiting were assessed by Index of Nausea, Vomiting and Retching (INVR) questionnaire. Pain intensity was measured with frequency of analgesics. Results: Vomiting both was significantly different between two groups at 12 hours (t=-2.18, p=0.03) and 24 hours (t=-2.64, p=0.01) after surgery. Total scores of nausea, vomiting and retching in experimental group was significantly lower than control group 24hours after surgery (t=-2.18, p=0.03). However, pain was not different between two groups. Conclusion: Nei-Guan (P6) acupressure may be considered as an effective nursing intervention to decrease PCEA-associated nausea and vomiting in older patients after TKRA.
The purpose of this study is to assess the effect of progressive muscle relaxation therapy on nausea, vomiting and anxiety experienced by children who were histologically diagnosed as malignancy and have received cancer chemotherapy. Twenty subjects with 10 in experimental group and 10 in control group, between the age of 7 to 12 years participated in this study. All subjects were admitted for inpatients chemotherapy at the S hospital between April to August 1998. The methods used for interventional muscle relaxation were both the 16 items of the progressive muscle relaxation therapy developed by Jacobson and the 4 items of the muscle relaxation therapy by Wolpe. The method for assessing nausea, vomiting was the Index of Nausea and Vomiting by Rhodes et al and that for anxiety was State-Trait Anxiety Inventory for Children by Spielberger. The data was analysed by repeated measures ANOVA and Scheffe test using SAS program. The results were as follows : 1. On measurement of nausea, vomiting, in the experimental group, the mean values of the second and the third measurements showed tendency to slightly decrease compared with the first measurement, but there was no statistically significance. And in the control group, there were no significant differences among first measurement, second measurement and third measurement. Also there was no significant difference in the mean values assessing nausea, vomiting between experimental and control group (F=1.33, p=0.2645). 2. The results assessing state anxiety showed, in the experimental group, progressive decrease in mean values at each measurement, while the control group showed no significant differences between the mean values of each measurement. Also there was statistically significant difference in state anxiety measurements between experimental and control group(F=4.36, p=0.05). In conclusion, muscle relaxation therapy for school-aged children with chemotherapy could not be effective to decrease nausea, vomiting, but it could be effective to decrease anxiety.
Purpose: This study was to confirm the effect of acupressure on the emesis control and the weight change among pediatric cancer patients receiving anti-cancer chemotherapy. Method: Forty pediatric cancer patients, receiving the induction stage of chemotherapy with MTX and vincristine, were divided into control(n=20) and the intervention group(n=20). Both groups received regular anti-emesis medication, but the intervention group was added acupressure maneuver for 5 minutes on P6 point for 3 times a day for 5days: before chemotherapy, lunch and dinner by investigator during the hospitalization and by mother at home. The instruments for this study were Rhode's(1986) Index of nausea, vomiting and retching(INVR), Cas electric scale and pamphlet developed by researcher. Result: Significant differences in the degree of nausea and vomiting were observed between the control and the intervention group as measured by INVR(t=4.73; p=.01). Repeated measures ANOVA also shows that the group effect was significant(F=22.39, P=.01) as was the time effect(F=380.35, P=.01). The group by time interaction was also significant(F=5.27, P=.01). Acupressure maneuver was apparently effective in reducing the degree of chemotherapy-induced nausea and vomiting. There were also statistically significant weight loss noted in the control group than the intervention group(t=5.42, p=.01). Conclusion: Acupressure on P6 point shows an effective adjunct maneuver in reducing the degree of nausea and vomiting and conserving the weight in pediatric cancer patients. Therefore, it is proposed that acupressure should be applied as supportive nursing intervention strategies to relieve chemotherapy induced nausea and vomiting and to prevent weight loss in pediatric cancer patients.
