인상채득 방법이 임플란트 보철 작업모형의 정확도에 미치는 영향 (EFFECT OF IMPRESSION TECHNIQUES ON ACCURACY OF MASTER CASTS FOR IMPLANT PROSTHESES)

  • Osseointegrated implant prostheses are to provide normal function without compromising the unique interaction between the tissue and the implant. The essential requirement for the osseointegrated implant prostheses is passive fit of abutment. Therefore, the impression must be accurate and reproduciable since the resultant master cast precisely duplicates the clinical condition. The purpose of this study was to compare the accuracy of the master casts obtained from five impression techniques. Group 1 : To take impression with indirect technique and Impregum F. Group 2 : To take impression with unsplinted direct technique and Impregum F. Group 3 : To take impression with splinted direct technique and Impregum F. Group 4 : To take impression with unsplinted direct technique and Xanthano. Group 5 : To take impression with splinted dierct technique and Xanthano. The results were as follow : 1. In taking impression of Impregum F, there was no significant difference between to use of indirect technique and unsplinted direct technique.(p<0.05) 2. Unsplinted direct technique with Impregum F is less accurate than splinted direct technique with Impregum F or Xanthano and unsplinted direct technique with Xanthano.(p<0.05). 3. There was no significant difference between splinted direct technique with Impregum F and unsplinted direct technique with Xanthano.(p<0.05) 4. Splinted direct technique reproduce more accurate than unsplinted direct technique.(p<0.05) 5. Impression plaster produced less distortion than polyether.(p<0.05) As a result, splinted direct technique with Xanthano was the most accurate technique. As a result, splinted direct technique with Xanthano was the most accurate technique in this study. In addition to dimensinal changes in the materials used, positional errors were also attributed to the mechanical components used in the transfer porocess. Although the errors measured were relatively small, this study demonstrates the potential for distortions with the transfer technique used. Further study is indicated that ?the technique will be able to reproduce the intraoral relationship of implant fixtures reliably and predictably.

  • In order to compare the accuracy of impression technique using the addition silicone putty and improved heavy body material, impression were taken for the natural 6 maxillary anterior teeth, and for the metal mast model that has full arch shape with 4 cylindrical abutment teeth. Marginal gingiva was retracted by routine method using retraction cord, and two techniques were tried to compare the length of impression materials in the gingival sulcus. This was aimed to see the effect of viscosity and hydraulic pressure of impression materials for the subgingival impression. Impressions for the full arch-shaped metal master model were taken to compare the linear stability of the different impression materials and their related techniques. The conclusions were as belows : 1. The one-step impression technique showed longer extension of impression material in gingival sulcus than two-step impression technique. 2. High viscosity and hydraulic pressure of impression material were useful to take subgingival impression. 3. There was no statistically significant difference for the two-dimensional accuracy of impression technique, but two-step technique showed better trend than one-step technique.

Comparison of the fit of cast gold crowns fabricated from the digital and the conventional impression techniques

  • PURPOSE. The purpose of this study was to compare the fit of cast gold crowns fabricated from the conventional and the digital impression technique. MATERIALS AND METHODS. Artificial tooth in a master model and abutment teeth in ten patients were restored with cast gold crowns fabricated from the digital and the conventional impression technique. The forty silicone replicas were cut in three sections; each section was evaluated in nine points. The measurement was carried out by using a measuring microscope and I-Soultion. Data from the silicone replica were analyzed and all tests were performed with ${\alpha}$-level of 0.05. RESULTS. 1. The average gaps of cast gold crowns fabricated from the digital impression technique were larger than those of the conventional impression technique significantly. 2. In marginal and internal axial gap of cast gold crowns, no statistical differences were found between the two impression techniques. 3. The internal occlusal gaps of cast gold crowns fabricated from the digital impression technique were larger than those of the conventional impression technique significantly. CONCLUSION. Both prostheses presented clinically acceptable results with comparing the fit. The prostheses fabricated from the digital impression technique showed more gaps, in respect of occlusal surface.

인상채득방법과 방향에 따른 인상체의 정밀성과 체적 안정성에 관한 연구 (A STUDY ON ACCURACY AND DIMENSIONAL STABILITY ACCORDING TO IMPRESS10N METHODS)

  • Accurate reproduction of intraoral tissue is essential in the fabrication of fixed prosthesis. This selection of impression material and method is very important. In this study, vinyl polysiloxane type impression material $Panasil^{(R)}$ was used. Three impression methods ; one-step technique, individual tray technique, and individual tooth tray technique using the vinyl polysiloxane type bite registration material $Futar^{(R)}$ Occlusion, were used to take horizontal and vertical impression. Improved stone models were fabricated. The amount of occlusal surface discrepaneies, interabutment distance discrepancies, amount of marginal defect were evaluated with the $X-PLAN360d^{(R)}$ and photoscanning. The results were as follows. 1. There was no significant difference in occlusal surface according to the directions and techniques of impression taking. 2. There was no significant difference in interabutment distance discrepancies according to the directions and techniques of impression taking 3. There was no significant difference in marginal discrepancies according to the direction of impression taking but there was significant difference between one-step technique and individual tray technique, individual tooth tray technique (P<0.05). Taken together, these results suggest that individual tray technique and individual tooth tray technique are more accurate for impression taking than one-step technique.

  • Statement of problem. Use of the conventional dental impression procedure is problematic in patients who have difficulty opening their mouth, difficulty breathing through their nose or tendency to gag. Purpose. It is necessary to make individual trays more comfortable for patients during impression taking procedure. It was reported at the KAP Annual Meeting 2001 Seoul that an improved impression technique was suitable for this purpose. In this study, the accuracy of the improved dental impression method for implant was compared with the conventional dental impression method. Material and methods. An oral simulator was made from clear acrylic resin block which had similar form of edentulous ridge. For setting up the standard, five fixtures were installed on it. Study casts were made using two kinds of impression techniques. One was the conventional method that was taken using silicone impression material and an individual resin tray under connection of inter-fixture relation. The other was the improved method in which was the connection of the impression coping and the thermoformed polymethyl methacrylate tray. In addition, two different study casts were made from the improved impression body. The coordinates of the fixture on the study model were measured by three-dimensional coordinate measuring equipment. Then the distances between each fixture were calculated and compared with that of oral simulator. Accuracy of the each impression method was also assessed. Results. The differences of inter-fixture dimension between study casts and simulator in the improved impression technique showed $0.014{\pm}0.016mm$ and $0.017{\pm}0.022mm$, respectively and that of the conventional method was $0.017{\pm}0.014mm$. There was no significant difference between the improved impression technique and conventional method. Conclusion. The improved impression technique is useful for multiple support implants.

Closed Mouth Impression technique에 관한 연구 (A STUDY OF THE CLOSED MOUSE IMPRESSION TECHNIQUE)

  • The author conducted an clinical study of closed mouth impression technique with a case of complete denture patient of favorable ridge contour to see the effect on the lingual border molding of lower denture. As a result, the shape of the border was formulated a quite different contour from the open mouth impression technique. The obtained contour had to be reshaped considerably. It was found that functional harmony with the surrounding tissue was difficult to be maintained. However, the contour is surely stable and favorable form for the retention of the lower denture provided patient is possible to adapt.

  • 단일 치아의 인상채득을 위해서는 바이트 트레이를 이용한 폐구인상법이 선호된다. 그러나 임플란트 인상채득을 위해서는 단일치와 다수치 구분 없이 편악 트레이를 이용한 개구인상법이 주로 사용되고 있다. 바이트 트레이를 이용하는 폐구인상법은 시간이 절약되고 재료가 적게 들며, 모형의 교합기 부착 시 상하악 악간 위치 관계 오차 발생 확률이 적다. 본 증례에서는 2 가지 종류의 폐구인상용 코핑을 이용해 바이트 트레이로 인상채득 후 단일 임플란트 고정성 보철물을 제작함에 있어 만족할 만한 결과를 얻어 보고하고자 한다.

Modified soft tissue cast for fixed partial denture: a technique

  • In process of fabrication of a fixed partial denture, dies are trimmed to expose margins of the preparations. The need for the soft tissue cast is quite evident as the soft tissue emergence profile that surrounds the prepared tooth is destroyed in the process of fabrication. This article describes a modified technique to fabricate the soft tissue cast for the conventional fixed partial denture. The impression made with the polyvinylsiloxane was first poured to prepare the die cast. After retrieval of the cast, the same impression was poured second time with the resin based resilient material to cover the facial and proximal gingival areas. The remaining portion of the impression was poured with the gypsum material. This technique does not require additional clinical appointment, second impression procedure, technique sensitive manipulations with impression, or cumbersome laboratory procedures. The simplicity of this technique facilitates and justifies its routine use in fabrication of the fixed partial denture.

  • In order to compare the accuracy of impression method using addition silicone putty impression material, metal master die was fabricated with 4 cylindrical abutments that were similar in shape to mandibular arch. Among the 4 abutments, two(A, D) with 8mm width and 7mm height were formed in the 2nd molar regions and the other two(B, C) with 6mm width and 7mm height were on the canine regions. Impressions were taken using one-step putty wash impression technique and two-step putty wash impression technique by three different types of impression materials(Perfect, Express, Exaflex). Upon measuring the distance between the abutments on the model by three dimensional measuring machine, the percent of devitaion of the materials were obtained, rendering the following results. The results obtained are as follows : 1. There was no significant difference in accuracy in regard with the impression method between one-step putty wash impression technique and two-step putty wash impression technique using addition silicone putty impression material. 2. There were no difference in accuracy among with three different addition silicone putty impression materials. 3. All the distances between abutments on improved stone models increased in comparison with those on the metal master model.

디지털 방식을 이용하여 제작한 전부 지르코니아 수복물의 변연 적합도 평가 (Evaluation of marginal fit of monolithic zirconia restoration by digital impression)

  • Purpose : The purpose of this study was to compare the marginal fit of monolithic zirconia restorations fabricated form the conventional and the digital impression technique. Materials and methods : Ten patients were restored with monolithic zirconia restorations fabricated from the digital and the conventional impression technique. Before definitive insertion, silicone replicas were produced from all twenty crowns. The silicone replicas were cut in three sections; each section was evaluated at two points. The measurement was carried out by using a measuring microscope (Olympus BX 51) and I-Solution. Data from the silicone replica scores were analyzed by Shapiro-Wilk test for normal distribution and the t-test for equality of two population's mean. All tests were performed with ${\alpha}-level$ of 0.05. Results : The average marginal gaps of monolithic zirconia restorations were $133.81{\pm}36.46{\mu}m$ for the conventional impression technique, $90.07{\pm}9.47{\mu}m$ for the digital impression technique. No statistical differences were found between the two impression techniques Conclusion : Both prostheses presented clinically acceptable results with comparing the marginal fit.
