Background: Faced with highly prevalent and recalcitrant cancer-related fatigue (CRF), together with the absence of any official guidelines on management, numerous groups have been striving to seek and test alternative therapies including acupuncture and moxibustion. However, different patients have various feedbacks, and the many clinical trials have given rise to varied conclusions. In terms of the therapeutic effect of acupuncture and moxibustion, there exist vast inconsistencies. Objective: The aim of the study was to evaluate the auxiliary effectiveness of acupuncture and moxibustion in the treatment of CRF, and to provide more reliable evidence to guide clinical practice. Methods: Randomized controlled trials (RCTs) published before December 2012 were all aggregated, focusing on evaluation of acupuncture or moxibustion for CRF. The quality of the included studies was assessed basing on Cochrane handbook 5.1.0, and the available data were analyzed with RevMan software (version 5.2.0). Descriptive techniques were performed when no available data could be used. Results: A total of 7 studies involving 804 participants were eligible. With real acupuncture versus sham acupuncture, subjects receiving true acupuncture benefited more in the reduction of fatigue. With real acupuncture versus acupressure or sham acupressure, fatigue level appeared 36% improved in the acupuncture group, but 19% in the acupressure group and only 0.6% with sham acupressure. When real acupuncture plus enhanced routine care was compared with enhanced routine care, the combination group improved mean scores for general fatigue, together with physical and mental fatigue. With real acupuncture versus sham acupuncture or wait list controls, the real acupuncture group displayed significant advantages over the wait list controls at 2 weeks for fatigue improvement and better well-being effects at 6 weeks. When moxibustion plus routine care was compared with routine care alone, the meta-analyses demonstrated the combination had a relatively significant benefit in improving severe fatigue and QLQ-C30. Conclusion: Up to the search date, there exist few high quality RCTs to evaluate the effect of acupuncture and moxibustion, especially moxibustion in English. Yet acupuncture and moxibustion still appeared to be efficacious auxiliary therapeutic methods for CRF, in spite of several inherent defects of the included studies. Much more high-quality studies are urgently needed.

The purpose of this paper is to explore the strategy of future Korean medicine(KM) clinical research through the study on the proposals for KM clinical research worldwide. In this study, the papers published in English through Pubmed were investigated mainly. Among them, we analyzed the methodological proposals from the clinical research papers that were published in the KM related fields. Various proposals for improving the problems in KM clinical studies are as follows. First, KM clinical research should be designed based on understanding for the theory, backgrounds, paradigms and worldviews of KM. In addition, considering the model validity, KM clinical research model should include the diagnosis, interventions and outcomes measurement methods reflecting the characteristics and real practice in KM. The internal validity and external validity should be also taken into account. One of the most important thing is to identify the contents about various and complex 'real practice' in KM. A prospective observational study was suitable for the purpose of this study. Finally, we suggested a few improvement directions for RCTs studies in KM. First, we would be able to improve the quality and the internal validity in KM clinical research using the checklists of CONSORT(Consolidated Standards for Reporting Trials) Statement and STRICTA(Standards for Reporting Interventions in Clinical Trials of Acupuncture). Second, we could use various clinical research methods and the modified research of RCTs such as PCT(pragmatic clinical trial) to reflect the characteristics of actual KM practice. Consequently, we would be able to improve the external validity. Third, the KM diagnosis and outcomes measurement methods should be developed based on an actual KM practice and it should reflect a real practice. The 'pattern identification(辨證)' is the core to KM diagnosis. But in order to be applied to the clinical research, the pattern identification(辨證) should be objectified and standardized. Future KM clinical research model should reflect the characteristics and a real practice in KM. In addition, it should include the advantage of rigorous RCTs research.Specially, the diagnosis, interventions and outcomes measurement methods in KM clinical research should reflect this view.
This study reviews empirical studies that investigated the impact of marketization of long-term care services on quality of care in Korea, England, Sweden, United States, and Japan. The trend and characteristics of marketization and regulatory system in those countries were compared. Since 2008 when long-term care insurance was introduced in Korea, we selected and reviewed empirical studies which were listed in the National Research Foundation, PubMed and Web of Science and examined the relationship between marketization and quality of care. Seven Korean, three English, two Swedish, seven American studies and one Japan study were identified and there was little evidence supporting that marketization would lead to better quality of care. Specifically, competition had no consistent relationship with quality of care while there was little evidence supporting that for-profit nursing homes tended to exhibit greater quality of care compared to non-for-profit nursing homes. The chains of for-profit nursing homes were revealed to have greater deficiencies. The findings of this review imply that improvement of quality of care may not be achieved by making for-profit nursing homes have chains. Reconstruction of nursing home markets should be needed, and the number and the proportion of public and non-for-profit nursing homes should be increased.

As a sequel of "special improvement act for gifted student education" legislated on January 2000, "regulation act for gifted student education" was published on April 2002 which is the time Korea has settled down its education for the gifted. Announced in the December 2007 "general plan for development of gifted student education" provided a platform for the gifted student education in Korea of growth in quantity, in which a plan of providing gifted student education up to 1 percent of the elementary and middle school level students (approximately 70 thousands) has been established while the education currently provides to 0.59 percent (40 thousands) of all students. Until recently, however, education for gifted students has been performed based on the way of concentrating on academic domains. and it has put more weights on mathematics and english domains. In order to overcome this drawbacks, there have been various attempts for growth in quality of education for gifted students, one of them is the our proposal of convergence of science and art education for cultivating 21 century creative humans through establishment of new type of institution. In this paper, education curriculum and management strategies appliable to the proposed convergence education institutions for gifted students. For this purpose we derived the implication points through analysis on education processes used in korea science school for the gifted students, a representative institution for the gifted students in Korea, and we suggested educational process curriculums for the science and art institute for gifted students along with the detailed contents of convergence subject which is an essential subject to the institute.

Recently, Depending on the quality of data increases, the problem of time-consuming to process the image is raised by being required to accelerate the image processing algorithms, in a traditional CPU and CUDA(Compute Unified Device Architecture) based recognition system for computing speed and performance gains compared to OpenMP When character recognition has been learned by the system to measure the input by the character data matching is implemented in an environment that recognizes the region of the well, so that the font of the characters image learning English alphabet are each constant and standardized in size and character an image matching method for calculating the matching has also been implemented. GPGPU (General Purpose GPU) programming platform technology when using the CUDA computing techniques to recognize and use the four cores of Intel i5 2500 with OpenMP to deal quickly and efficiently an algorithm, than the performance of existing CPU does not produce the rate of four times due to the delay of the data of the partition and merge operation proposed a method of improving the rate of speed of about 3.2 times, and the parallel processing of the video card that processes a result, the sequential operation of the process compared to CPU-based who performed the performance gain is about 21 tiems improvement in was confirmed.

Idol music, which began to gain huge popularity in the domestic music market in the mid-1990s, has become one of the major global contents thanks to the global popularity of K-POP. As a result, the Korean music market continues to grow, forming the world's sixth-largest music market, and domestic music agencies are focusing more on fostering idol groups and producing idol music. The global success of idol music is surprising, but this has resulted in the domestic music market being biased toward idol music. As a result of the study, it was confirmed that there are several problems with the phenomenon of being biased toward similar types of musicians and music content. First, just as trend-oriented cultural contents face the problem of life expectancy all the time, the "Korean Wave" is also forced to think in terms of identity and sustainability. Second, it was observed that only consumers of a certain age may cause cultural alienation of other age groups, and thirdly, various problems such as shrinking creative paths due to the size of the cost required for idol group production and the lifespan of idol musicians' art activities. This paper derives the reality of the domestic idol bias phenomenon through comparative analysis of the English-American music market and the domestic music market, which have had a profound influence on the global music market in popular music history, and verified the theory and results through an expert survey using the Likert scale. In addition, the problems caused by the idol bias phenomenon were considered based on the theory of cultural diversity, and improvement directions were also suggested to solve this problem.
The purpose of this study was to analysis of food adaptation and dietary behavior of marriage migrant women in according to their acculturation type, and find out their relationship. The survey was conducted by well-trained researchers using questionnaires translated into English, Chinese and Vietnamese. Food adaptional score was the highest in the assimilation type, and the separation type was the lowest. Dietary behavioral score in daily intake of milk and dairy foods was the highest in the assimilation type. There were positive correlations occurred between food adaptional score and dietary behavioral score, and also, between food adaptional score and assimilation score. Analysis of structural equation models shows that only assimilation affects food adaptational score, and food adaptational score affects the improvement of dietary behavior. In conclusion, marriage migrant women's positive attitude of new cultures, assimilation, might help them not only adapt to new cultures, but food adaptation and right dietary behaviors.
Given that single blockade with angiotensin-converting enzyme inhibitors (ACEIs) or angiotensin receptor blockers (ARBs) can achieve only partial and undurable suppression of the Renin Angiotensin System (RAS), it has been hypothesized that dual blockage would be more beneficial in the management of blood pressure (BP) reduction and prevention of progressive chronic kidney disease (CKD) than either agent alone. Thus, it has been suggested that the combination of an ACEI and an ARB might provide renal benefits to hypertensive patients over and above BP reduction. However, this might also expose patients to additive or synergistic side effects. We attempted to conduct a systematic review to evaluate the benefits and harms of combination therapy in hypertensive patients with or without kidney diseases. MEDLINE and KoreaMed were searched for relevant randomized clinical trials in adult hypertensive patients with or without diabetes (restricted to 1997, limited to trials published in English). Results were summarized using the random-effects model, and between-studies heterogeneity was estimated with $I^2$. A final analysis of ten trials (23,928 patients) revealed that the combination of an ACEI and an ARB reduced blood pressure (SBP/DBP) by 3.95/2.02 mmHg (95% confidence interval [CI], -4.38 to -3.53/-2.33 to -1.71) compared with ACEI monotherapy, and 2.83/2.64 mmHg (95% CI, -3.25 to -2.41/-4.95 to -0.33) compared with ARB monotherapy. Eight trials (391 patients) demonstrated a significant reduction in 24h-proteinuria (weighted mean difference, 0.16 g/day, 95% CI, -0.26-0.05), but they did not translate into an improvement in GFR. Tests for heterogeneity showed no difference in effect among the studies. The combination therapy reduced proteinuria by 30% (95% CI, 23% to 37%) and 39% (95% CI, 31% to 48%) compared with ACEI monotherapy and ARB monotherapy, respectively. However, in patients who had proteinuria more than 0.5 g/day, the combination therapy failed to show significant reduction in urinary protein excretion. The current cumulative evidence suggests that diabetic patients with proteinuria on dual RAS blockade have an increase risk of adverse events such as hyperkalemia, hypotension, and so on, compared with ACEI or ARB alone. It is, therefore, proposed that the combination therapy should not be routinely used for the treatment of hypertension with or without compelling indications.
In this paper, we propose a new acoustic model for characterizing segmental features and an algorithm based upon a general framework of hidden Markov models (HMMs) in order to compensate the weakness of HMM assumptions. The segmental features are represented as a trajectory of observed vector sequences by a polynomial regression function because the single frame feature cannot represent the temporal dynamics of speech signals effectively. To apply the segmental features to pattern classification, we adopted segmental HMM(SHMM) which is known as the effective method to represent the trend of speech signals. SHMM separates observation probability of the given state into extra- and intra-segmental variations that show the long-term and short-term variabilities, respectively. To consider the segmental characteristics in acoustic model, we present segmental-feature HMM(SFHMM) by modifying the SHMM. The SFHMM therefore represents the external- and internal-variation as the observation probability of the trajectory in a given state and trajectory estimation error for the given segment, respectively. We conducted several experiments on the TIMIT database to establish the effectiveness of the proposed method and the characteristics of the segmental features. From the experimental results, we conclude that the proposed method is valuable, if its number of parameters is greater than that of conventional HMM, in the flexible and informative feature representation and the performance improvement.
The purpose of this study is 1) to survey to identify how high school students think about career guidance and job and 2) to pursue effective career guidance. According to the survey result, first, most students have thought about their career. They chose friends as a counselor for their future. Among respondents, 39.84% of them said they did not consult with teachers. They said academic record is a stumbling block in choosing career. Second, many students responded career guidance should start at 'Junior High school'. 76.56% of respondents said they were lack of career guidance resources. With respect to career, an important subject was shown as the following order: social study, English, Korean, Science and Math. Third, students recognized the face that aptitude and ability should be the utmost important factors in choosing career. They chose mass media as a main source to gain information relative to career, which accounted for 52.34% and 20.31% for teachers. In the case of students who have rather 'poor'home background, teachers were the main sources for career information, whose proportion was relatively high. Forth, parents were the most influential people who helped students choose which high school to go(40.69%). In terms of scope of higher education, 65.63% said they wanted to graduate university. Students who has good academic records and good home environment wanted to pursue Master's or Doctor's degrees. 70.31% of respondents said they attended a high school just because they wanted to have a desired job.
